1.Correlation between cognitive dysfunction and autonomic nerve function damage in patients with cerebral small vessel disease

Objective To study the correlation of cognitive dysfunction with autonomic nerve function damage in patients with cerebral small vessel disease (CSVD). Methods A total of 310 patients with CSVD admitted from June 2022 to June 2025 were included. Montreal Cognitive Assessment Scale (MoCA) was applied for cognitive function assessment, and Scale for Outcomes in PD for Autonomic Symptoms (SCOPA-AUT) was used to evaluate autonomic nerve function damage. The incidence rate of autonomic nerve function damage, total score and dimensions scores of SCOPA-AUT were counted. According to the presence or absence of autonomic nerve function damage, the enrolled patients were categorized into damage group and non-damage group. The total score of MoCA and scores of dimensions were compared between both groups. Pearson correlation analysis model was utilized to analyze the correlation between total score of MoCA and scores of dimensions of SCOPA-AUT in patients with CSVD. Results Among the 310 patients, 63.87% cases (198 / 310) had autonomic nerve function damage. The SCOPA-AUT total score and scores of dimensions of gastrointestinal function, urinary system function, cardiovascular function, body temperature regulation, pupil movement and sexual function were (34.16 ± 10.41) points, (10.25 ± 3.31) points, (9.44 ± 2.89) points, (4.02 ± 1.66) points, (6.41 ± 2.35) points, (1.35 ± 0.49) points and (2.69 ± 0.81) points. The total score of MoCA and scores of language function, naming, delayed recall and attention in the damage group were lower than those in the non-damage group (P < 0.05). Pearson correlation analysis results revealed that the total score, urinary system score, cardiovascular function score and body temperature regulation score of SCOPA-AUT were negatively correlated with the total score of MoCA (r = -0.545, -0.598, -0.607, -0.615, P < 0.05), and the gastrointestinal function score, pupil movement score and sexual function score were not significantly associated with total score of MoCA (P > 0.05). Conclusion The risk of autonomic nerve function damage is high in patients with CSVD, and it involves multiple systems and is closely related to the cognitive function of patients.
2.Autogenous rib cartilage combined with nasal Pitanguy ligament reconstruction technique for correction of nasal tip droopiness

Objective:To evaluate the clinical efficacy of autologous costal cartilage combined with nasal Pitanguy ligament reconstruction for correcting downward rotation of the nasal tip following rhinoplasty.Methods:A retrospective analysis was conducted on the clinical data of patients who underwent this combined technique at the Department of Plastic Surgery, Chongqing Contemporary Plastic Surgery Hospital, between January 2019 and January 2021. Autologous costal cartilage was carved into a bifurcated nasal columella support graft (3 cm in length × 8 mm in width × 2 mm in thickness, with a 1 cm bifurcation at the distal end). This graft was fixed across the caudal septum to establish a costal cartilage support framework. A piece of costal perichondrium (approximately 3 cm × 1 cm) was harvested. Using 5-0 PDS sutures, the cephalic end of the perichondrium was meticulously fixed to the residual stump of the Pitanguy ligament on the nasal dorsum, while the caudal end was anchored to the nasal tip scaffold, thereby reconstructing the ligamentous tension system. Postoperative complications were monitored. Preoperatively and at 12 months postoperatively, patients’ photographs were processed using Adobe Photoshop CS 6 to measure the nasolabial angle, nasofrontal angle, and columellar-lobular angle. Patient satisfaction was assessed using the rhinoplasty outcome evaluation (ROE) questionnaire (total score range: 0-24; higher scores indicate greater satisfaction). The paired t-test was employed for statistical comparison of these measurements and scores, with a P-value < 0.05 considered statistically significant. Results:A total of 76 patients were included (16 males, 60 females), with a mean age of 26.5 ± 4.3 years (range: 18-42 years). The mean follow-up duration was 13.2 months (range: 12-15 months). All patients recovered well, with no complications, such as nasal tip graft visibility, graft displacement, or breathing difficulties were observed. Natural contouring between the nasal tip and dorsum was achieved, and the fullness of the infratip lobule was significantly improved. Statistically significant differences ( P < 0.01 for all) were found between preoperative and 12-month postoperative measurements: nasofrontal angle (143.54° ± 5.08° vs. 134.12° ± 8.63°), nasolabial angle (82.28° ± 7.02° vs. 95.36° ± 3.76°), columellar-lobular angle (50.72° ± 6.67° vs. 43.05° ± 4.52°), and ROE scores [(9.05 ± 2.03) points vs. (22.07 ± 2.45)points]. Conclusion:The combined technique utilizing autologous costal cartilage and Pitanguy ligament reconstruction effectively corrects downward rotation of the nasal tip. Through the synergistic action of costal cartilage structural support and anatomical restoration of the Pitanguy ligament, it achieves a natural nasal tip contour, functional stability and high patient satisfaction.
3.Development and verification of a risk prediction model for social dysfunction in middle-aged and elderly patients with ischemic stroke

