1.Study on the Mechanism of Shuangshen Ningxin Capsules in Treating Coronary Microvascular Disease Based on Transcriptomic

Objective To explore the potential mechanism of Shuangshen Ningxin Capsules in the treatment of coronary microvascular disease(CMVD)using transcriptomics;To verify it.Methods Totally 30 male SD rats were randomly divided into sham-operation group,model group and Shuangshen Ningxin Capsules group,with 10 rats in each group.Shuangshen Ningxin Capsules group was given Shuangshen Ningxin Capsules solution(90 mg/kg)in advance for 7 days by gavage,while the sham-operation group and model group were given normal saline.CMVD rat model was established through left ventricular injection of embolic microspheres 2 hours after the last administration.The sham-operation group underwent open chest surgery and injected normal saline.24 hours after modeling,ultrasound was used to detect cardiac function[left ventricular systolic anterior wall thickness(LVAWs),left ventricular diastolic anterior wall thickness,left ventricular systolic posterior wall thickness,left ventricular diastolic posterior wall thickness,left ventricular end systolic diameter(LVIDs),left ventricular end diastolic diameter,left ventricular end diastolic volume,left ventricular end systolic volume(LVVs),stroke volume(SV),cardiac output(CO),ejection fraction(EF),fractional shortening(FS)];the reagent kit was used to detect the contents of serum CK,CK-MB and LDH;TTC staining was used to detect myocardial infarction area;HE staining was used to observe myocardial tissue morphology;transcriptome sequencing was used to detect common differentially expressed genes among the sham-operation group,model group and Shuangshen Ningxin Capsules group,GO and KEGG pathway enrichment analysis was performed on differentially expressed genes,and mRNA expression of differentially expressed genes were validated through RT-PCR.Results Compared with the sham-operation group,LVAWs,SV,CO,EF and FS of the model group significantly decreased,LVIDs and LVVs significantly increased(P<0.05,P<0.01),serum CK,CK-MB and LDH contents significantly increased(P<0.05,P<0.01),the area of myocardial infarction increased,the arrangement of myocardial cells was disorderly,and there was obvious infiltration of inflammatory cells.Compared with the model group,the rats in Shuangshen Ningxin Capsules group showed a significant decrease in LVIDs and LVVs,a significant increase in LVAWd,LVPWs,EF and FS(P<0.01),a significant decrease in serum CK,CK-MB and LDH contents(P<0.05,P<0.01),and a significant reduction in myocardial infarction area(P<0.01),the arrangement of myocardial cells was relatively neat,and the infiltration of inflammatory cells was reduced.The transcriptome sequencing results showed that there were 287 common differentially expressed genes among the sham-operation group,model group and Shuangshen Ningxin Capsules group,mainly enriched in chemokine signaling pathway,JAK-STAT signaling pathway,regulation of actin cytoskeleton,adrenergic signaling in cardiomyocytes,PPAR signaling pathway,NOD-like receptor signaling pathway,etc.RT-PCR validation results showed that Shuangshen Ningxin Capsules could significantly down-regulate the mRNA expressions of JAK2,STAT1,CXCL10,CXCL13 and CCR1 in myocardial tissue of model rats(P<0.05,P<0.01).Conclusion Shuangshen Ningxin Capsules can significantly improve cardiac function and reduce myocardial infarction area in CMVD rats,possibly by inhibiting the expressions of JAK-STAT signaling pathway JAK2 and STAT1,reducing the expressions of chemokines and their receptors(CXCL10,CXCL13,CCR1),reducing the recruitment of inflammatory cells,and exerting anti-inflammatory effects.
2.Efficacy observation of cetrorelix combined with aspirin in preventing early-onset ovarian hyperstimulation syndrome

