1.Construction and empirical research of the nursing quality evaluation index system for interventional operating rooms under the context of medical alliance system

Objective Based on the interventional operating room,the nursing quality evaluation index system was con-structed under the background of medical union and empirical analysis was conducted,so as to provide support and basis for accu-rate and efficient evaluation of nursing quality in interventional operating room.Methods From July to September 2024,based on evidence-based methods,the relevant literature was retrieved and evaluated,and the structure-process-outcome theory was taken as the basic framework,and the relevant nursing quality indicators were preliminarily drawn up in interventional operating rooms.In October 2024,a total of 28 experts were selected to conduct expert letter consultation(2 rounds)with Delphi method,from which the indicators were revised and finally clarified,and empirical analysis was carried out.Results There were two rounds of expert consultation in this paper.The questionnaire recovery rate of the first round was 85.71%(24/28).The Cs,Ca and Cr of the first round were 0.895,0.942 and 0.919,respectively.The recovery rate of the second round questionnaire,Cs,Ca and Cr of the experts were 100.00%(24/24),0.891,0.941 and 0.916,respectively,and Kendall's W value was within 0.088-0.301(P<0.05).The final nursing quality sensitive index system consists of 1 structure,5 process and 2 outcome inde-xes,respectively.Fifty patients undergoing interventional surgery were included before and after intervention.Empirical analysis showed that the incidence of adverse events after intervention(4.00%)was lower than that before intervention(16.00%)(P<0.05).Conclusion The sensitive index of nursing quality based on the background of medical union is practical and scientific,and can provide practical guidance for the evaluation,continuous improvement and optimization of nursing quality in interventional operating room,and reduce the incidence of adverse events.
2.Analysis of the gene mutation of patients with congenital plasminogen deficiency

Objective:To analyze the gene mutations of 18 patients with plasminogen (PLG) deficiency and to explore the clinical manifestations caused by PLG gene mutations.Methods:This study belongs to observational study-descriptive study: case series.Clinical data from 18 patients with PLG deficiency admitted to the First Affiliated Hospital of Wenzhou Medical University from January 1st, 2021 to May 31st, 2025 were collected. The age ranged from 16 to 70 years old, with an average of 48 years old. Among them, there were 10 males and 8 females. Anticoagulant blood samples were taken before treatment to measure and analyze plasminogen activity (PLG:A), plasminogen antigen (PLG:Ag), protein C activity, protein S activity, fibrinogen, antithrombin activity, D-dimer, and fibrin (fibrinogen) degradation products. PCR direct sequencing was used to analyze the 19 exons and flanking sequences of the PLG gene in these patients, and reverse sequencing was employed to verify the suspected mutations.Results:For the 18 patients, cranial MRI showed fresh cerebral infarction lesions, and PLG:A levels ranged from 19% to 67%, while no other lab indicators showed significant abnormalities, all presenting with dysplasminogenemia. Genetic analysis revealed five types of PLG gene mutations: c.1858G>A (p.Ala620Thr) heterozygous mutation, c.1858G>A (p.Ala620Thr) homozygous mutation, c.398A>G (p.His133Arg) heterozygous mutation, c.2108G>A (p.Gly703Asp) heterozygous mutation, and c.1702G>A (p.Gly568Arg) heterozygous mutation. Among the above, the c.1858G>A heterozygous mutation was the most common, and c.398A>G and c.1702G>A were identified for the first time.Conclusion:Patients with plasminogen deficiency caused by PLG gene defects are prone to occur cerebral infarction events, which may be related to impaired fibrinolytic function due to PLG gene mutations.
3.Intelligent recognition and automatic measurement of uterine fibroids based on ultrasonic images

Objective:To develop an intelligent recognition and precise segmentation technique using ultrasonic images,and to enhance diagnostic efficiency and accuracy.Methods:A total of 1,430 patients diagnosed with uterine fibroids through transvaginal ultrasonography at the Maternal and Child Health Hospital of Guangming from November 2020 to October 2024 were retrospectively included. Ultrasonic images were manually annotated by two experienced physicians and reviewed by a senior expert. The Mask DINO deep learning model was used for lesion segmentation,and the segmentation results were optimized using ellipse fitting technology. Model performance was evaluated using the Dice coefficient,intraclass correlation coefficient(ICC),mean absolute error(MAE),and measurement accuracy.Results:In the test set of 286 cases,the average Dice coefficient of model prediction was 0.992,indicating extremely high segmentation accuracy. The average accuracy of lesion identification by the model was 0.909,with 241 correctly identified samples,19 basically correct samples,and 26 incorrect samples. In terms of long and short axis measurements,the ICC of the model's direct predictions were 0.871(short axis)and 0.784(long axis),with MAE of 0.436 cm(short axis)and 0.508 cm(long axis). After optimization with ellipse fitting,the ICC increased to 0.893(short axis)and 0.866(long axis),and the MAE decreased to 0.191 cm(short axis)and 0.274 cm(long axis),the measurement accuracy improved significantly.Conclusions:The intelligent recognition and precise segmentation technique for uterine fibroids based on ultrasonic images constructed in this study performed excellently in lesion segmentation and measurement,it can significantly improve the efficiency and accuracy of diagnosis.
4.Advances in the vortex vein and choroidal vasculature of central serous chorioretinopathy

