Objective:To systematically synthesize the illness stigma experiences of stroke patients.Methods:A systematic search was conducted in PubMed, Cochrane Library, Embase, Web of Science, China Biology Medicine disc, China National Knowledge Infrastructure, Wanfang Data, and VIP for qualitative studies exploring illness stigma in stroke patients. The search period extended from database inception to September 30, 2024. A Meta-synthesis approach was employed to integrate the findings.Results:A total of fourteen articles were included and ten new categories were extracted and synthesized into four integraion results: sources of illness stigma, key influencing factors of illness stigma, manifestations and impacts of illness stigma, and coping strategies illness stigma.Conclusions:Illness stigma is a significant barrier to stroke rehabilitation. Nurses should be aware of the characteristics of illness stigma, conduct early identification, and develop individualized strategies for prevention and intervention. It is essential to help patients build multi-dimensional support systems involving society and family, and to guide them in gradually accepting and adapting to their new self.

3.Meta-synthesis of qualitative researches on experience of fear of falling in the elderly

Objective:To systematically evaluate the qualitative research on experience of fear of falling in the elderly, so as to provide theoretical basis for nursing practice.Methods:Qualitative researches on the fear of falling among the elderly were systematically searched in PubMed, Web of Science, Embase, Cochrane Library, China National Knowledge Infrastructure, WanFang Data, VIP, and China Biology Medicine disc. The search period was from database establishment to November 5, 2023. The quality of literature was evaluated using the quality evaluation criteria for qualitative research of the Australia Joanna Briggs Institute Evidence-Based Health Care Center, and the results were integrated using the Meta-synthesis.Results:A total of 13 articles were included, 44 themes were extracted, categorized into 12 categories, and refined into four integrated results, namely perception of fear of falling, influencing factors of fear of falling, coping strategies in attitude and behavior, and multidimensional needs for rehabilitation nursing.Conclusions:Medical and nursing staff should pay attention to and understand the fear of falling among the elderly, evaluate their fear of falling from multiple dimensions and dynamically, provide personalized guidance based on coping behaviors, create support systems to alleviate their fear of falling and meet their multidimensional needs, so as to improve their quality of life.
4.Comparison of nursing delirium screening scale and intensive care delirium scale in the evaluation of delirium after percutaneous coronary intervention

Objective:To compare the evaluation ability and effect of the nursing delirium screening scale (Nu-DESC) and the intensive care delirium screening checklist (ICDSC) on the patients with delirium after percutaneous coronary intervention(PCI).Methods:A total of 128 patients who were admitted to cardiovascular surgery from June 2018 to June 2019 for PCI intervention were selected by convenient sampling method.Data were collected by general data questionnaire, restless sedation scale (RASS), Nu-DESC and ICDSC.SPSS 18.0 was used to analyze the collected data.The patients' receiver operating characteristic(ROC) curve and area under the ROC curve(AUC) were used to analyze Nu-DESC scale and ICDSC scale to evaluate the delirium ability of patients after PCI. Kappa value was used to test the consistency of the scale.Bayes discriminant analysis was used to analyze the accuracy of Nu-DESC and ICDSC in judging delirium after PCI.Results:The AUC of Nu-DESC and ICDSC was 0.902 and 0.857 respectively, and the difference between them was AUC=0.045 ( Z=3.489, P<0.001); the best critical value of Nu-DESC and ICDSC for delirium evaluation was 2 and 4. The cross validation accuracy of Nu-DESC and ICDSC for delirium determination was 87.5% and 83.6%, respectively.The delirium rates of DSM-Ⅳ, Nu-DESC and ICDSC were 33.6%, 34.4% and 43.7%, respectively. Conclusion:Compared with ICDSC, Nu-DESC is more suitable for the evaluation of delirium after PCI.

6.The application effect of ICU checklist of medical cooperation to prevent ventilator-associated pneumonia

Objective To explore the application effect of ICU checklist of medical cooperation on prevention of ventilator-associated pneumonia. Methods Totally 728 patients with mechanical ventilation from July 2015 to July 2016 in ICU were assigned to the experimental group with using of the checklist; and 685 patients with mechanical ventilation from June 2014 to June 2015 were set as the control group who did not use the checklist. The incidence of ventilator-associated pneumonia and medical cooperation were compared before and after the implementation of the checklist. Results The incidence of ventilator associated pneumonia were 6.72% (46/685) and 3.02% (22/728), the intervention strategy execution rates were 63.36% (434/685) and 84.89% (618/728) before and after the application of checklist, there was significant difference between two groups (χ2=10.51, 86.03, P<0.01). Medical cooperation (doctors and nurses) and satisfaction scored 18.53 ± 2.39, 21.91 ± 2.16, 30.93 ± 2.18 and 26.71 ± 2.21, 28.64 ± 2.55, 45.33 ± 1.74 before and after the application of checklist, there was significant difference between two groups (t=6.14, 14.25, 2.39, P<0.01 or 0.05). Conclusions The application of checklist can improve the medical cooperation on prevention of ventilator-associated pneumonia management, reduce the incidence of ventilator-associated pneumonia, improve medical cooperation, nursing satisfaction and the management efficiency, which is worthy of popularization and application.
7.Comparative study of laparoscopic and transabdominal radical hysterectomy's impact on the cellular immunity

