Objective To investigate the causes and factors affecting unplanned reoperation,and to provide a reference basis for reducing the incidence of unplanned return of the patient to the operating room for reoperation.Methods Surgical data from the hospital was extracted spanning from January to December 2022,and subjected to a descriptive analysis of the overall situation,departmental distribution,primary reasons,and patient referrals related to unplanned reoperations in the hospital,and analyzed factors contributing to unplanned reoperations in the hospital using binary logistic regression.Results In 2022,130 unplanned reoperations were reported in this hospital,corresponding to an incidence of 0.35％.Patients who required unplanned reoperation were predominantly male(63.08％).The majority had surgical incision grade of category 0(46.92％),and surgeries were classified as levels 3 and 4(80.77％).Furthermore,88.46％of the surgeries were performed by surgeons with advanced degrees or higher.The common causes were postoperative bleeding,failure to achieve the desired result,need for the condition,probing for the cause,and occurrence of leakage or fistula,collectively accounting for 50.00％of the cases.Key factors contributing to unplanned reoperations were sex,type of surgical incision,and incision healing grade;among which male patients(OR=1.733,P=0.006),patients with class Ⅰ surgical incision(OR=2.909,P=0.004),and patients with incision grade B healing(OR=6.565,P＜0.001)showed a higher propensity for unplanned reoperations.Conclusion Hos-pitals should emphasize monitoring and managing unplanned reoperations by improving perioperative supervision,conducting thorough root cause analyses,and focusing on continuous quality improvement to enhance surgical outcomes and patient safety.

Objective To understand the current status and development trend of the research on Prunellae Spica;To provide reference for the subsequent research.Methods The research literature related to Prunellae Spica was searched in China Knowledge Resources Database(CNKI),Chinese Science and Technology Journal Database(VIP),Chinese Academic Journal Database(Wanfang Data),and Chinese Biomedical Literature Database(CBM)from January 1,2002 to March 23,2023.After merging and removing duplicates using NoteExpress 3.7.0.9258 software,CiteSpace 6.2.R1 software was used to visualize and analyze authors,institutions and keywords.Results Totally 3 136 articles were included,and the annual number of publications showed a fluctuating upward trend.Shaanxi Journal of Traditional Chinese Medicine had the most publications(93).The core authors included Lin Limei(34),Guo Qiaosheng(27),and Zhang Mingzhi(20),and core research institutions included Shandong University of Traditional Chinese Medicine(37),Tianjin University of Traditional Chinese Medicine(30),and Nanjing University of Chinese Medicine(28),with less cooperation among authors and institutions.The analysis of keywords showed that the research on the treatment of diseases were mainly"hypertension","breast cancer","hyperplasia of mammary glands","vertigo","insomnia".The chemical composition studies were mainly on"rosmarinic acid"and"ursolic acid".The type of literature was mainly"experience of famous doctors"and"clinical observation".Conclusion The research field of Prunellae Spica is in the rising stage of development,and the research content mainly focuses on clinical application and chemical composition research,etc.Summarizing the law of medication,exploring its mechanism of action and action target through data mining and molecular docking technology are the research trends.

Objective To understand the research hotspots and trend of Scrophulariae Radix from 2000 to 2023;To provide reference for related research.Methods The relevant literature of Scrophulariae Radix were retrieved from CNKI,VIP,Wanfang Data and CBM from January 1,2000 to June 1,2023.NoteExpress 3.7.0.9296 software was used to manage files and weight,and CiteSpace 6.2.R4 software was used for visualization analysis of authors,institutions and keywords.Results A total of 2 240 articles were included,and the annual number of publications showed a dynamic upward trend.The source journals with more publications included Shaanxi Journal of Traditional Chinese Medicine,Journal of Traditional Chinese Medicine,Shizhen Journal of Traditional Chinese Medicine Research and so on.The core authors were Zhang Ning,Liu Shumin,Liu Bin,etc;the main research institutions included Beijing University of Chinese Medicine,Nanjing University of Chinese Medicine,Guangzhou University of Chinese Medicine,etc.;high-frequency keywords were"harpagoside","quality standard","clinical observation"and so on.Conclusion The research of Scrophulariae Radix mainly focuses on chemical composition,disease treatment and compatibility application.It is a research trend in recent years to explore the medication law and mechanism of Scrophulariae Radix by using data mining,network pharmacology and molecular docking.

