1.Neurovascular coupling in patients with depression:a study based on multimodal magnetic resonance imaging

Objective:To investigate altered neurovascular coupling in patients with depression(DEP)using resting-state functional magnetic resonance imaging(MRI)and arterial spin labeling perfusion MRI,as well as its association with depressive symptoms.Methods:Neuropsychological assessment and multimodal MRI scans were performed for 25 DEP patients and 35 healthy controls(HCs).Arterial spin labeling perfusion MRI was used to calculate cerebral blood flow(CBF),and functional MRI was used to calculate regional homogeneity(ReHo).The Pearson correlation coefficient between CBF and ReHo was calculated to obtain neurovascular cou-pling.Results:At the whole-brain level,CBF-ReHo coupling was reduced in DEP patients compared with HCs.At the brain region level,CBF-ReHo coupling was reduced in 26 brain regions in DEP patients,which were mainly located in the visual network,the default network,and the auditory network.The correlation analysis showed that the coupling values of the left suboccipital gyrus,the left angular gyrus,and the left thalamus were negatively correlated with Hamilton Depression Scale score.Conclusion:There is a sig-nificant reduction in neurovascular coupling in DEP patients,which is correlated with the severity of DEP.
2.Study on quality standard of wine-processed Coptidis Rhizoma standard decoction

Objective:To establish the quality standard of the standard decoction of wine-processed Coptidis Rhizoma by studying the extraction rate, fingerprint and component quantitative analysis.Methods:ccording to the Technical Requirements for Quality Control and Standard Formulation of Chinese Medicine Formula Granules, 15 batches of the standard decoction of wine-processed Coptidis Rhizoma were prepared, and the paste rate was determined; HPLC fingerprints of 15 batches of standard decoction of wine-processed Coptidis Rhizoma were established, and evaluated by combining similarity evaluation, clustering analysis, principal component analysis and orthogonal partial least squares discriminant analysis; the contents of berberine, epiberberine, pamadine, and safranine in the samples of the 15 batches were determined and analyzed their transfer rates.Results:A total of 15 batches of standard decoction samples were calibrated with 11 common peaks, referring to the recognition of 8 components. The similarity between the samples and the control product was greater than 0.900; the clustering analysis could cluster the 15 batches of samples into 2 classes; the results of the principal component analysis showed that the cumulative variance contribution rate of the 3 principal component factors was 89.388%; the OPLS-DA screened out the 3 components of the quality difference; the 15 batches of samples out of the paste rate was 15.7% -20.8%, and the mass fractions of berberine, epiberberine, safranine, and palmatine were 18.47%-24.38%, 2.82%-3.49%, 5.08%-6.69%, and 4.84%-6.68%, respectively, with transfer rates of 41.7%-61.7%, 46.9%-68.7%, 39.8%-61.5%, and 43.8%-65.2%.Conclusion:The fingerprint and content determination method established in this study is accurate, stable, simple, and can be used for the quality control and evaluation of the standard decoction of wine-processed Coptidis Rhizoma.

Performance appraisal of public hospitals have given a guidance for the development of public hospitals at all levels.A Class A tertiary hospital reviewed the problems in the development of the hospital at the present stage and focused on the following four aspects:①insufficient fine management;②No clear orientation of discipline development;③The bottleneck of the improvement of medical operation efficiency;④New challenges in the reform of payment mode.The tertiary hospital launched a fine management practice in May 2022,in order to solve the problems by taking the Department of Surgery as a pilot area,laying the foundation for fine management through information system construction,improving the efficiency of medical operation through management process optimization,improving the overall competitiveness of disciplines through the construction of sub-specialty and Discipline Alliance and adjusting the performance appraisal index system to play the role of performance incentives.The measures effectively improve the overall capacity and efficiency of hospital medical services and help the hospital to achieve high-quality development.
6.Iodine nutritional status and influencing factors of thyroid disease of pregnant women in Beijing from 2021 to 2023

