Autism spectrum disorder(ASD),characterized by unknown etiology and high heterogeneity,ne-cessitates precise diagnostic and intervention strategies.Neuroimaging techniques have shown great promise in un-covering the neural mechanisms of ASD,providing a foundation for aided diagnosis and transcranial magnetic stim-ulation(TMS)interventions.This review highlights that integrating multimodal neuroimaging and developing indi-vidualized indices with developmental specificity can significantly improve the accuracy of ASD diagnosis and clas-sification.Furthermore,TMS interventions guided by functional connectivity derived from functional magnetic reso-nance imaging(fMRI)offer a personalized approach to ASD treatment.
2.Exploring the Material Basis of Guben Qushi Huayu Prescription in the Treatment of Psoriasis Recurrence Based on Constituents Absorbed into Blood Analysis and Molecular Docking Techniques

Objective To clarify the active ingredients and the potential molecular mechanism of Guben Qushi Huayu Prescription in treating psoriasis recurrence.Methods An ultra performance liquid chromatography-quadrupole time-of-flight mass spectrometry(UPLC-Q-TOF/MS)was applied to analyze the whole formula and the constituents absorbed into blood of Guben Qushi Huayu Prescription,and molecular docking technology was used to study the binding affinity of the constituents absorbed into blood with psoriasis-related immunomodulatory proteins such as CD69 and CD103 proteins.Results Mass spectrometry analysis identified 21 active ingredients such as paeoniflorin in Guben Qushi Huayu Prescription,including several known anti-inflammatory and immunomodulatory compounds.Analysis of the constituents absorbed into blood identified 11 ingredients,including paeoniflorin,that may affect the course of psoriasis through blood circulation.Molecular docking studies revealed that the constituents absorbed into blood,including astilbin,isoastilbin,chlorogenic acid,neochlorogenic acid,cryptochlorogenic acid,helicine,paeoniflorin,ononin,all had high binding affinities with CD69 and CD103 proteins.Conclusion This research reveals the main active ingredients of Guben Qushi Huayu Prescription and their potential mechanism for regulating the recurrence of psoriasis by mass spectrometry and molecular docking technology,contributing to providing scientific basis for further pharmacological research and clinical application.
3.Proteomic analysis of serum proteins in mice with carbapenem-resistant hypervirulent Klebsiella pneumoniae bloodstream infection

OBJECTIVE To observe the changes of serum proteins in the mice with carbapenem-resistant hyperviru-lent Klebsiella pneumoniae(CR-hvKP)bloodstream infection by using liquid chromatogram-mass spectrometer(LC-MS),find out the differentially expressed proteins and carry out corresponding biological function analysis.METHODS The models of ICR mice with CR-hvKP and classical Klebsiella pneumoniae(cKP)bloodstream infec-tions were established,the serum specimens were collected from the mice at 12 hours of establishment of the in-fection models and were detected by using LC-MS.The result of LC-MS was identified by using Maxquant soft-ware.The corresponding bioinformatics analysis was performed for the differentially expressed proteins.RESULTS As compared with the normal control group,there were 24 upregulated proteins and 20 downregulated proteins in CR-hvKP group.As compared with cKP group,there were 107 upregulated proteins in the CR-hvKP group.The 134 differentially expressed proteins were retrieved one by one from Uniprot database,it was found that the pro-teins were involved in biological regulation,immune process,biological interaction,movement,metabolic process,response to stimulation,signal transduction,inflammatory response,oxidative stress and angiogenesis.The signal pathway analysis involved multiple metabolism-related pathways,including complement and coagula-tion cascades,cholesterol metabolism,bacterial infection,heme clearance,and assembly,remodeling and clear-ance of plasma lipoprotein.In terms of the proteins being attached great importance,the expression levels of kal-likrein B1(KLKB1)and serpin family A(Serpina)1b were downregulated as compared with the normal control group and were upregulated as compared with the cKP group;the expression levels of serum amyloid A protein(SAA)1,SAA2,Vanin-3(VNN3)and hemoglobinβpolypeptide chain b2(HBB-b2)were upregulated as com-pared with both the cKP group and the normal control group.CONCLUSIONS The CR-hvKP bloodstream infection involves the activation and action of multiple proteins.The rise of SAA1,SAA2,VNN3 and HBB-b2 or the de-cline of KLKB1 and Serpina1b may indicate the CR-hvKP bloodstream infection.