OBJECTIVE To observe the efficacy of cetrorelix combined with aspirin in preventing early-onset ovarian hyperstimulation syndrome (OHSS). METHODS A retrospective analysis was conducted on clinical data from 38 early-onset OHSS patients, who received treatment in our hospital from January 1st to July 1st, 2022. These patients were divided into intervention group (19 cases) and control group (19 cases) according to the therapy regimen. On the first day after oocyte retrieval surgery, the control group was given aspirin enteric-coated tablets 100 mg orally until menstruation began. The intervention group was given cetrorelix for injection 0.25 mg subcutaneously, for consecutive 3 days+aspirin enteric-coated tablets (same usage and dosage as the control group). The first luteal phase, the degree of OHSS, and the ovarian volume, ascites volume, serum estradiol (E2), white blood cell count (WBC), hematocrit (HCT), neutrophil ratio (NEUT%), D-dimer (DD), prothrombin time (PT), fibrinogen (Fib) after oocyte retrieval surgery were observed and measured in 2 groups. RESULTS The first luteal phase was significantly shorter, and the proportions of median and severe OHSS cases were significantly lower in the intervention group compared to the control group (P<0.05 or P<0.01). After oocyte retrieval surgery, the intervention group showed significantly lower ovarian volume, ascites volume, serum E2, WBC, NEUT%, HCT, DD and Fib compared to the control group, but PT of intervention group was signiticantly higher than that of control group (P<0.05). CONCLUSIONS Cetrorelix combined with aspirin is more effective in preventing early-onset OHSS than aspirin alone.
3.Establishment and evaluation of a rat model of coronary microvascular disease with qi deficiency and blood stasis syndrome

Objective The incidence of coronary microvascular disease(CMVD)is increasing annually.According to traditional Chinese medicine(TCM),CMVD belongs to the category of"collaterals",and qi deficiency and blood stasis are the main syndrome type of CMVD.Notably however,no studies have reported on the use of animal models of CMVD with qi deficiency and blood stasis.The current study therefore aimed to establish and evaluate a rat model of CMVD with qi deficiency and blood stasis syndrome.Methods Forty-five male SD rats were divided randomly into sham group,CMVD group,and CMVD + QXXY group(n = 15 rats per group).Rats in the CMVD + QXXY group were randomly deprived of sleep for 14~16 h/day for 6 weeks,and the model of qi deficiency syndrome was established.Animals in the sham group and the CMVD group were fed normally for 6 weeks.After 6 weeks,rats in the CMVD and CMVD + QXXY groups were anesthetized,their chests were opened,and embolic microspheres(40~120 μm)were injected into the left ventricle.Rats in the sham group underwent thoracotomy without injection of embolic microspheres.On day 7 after operation,relevant detection indicators were measured in each group.Results Compared with the sham group,the CMVD group showed a significant decrease in left ventricular ejection fraction and left ventricular shortening rate,while the activities of creatine kinase MB isoenzyme(CK-MB)and lactate dehydrogenase(LDH)were significantly increased.Heart function,hemorheology,myocardial enzyme index,and the degree of myocardial cell damage differed significantly between the CMVD + QXXY group compared with the sham group.Conclusions A rat model of CMVD + qi deficiency + blood stasis syndrome can be successfully established by sleep deprivation combined with intraventricular injection of embolic microspheres.This model will be suitable for the study of the pathogenesis of CMVD and the mechanisms of TCM.
4.Effect of Buyang Huanwutang on Platelet Function and Inflammatory Cytokine in Rat Model of Acute Blood Stasis Induced by Multiple Factors

