1.Analysis of Inflammatory Indexes and Microecological Characteristics of Vaginal Secretions in Patients with Fungal Vaginitis

Objective To explore the inflammatory indexes and microecological characteristics of vaginal secretions in patients with fungal vaginitis. Methods 320 patients with fungal vaginitis admitted to Wuxi Xishan People's Hospital from January 2019 to January 2022 were selected. According to pathological examination,the patients were divided into A:[simple fungal vaginitis group (n=216)]and B:[fungal vaginitis with cervical lesions group (n=104)]. The clinical data and vaginal microbiological indexes of the two groups were compared. The risk factors of cervical lesions were analyzed using a random forest algorithm. Results The number of patients with vaginal pH>4.5,the numbet of patients with vaginal secretions in grade Ⅲ to Ⅳ and the levels of TNF-α,IL-2 and IL-12 in group B were higher than those in group A(t/x2=4.862~12.924),CRP,IL-4,IL-6 and IL-10 were significantly lower than those in group A(t=9.025~12.962),with statistical significance(all P<0.05),respecitively. Group A has the largest proportion of Lactobacillus crispatus (18.89%) and group B has the largest proportion of Lactobacillus iners (27.05%). The diversity of vaginal microflora in group B was significantly higher than that in group A. The dominant bacterial species in group A were Lactobacillus crispatus and Lactobacillus helyeticus,and the dominant bacterial species in group B were Staphylococcus aureus,Bifidobacterium breve,Lactobacillus iners and Bacteroides fragilis. Compared with group A,the abundances of Glycosyltransferases,Fatty acid biosynthesis and Transporters in group B were significantly reduced. Nicotinate and nicotinamide metabolism and Glycerophospholipid metabolism. The abundances of pathways such as metabolism,Prenyltransferases and Amino acid related enzymes were significantly increased. Random forest analysis showed that Lactobacillus iners,Lactobacillus crispatus,Staphylococcus aureus,Gardnerella vaginalis,Bacteroides fragilis and Bifidobacterium breve were important bacteria groups affecting cervical lesions. Conclusion The microbial composition ratio in vaginal secretions of different patients is different,and the diversity of vaginal flora in patients with fungal vaginitis accompanied by cervical lesions is higher than that in patients with simple fungal vaginitis,Lactobacillus iners,Lactobacillus crispatus,Lactobacillus crispatus,Lactobacillus iners,Lactobacillus crispatus,Staphylococcus aureus,Gardnerella vaginalis,Bacteroides fragilis and Bifidobacterium breve are important groups of bacteria affecting cervical lesions.
2.Intervention effect of puerarin on rats with bacterial periodontitis by regulating NLRP3/Caspase-1 signaling pathways

OBJECTIVE To observe the intervention effect of puerarin on the rats with bacterial periodontitis by reg-ulating nucleotide-binding oligomeric domain-like receptor protein 3/Caspase-1(NLRP3/Caspase-1)signaling pathways.METHODS Totally 44 male SPF rats were chosen as subjects and were divided into the blank group,the model group,the puerarin group and the puerarin plus inhibitor group.The models of rats with bacterial periodon-titis were successfully established.The periodontal indexes and alveolar resorption were observed and compared a-mong the groups.RESULTS The sulcus bleeding index(SBI)and plaque index(PLI)scores of the model group and the puerarin plus inhibitor group were higher than those of the blank group;the NLRP3 level was 0.87±0.09 in the model group,0.85±0.08 in the puerarin plus inhibitor group,higher than 0.18±0.03 in the blank group;the Caspase-1 level was 1.15±0.11 in the model group,1.03±0.10 in the puerarin plus inhibitor group,higher than 0.35±0.04 in the blank group.The trabecular separation spacing(Tb.Sp),cemento-enamel junction-facial bone crest(CEJ-FBC),interleukin--1β(IL-1β)level,interleukin-6(IL-6)level,tumor necrosis factor-α(TNF-α)level and osteoclast counts of the model group and the puerarin plus inhibitor group were higher than those of the blank group;while the trabecular thickness(Tb.Th),bone volume/tissue volume(BV/TV)and bone mineral density(BMD)of the model group and the puerarin plus inhibitor group were lower than those of the blank group.The SBI and PLI scores of the puerarin group were lower than those of the model group;the NLRP3 level of the puerarin group was 0.20±0.04,lower than 0.87±0.09 of the model group;the Caspase-1 level of the puerarin group was 0.37±0.05,lower than 1.15±0.11 of the model group.The Tb.Sp,CEJ-FBC,IL-1βlevel,IL-6 lev-el,TNF-α level and osteoclast counts of the puerarin group were lower than those of the model group;while the Tb.Th,V/V and BMD of the puerarin group were higher than those of the model group.The SBI and PLI score of the puerarin plus inhibitor group were higher than those of the puerarin group;the NLRP3 level of the puerarin plus inhibitor group was 0.85±0.08,higher than 0.20±0.04 of the puerarin group;the Caspase-1 level of the pu-erarin plus inhibitor group was 1.03±0.10,higher than 0.37±0.05 of the puerarin group.The Tb.Sp,CEJ-FBC,IL-1β level,IL-6 level,TNF-α level and osteoclast counts of the puerarin plus inhibitor group were higher than those of the puerarin group,while the Tb.Th,V/V and BMD of the puerarin plus inhibitor group were low-er than those of the puerarin group,there were significant differences in the above comparisons(P<0.05).CONCLUSIONS Puerarin can effectively reduce the periodontal plaque index and bleeding index of the rats with bacterial,reduce the levels of inflammatory factors and alleviate the damage of tissue.The action mechanisms may be associated with the NLRP3/Caspase-1 signaling pathways.

5.Perioperative Rehabilitation Nursing of Thoracic Breathing Reconstruction for Cervical Spinal Cord Injury

Objective To summarize experience of nursing for thoracic breathing reconstruction in patients after cervical spinal cord in-jury (CSCI). Methods 6 cases accepting thoracic breathing reconstruction in our hospital from March 2013 to June 2014 were reviewed from 1 week before operation to 4 weeks after operation. Results All the patients, aged (41.7 ± 16.2) years, received tracheotomy (6.0 ± 5.8) days after injury because of weakened cough, retention of respiratory secretions and dyspnea;the ventilator was used for (45.3±25.9) days, and they received thoracic breathing reconstruction surgery (84±26.7) days after injury. Their vital capacity was (1085±92) ml before opera-tion and was (1680±283) ml 2 weeks after operation with 55%increment (P<0.01). Only 1 patient underwent a delayed wound healing. No pulmonary infection, urinary tract infections and pressure sore occurred until 4 weeks after operation. Conclusion Enhancement of airway care and prevention of pressure sore are the keys to reduce postoperative complications of thoracic breathing reconstruction.