5.Clinical efficacy comparison of single and two interbody fusion devices in treatment of lumbar instability under intervertebral foraminoscopy

Objective To compare the clinical efficacy and safety of lumbar intervertebral space implantation with single and two fusion devices in treatment of lumbar instability under intervertebral foraminoscopy.Methods 101 patients(101 vertebral bodies)diagnosed with L4,L5 or S1 segment Ⅰ° to Ⅱ° spondylolisthesis from January 2019 to June 2022 were enrolled and the patients were randomly divided into group A(single fusion device group)and group B(double fusion devices group)by intervertebral foraminoscopy.Among of them,there were 51 cases of vertebral spondylolisthesis in group A and 50 cases of vertebral spondylolisthesis in group B.The patients were followed up at 1 week,1 month,6 months,9 months and 15 months after surgery,and the differences in spondylolisthesis distance,intervertebral space height,intervertebral space fusion,vertebral sedimentation rate,lumbar pain visual analogue scale(VAS)and lumbar Oswerry disability index(ODI)between the two groups were compared.Results All the 101 patients were followed up throughout the whole process,and there were no significant differences in operation time,intraoperative blood loss,and postoperative drainage between the two groups(P>0.05),and the VAS was(7.32±0.57)in group A and(7.14±0.61)in group B one week after operation,there was no significant difference between the two groups(P<0.05).At 6 months after surgery,the ODI of group A was(27.44±9.52)%and group B was(21.89±8.46)%,and there was no significant difference between the two groups(P>0.05).And at 6,9 and 15 months follow-up,the fusion rates of group A were 11.76%,68.63%and 90.20%,respectively,and the fusion rates of group B were 18.00%,80.00%and 96.00%,respectively,and there was significant difference between the two groups(P<0.05).At 15 months postoperative follow-up,the intervertebral space height of group A(0.45±0.07)cm was significantly smaller than that of group B(0.58±0.06)cm,and the difference was statistically significant(P=0.038).There was no significant difference between group A and group B(0.33±0.18)mm and(0.28±0.21)mm(P=0.079)of spondylolisthesis distance.Conclusion Implantation of the double fusion devices can provide more contact area and better stability,shorten the vertebral fusion time,increase the vertebral fusion rate,and reduce the occurrence of vertebral settlement.It is worthy of clinical promotion and application.

8.Reconstruction of pulp defects of two neighbouring digits with free great toe fibular flaps of both feet: a study on 7 cases

Objective:To explore the clinical effect of using great toe fibular flaps of both feet on reconstruction of pulp defects of two neighbouring digits.Methods:A total of 14 digit pulp defects in 7 cases were repaired in Zhoukou Huaihai Hospital using great toe fibular flaps of both feet from August 2020 to January 2023. Of the 7 cases, there were 4 males and 3 females, with an average of 28 years old, ranging from 19 to 45 years old. Meanwhile, there were 4 cases in left hand and 3 cases in right hand. There were 3 cases of digit pulp defects in index and middle fingers, 2 in middle and ring fingers, and 2 in thumb and index fingers. The area of soft tissue defect in 1.2 cm×1.5 cm-3.0 cm×2.5 cm, and flap was 1.5 cm ×1.8 cm-3.2 cm×2.8 cm. Furthermore, 1 case underwent emergency surgery and 5 were repaired in elective surgery. The donor site of the flap was closed directly, and an intermediate-thickness skin graft was prepared from the medial plantar area for transfer in the case of high suture tension at the wound edge. After surgery, patients received postoperative by outpatient clinic and WeChat to observe the appearance, sensation, functional recovery and flap contracture of digits, as well as the movement of the great toes of both feet.Results:After the surgery, all flaps in the 7 cases survived smoothly and the donor sites healed. All patients entered scheduled follow-ups postoperatively for 6 months to 2 years, with an average of 9 months. The flap showed an aesthetic appearance and excellent sensation, with a TPD of 3-6 mm, and satisfactory digit function. The donor site of the great toe fibular flap left linear scars only, without abnormality in range of motion and gait in walking. In addition, there were 5 in excellent and 2 in good according to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association.Conclusion:Application of great toe fibular flaps of both feet is an ideal option for the simultaneous repair of pulp defects of two neighbouring digits, which can achieve good reconstructive results.
9.Inhibition of gastric cancer cell proliferation and invasion by circ-BCAR3 through regulation of miR-145-5p/VEGFR-2 signaling pathway