Objective:To understand the iodine nutritional status of pregnant women in Beijing, and analyze the influencing factors of thyroid disease.Methods:From 2021 to 2023, a stratified random sampling method was used to select at least 100 pregnant women from each of the 16 administrative districts under the jurisdiction of Beijing each year, using cross-sectional survey. Random urine samples and household salt samples were collected to determine urinary iodine and salt iodine levels, and were analyzed iodine nutritional status. Binary logistic regression model and restricted cubic spline(RCS) model were sued to analyze the influencing factors of thyroid disease in pregnant women (age, residential area, ethnicity, pregnancy, etc.).Results:A total of 4 878 pregnant women were investigated from 2021 to 2023, and the overall median urinary iodine level was 129.11 μg/L, indicating a deficiency in iodine nutrition (< 150 μg/L). There was a statistically significant difference in the urinary iodine levels of pregnant women between different years ( H = 20.22, P < 0.001). There was a statistically significant difference in median urinary iodine levels of pregnant women in different pregnancy stage ( H = 11.57, P = 0.003). The median of salt iodine value was 24.00 mg/kg, and the consumption rate of qualified iodized salt was 75.05% (3 661/4 878). The comparison of salt iodine levels between different years showed statistically significant differences ( H = 14.20, P < 0.001). Binary logistic regression analysis showed that the risk of thyroid disease in urban area was higher than that in suburban area ( OR = 2.01, P < 0.001), while the risk of disease in central government areas was lower ( OR = 0.52, P = 0.044). Compared with late pregnancy, the risk of the disease was lower in the early pregnancy ( OR = 0.60, P = 0.003) and the middle pregnancy ( OR = 0.71, P = 0.046). Supplementing with iodine can reduce the risk of the disease ( OR = 0.48, P < 0.001). The analysis of the RCS model showed that there was a nonlinear dose-response relationship between the risk of thyroid diseases in pregnant women and urinary iodine, with a U-shaped curve and cut-off values of 127.74, 472.66 μg/L. Conclusion:From 2021 to 2023, the iodine nutrition of pregnant women in Beijing is in iodine deficiency state, and factors affecting thyroid disease include residential area, pregnancy, supplementing with iodine, and urinary iodine.

Objective To establish an ultra-high liquid chromatography(UPLC)characteristic spectrum of Rheum tanguticum Maxim.ex Balf.water decoction and conduct chemical pattern recognition analysis,and to identify the medicinal materials of different origins and different processed products.Methods:UPLC was adopted to establish the characteristic spectra of 15 batches of Rheum tanguticum Maxim.ex Balf.Cluster analysis combined with principal component analysis was used to analyze their quality.Rhei Radix et Rhizoma from different origins and different processed products of Rheum tanguticum Maxim.ex Balf.were identified.Results:The characteristic spectrum of Rheum tanguticum Maxim.ex Balf.water decoction was established,18 common peaks were identi-fied,and 15 batches of Rheum tanguticum Maxim.ex Balf.were divided into 2 categories according to their origins by cluster analysis.The similarity between 15 batches of samples from different origins and the control spectrum was greater than 0.900.According to OPLS-DA analysis,a total of 6 markers(rhein-8-O-β-D-glu-cosid,resveratrol-4'-O-β-D-(6''-O-D-gallyl)glucopyranside,isolindleyin,rhein,epicatechin-3-O-D-gallate,and catechin)affecting the quality of Rheum tanguticum Maxim.ex Balf.water decoction samples were found.Rhei Radix et Rhizoma from different origins and different processed products of Rheum tanguticum Maxim.ex Balf.can be effectively distinguished.Conclusion:The established characteristic spectrum method is easy to operate and has good repeatability.It can be used for the quality control of Rheum tanguticum Maxim.ex Balf.water decoction,and can provide reference for the formulation of quality standard of formula granules of Rhei Radix et Rhizoma.
8.Research progress in asymptomatic lesions of neuromyelitis optica spectrum disorders

