Psoriasis vulgaris is notoriously difficult to treat and prone to recurrence. Traditional Chinese medicine (TCM), however, has shown considerable efficacy in mitigating or suppressing such recurrence. The underlying reason lies in the TCM concept of "latent pathogens", which are prone to be reactivated by external pathogenic factors, thereby triggering relapse. At the early stage of recurrence, manifestations of "latent fire" often appear externally. If treatment is not thorough, the condition may shift into a state of "stalemate between healthy Qi and pathogenic factors", in which the disease appears on the skin but is rooted in deeper pathological layers, remaining unresolved and accumulating internally. Over time, blood stasis arises from fire, and the fire further congeals due to stasis, leading ultimately to recurrent flare-ups. This aligns with the modern immunological concept of "immunological memory" mediated by tissue-resident memory T cells (TRM) in the skin, which corroborates the TCM view of "latent fire inducing blood stasis". The interaction between TRM and keratinocytes (KC) parallels the entanglement of latent fire and latent stasis, both of which are deeply entrenched and difficult to resolve. The core pathogenesis of recurrent psoriasis vulgaris lies in "latent fire causing blood stasis". The hallmark is the deep concealment and persistence of latent fire and stasis, which linger and await an opportunity to reemerge. Based on this understanding, Xijiao Dihuangtang is employed to cool the blood, resolve stasis, and eliminate latent pathogens, and treatment is tailored according to the disease stage through three-phase syndrome differentiation. In the progressive stage, both exterior and interior are treated, with emphasis on clearing latent fire. In the stationary stage, the focus shifts to dispelling latent stasis and simultaneously regulating the Zang-fu organs. In the regressive stage, efforts are made to prevent the retention of latent pathogens and to strengthen healthy Qi. Accordingly, drugs effective in dispersing wind and clearing heat, pungent-moistening and dredging the collaterals, and tonifying deficiency and moistening dryness are often employed to achieve optimal outcomes. The precise mechanisms by which Xijiao Dihuangtang treats recurrent psoriasis vulgaris remain to be fully elucidated. Current research suggests it may intervene in the recurrence process through inhibiting KC proliferation via the PI3K/Akt/mTOR signaling pathway and glycolysis, regulating the Th1/Th2 and Th17/Treg cell balances to restore immune homeostasis, suppressing inflammatory cytokine production to alleviate the inflammatory response, modulating angiogenesis-related factors, such as vascular endothelial growth factor A (VEGF-A) and matrix metalloproteinase-9 (MMP-9), to control disease progression, and restructuring the gut microbiota to modulate systemic immunity and thereby influence the course of disease recurrence.
3.Association Between Epicardial Atrioventricular Groove Fat Thickness and Prognosis of Patients With Dilated Cardiomyopathy

Objectives:To investigate the predictive value of epicardial fat volume(EFV)and atrioventricular groove fat thickness(AVGT)—morphological biomarkers of epicardial adipose tissue—for major adverse cardiovascular events(MACE)in patients with dilated cardiomyopathy(DCM).Methods:This study enrolled 216 DCM patients.EFV and AVGT were obtained from cardiac magnetic resonance imaging(CMR).Patients were divided into event-free group(n=142)and event group(n=74)based on MACE occurrence during follow-up.Receiver operating characteristic(ROC)curve analysis was used to determine optimal cutoff values.Survival differences were assessed using Kaplan-Meier analysis,Cox proportional hazards regression analysis was used to identify independent risk factors,and restricted cubic spline(RCS)models were used to evaluate dose-response relationships.Results:AVGT and EFV were significantly higher in the event group than in event-free group(both P<0.05).ROC analysis identified optimal MACE-predicting cutoffs as follows:AVGT≥7.74 mm(area under the curve[AUC]=0.57)and EFV≥78.6 ml(AUC=0.62).Kaplan-Meier analysis revealed significantly lower MACE-free survival rates in patients with AVGT≥7.74 mm and EFV≥78.6 ml(both P<0.05).Cox regression analysis confirmed that AVGT(HR=2.18,95%CI:1.34-3.54)and EFV(HR=1.81,95%CI:1.11-2.96)were independent MACE risk factors(both P<0.05)in this patient cohort.RCS models demonstrated the significant linear associations between EFV/AVGT and MACE risk(bothoverall P<0.05).Conclusions:EFV and AVGT,the non-invasive imaging biomarkers quantifying and characterizing fat distribution,are independently correlated with elevated MACE risk in DCM patients.These metrics serve as potential prognostic indicators,enriching risk stratification indicators for early identification of high-risk patients and guiding personalized medication strategies.

