The Global Initiative for Asthma (GINA) 2025 has conducted a comprehensive revision of the diagnosis and management of asthma in children aged 5 years and younger.The most important update is the first-time proposal of the specific conditions for asthma diagnosis in this age group.This article aims to interpret the relevant content of diagnosis, exacerbation management, assessment and treatment of asthma in children aged 5 years and younger in GINA 2025.It is hoped that this will provide a reference for clinical practice and enhance the management of asthma in children aged 5 years and younger.

The Global Initiative for Asthma (GINA) continues to optimize asthma treatment strategies and has introduced the concepts of anti-inflammatory reliever (AIR) and maintenance and reliever therapy(MART). AIR mainly includes Budesonide-Formoterol, Beclomethasone-Formoterol, and inhaled corticosteroids(ICS)combined with short-acting beta2-agonist.MART refers to a treatment regimen that utilizes the same ICS-Formoterol combination for both maintenance and reliever therapy.Currently, AIR and MART are recommended for the treatment of asthma in children aged 6 years and older, adolescents and adults.This article provides an interpretation of the pediatric AIR and MART content in GINA 2025, highlighting their clinical application in pediatric asthma treatment and their role in asthma action plans, with the aim of providing a reference for clinicians to optimize the treatment of pediatric asthma.

5.Design and application effect of a multisensory supported device for preterm infants

Objective To design a multisensory supported device and evaluate its effectiveness on preterm infants(born before 34 weeks)during NICU hospitalization.Methods The multisensory supported device is composed of a basement,several soft cushions and an adjustable eye mask.The inner layer of the device comprises of the head and tail boundaries,serves as uterine wall-like circular boundaries.The outer skeleton is equipped with multisensory stimulation modules to provide visual,hearing,and tactile sensory stimulations for premature infants.The study was conducted in a NICU of a tertiary A children specialist hospital in Shanghai,China.The convenience sampling method was used and based on the ratio of 1∶2 between the experimental and the control group in this study.The control group was treated by standard nursing care,while the experiment group was treated with the multisensory supported device in addition to NICU conventional care.All infants were assessed during the week of admission and again at corrected gestational age of 36 weeks.The actigraphy watch which was used for 72 hours continuous record of the activities of study infants,allows the researcher to compare the activity scores,wakefulness and sleep indicators of 2 groups of infants.Results 71 preterm infants were enrolled in the study,and 60 preterm infants completed data collection for study data analysis,including 20 in the experimental group and 40 in the control group.There were no statistical differences in demographic characteristics and clinical status regarding wakefulness,sleep and physical development between the 2 groups in baseline(P>0.05).At 36 weeks of corrected gestational age,the activity score of the experimental group was(46.61±12.14)points,and that of the control group was(57.33±18.36)points,with statistically significant differences in 2 groups(P=0.024).The total waking time of the experimental group was(384.85±169.42)min,and that of the control group was(492.08±220.45)min,with statistically significant differences in 2 groups(P=0.049).There was no statistical difference in other indicators between 2 groups(P>0.05).Conclusion The multisensory supported device can reduce high-frequency unpleasant activity as well as frequent wakefulness status,which could promote the sleeping quality of preterm infants.Further studies are needed to verify further effects of the device on premature infants'physical development.

无义介导的信使RNA(mRNA)降解途径(nonsense-mediated mRNA decay,简称为NMD)是真核生物细胞内一种重要的基因转录后表达调控机制,它积极参与一系列细胞生理和生化过程,控制细胞命运和生命体的组织稳态。NMD的缺陷会导致人类疾病,如神经发育障碍、肿瘤发生和自身免疫疾病等。UPF3 (Up-frameshift protein 3)是一个核心的NMD因子,它最早在酵母中被发现。UPF3A和UPF3B是UPF3在生物进化到脊椎动物阶段出现的两个旁系同源物,在NMD中具有激活或抑制的作用。以往研究发现,UPF3B蛋白几乎在所有哺乳动物器官中均有表达,而UPF3A蛋白在除睾丸外的大多数哺乳动物组织中难以被检测到。解释这一现象的假说为:在NMD途径中,UPF3B具有比UPF3A更高的竞争性结合UPF2的能力,UPF3B和UPF2的结合促使UPF3A成为游离状态,而游离的UPF3A蛋白不稳定且易被降解。此假说提示UPF3A和UPF3B在NMD中存在拮抗作用。在本研究中,我们重新定量评估了UPF3A和UPF3B在野生型成年雄性和雌性小鼠的9个主要组织和生殖器官中的mRNA和蛋白表达,结果证实UPF3A在雄性生殖细胞中表达量最高。令人惊讶的是,我们发现在包括大脑和胸腺在内的大多数组织中,UPF3A与UPF3B的蛋白水平相当,而在小鼠脾、肺组织中,UPF3A表达高于UPF3B。公共基因表达数据进一步支持了上述发现。因此,我们的研究表明了UPF3A是小鼠组织中普遍表达的NMD因子。同时,该研究结果推测:在生理条件下,UPF3A和UPF3B蛋白之间不存在竞争抑制,且UPF3A在多种哺乳动物组织的稳态中发挥重要作用。

7.Epidemiological characteristics of sexually transmitted diseases in Zhejiang Province from 2018 to 2022