Objective To analyze the research hotspots and trend of Raphani Semen from 2000 to 2023;To provide reference and direction for the follow-up research.Methods The literature related to Raphani Semen was retrieved from CNKI,VIP,Wanfang Data and CBM from January 1,2000 to July 20,2023.NoteExpress 3.7 software was used to organize the bibliothems,and CiteSpace 6.2.R2 software was used to visualize the authors,research institutions and keywords.Results After screening,a total of 1 023 articles were included,and the annual publication volume showed a trend of first fluctuating and then slowly decreasing.The source journals were 421,and the journal with the most articles was Shaanxi Journal of Traditional Chinese Medicine(75 articles).It involved 682 authors,the core authors included Zhou Honglei,Lü Wenhai,Zhu Liqiao,etc.Core research institutions included Shandong University of Traditional Chinese Medicine,Beijing University of Chinese Medicine,Affiliated Hospital of Shandong University of Traditional Chinese Medicine,etc.High-frequency keywords included"constipation","data mining","clinical observation"and so on.Conclusion The current research hotspots in Raphani Semen are disease treatment,chemical composition,compatibility of prescriptions,related prescriptions and so on.The key research trend in this field is to summarize the clinical medication law by data mining technology and the treatment of spleen and stomach diseases.

Objective:To establish a micro kinetic-chromogenic method and verify its feasibility for bacterial endo-toxin testing.Methods:Establish an addition system for micro kinetic-chromogenic lysate reagent with 25 μL and 25 μL sample,respectively.Methodological validation was conducted by examining accuracy and precision,linear-ity and range,minimum detection limit,and reagent durability.Two methods,kinetic-chromogenic method and micro kinetic-chromogenic method,were used to compare the applicability of the method to 39 different varieties.Results:The recovery rates of three doses of 0.03,0.3,and 3.0 EU·mL-1 in six repeated experiments were(96.19±16.68)％,(92.53±10.17)％,and(89.54±25.84)％,respectively.The absolute value of the correlation coefficient(r)for the six standard curves is(0.997 7±0.002 3).When the minimum sensitivity concentration of lysate reagent is 0.01 EU·mL-1,the reaction time is(3 896.0±279.6)s,which is lower than the reaction time of the negative control.The durability test results meet the requirements by two micro kinetic-chromogenic lysate reagents from different manufacturers.The research results on variety adaptability show that the recovery rates of 39 varieties are between 50％and 200％,the effectiveness of the two methods is the same.Conclusion:The accuracy and precision,linearity and range,minimum detection limit,and reagent durability of micro kinetic-chromogenic method all meet the requirements of method validation.The variety applicability is equivalent to that of the traditional kinetic-chromogenic method,so this method can be used for bacterial endotoxin detection of routine samples.

Objective Analyze the implementation effects of the specialized disease management policy(hereinafter re-ferred to as the policy)to provide reference for strengthening operational management and enhancing the integration of business and finance in public hospitals.Methods Patient information of inpatients from a large comprehensive tertiary hospital from Jan-uary 2022 to June 2023 was collected.Four out of ten single diseases focused on by the national tertiary public hospital perform-ance assessment were included in the study group for specialized disease management,while the remaining six were used as the control group.A difference-in-differences model was adopted to analyze the changes in relevant indicators such as per capita inpa-tient expenses,average length of stay,per capita drug expenses,per capita consumable expenses,and per capita examination and treatment expenses before and after the policy implementation.Additionally,the effects of policy implementation were further analyzed based on different medical insurance types.Results A total of 22 457 patients were included,with 5 879 patients in the study group(3 164 cases before policy implementation and 2 715 cases after policy implementation),and 19 741 patients in the control group.The difference-in-differences model analysis showed that after the policy implementation,per capita inpatient expenses for specialized disease management patients decreased by 32.7％,average length of stay decreased by 31.4％,drug ex-penses decreased by 49.5％,consumable expenses decreased by 24.2％,and examination and treatment expenses decreased by 29.2％.Moreover,compared to patients under non-Wuhan medical insurance,patients under Wuhan medical insurance experi-enced a greater reduction in related indicators.Conclusion The implementation of the policy effectively reduces the burden on patients seeking medical care,improves medical treatment efficiency,contributes to hospital refined management methods,deep-ens the integration of business and finance,and facilitates the transition towards leaner operations.