AIM:To study the mechanism of oxy-sophoridine(OSR)in relieving neuropathic pain(NP).METHODS:The analgesic effect of OSR was observed by measuring mechanical pain sensitivity.By combining OSR with agonists and antagonists re-lated to synthesis and metabolism of gamma ami-nobutyric acid(GABA),the mechanism related to analgesic effects of OSR and GABA nervous system is studied.The expression of c-Fos immunopositive cells and the expression of c-Fos and Glutamic acid decarboxylase(Glutamic acid decarboxylase 67)in brain and spinal cord were detected by immunoflu-orescence staining.Co-expression of GAD67,GABA transporter 1(gamma-Aminobutyric acid Transport-er 1,GAT-1)immunopositive cells.RESULTS:Com-pared with Sham group,spared nerve injury(SNI)group showed increased mechanical pain sensitivi-ty,increased expression of c-Fos immunopositive cells,decreased and increased co-expression of c-Fos and GAD67 and GAT-1 immunopositive cells,re-spectively.Compared with SNI group,mechanical algesia in OSR 500 and 1 000 mg/kg groups was de-creased,algesia in OSR 250 mg/kg combined with antagonist group was decreased,and algesia in OSR 500 mg/kg combined with agonist group was increased.The co-expression of c-Fos and GAD67 immunopositive cells in the brain and spinal cord of OSR 500 mg/kg group increased,while the co-ex-pression of c-Fos and GAT-1 decreased.CONCLU-SION:OSR has a good analgesic effect on NP mice induced by SNI.The mechanism is that OSR increas-es the content of central GABA by up-regulating GABAergic neurons in brain and spinal cord.
5.Exploring the Impact of Compound Jinguanlan on Acne Based on the p38 MAPK Signaling Axis

Objective To study the effect of Compound Jinguanlan preparation on acne vulgaris through the p38MAPK signaling pathway.Methods New Zealand White Rabbits were randomly divided into high,medium,and low concentration groups of Jinguanlan,a positive control group,a model control group,and a blank control group.A rabbit ear acne model was constructed and each group was given the corresponding drug intervention.The gross changes in the skin tissue of the rabbit ears were observed.HE staining was used to observe pathological changes.ELISA was used to detect the levels of IL-6 and TNF-α in the serum.Western blot was used to detec the levels of p38MAPK protein expression.Results Jinguanlan significantly inhibited the inflammation of acne vulgaris,reducing swelling,erythema,hyperkeratosis,and inflammatory cell infiltration in the rabbit ears.ELISA results showed that compared with the model control group,the levels of IL-6 and TNF-α in the serum of each concentration of Jinguanlan group were significantly reduced,with the high concentration group showing a more pronounced decrease(P<0.01).Western blot results indicated that compared with the model group,the relative expression of p38 MAPK protein in the Jinguanlan group was reduced.Conclusion Jinguanlan can inhibit the inflammatory response of acne vulgaris,and its effect may be achieved by regulating the p38MAPK signaling pathway and reducing the expression levels of IL-6 and TNF-α in the serum.
6.The postoperative care of a patient with uremic complicated by ventricular tachycardia undergoing catheter radiofrequency ablation therapy

To summarize the nursing experience of a patient with uremia on maintenance hemodialysis complicated by recurrent ventricular tachycardia and treated with transcatheter radiofrequency ablation.Key nursing interventions included:dynamically assessing the patient's coagulation and bleeding status,being vigilant against the occurrence of deep vein thrombosis,and preventing local and major organ bleeding;implementing goal-oriented volume management strategies to prevent electrolyte disorders;the strengthened management of vascular access to reduce risks of stenosis or occlusion in the arteriovenous fistula;conducting precise assessment and comprehensive intervention to reduce the patient's psychological and mental burden.After careful treatment and nursing care,the patient was stable and discharged on the 6th postoperative day.During the 2-month outpatient follow-up,cardiac function indicators were normal,and the fistula was unobstructed,and the patient recovered well.

Objective To explore the value of preoperative ultrasound examination in the prediction of restenosis of arteriovenous fistula(AVF)after percutaneous transluminal angioplasty(PTA)in hemodialysis patients.Methods A case-control trial was conducted on 225 hemodialysis patients who undergoing PTA due to AVF in our hospital January 2023 to May 2024.After 3 months of follow-up,they were divided into a patency group(n=204)and a restenosis group(n=21)according to the occurrence of postoperative restenosis.The preoperative clinical data and ultrasound parameters were compared between the groups.Binary logistic regression analysis was used to identify the independent factors for AVF restenosis after PTA.Receiver operating characteristic(ROC)curve was drawn to evaluate the value of preoperative stenosis length in the prediction of the restenosis after PTA.Results There were significant differences in preoperative internal diameter at the site of stenosis,stenosis length,stenosis number,intimal thickness,and brachial artery flow between the 2 groups(P<0.05).Preoperative stenosis length(OR=1.856,95%CI:1.350~2.552,P<0.001)was an independent factor of AVF restenosis in hemodialysis patients after PTA.ROC curve analysis showed that the area under the curve of preoperative stenosis length in predicting restenosis after PTA was 0.868(95%CI:0.784~0.953,P<0.001),with a sensibility and specificity of 85.7%and 80.4%,respectively.Conclusion Preoperative stenosis length may be an independent factor for AVF restenosis after PTA in hemodialysis patients.
