1.Salvianolic acid C alleviates cuproptosis and inflammatory injury in cardiomyocytes after myocardial infarction via activating Nrf2/HO-1 signaling pathway

OBJECTIVE To investigate the regulatory effects of salvianolic acid C(SAC)on the level of cuproptosis and inflammatory injury in cardiomyocytes after myocardial infarction(MI).METHODS①C57BL/6 mice were divided into a sham group,an MI model group,and SAC(5,10 and 20 mg·kg-1)groups,with 10 mice in each group.Mice in the SAC groups were pretreated with oral gavage of SAC for 1 week,while those in the sham and model groups received an equal volume of saline.One week later,an MI model was established in the model and SAC groups by ligating the left anterior descending coronary artery,while the sham group underwent thoracotomy without ligation.MI size was assessed using triphenyltetrazolium chloride(TTC)staining.Cardiomyocyte apoptosis was evaluated by TUNEL staining.The ultrastructure of cardiomyocyte mitochondria was observed under a transmission electron microscope.② Mouse cardiomyocytes HL-1 were divided into a control group,an oxygen-glucose deprivation(OGD)model group,OGD+SAC 1,5 and 10 μmol·L-1 groups,and a OGD+SAC(5 μmol·L-1)+nuclear factor erythroid 2-related factor 2(Nrf2)inhibitor ML385(2 μmol·L-1)group.Cells in the OGD+SAC groups were pretreated with SAC for 24 h while those in the OGD+SAC+ML385 group were pretreated with both SAC 5 μmol·L-1 and ML385 2 μmol·L-1 for 24 h.Except for the control group,an OGD model was established in HL-1 cells.ELISA was used to detect the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and IL-1β in mouse serum and HL-1 cell culture supernatants.The Cu+detection kit was used to measure Cu+levels in myocardial tissue and HL-1 cells.Cell viability was assessed using the CCK-8 kit.Apoptosis rates of HL-1 cells were detected by flow cytometry.Reactive oxygen species(ROS)levels in HL-1 cells were measured using a ROS detection kit.Western blotting analysis was performed to detect the expression levels of Nrf2,heme oxygenase-1(HO-1),and cupro-ptosis markers,ferredoxin 1(FDX1)and solute carrier family 31 member 1(SLC31A1)in myocardial tissue and HL-1 cells.RESLUTS ① Compared with the sham group,the MI model group exhibited increased myocardial infarction size,elevated cardiomyocyte apoptosis rates,mitochondrial swelling,vacuolation,and cristae rupture in cardiomyocytes,increased serum levels of TNF-α,IL-6,and IL-1β,elevated Cu+levels and expressions of FDX1 and SLC31A1 in myocardial tissue,and decreased expressions of Nrf2 and HO-1(P<0.01).Compared with the model group,the SAC 5,10 and 20 mg·kg-1 groups showed reduced MI size,decreased cardiomyocyte apoptosis rates,alleviated mitochondrial swelling,vacuola-tion,and cristae rupture,lower serum levels of TNF-α,IL-6 and IL-1β,decreased Cu+levels and expres-sions of FDX1 and SLC31A1 in myocardial tissue,and increased expressions of Nrf2 and HO-1(P<0.05,P<0.01).② Compared with the cell control group,the OGD model group demonstrated signifi-cantly decreased HL-1 cell viability,increased cell apoptosis rates,Cu+and ROS levels,expressions of FDX1 and SLC31A1,elevated levels of TNF-α,IL-6 and IL-1β in cell culture supernatants,and decreased expressions of Nrf2 and HO-1(P<0.01).Compared with the OGD model group,the SAC 1,5 and 10 μmol·L-1 groups showed increased HL-1 cell viability,decreased cell apoptosis rates,Cu+and ROS levels,expressions of FDX1 and SLC31A1,reduced levels of TNF-α,IL-6 and IL-1β in cell culture supernatants,and increased expressions of Nrf2 and HO-1(P<0.05,P<0.01).Compared with the SAC 5 μmol·L-1 group,the SAC 5 μmol·L-1+ML385 2 μmol·L-1 group exhibited decreased cell viability,increased cell apoptosis rates,Cu+and ROS levels,expressions of FDX1 and SLC31A1,elevated levels of TNF-α,IL-6,and IL-1β in cell culture supernatants,and decreased expressions of Nrf2 and HO-1(P<0.01).CONSLUSION SAC can activate the Nrf2/HO-1 signaling pathway,alleviate cuproptosis in cardiomyocytes after MI,and reduces inflammatory damage.