Central serous chorioretinopathy(CSC), the first described pachychoroid disease, is characterized by visual distortions and loss of vision, which are commonly seen in middle-aged male. Research has demonstrated that ocular blood flow in CSC is in a state of overload, characterized by the dilation of vortex vein ampullae and choroidal vasculature. The obstruction of venous outflow is linked to scleral thickness, while the choriocapillaris exhibits perfusion deficits due to compression from the engorged vascular layer. Over time, vascular remodeling occurs, with venous anastomoses forming to create alternative drainage pathways and mitigate blood stasis. These abnormalities in vortex vein dynamics and choroidal circulation play a critical role in elucidating the underlying pathogenesis and clinical manifestations of CSC. This review highlights the alterations in vortex vein and choroidal vasculature in CSC, hoping to understand how the changes of blood flow affect the course of CSC and their correlation with treatment response. By evaluating blood flow dynamics, we aim to determine the disease stage more accurately, optimize therapeutic strategies, and ultimately enhance patient outcomes.
5.Clinical study on diagnosis of intrauterine infection by dynamic quantitative detecting cytomegalovirus antibody

Objective:To investigate the significance of dynamic quantitative detection of(cytomegalovirus)CMV antibodies in assessing the risk of fetal CMV infection.Methods:A total of 171 pregnant women with positive/weakly positive CMV IgM antibody ELISA screening results from Huizhou Central People's Hospital from August 2019 to March 2022 were selected as the study objects.IgM and IgG levels were measured by electrochemical luminescence(ECLIA),and re-examined 2 to 4 weeks later;at the same time,CMV nucleic acid was detected by real-time PCR fluorescent probe,and the fetal outcome was followed up.Results:In 171 cases,135 CMV IgM antibody ELISA(+)/ECLIA(-)specimens were positive for IgG antibodies and had stable levels after 2~4 weeks,one case was diagnosed with severe β thalassemia at 19+5 weeks,and the remaining fetuses were in good condition.There were 36 cases of IgM antibody ELISA(+)/ECLIA(+)specimens,one was negative for IgG antibody;35 cases had positive IgM antibody and stable IgG antibody level after 2~4 weeks,the fetus was in good condition;one case had IgM antibody level decreased and IgG antibody level increased significantly,urinary CMV DNA level gradually decreased,and was diagnosed as disseminated congenital CMV infection.Conclusion:The false positive rate of CMV IgM antibody detected by ELISA method is significantly higher than that of ECLIA method,and the dynamic quantitative detection can effectively eliminate interference.Maternal women with significant changes in CMV IgM,IgG antibodies and urinary DNA levels are at high risk of intrauterine infection and had a high probability of fetal congeni-tal CMV infection.

10.Cardiovascular health status in Chinese school aged children

Objective:
The American Heart Association released the Life s Essential 8 (LE 8) for the overall evaluation of cardiovascular health (CVH) on individual level. The present study aimed to describe the overall CVH in Chinese school aged children using LE 8 metrics.
Methods:
Data of the present analysis came from a national representative multicentered cross sectional study conducted in 7 provinces of China in 2013. The original study used a multistage cluster sampling method. A total of 10 326 children aged 5 to 19 years with complete data of health behaviors and health outcomes were included in the study. Children s health behavior indicators included diet, physical activity, nicotine exposure and sleep health. Health outcome factors included body mass index, fast blood glucose, lipid profile and blood pressure.
Results:
The median CVH score was 73.3 ( IQR =14.4) in boys and 73.4 ( IQR = 13.5) in girls. Compared to children aged ≤9 years, the health behavior scores were lowest in the 13-15 age group, with boys scoring 7.73 lower (95% CI =-8.35--7.12, P <0.01) and girls scoring 9.15 (95% CI =-9.83--8.48, P <0.01) lower. The ≥16 age group had the lowest health outcome scores, with boys scoring 7.85 (95% CI =-9.07--6.63, P <0.01) lower and girls scoring 6.11 (95% CI =-7.12--5.09, P <0.01) lower.
Conclusions
Chinese school aged children are generally at a moderate level of cardiovascular health. Specific LE 8 components vary substantially between groups and therefore require targeted intervention strategies.