ObjectiveTo explore the effect of Buyang Huanwutang (BYHWT) on platelet function and inflammatory cytokines in the rat model of acute blood stasis. MethodThe model of acute blood stasis was established with SD rats by ice water bath combined with injection of epinephrine. Rats were randomly assigned into four groups: normal group, model group, BYHWT (3.2 g·kg-1) group, and aspirin (60 mg·kg-1) group. The rats were injected with epinephrine hydrochloride on day 8 after 7 days of modeling. The macroscopic indexes of triditional Chinese medicine (TCM) syndrome including tongue manifestation and pulse manifestation were observed, while hemorheological indexes, blood coagulation, and platelet aggregation were detected. The serum levels of the inflammatory cytokine matrix metalloprotein-9 (MMP-9) and the adhesion factor intercellular adhesion molecule-1 (ICAM-1) and were determined by enzyme-linked immunosorbent assay (ELISA). ResultThe pulse distention of rats in the model group was lower than that in the normal group (P<0.01), while BYHWT improved the pulse distention of the rats with the syndrome of blood stasis (P<0.01). In the model group, the tongue showed the characteristics of blood stasis syndrome, with dark purple veins at the tongue bottom and lower values of R, G, B on the tongue surface than those in the normal group (P<0.01), which, however, can be recovered by BYHWT (P<0.01). The blood viscosity at high, medium, and low shear stress and the plasma viscosity in the model group were higher than those in the normal group (P<0.01, P<0.05). Compared with the model group, BYHWT restored the whole blood viscosity under high, medium and low shear stress and plasma viscosity (P<0.05,P<0.01). The model group had shorter prothrombin time (PT), shorter thrombin time (TT), and higher fibrinogen (FIB) than the normal group (P<0.05, P<0.01). BYHWT improved the TT and reduced the FIB in the rats with blood stasis syndrome (P<0.01). The platelet aggregation rate induced by arachidonic acid (AA) and adenosine diphosphate (ADP) in the model group was higher than that in normal group (P<0.01) and BYHWT decreased the platelet aggregation rate of the rats with blood stasis syndrome (P<0.01). The results of scanning electron microscopy showed that the model group exhibited excessive platelet activation, obvious pseudopodia, and increased aggregation of platelets compared with the normal group, while platelet activation and aggregation were rare in the BYHWT group. The serum levels of MMP-9 and ICAM-1 in the model group were higher than those in the normal group (P<0.01), which were decreased in the BYHWT group (P<0.05, P<0.01). ConclusionThe SD rats with the syndrome of acute blood stasis induced by ice water bath combined with injection of epinephrine demonstrate obvious changes in platelet function and morphology, inflammation, and abnormal cell adhesion. In the treatment of acute blood stasis in rats, BYHWT may reduce thrombosis and improve blood consistency and cohesion by mitigating inflammation, down-regulating cell adhesion factor overexpression, and improving platelet shape and function.
5.Establishment and Evaluation of Rat Model of Acute Coronary Syndrome with Combined Blood Stasis and Poison

ObjectiveTo compare the feasibility of establishing the rat model of acute coronary syndrome with combined blood stasis and poison by lipopolysacharide (LPS) injection, ligation of coronary artery and different combinations of the two methods. MethodA total of 225 male SD rats were randomly divided into sham operation group, simple coronary artery ligation group, first injected LPS group [LPS(5 mg·kg)injection 24 h before coronary artery ligation] and follow injected LPS group [LPS(5 mg·kg)injection 10 min after coronary artery ligation]. The indexes of each group were detected at 3, 24, 72 h after modeling, and the model was comprehensively evaluated. The general state and macroscopic evaluation indexes of traditional Chinese medicine (TCM) syndrome (tongue and pulse) of rats in each group were observed. ECG and echocardiography were used to evaluate cardiac function, and the myocardial ischemia and infarction areas were measured by Evans blue/2,3,5-triphenyltetrazolium chloride (TTC) staining. The content of creatine kinase isoenzyme (CK-MB), lactate dehydrogenase (LDH), creatine kinase (CK), and troponin T (cTnT) in serum as well as interleukin-1 β (IL-1β) and IL-6 changes were determined by biochemical method or enzyme-linked immunosorbent assay (ELISA). Hematology analyzer was adopted to determine the white blood cell (WBC) count, and the four coagulation indexes, platelet aggregation rate, hemorheology and other coagulation evaluation indexes were also detected. The myocardial tissue was observed by hematoxylin-eosin(HE)staining. ResultAfter 3 h of modeling, compared with the conditions in sham operation group, the R, G and B values of tongue of rats (P<0.01), pulse amplitude (P<0.01), and cardiac function in simple coronary artery ligation group were decreased, and the color of hypoglossal veins became purple(P<0.01). The content of CK, LDH, cTnT, IL-1β and IL-6 in serum(P<0.05), myocardial infarction area(P<0.01), and total number of WBCs (P<0.05)were increased. Compared with simple coronary artery ligation group, first injected LPS group and follow injected LPS group had increased hypoglossal veins, decreased R value of tongue and elevated cTnT content (P<0.01), while follow injected LPS group had reduced B value of tongue, decreased cardiac output (CO)(P<0.05), increased IL-1β content, and thinned left ventricular anterior walls at end-systole (LVAWs)(P<0.01). After 24 h of modeling, compared with sham operation group, simple coronary artery ligation group presented significantly decreased R, G and B values of tongue, lengthened purplish dark hypoglossal veins (P<0.01), reduced pulse amplitude(P<0.01) and cardiac function, enlarged myocardial infarction area(P<0.01), increased whole blood viscosity, platelet aggregation rate, fibrinogen (FIB), shortened prothrombin time (PT) and thrombin time (TT)(P<0.01), and elevated total number of WBCs (P<0.01)and content of CK, LDH, cTnT and IL-6 in serum(P<0.05). Compared with the conditions in simple coronary artery ligation group, the pulse amplitude, R, G and B values of tongue (P<0.01), and ejection fraction (EF) and fractional shortening (FS) scores (P<0.05)dropped, and hypoglossal veins were deepened and lengthened(P<0.05), and cTnT content was increased(P<0.01)in first injected LPS group and follow injected LPS group. However, follow injected LPS group had thinned LVPWs, increased LDH content, platelet aggregation rate(P<0.05), myocardial infarction area, and total number of WBC, level of IL-1β(P<0.05), and shortened TT(P<0.01). Additionally, 72 h after modeling, compared with sham operation group, simple coronary artery ligation group showed significantly reduced pulse amplitude, lowered R, G and B values of tongue, thickened and lengthened hypoglossal veins(P<0.01), decreased cardiac function, and increased content of cTnT, FIB, whole blood viscosity(P<0.01),platelet aggregation rate, level of IL-6 and IL-1β(P<0.05). Compared with the conditions in simple coronary artery ligation group, the hypoglossal veins of the first injected LPS group and the follow injected LPS group were more purple, and the content of cTnT was boosted(P<0.01), whereas follow injected LPS group had decreased pulse amplitude, R, G and B values of tongue, EF and FS scores (P<0.05), and enlarged myocardial infarction area(P<0.01). ConclusionCompared with the other modeling methods and models at different modeling time, the established model by LPS injection 10 min after coronary artery ligation for 24 h was more consistent with the clinical characteristics of acute coronary syndrome with combined blood stasis and poison.
6.Clinicopathological and molecular genetic characteristics of adult IDH wild-type diffuse gliomas

