1.Penyan pill combined with thunder fire moxibustion in patients with sequelae of pelvic inflammatory disease:a randomized controlled trial

Objective To observe the clinical efficacy of Penyan pill combined with thunder fire moxibustion(TFM)in patients with sequelae of pelvic inflammatory disease(SPID).Methods This study was conducted at the department of gynecology,the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from May 2023 to May 2024.Patients with SPID were recruited and randomly and equally divided into group A(treated with ornidazole),group B(treated with ornidazole combined with Penyan pill),and group C(treated with ornidazole combined with Penyan pill and TFM).The primary outcome was the treatment effective rate,the secondary indicators were the Traditional Chinese Medicine Syndrome Integral(TCMSI),Visual Analogue Scale(VAS)score,short-form 36 health survey(SF-36)score,Self-rating Anxiety Scale(SAS)score,inflammatory factors[interleukin-6(IL-6),tumor necrosis factor(TNF-α),and C-reactive protein(CRP)],and the incidence of adverse drug reactions.All observation indicators were evaluated before treatment and 12 weeks after treatment.Results A total of 195 patients with SPID were included,and 16 patients failed to complete the study due to poor compliance.Therefore,the final number of cases was 59 in group A,60 in group B,and 60 in group C.The treatment effective rate in group C was 95.00%,which was significantly higher than that in group B(80.00%)and group A(71.19%)(P<0.017).However,there was no significant difference in the effective rates between group A and group B(P>0.017).After treatment,the TCMSI total score,VAS score,SF-36 score,SAS score,and the levels of serum IL-6,TNF-α,and CRP in the three groups were significantly improved compared with those at enrollment(P<0.05).In addition,all the indicators in group C were significantly better than those in group B and group A(P<0.05).No adverse reactions were observed in the three groups.Conclusion Penyan pill combined with TFM can improve the pain degree,quality of life and anxiety state in SPID patients,which may be related to the regulation of inflammatory factor expression.
2.Construction and application of a graded early mobility path for critically ill adult patients

Objective To develop a graded early mobility implementation pathway for critically ill adult patients in tertiary hospitals in Beijing and to preliminarily validate its feasibility and effectiveness.Methods Based on the"goal-directed"early mobility concept,a graded early mobility implementation pathway for critically ill patients was developed through evidence synthesis and the Delphi method,consisting of 3 components:patient inclusion,mobility implementation,and mobility evaluation.Using convenience sampling,patients meeting inclusion criteria in the general ICU of a tertiary hospital in Beijing from October 2024 to January 2025 were selected as participants.Among them,25 patients admitted from December 2024 to January 2025 were assigned to an experimental group and received early mobility interventions following the developed pathway.25 patients admitted from October to November 2024 served as a control group and received routine ICU mobility care.Outcomes including diaphragm excursion,muscle strength,ICU length of stay,and adverse events were compared between the 2 groups.Results The graded early mobility pathway achieved an implementation rate of 70.05%in the experimental group,significantly higher than it in the control group(P<0.001),without increasing adverse events.Post-intervention diaphragm excursion in the experimental group was significantly greater than that in the control group(P=0.018).Conclusion The developed graded early mobility implementation pathway for ICU patients demonstrates scientific rigor and clinical practicality.It provides a reference for the widespread and effective implementation of early mobility in ICUs,standardizing its clinical application.

5.Association between dietary patterns and sexual function in people of childbearing age

Objective:To explore the relationship between dietary pattern and sexual function in people of childbearing age.Methods:This study adopted a cross-sectional method and included all women and men who visited Reproduction Center of Maanshan Maternal and Child Health Hospital, Reproductive Medicine Center of the 901th Hospital of the Joint Logistics Support Force of People's Liberation Army and Reproduction Center of Anhui Wanbei Coal Power Group General Hospital from December 2020 to March 2022. The Female Sexual Function Index (FSFI) and International Index of Erectile Function-15 (IIEF-15) were used to assess sexual function. Factor analysis was used to establish dietary patterns, and multivariate logistic regression was used to analyze the association between dietary patterns and sexual function.Results:A total of 1 290 females and 1 331 males were included in the study, including 1 031 females and 899 males with sexual dysfunction. There were 289 women and 272 men with balanced dietary pattern, 473 women and 395 men with traditional dietary pattern, 272 women and 324 men with processed dietary pattern, and 256 women and 340 men with beverage dietary pattern. After adjusting for confounding factors such as residence and age, it was found that balanced dietary pattern was negatively correated with female sexual desire disorder ( OR=0.904, 95% CI: 0.820-0.995, P=0.039), sexual arousal disorder ( OR=0.840, 95% CI: 0.759-0.929, P=0.001), vaginal lubrication disorder ( OR=0.833, 95% CI: 0.710-0.979, P=0.026), orgasmic disorder ( OR=0.764, 95% CI: 0.680-0.858, P<0.001), low sexual satisfaction ( OR=0.887, 95% CI: 0.796-0.987, P=0.028), sexual dysfunction ( OR=0.805, 95% CI: 0.714-0.907, P<0.001), and male orgasmic disorder ( OR=0.859, 95% CI: 0.763-0.967, P=0.012). The traditional dietary pattern was negatively correlated with female sexual desire disorder ( OR=0.879, 95% CI: 0.786-0.983, P=0.024), sexual arousal disorder ( OR=0.884, 95% CI: 0.784-0.995, P=0.042), male erectile disorder ( OR=0.736, 95% CI: 0.634-0.855, P<0.001), sexual desire disorder ( OR=0.753, 95% CI: 0.648-0.876, P<0.001), and sexual dysfunction ( OR=0.769, 95% CI: 0.653-0.907, P=0.020). The processed dietary pattern was positively correlated with erectile disorder ( OR=1.162, 95% CI: 1.049-1.287, P=0.004), orgasmic dysfunction ( OR=1.207, 95% CI: 1.091-1.337, P<0.001), sexual desire disorder ( OR=1.199, 95% CI: 1.081-1.330, P=0.001) and sexual dysfunction ( OR=1.134, 95% CI: 1.020-1.261, P=0.002). Beverage dietary pattern was not associated with sexual dysfunction in men and women (all P>0.05). Conclusion:Balanced, traditional dietary patterns were related to the reduce risk of sexual dysfunction in both women and men of childbearing age, while processed dietary patterns were related to the increased risk of sexual dysfunction in men of childbearing age.

