1.Evaluation of the Pharmacological Efficacy and Network Pharmacological Mechanisms of Total Lignans of Syringae Ramuls Against Liver Cancer

Objective To explore the effect and potential mechanism of total lignans of syringae ramuls(TLS)against liver cancer u-sing in vitro cellular assays,network pharmacological and molecular docking.Methods Flow cytometry was performed to detect the effect of TLS on apoptosis and cycle of HepG2 cells.TL components were collected by literature search;drug-like properties and synthetic scores of components were assessed by ADMETlab;toxicological parameters of components were predicted by ProTox-Ⅱ;TLS component targets were predicted by Swiss Target Prediction website;GeneCards,OMIM,DisGeNET,and PharmGKB databases were used to screen the related targets of liver cancer;The"component-disease-target"network diagram was constructed by Cytoscape software and the"protein-protein interaction"(PPI)network diagram was constructed by STRING platform;and DAVID database was used to analyse GO,KEGG and WIKI metabolic pathways.Results TLS at 50-200μg/ml could significantly inhibit the proliferation of HepG2 cells,induce apoptosis and block the cell cycle.The main components of TLS in anti-liver cancer were 31 active components,such as(-)matairesinol,(-)-secoisolariciresinol,and pinnatifolin A,which mainly acted on 82 key targets such as Akt1,Bel-2 and EGFR,which were mainly enriched in the cancer pathway,EGFR tyrosine kinase inhibitor resistance,cycle and apoptosis signaling pathways.Conclusion TLS may play an anti-liver cancer effect through multi-components,multi-targets,and multi-pathway.
2.Effects of different volumes of ropivacaine for iliopsoas muscle plane block on quadriceps strength in patients undergoing hip arthroplasty

Objective:To evaluate the effects of different volumes of ropivacaine for iliopsoas muscle plane block on quadriceps strength in patients undergoing hip arthroplasty.Methods:In this prospective study, 75 patients of either sex, aged 40-80 yr, with a body mass index of 18.5-30.0 kg/m 2, of American Society of Anesthesiologists Physical Status classification Ⅰ-Ⅲ, undergoing elective hip arthroplasty from June to December 2023 at Baoding First Central Hospital, were randomized into 3 groups ( n=25 each) by using a computer-generated randomization sequence: ropivacaine 5 ml group (group Ⅰ), ropivacaine 10 ml group (group Ⅱ), and ropivacaine 15 ml group (group Ⅲ). Before induction of anesthesia, iliopsoas muscle plane block was performed with 0.5% ropivacaine 5, 10 and 15 ml in Ⅰ, Ⅱ and Ⅲ groups, respectively. The lateral femoral cutaneous nerve block was carried out using 0.5% ropivacaine 5 ml in all the three groups. The quadriceps muscle strength and status of quadriceps motor block in the affected limb were assessed on admission to postanesthesia care unit (PACU) and at 2, 4, 6, 24 and 48 h after surgery. The consumption of dezocine, score for patient′s satisfaction with analgesia, and occurrence of complications were recorded. Results:Compared with group Ⅰ, the quadriceps muscle strength scores were significantly decreased on admission to PACU and at 2, 4 and 6 h postoperatively in group Ⅲ, the rate of quadriceps motor block was significantly increased on admission to PACU in group Ⅱ, and the rate of quadriceps motor block was significantly increased on admission to PACU and at 2 and 4 h postoperatively in group Ⅲ ( P<0.05), and the consumption of dezocine was significantly decreased in group Ⅱ and group Ⅲ ( P<0.05). Conclusions:The recommended volume of ropivacaine for iliopsoas plane block is 10 ml in patients undergoing hip arthroplasty.
3.Effectiveness of IDEAS model-based preoperative visits in patients undergoing cataract surgery