Objective:To analyze the clinicopathological and molecular features and prognostic implications of adult isocitrate dehydrogenase wild type (IDH-wt) diffuse gliomas.Methods:A total of 87 cases of adult IDH-wt diffuse gliomas from 2016 to 2020 in Xuanwu Hospital of Capital Medical University were retrospectively collected. The clinicopathological characteristics and prognosis were analyzed. Molecular characteristics were also analyzed using Sanger sequencing and next generation sequencing.Results:There were 53 males and 34 females, aged from 19 to 78 years (mean 53 years). Histopathologically, there were 63 (72.4%) glioblastomas, 16 (18.4%) anaplastic astrocytomas, six (6.9%) diffuse astrocytomas, and one (1.1%) each of anaplastic oligodendrocytoma, and anaplastic oligodendroglioma. Common molecular genetic changes in IDH-wt gliomas included TERT promoter mutation which was found in 60 cases (69.0%); MGMT promoter methylation in 43 cases (49.4%); EGFR mutation in 38 cases (43.7%); PTEN mutation in 35 cases (40.2%) and TP53 mutation in 32 cases (36.8%). In addition, PDGFRA mutation was detected in 17 cases (19.5%), CDK4 amplification in 15 cases (17.2%) and MDM2 amplification in 11 cases (12.6%). In IDH-wt diffuse gliomas, there was no significant difference in the overall survival between TERT promoter, EGFR, PTEN, TP53, PDGFRA, CDK4, MDM2 mutations and the wild-type, since these gene mutations could co-occur in any case ( P>0.05). Also there was no significant difference in the overall survival between the WHO grade Ⅱ/Ⅲ gliomas and glioblastoma patients with these gene mutations ( P>0.05). Conclusions:TERT promoter, EGFR, PTEN, TP53, PDGFRA, CDK4 and MDM2 gene mutations are common molecular genetic changes in adult IDH-wt gliomas, and are associated with poor prognosis. It is suggested that these genes are potentially useful for predicting the prognosis and should be tested in adult IDH-wt gliomas.
7.Gene variant analysis of a child presented with neonatal diabetes and multiple organ malformations.