Neuromyelitis optica spectrum disorders (NMOSD) are a group of autoimmune-mediated demyelinating diseases of the central nervous system. A growing number of studies have identified asymptomatic lesions on imaging in patients with NMOSD, including asymptomatic optic neuropathy, asymptomatic spinal lesions, and asymptomatic brain lesions, even in the absence of clinical symptoms. The characteristics of asymptomatic lesions of NMOSD, influencing factors, relationship with relapse and disability progression, and management strategies were reviewed in this article.
9.Analysis of the prevalence and risk factors of goiter among children aged 8 to 10 years old in Beijing in 2018

Objective:To analyze the prevalence of goiter among children aged 8 to 10 years old in Beijing and study the influencing factors of goiter in children.Methods:In 16 districts of Beijing, each district was divided into 5 sampling districts according to east, west, south, north and middle locations in 2018. One town/street was selected from each sampling district, one primary school was selected from each town/street, and 40 non-boarding students aged 8 to 10 years old were selected from each primary school. The size of thyroid was measured by B-ultrasound method, the volume of thyroid was calculated and whether goiter or not was determined according to the "Diagnostic Criteria for Endemic Goiter". Random urine samples were collected, urinary iodine was detected by arsenic-cerium catalytic spectrophotometry. Height and weight were measured, body mass index (BMI) was calculated, and the correlation of thyroid volume with height and weight was analyzed.Results:A total of 3 268 children were surveyed, in which 154 children with goiter; the goiter rate was 4.7%. The goiter rate in girls [5.7% (93/1 641)] was higher than that in boys [3.7% (61/1 627), χ 2 = 6.694, P < 0.05]. The goiter rates of children aged 8, 9 and 10 years old were 4.3% (15/346), 5.5% (85/1 534) and 3.9% (54/1 388), respectively, there was no significant difference between the ages (χ 2 = 4.544, P > 0.05). The median of urinary iodine in children was 175.3 μg/L, which was in an iodine appropriate level. The thyroid volumes of children aged 8, 9 and 10 years old were positively correlated with height and weight ( r = 0.143, 0.225, 0.323, 0.338, 0.321, 0.346, P < 0.01). Conclusion:The goiter rate of children in Beijing is high, children's thyroid volume is greatly affected by height and weight, and it is recommended to consider the influence of height and weight when study the diagnostic criteria of children's goiter.
10.Contribution of NOD2 signal pathway to Chlamydia pneumonia Cpn0423-induced inflammatory response

Objective:To understand and determine the biological properties of Chlamydia pneumonia (Cpn) hypothetical protein Cpn0423 and the mechanisms of which involved in Cpn0423-induced inflammatory response. Methods:The biological properties of Cpn0423 gene were analyzed using bioinformatic software. The subcellular localization of nucleotide-binding oligomerization domain-like receptor 2 (NOD2) in bone marrow-derived macrophages (BMDMs) was detected by confocal microscope. NOD2-siRNA was used to inhibit the expression of NOD2 at mRNA level. Cpn0423-induced macrophage inflammatory protein 2 (MIP-2) and IL-6 production in BMDMs were detected by ELISA. PCR was performed to detect Cpn0423 DNA in bronchoalveolar lavage fluid (BALF) of Cpn-positive patients.Results:The homology between Cpn0423 and other type Ⅲ secretion system effector proteins of Chlamydia ranged from 85% to 93%. NOD2-siRNA could effectively inhibit the expression of NOD2 at mRNA level in BMDMs ( P<0.001). Moreover, Cpn0423-induced production of MIP-2 [(920.5±99.1) pg/ml vs (130.1±11.5) pg/ml, P<0.001] and IL-6 [(266.2±58.4) pg/ml vs (165.7±21.5) pg/ml, P<0.001] in BMDMs were decreased following NOD2-siRNA pre-treatment. Cpn0423 DNA was detected in the BAlF of 83.3% (10/12) of Cpn-positive cases, but not in Cpn-negative cases. Conclusions:Cpn0423 induced inflammatory response in host cells through NOD2 pathway, which was closely related to the chronic inflammatory injury caused by Cpn.