Objective:To investigate the effect of IDEAS model-based preoperative visit in patients undergoing cataract surgery.Methods:From January 2022 to December 2023, 120 patients with cataract surgery admitted to the Department of Ophthalmology of the First Affiliated Hospital of Zhengzhou University were selected for the study using the convenience sampling method. The 53 patients included from January to December 2022 were set up as the control group, and the 67 patients from January to December 2023 were set up as the experimental group. Conventional preoperative visits were used in the control group and IDEAS model-based preoperative visits were used in the experimental group. Before and after the intervention, anxiety was compared between the two groups using the State Trait Anxiety Inventory Form Y-I (S-AI), while the physiologic indices (systolic blood pressure, diastolic blood pressure, and pulse rate) of the two groups were compared.Results:After the intervention, the S-AI score, systolic blood pressure, diastolic blood pressure, and pulse rate of the experimental group were lower than those of the control group, and the differences were statistically significant (all P<0.05) . Conclusions:Preoperative visits based on the IDEAS model have a positive effect on alleviating anxiety in patients undergoing cataract surgery.
4.Effects of Remimazolam combined with Esketamine on cellular immunity,cognitive function,and sleep quality in elderly patients with lung cancer

Objective To investigate the effects of Remimazolam combined with Esketamine on cellular immunity,cognitive function,and sleep quality in elderly patients with lung cancer during surgery.Methods A total of 106 elderly lung cancer patients who underwent elective thoracoscopic lobectomy from September 2023 to March 2025 were selected and divided into the control group(n=53)and the research group(n=53)according to random number table method.The control group received Midazolam combined with Esketamine for general anesthesia,while the research group received Remimazolam combined with Esketamine for general anesthesia.The intraoperative and postoperative conditions,hemodynamic parameters[heart rate(HR),mean arterial pressure(MAP)]before anesthesia induction(T0),at tracheal intubation(T1),at 1 min before one-lung ventilation(OLV)(T2),at skin incision(T3),at 1 h after OLV(T4),and immediately after extubation(T5),as well as pain and inflammatory factors[5-hydroxytryptamine(5-HT),prostaglandin E2(PGE2),high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)],cellular immunity[peripheral blood natural killer(NK)cells,CD3+,CD4+,CD4+/CD8+],the levels of serum interleukin(IL)-2,interferon-γ(IFN-γ),cognitive function[Mini-Mental State Examination(MMSE)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)],and recovery quality[Postoperative Quality of Recovery-15(QoR-15)score]were compared between the two groups of patients.The adverse reactions and complications were also compared between the two groups.Results There was no significant difference in duration of surgery,duration of anesthesia,intraoperative blood loss,and time to spontaneous breathing recovery of patients between the two groups(P>0.05).The total dosage of Sufentanil in the research group was less than that in the control group,and time to postoperative eye opening,duration of recovery room stay and length of postoperative hospitalization were shorter than those in the control group(P<0.05).At T0 and T5,there was no significant difference in HR and MAP between the two groups(P>0.05),and at T1-T4,HR and MAP in the research group were higher than those in the control group(P<0.05).At the time of recovery,the levels of serum 5-HT,PGE2,hs-CRP,and TNF-α in the research group were lower than those in the control group(P<0.05),and at the time of recovery,the peripheral blood NK cells,CD3+,CD4+,and CD4+/CD8+ratio,serum IL-2 and IFN-γ levels in the research group were higher than those in the control group(P<0.05).At 1 d after surgery,the MMSE and QoR-15 scores of the research group were higher than those of the control group,while the PSQI score was lower than that of the control group(P<0.05).The incidence of adverse reactions and complications in the research group[5.66%(3/53)and 3.77%(2/53)],was lower than that of the control group[18.87%(10/53)and 15.09%(8/53)](P<0.05).Conclusion The combination of Remimazolam and Esketamine in elderly patients undergoing thoracoscopic lobectomy can better maintain intraoperative hemodynamics,stabilize cellular immune function,reduce serum pain and inflammatory factor levels,improve postoperative cognitive function and sleep quality,reduce adverse reactions and complications,and promote postoperative recovery.