Objective To compare the difference of impact on the cellular immunity between laparoscopic and transabdominal radical hysterectomy. Methods 60 patients with early cervical cancer (Ⅰa2~Ⅱa1), half of them were assigned to do abdominal radical hysterectomy (ARH) and the other half for laparoscopic radical hysterectomy (LRH). Adopt flow cytometry (FCM) detect peripheral blood T lymphocyte subsets, NK cells, CIK cells and T lymphocyte ac-tivation function on one day before surgery, one day, five days, and 28 days after the surgery separately. Compare the changes of immune status. Results After one day, the number of T lymphocyte subsets declined compared with preoperative one day(P <0.05). After five days, each index in LRH group was increased compared with postoperative one day, and the degree of decline is less than the ARH group ( P< 0.05), and recovered faster. After one day, the number of HLA-DR+CD3+in LRH group compared with the preoperative one day declined and HLA-DR+CD8+in-creased, and the degree of change is less than the ARH group, and recovered faster. Conclusions Immune function in patients after laparoscopic group was less changed, and recovered quickly, while the immune function were less inhibited, it may protect patients' immune function better.
8.Effect of Danchi Decoction on Serum Interleukin of Endometriosis

Objective Based on the immunologic balance, to investigate the mechanisms of Danchi decoction for controlling the recurrence of endometriosis (EMs) of qi stagnation and blood stasis after conservative operation. Methods A multi-center and randomized clinical trial was adopted. EMs patients were divided into Chinese medicine group (CG, 20 cases) and western medicine group (WG, 10 cases). The two groups were given Danchi decoction and GnRH-a or gestrinone respectively for 3 months. The serum IL-2, IL-6 and IL-2/IL-6 were detected before operation, after 3 months of treatment and 3 months after withdrawal, and compared with the control group (5 cases of simple ovarian cysts). Results Before operation, the serum IL-2 level of CG and WG was lower than control group (P<0.05), IL-6 was higher than control group (P<0.05), the ratio of IL-2/IL-6 was lower than control group (P<0.05). After treatment and 3 months after withdrawal, IL-2 level of CG and WG was increased than before operation (P<0.05), IL-6 was decreased (P<0.05), IL-2/IL-6 ratio was increased (P<0.05). There was no significant difference in IL-2/IL-6 between CG and control group (P>0.05), but 3 months after withdrawal, IL-2/IL-6 of WG was significantly lower than that of control group (P<0.05). Conclusion Immune imbalance is closely related to the occurrence and development of EMs. Danchi Decoction or GnRH-a or gestrinone can prevent the recurrence of EMs by adjusting the disorder of immune response, and Danchi Decoction has better effect on long-term result.
9.Screening of mutations in superoxide dismutase 1 gene in Chinese with familiar amyotrophic lateral sclerosis

Objective To identify the mutations in Cu/Zn superoxide dismutase ( SOD1 ) gene in three Chinese kindreds with amyotrophic lateral sclerosis ( ALS), compare the genotypes with those found in other ethnic groups and to analyze the clinical characteristics.Methods The diagnosis of ALS met El Escorial ALS diagnostic criteria.Genomic DNA was extracted from peripheral blood in ALS patients using standard procedure.PCR amplifications of five exons of SOD1 were performed using primers as described in the previous publication.The PCR products were directly sequenced.Results A heterozygous mutation H46R was found in four affected members in a family with middle age onset and slowly progressive ALS.A heterozygous mutation of G72C was identified in a 20-year-old male who died of respiratory failure after two years of ALS.His father carried the same mutation without clinical phenotype.In the third family with 20affected members with middle age onset and rapidly progress, a mutation of E13V was identified in 5 affected subjects.Conclusions This study is the first large screening of SOD1 mutation in Chinese familiar ALS patients.H46R has previously been found only in Japanese and Pakistanis; this is the first report in Chinese, suggesting H46R may be specific to Asians.The family with mutation G72C presented decreased penetrance, therefore screening SOD1 mutation in sporadic cases and unaffected family members is necessary.E133V is the first reported mutation and needs more study to investigate its effect on the disease.
10.The rehabilitation nursing for refractory pressure sore in patients with low position paraplegia

Objective To investigate the function and effect of early rehabilitation for patients with low position paraplegia and refractory pressure sore. Methods 15 patients with low position paraplegia and refractory pressure sores were given modified vacuum-assisted closure treatment, at the same time, rehabilitation and intervention for functional training of limbs, bladder, intestinal was carried out to enhance capacity of psychosocial adjustment in patients. Results 15 cases of paraplegia patients with refractory low pressure sores were all cured, no pressure sores relapsed at follow-up visit six months later. Conclusions Early intervention can improve the rehabilitation of self-care capabilities, improve patients' individual skin-care ability, speed up the healing of pressure sores in patients with paraplegia, so as to reduce pressure sore recurrence.