Objective To develop and validate a risk prediction model for social dysfunction in middle-aged and elderly patients with ischemic stroke.Methods A non-matched case-control study was conducted among ischemic stroke patients admitted to the neurology department of a tertiary hospital in Tangshan between August 2022 and March 2023.Patients who developed social dysfunction within 3 months after discharge were assigned to a case group,while those without it were assigned to a control group.Multivariate logistic regression was used to identify significant predictors and construct a nomogram-based prediction model.The model's discrimination and calibration were assessed using the area under the receiver operating characteristic curve(AUC)and the Hosmer-Lemeshow test.Internal validation was performed via bootstrap resampling,and clinical utility was evaluated using decision curve analysis.Results Logistic regression identified the following as significant risk factors for social dysfunction(P<0.05):male gender,age≥60 years,primary education or below,rural residence,income<3 000,cognitive impairment,low disability acceptance,poor self-management ability,suboptimal utilization of chronic disease resources,low future-oriented coping,and high cumulative ecological risk.The nomogram achieved an AUC of 0.874,with a sensitivity of 79.4%and specificity of 80.7%.The Hosmer-Lemeshow test indicated good calibration(x2=3.631,P=0.88).Conclusion The developed nomogram provides an effective tool for predicting the risk of social dysfunction in middle-aged and elderly ischemic stroke patients,facilitating early identification of high-risk individuals.

5.Effects of dietary naringenin on cadmium-induced endoplasmic reticulum stress and autophagy in chickens and its hepatoprotective effect

This study aims to investigate the protective effect of naringenin(Nar)on cadmium(Cd)-induced liver injury in chickens and its regulatory role in endoplasmic reticulum stress(ERS)and autophagy.Forty-eight 1-day-old Hyland Brown egg roosters were divided into four groups:The control group,the Cd group(150 mg/kg CdCl2),the Nar group(250 mg/kg Nar),and the Cd+Nar group(150 mg/kg CdCl2+250 mg/kg Nar),and the rearing period was 60 days.On the 60th day,6 chickens were randomly selected from each group,and after fasting for 24 hours,blood was collected via vein and then removed from the neck and executed,the livers were collected and organ coefficients were calculated,changes in the content of liver function-related indexes were de-termined,pathological sections were made and observed,and the expression of mRNA and protein of related genes were detected by fluorescence quantitative PCR and Western blot method.The re-sults showed that Cd treatment resulted in a highly significant decrease(P<0.01)in body mass,a highly significant increase(P<0.01)in liver coefficient,a highly significant increase(P<0.01)in serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),and lactate dehydrogenase(LDH)levels were highly significantly increased(P<0.01),hepatic antioxidant capacity was highly significantly reduced(P<0.01),and ERS-related genes(GRP-78,PERK,eIF-2α,CHOP,ATF-4,ATF-6)and autophagy-related genes(Atg-5,Beclin-1,LC3)mRNA expression was highly significantly up-regulated(P<0.01),and the expression of mRNA for p62 was highly significantly reduced(P<0.01).After the addition of Nar,Cd-induced liver injury was significantly alleviated,liver function indexes improved,hepatic antioxidant capacity was highly significantly el-evated(P<0.01),mRNA expression of ERS-related genes(GRP-78,PERK,eIF-2α,CHOP,ATF-4,ATF-6)and autophagy-related genes(Atg-5,Beclin-1,LC3)was highly significantly decreased(P<0.01)and mRNA expression of P62 was highly significantly increased(P<0.01).This study suggests that Nar has potential in alleviating Cd-induced liver damage in chickens through the reg-ulation of ERS and autophagy,providing theoretical support for its application in livestock health.