Objective:To analyze the development status, research hotspots and development trend of Tianma Gouteng Decoction.Methods:Research literature on Tianma Gouteng Decoction in the medical field from January 1, 2002 to December 31, 2022 was retrieved from CNKI, WanFang, VIP, and SinoMed. The NoteExpress 3.7.0 software was used to merge and remove the duplicates. CiteSpace 6.1.R6 software was used to visually analyze the information such as the publication trend, authors, institutions and keywords of the included literature, and to display the cooperation network of authors and institutions, as well as the co-occurrence, clustering and burst map of keywords.Results:Totally 2 137 articles were finally included, with an overall increasing trend of annual publication. The distribution of journals was mainly in TCM, among which the Journal of Practical Traditional Chinese Internal Medicine had the most publications. Xian Shaoxiang (First Affiliated Hospital of Guangzhou University of Chinese Medicine) was the author with the most publications (17 articles). The institution with the most publications was Tianjin University of Traditional Chinese Medicine (23 articles), and the cooperation between research institutions was mainly in Chinese medicine colleges and their affiliated hospitals. The results of keyword analysis showed that the high-frequency keywords were hypertension, vertigo, hyperactivity of liver yang, clinical efficacy, migraine, etc. The keywords formed 10 clusters. The keywords with a long time span were syndrome differentiation and treatment, hypertension, experience of famous doctors, and clinical observation. In recent years, the emergent keywords were adverse reactions, blood pressure levels, vertigo, addition and subtraction treatment, and medication laws. Conclusions:The current research field of Tianma Gouteng Decoction is in a rising stage of development, and it is still necessary to strengthen academic exchanges and cooperation across regions and teams. Relevant research mainly focuses on the clinical application of cardiovascular and cerebrovascular diseases. The commonly used treatment is comprehensive therapy of TCM, focusing on blood pressure changes, adverse reactions and so on. The application of Tianma Gouteng Decoction in the treatment of vertigo, the study of adverse drug reactions and the discussion of medication rules have gradually become research hotspots.

Objective To understand the research status,hotspots and trends of Wuzhuyu Decoction;To provide reference for relevant research.Methods The literature related to Wuzhuyu Decoction was retrieved from CNKI,VIP,Wanfang Data and CBM databases from the establishment to February 28,2023.NoteExpress 3.6 was used to merge and deduplicate,and the author,organization and keywords were mapped and interpreted by CiteSpace 6.1.R6 software.Results A total of 822 articles were included,and the number of publications showed a wave upward trend;the top journals were New Chinese Medicine,Henan Traditional Chinese Medicine and Sichuan Traditional Chinese Medicine;the main research institutions were Beijing University of Chinese Medicine,China Academy of Chinese Medical Sciences and Shandong University of Traditional Chinese Medicine;a total of 566 authors were involved,and the authors with more publications included Wang Zhimin(12),Gong Muxin(9)and Bi Kaishun(5);high-frequency keywords included"headache","Shang Han Lun","TCM therapy"and so on.Conclusion Wuzhuyu Decoction is effective in treating chronic gastritis,hypertension and other primary diseases,which is a research hotspot in this field.It is a research trend in this field to explore its active components by high performance liquid chromatography and explain its action mechanism and target at the molecular level.