9.Chinese Herbal Prescription Against Esophageal Cancer: A Review

Esophageal cancer is a type of upper gastrointestinal malignant tumor with a high degree of malignancy, strong invasion ability, and poor prognosis, which belongs to the category of "dysphagia" in traditional Chinese medicine (TCM). In addition to tumor resistance, Chinese herbal prescription plays a role in sensitization. In light of information in literature, the syndrome elements of esophageal cancer include Qi stagnation, phlegm, Qi deficiency, blood stasis, and Yin deficiency. After clinical differentiation, the syndromes of esophageal cancer are divided into phlegm and Qi obstruction, Qi and Yin deficiency, fluid deficiency and heat accumulation, Qi deficiency and phlegm dampness, combined phlegm and heat, combined phlegm and stasis, combined heat toxin and stasis, healthy Qi deficiency and toxin accumulation, et al. Dabanxiatang, Qigesan, Xuanfu Daizhetang, Liujunzitang, Shashen Maidongtang, and Tongyoutang are the common clinical prescriptions, where Qigesan, Liu Junzitang, and Tongyoutang have been proved by in vitro and in vivo experiments to exert anti-esophageal cancer effect by directly inhibiting tumor cell proliferation, promoting apoptosis, affecting tumor microenvironment, regulating cell energy metabolism, and inhibiting angiogenesis. In addition, an increasing number of studies have been conducted on anti-esophageal cancer effect of Chinese herbal prescription by targeting non-coding single-stranded microRNA. The specific mechanisms of Da Banxiatang, Shenzhe Peiqitang, Xiao Xianxiongtang, Renshen Banxiatang, and Liushenwan have been scarcely reported despite good clinical efficacy. Wuzhuyu Tang and Tongguansan recorded in ancient books have been rarely applied in modern times. Therefore, the present study reviewed the special drugs and prescriptions mentioned in ancient TCM classics, the commonly used Chinese herbal prescriptions in modern clinical practice, and experimental research progress, to promote treatment methods of Chinese herbal prescriptions against esophageal cancer.
10.Cause of death of children with intraocular retinoblastoma in the Beijing Children's Hospital 2009-2017

Objective:To observe and analyze the clinical characteristics of children who died of intraocular retinoblastoma (RB).Methods:A retrospective clinical study. Fourteen children (23 eyes) with intraocular RB who died after receiving treatment in Beijing Children's Hospital from 2009 to 2017 were included in the study. Among the children, there were 7 males (10 eyes) and 7 females (13 eyes); 5 had unilateral and 9 had bilateral tumor. Age were 17.2±15.5 months. All children underwent RetCam examination. RB was staged according to the international intraocular RB classify. Among the 23 eyes, 1 eye was in stage B, 2 eyes were in stage C, 12 eyes in stage D, and 8 eyes in stage E. Treatment methods included a systemic (vincristine, etoposide and carboplatin) chemotherapy (VEC chemotherapy), enucleation surgery, and vitrectomy. The basic conditions including age, time of diagnosis, pathological diagnosis, treatment and main causes of death were retrospectively analyzed.Results:Among the 14 cases, the first symptom was leukemia in 12 cases, red eye in 1 case, and squintin in 1 case. Systemic VEC chemotherapy was used for 1-6 courses of treatment; 5 cases were enucleated, 3 cases underwent histopathological examination; 3 cases were treated with vitrectomy. Among the 3 cases who underwent histopathological examination, the sclera and optic nerve, optic nerve and optic disc were invasted respectively. Seven patients died of tumor metastasis and/or intracranial lesions (50.0%, 7/14); the median survival time was 19 months. Four patients died of treatment (28.6%, 4/14), including 3 patients died of chemotherapy-related side effects, and 1 died of organ failure after enucleation surgery (7.1%); the median survival time was 3.5 months. Early abandonment of treatment died in 3 cases (21.4%, 3/14); the median survival time was 15 months.Conclusion:Intracranial metastasis is the main cause of death in children with intraocular RB.