7.Research progress of early activity in ICU patients with continuous renal replacement therapy

ICU patients with continuous renal replacement therapy (CRRT) are at high risk of intensive care unit acquired weakness (ICU-AW) and early activity is an important measure to prevent ICU-AW. This article reviews the concept and significance, influencing factors, safety and implementation methods of early activities for ICU patients during CRRT treatment, in order to provide a reference for constructing early activity programs for ICU CRRT patients.
8.Construction of a shared decision-making scheme for early activities of ICU patients

Objective:To construct a shared decision-making scheme for early activities of ICU patients, so as to provide guidance for carrying out early activities of ICU patients and improving patient activity compliance.Methods:From July to December 2021, a total of 16 medical and nursing experts engaged in shared decision-making research were selected as correspondence experts using the purposive sampling method, and the Delphi expert correspondence method was used to construct a shared decision-making plan for early activities of ICU patients.Results:A total of 2 rounds of expert correspondence were conducted. The effective recovery rates of the questionnaires were 100%, the expert authority coefficients were 0.94 and the Kendall's harmony coefficients were 0.254 and 0.393, respectively ( P<0.01) . Finally, a shared decision-making plan for early activities of ICU patients with 3 stages (preparation stage, decision-making and execution stage and evaluation stage) , 12 basic steps and 65 specific contents was formed. Conclusions:The shared decision-making scheme for early activities of ICU patients constructed in this study is scientific and feasible, which can provide a reference for improving the activity compliance of ICU patients.
9.Effect of clustering nursing on the prevention of urinary tract infection associated with urinary catheter in patients with spinal fracture during perioperative period

Objective:To investigate the clinical effect of cluster nursing on the prevention of perioperative urinary catheter associated urinary tract infection (CAUTI) in patients with spinal fracture.Methods:A retrospective case-control method was used to select 160 patients with spinal fracture admitted to the department of spinal cord injury in emergency care from January 2018 to March 2020 in the Department of Orthopaedics General Hospital of Chinese People′s Liberation Army as study subjects, 80 patients admitted to the department of spinal cord injury from January to December 2018 were set as the control group, and 80 patients admitted from January 2019 to March 2020 as the observation group. The control group was given standardized clinical nursing measures during the perioperative period, while the observation group was given cluster nursing measures. The duration of indwelling of catheters, the incidence of catheter reinsertion and the incidence of CAUTI were compared between the two groups.Results:The catheter indwelling time in the observation group was (3.37±0.64) days, lower than that in the control group (6.35±1.52) days. The incidence of catheter insertion and CAUTI was 5.00%(4/80), 3.80%(3/80) in the observation group, lower than 11.30%(9/80), 11.60%(13/80) in the control group. The differences were statistically significant ( χ2 values were -2.927, -2.654, all P<0.05). Conclusions:Cluster nursing intervention for patients with spinal fracture in the perioperative period can effectively shorten the catheter indwelling time, reduce the incidence of catheter re-insertion and the incidence of patients with CAUTI, significantly improve the quality of clinical care in the department, and have positive significance for the treatment and rehabilitation of patients.
10.Relationship between peripheral blood neutrophil lymphocyte ratio and osteoporosis in Chinese elderly women

Objective:To explore the association of peripheral blood neutrophil lymphocyte ratio(NLR) with primary osteoporosis (POP) in Chinese elderly women.Methods:From December 2015 to April 2019, a total of 788 elderly women in Chongqing Three Gorges Central Hospital were included in this study.They were divided into three groups according to bone mineral density: 242 cases of osteopenia, 206 cases of normal bone mineral density and 340 cases of osteoporosis.Their height, weight, statuses of bone of left femoral neck and results of blood tests were recorded, and the values of BMI, NLR and OSTA were calculated.The SPSS statistics software was used to analyze the differences of parameters among the three groups.Logistic regression analysis was used to get significant independent related factors for osteoporosis.The relationship of NLR and OSTA with T-score of left femoral neck was confirmed by Pearson correlation test.ROC curves were used to define the values of NLR and OSTA in diagnosis of osteoporosis.Results:There were statistically significant differences in age(median values: 66, 68 and 70, χ 2=71.596, P<0.001), BMI(median values: 23.46, 24.04 and 25.415, χ 2=64.936, P<0.001) and NLR(median values: 1.97, 2.435 and 2.84, χ 2=106.218, P<0.001) among the three groups(all P<0.01). Logistic regression analysis demonstrated that age, BMI and NLR were all independent significant related factors.There was correlation of NLR, OSTA with T-score of left femoral neck.In diagnosis of osteoporosis, the cut-off value, maximum Youden index, sensitivity and specificity were 2.395, 0.299, 70.3%, 59.6% for NLR, and -1.315, 0.174, 73.6%, 43.5% for OSTA. Conclusion:NLR has intermediate sensitivity and specificity in screening POP in Chinese elderly women, and deserves future researches.