Objective:To analyze the expression and significance of circ-BCAR3 in gastric cancer tissues, and to explore the effect and molecular mechanism of silencing circ-BCAR3 on the proliferation and invasion of gastric cancer cells. Methods:The expression of circ-BCAR3 in gastric cancer tissues and adjacent normal tissues was analyzed using the UCSC database. The relationship between circ-BCAR3 expression and the survival rate of gastric cancer patients was analyzed by Kaplan-Meier method. Small interfering RNA (siRNA)-negative control (si-NC) and si-circ-BCAR3 were transfected into gastric cancer BGC823 cells, and recorded as si-circ-BCAR3 group and si-NC group, respectively. Real-time reverse transcription-PCR (RT-qPCR) was used to detect the expression of circ-BCAR3 gene in gastric cancer NCI-N87, AGS, BGC823, HS-746T, SGC7901 cells, and immortalized gastric mucosal epithelial GES-1 cells. The proliferation and invasion abilities of BGC823 cells were detected by clone formation assay and Transwell assay, respectively. The binding sites of circ-BCAR3 and miR-145-5p were predicted using the online Circinteractome platform, and their targeting relationship was verified by a dual luciferase reporter gene assay. The relationship between the expression of circ-BCAR3 and miR-145-5p in gastric cancer tissues was analyzed using the UCSC database. The expression levels of vascular endothelial growth factor receptor-2 (VEGFR-2) signaling pathway proteins, including phosphorylated VEGFR-2 (p-VEGFR-2), phosphorylated phospholipase C-λ (p-PLC-λ), phosphorylated extracellular signal-regulated kinase (p-Erk), and phosphorylated P38 (p-P38), were detected in BGC823 cells using Western blotting analysis. Results:The relative expression level of circ-BCAR3 in gastric cancer tissues (8.04±2.34) was significantly higher than that in adjacent normal tissues (2.53±1.74), and the difference was statistically significant ( P<0.01). The survival rate of gastric cancer patients with low circ-BCAR3 expression was higher than that of gastric cancer patients with high circ-BCAR3 expression, and the difference was statistically significant ( P<0.05). The relative expression levels of circ-BCAR3 in NCI-N87, HS-746T, AGS, BGC823, and SGC7901 cells (5.05±0.36, 3.50±0.21, 2.87±0.38, 6.80±0.29, and 3.77±0.53) were all higher than those in GES-1 cells (1.02±0.21), and the differences were statistically significant (all P<0.01). The number of clones formed in the si-NC group and si-circ-BCAR3 group were [(126.70±21.78) and (44.89±8.33) unit], respectively, and the number of invasive cells were [(53.81±5.92) and (24.91±4.56) unit], respectively, with statistically significant differences ( P<0.01). The AACUGGA sequence in circ-BCAR3 was found to have the capacity to bind to the UUGACCU sequence in miR-145-5p. The luciferase activity of wild type circ-BCAR3 (WT- circ-BCAR3)+ miR-145-5p group (0.26±0.03) was lower than that of WT- circ-BCAR3+miR-NC group (1.00±0.05), and the difference was statistically significant ( P<0.01). In contrast, for the miR-NC, the luciferase activities of the mutant- circ-BCAR3 (MUT- circ-BCAR3)+ miR-145-5p and MUT- circ-BCAR3+ miR-NC groups were (1.07±0.12) and (1.00±0.09), and the differences were not statistically significant ( P>0.05). The expression of circ-BCAR3 and miR-145-5p showed a negative correlation in gastric cancer tissues ( R=0.82, P<0.01). The relative expression levels of VEGFR-2 signaling pathway proteins p-VEGFR-2, p-PLC-λ, p-Erk, and p-P38 in BGC823 cells in the si-circ-BCAR3 group (1.08±0.21, 1.50±0.21, 3.01±0.25, and 0.46±0.10) were lower than those in the si-NC group (5.22±0.17, 5.56±0.19, 6.63±0.31, and 4.24±0.16), and the differences were statistically significant (all P<0.05). Conclusions:The expression of circ-BCAR3 is elevated in gastric cancer tissues and cells. Furthermore, silencing of circ-BCAR3 may inhibit the proliferation and invasion of BGC823 cells by regulating the miR-145-5p/VEGFR-2 signaling pathway.
10.Reconstruction of soft tissue defect of hand and foot with a free superficial circumflex iliac artery perforator flap in children

Objective:To explore the clinical efficacy of free superficial circumflex iliac artery perforator flap (SCIAPF) in reconstruction of soft tissue defects of hand and foot in children.Methods:From August 2021 to August 2023, free SCIAPFs were used to reconstruct soft tissue defects of hands and feet in 8 children at the Department of Hand and Foot Surgery, Zhoukou Huaihai Hospital. There were 6 boys and 2 girls aged between 2 and 7 years old. The sites of soft tissue defect were: 1 of metacarpophalangeal joint in the functional area of left thumb, 2 of dorsal right hand, 1 of dorsal left foot, 2 of medial malleolus of right foot, and 2 of right forefoot, and all were accompanied with tendon and bone exposure. The size of soft tissue defects ranged from 2.5 cm×3.0 cm to 6.0 cm×3.5 cm. After emergency debridement, the children received flap transfers, the flaps sized from 3.0 cm×3.5 cm to 7.0 cm×4.0 cm. Donor sites were closed by running intradermal suture. All children were included in the scheduled postoperative follow-up by visit of outpatient clinic, WeChat interviews and home visits to observe the appearance, colour, texture and sensation of the flaps, the functional recovery of the affected hands and feet, as well as the healing of donor sites.Results:After surgery, all 8 flaps survived beside 1 had a partial necrosis at the edge and healed with a scab after dressing change. Flap donor sites healed in one stage. Scheduled postoperative follow-up lasted from 7 to 22 months, with an average of 10 months. The flaps presented a slightly bloated appearance, soft texture with a restored protective sensation and satisfactory functional recovery of hands and feet. Linear scars were left at the donor sites. According to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, 1 child was of excellent and 2 children of good in hand function of the 3 children with hand injury. The Maryland Foot Score revealed 3 children were of excellent and 2 of good in foot function of the 5 children with foot injury.Conclusion:Free SCIAPF is an ideal option for reconstruction of soft tissue defects of hand and foot in children, with concealed donor site, convenient flap harvesting, fewer complications and satisfactory reconstructive effect.