Objective:
To investigate the epidemiological characteristics of sexually transmitted diseases (STDs) in Zhejiang Province from 2018 to 2022, so as to provide insights into improving STDs prevention and control strategies.
Methods:
Data of syphilis, gonorrhea, condyloma acuminatum, genital herpes and genital Chlamydia trachomatis infections reported in Zhejiang Province from 2018 to 2022 were collected from the Infectious Disease Surveillance System of Chinese Disease Prevention and Control Information System. The temporal, spatial and population distribution of STDs were analyzed, and the trends in incidence of STDs were evaluated using annual percent change (APC).
Results:
A total of 386 770 STDs cases were reported in Zhejiang Province from 2018 to 2022, with mean annual reported incidence of 127.80/105. The mean annual reported incidence from high to low was syphilis (44.11/105), genital Chlamydia trachomatis infections (40.20/105), gonorrhea (24.92/105), condyloma acuminatum (13.32/105), and genital herpes (5.34/105). The incidence of STDs appeared a tendency towards a decline from 2018 to 2022 (APC=-9.332%, P<0.05), with an average annual growth rate of -9.41%. Syphilis (APC=-11.536%, P<0.05), condyloma acuminatum (APC=-7.509%, P<0.05), and genital Chlamydia trachomatis infections (APC=-9.009%, P<0.05) showed a appeared a tendency towards a decline. The top three highest reported incidence were reported in Hangzhou City (185.98/105), Taizhou City (133.85/105) and Lishui City (122.73/105), respectively. The mean annual reported incidence of females was higher than that of males (140.60/105 vs. 117.83/105; P<0.05). Most STDs cases were at ages of 20 to 49 years (265 314 cases, 68.60%). Farmers (118 096 cases, 30.53%), household and unemployed personnel (85 924 cases, 22.22%) were the predominant occupation.
Conclusions
The incidence of STDs appeared a tendency towards a decline in Zhejiang Province from 2018 to 2022, with regional differences in incidence. People at ages of 20 to 49 years, farmers, household and unemployed personnel are high-risk populations for STDs.
8.The therapeutic effect of Chaihu Longgu Muli Decoction combined with western medicine in the treatment of post-stroke depression

Objective:To evaluate the clinical efficacy of Chaihu Longgu Muli Decoction in the treatment of post-stroke depression.Methods:A total of 130 patients with post-stroke depression who met the inclusion criteria from April 2019 to December 2020 were enrolled in the study. The patients were randomly divided into two groups with 65 cases in each group. The control group was treated with Flupentixol and melitracen tablets on the basis of routine treatment, and the observation group was treated with Chaihu Longgu Muli Decoction on the basis of routine treatment. Both groups were treated for 4 weeks. TCM syndrome scores were performed before and after treatment. The levels of serum Hcy and hs CRP, Plasma 5-HT and brain-derived neurotrophic factor (BDNF) were detected by ELISA, and the ratio of neutrophils to lymphoblasts (NLR) was recorded.Results:The response rate was 92.3% (60/65) in the observation group and 75.4% (49/65) in the control group. There was significant difference between the two groups ( χ2=6.87, P=0.009). After treatment, the serum Hcy [(11.87±1.56) μmol/L vs. (16.69±1.77) μmol/L, t=16.47] and hs-CRP [(7.81±1.66) mg/L vs. (16.45±4.87) mg/L, t=13.54] in the observation group were significantly lower than those in the control group ( P<0.01), and Plasma 5-HT [(150.41±17.54) μg/L vs. (123.24±16.02) μg/L, t=9.22], BDNF [(19.45±2.48) μg/L vs. (12.56 ± 2.23) μg/L, t=16.66] were significantly higher than those in the control group ( P<0.01). After treatment, the NLR and TCM syndrome scores in the observation group were significantly lower than those in the control group ( t values were 6.03 and 7.15, respectively, all Ps<0.01). Conclusion:The Chaihu Longgu Muli Decoction combined with western medicine treatment can correct the levels of Hcy, hs-CRP, 5-HT, BDNF and NLR in peripheral blood, alleviate the state of depression and anxiety of patients with post-stroke depression.
9.Post-COVID-19 condition in children

Post-COVID-19 condition in children refers to the presence of physical symptoms or syndromes lasting more than 12 weeks after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, which can be manifested as a combination of various signs and symptoms.Girls, school-aged, adolescents, and children with severe illness in the acute phase of COVID-19, with underlying allergic diseases and poor baseline physical and mental health are high-risk factors for post-COVID-19 condition in children.Pathogenesis may be related to viral persistence, autoimmunity, chronic inflammatory responses, chronic inflammation of vascular endothelial cells, and microthrombosis.A comprehensive treatment scheme including symptoms relieving treatment, rehabilitation, and psychological support.This article reviews the epidemiology, clinical manifestations, pathogenesis, diagnosis, treatment, and management of the post-COVID-19 condition in children to improve clinicians′ comprehension of the disease.
10.A case report of leprosy reaction combined with bullous pemphigoid after treatment of paucibacillary leprosy

ObjectiveTo investigate the clinical characteristics of leprosy-related neuritis with bullous pemphigoid after treatment of paucibacillary leprosy. MethodsThe treatment of leprosy reaction combined with bullous pemphigoid of a cured case of leprosy was analysed. ResultsFive years after standard treatment for leprosy, erythema and vesicles appeared in the limbs without obvious inducement, and the disease became more and more severe. With clinical diagnosis and pathological examination, pemphigoid was confirmed, and the patients were given hormone treatment for leprosy reaction and anti-immunotherapy, as well as symptomatic supportive treatment. ConclusionLeprosy reaction and pemphigoid are both related to immunity, but the occurrence of both at the same time is relatively rare, so in the clinical process we should attach great importance to early detection, early diagnosis and prompt treatment to prevent further harm to the patient.