OBJECTIVE:
To explore the genetic basis for an infant with neonatal diabetes (NDM) and multiple malformations.
METHODS:
Genetic variants were detected by next generation sequencing (NGS). Suspected variant was verified by Sanger sequencing.
RESULTS:
A de novo heterozygous variant, c.1454_1455del(p.K485Rfs), was detected in exon 5 of the GATA6 gene. The variant was undetected in his parents and unreported previously. Bioinformatic analysis predicted the variant to be pathogenic.
CONCLUSION
The heterozygous variant of c.1454_1455del(p.K485Rfs) of the GATA6 gene probably underlies the disease in this child. Genetic testing can facilitate diagnosis and genetic counseling for NDM.

8.Ultrasound diagnosis and grading of neonatal respiratory distress syndrome: a multicenter prospective study

Objective:Lung ultrasound (LUS) has been used in the diagnosis of neonatal respiratory distress syndrome(RDS) successfully, but there have been no multicenter prospective studies to verify its reliability or determine how to grade RDS with LUS findings.This study aimed to discuss the necessity and feasibility of using LUS findings to determine RDS grades through a multicenter prospective study.Methods:Every researcher participated in the National Neonatal Lung Ultrasound Training Course and receiving 3-6 months of lung ultrasound system training at the National Neonatal Lung Ultrasound Training Center.Patients between June 2018 and May 2020 who met the RDS ultrasound diagnostic criteria and had full available clinical data were included in this study.The LUS examination was completed immediately after the patients were admitted to the hospital.Some of them also underwent chest X-ray examination.Arterial blood gas analysis was completed immediately before or after the LUS ultrasound examination.RDS grading was performed according to the LUS findings and whether the patient had serious complications.Results:A total of 275 qualifying cases were included in this study, which included 220 premature infants and 55 full-term infants, and the primary RDS occurred in 117 cases (42.5%), and secondary RDS occurred in 158 cases (57.5%). LUS manifestations of RDS patients can be divided into three categories: (1)A ground-glass opacity sign: which could be found among 50 infants when they were admitted to the hospital (that was, at their first LUS examination). Twenty-eight of these infants were considered to have wet lungs and were not sent for special management on admission, but LUS showed typical snowflake-like lung consolidation within 0.5 to 4 hours.Twenty-two of them were given mechanical ventilation with exogenous pulmonary surfactant; Eighteen cases were controlled within 6-12 hours, but the lung lesions became more severe in the other 4 infants (due to severe intrauterine infection). (2)Snowflake-like lung consolidations: the first LUS on admission showed typical snowflake-like lung consolidation involving areas ranging from 1-2 intercostal spaces to 12 lung divisions in 204 cases.Thirty-eight infants among them the lung consolidation only had involvement of 1-2 intercostal spaces at the time of admission; Fifteen of them received invasive respiratory support and recovered within 4-12 hours.Twelve patients received noninvasive respiratory support; Seven of them recovered, while five cases developed severe lung illness.The remaining 11 patients who were not given any form of ventilator support developed severe conditions within 1-4 hours.Thirty of them showed snowflake signs involving 12 lung regions at admission.The remaining 136 patients had lung consolidation degree between the two degree above condition.(3)Snowflake-like sign with complications: Twenty-one patients had severe complications such as pneumothorax, pulmonary hemorrhage or/and persistent pulmonary hypertension of the newborn or large area atelectasis, etc, although snowflake lung consolidation did not involve all lung regions.Conclusion:(1) LUS is reliable and accurate for diagnosing RDS.RDS has the same characteristics on ultrasound for both preterm and full-term infants, both primary and secondary RDS.(2) To facilitate the management of RDS, it is necessary to classify RDS according to the ultrasound findings and the presence of severe complications.(3) Based on the results of this study, it is recommended that RDS can be divided into mild, moderate and severe degrees.The exact standards for grading are as follows: Mild RDS: the early stage of RDS, in which lung consolidation shows as a ground-glass opacity sign on ultrasound; Moderate RDS: lung consolidation shows a snowflake sign on ultrasound, not all of the lung fields are involved; Severe RDS meets one or more of the following criteria: lung consolidation shows as a snowflake sign on ultrasound and all lung regions are involved, or regardless of its degree and extent, lung consolidation has caused serious complications, such as pulmonary hemorrhage, pneumothorax, persistent pulmonary hypertension of the newborn, or/and a large area of pulmonary atelectasis.