8.A survey report on the status of emergency radiology in China

Objective:To investigate the application status of emergency radiology in China, and to provide data support for the standardized development, scientific management and big data research of emergency radiology.Methods:From August 12th to October 19th, 2022, a questionnaire survey was conducted through WeChat"Questionnaire Star"to send targeted questionnaires to investigate the relevant data of the current status of emergency radiology in China, mainly including digital radiography (DR) and computed tomography (CT). This study was initiated by the Chinese Emergency Radiology Database Collaboration Group, and comprehensively investigated emergency imaging personnel, equipment, workload, critical value reporting process, and artificial intelligence (AI) application status.Results:There were 123 hospitals in the study. The survey showed that emergency DR/CT reports were mainly completed by residents and above (69.1%). There were 21 DR brands, 10 CT brands and 8 MR brands used for emergency imaging examinations. The median number of DR examinations in tertiary hospitals and secondary hospitals investigated from January to June 2022 was 4 642 and 2 015 cases respectively, and the median number of CT examinations was 16 512 and 3 762 cases respectively. The average single-shift workload of DR in the emergency radiology department during the day and night shift in tertiary hospitals was mainly ≤20 copies and 21-50 copies, and the average single-shift workload of CT in the emergency radiology department during the day and night shift was mainly 21-50 copies and 51-100 copies, while the average single-shift workload of DR/CT in the emergency radiology department during the day/night shift in secondary hospitals was mainly ≤20 copies. In terms of critical value reporting process, 74.8% of emergency imaging doctors and 84.6% of emergency imaging technicians took the way of phone/text message to notify the clinical doctor or the patients′ family. The overall deployment rate of AI in emergency imaging was about 60.2%. 75% of the respondents believed that in the future, AI can improve emergency radiology work from aspects such as emergency screening, aided diagnosis and process optimization.Conclusions:The emergency medical imaging mainly based on DR and CT has the current situations such as generally low seniority of doctors, diverse brands of imaging equipments, large volume of examinations and intense workload per doctor, especially in tertiary hospitals, and dependence on traditional means for critical value reporting. At present, AI is emerging in the field of emergency imaging, and there is still a long way to go to play the huge potential of AI in the intelligent whole process of emergency imaging in the future.
9.Bone marrow mesenchymal stem cell-derived exosomal miR-30e-5p ameliorates high-glucose induced renal proximal tubular cell pyroptosis

【Objective】 To study the effects of miR-30e-5p from bone marrow mesenchymal stem cell-derived exosomes(BMSC-exos) on high glucose (HG)-induced HK-2 cell pyroptosis and explore an alternative strategy to manage diabetic kidney disease (DKD). 【Methods】 BMSC-exos were isolated and internalized into HK-2 cells treated with HG to measure viability and cytotoxicity. The secretion of IL-1β and IL-18 was measured by ELISA. Pyroptosis was assessed by flow cytometry. The levels of miR-30e-5p, IL-1β, and IL-18 were measured. The expression of pyroptosis-associated cytokine proteins was determined. 【Results】 BMSC-exos decreased LDH, IL-1β, and IL-18 secretion and inhibited the expression of the pyroptosis-related factors (IL-1β, caspase-1, GSDMD-N, and NLRP3) in HG-induced HK-2 cells. Moreover, miR-30e-5p depletion in BMSC-exos promoted HK-2 cell pyroptosis. 【Conclusion】 BMSC-derived exosomal miR-30e-5p inhibits caspase-1-mediated pyroptosis in HG-induced HK-2 cells, which might provide a new strategy for treating DKD.
10.Recent advances in developing small-molecule inhibitors against SARS-CoV-2.

The COVID-19 pandemic caused by the novel SARS-CoV-2 virus has caused havoc across the entire world. Even though several COVID-19 vaccines are currently in distribution worldwide, with others in the pipeline, treatment modalities lag behind. Accordingly, researchers have been working hard to understand the nature of the virus, its mutant strains, and the pathogenesis of the disease in order to uncover possible drug targets and effective therapeutic agents. As the research continues, we now know the genome structure, epidemiological and clinical features, and pathogenic mechanism of SARS-CoV-2. Here, we summarized the potential therapeutic targets involved in the life cycle of the virus. On the basis of these targets, small-molecule prophylactic and therapeutic agents have been or are being developed for prevention and treatment of SARS-CoV-2 infection.