3.Clinical characteristics of family clustering pediatric and adult cases with severe acute respiratory syndrome coronavirus 2 Omicron variant infection

Objective:To investigate the clinical characteristics of family clustering pediatric and adult cases with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron variant infection in Shanghai City.Methods:A field investigation among the pediatric cases with Omicron variant infection and their household contacts from April 4 to April 30, 2022 in Children′s Hospital of Fudan University was conducted. The informations on case finding, clinical manifestations and SARS-CoV-2 vaccination status were collected. The epidemiological and clinical characteristics were compared between pediatric cases and adult cases. The independent sample t test or chi-square test was used for statistical analysis, and the relative risk ( RR) and 95% confidence interval (95% CI) were used to evaluate the protective effect of vaccination on the infection of Omicron variant. Results:There were 1 274 family members in 297 families including 370 children and 904 adults of whom 1 110(87.13%) were infected with Omicron variant, with 989(89.10%) symptomatic and 121(10.90%) asymptomatic. There were 355 children infected with Omicron variant, of whom 337(94.93%) were symptomatic, and the main manifestations were fever (96.74%(326/337)) and cough (40.36%(136/337)). Only one pediatric case with Rett syndrome developed critically severe pneumonia. A total of 194 pediatric cases had imaging examination, 64(32.99%) showed pulmonary inflammatory lesions. There were 755 adult cases infected with Omicron variant, of whom 652(86.26%) reported symptoms, and the main manifestations were fever (73.16%(477/652)) and cough (49.85%(325/652)). Among symptomatic cases, fever was more common in pediatric cases than in adult cases, while cough was more common in adult cases than in pediatric cases, and the differences were both statistically significant ( χ2=80.87 and 8.04, respectively, both P<0.01). The fever spike was higher in pediatric cases than in adult cases ((39.3±0.7) ℃ vs (38.6±0.6) ℃), and the difference was statistically significant ( t=9.85, P<0.001). The interval from the onset of symptoms to cycle threshold (Ct) value of the nucleic acid of Omicron variant≥35 was longer in pediatric cases than in adult cases ((13.0±3.1) d vs (10.9±3.6) d), and the difference had statistically significance ( t=2.97, P=0.004). Among 160 children aged 3 to 18 years, 54 (33.75%) received two-dose vaccination. Among the 904 adults, 388 (42.92%) received two-dose vaccination and 293 (32.41%) received a booster dose. In the adult cases, the risk of symptomatic infection was reduced by only 8% ( RR=0.92, 95% CI 0.86 to 0.98, P=0.014) following two-dose vaccination, and the risks of fever and cough following booster vaccination were reduced by 42%( RR=0.58, 95% CI 0.49 to 0.67, P=0.001) and 50% ( RR=0.50, 95% CI 0.34 to 0.78, P=0.001), respectively. Conclusions:Secondary attack rate and symptomatic rate of household infection are high in the context of the Omicron variant outbreak in Shanghai. Symptomatic infection is common in children and adults in household setting. Fever is the most common symptom and fever duration is short. Booster vaccination may provide certain protection against common symptoms caused by Omicron variant infection.
4.Internal fixation of lateral and medial borders for displaced scapular body fractures via minimally invasive approach:results of 23 cases

OBJECTIVE: To evaluate the efficacy of internal fixation of lateral and medial borders for displaced scapular body fractures via the minimally invasive approach. METHODS: The internal fixation of lateral and medial borders via minimally invasive approach was applied in surgical treatment of 23 patients with scapular body comminuted fractures from January 2014 to June 2018. The lateral approach was made straightly orienting over the lateral border of scapula. The dissection was taken down to the deltoid fascia. The deltoid was retracted cephalically, revealing the external rotators. Blunt dissection was used down to the lateral border between infraspinatus and teres minor, exposing the fracture site. The medial incision was done along the medial border of the scapula over site of the fracture. Dissections were taken down to the fascia and the periosteum. A subperiosteal dissection was then performed to elevate the infraspinatus to the degree necessary to visualize the fracture. The medial and lateral borders of scapula body were fixed with plates and screws in a frame-like way. RESULTS One patient developed the delayed healing of the incisions due to liquefactive fat necrosis. The other 22 patients showed no complications of the incisions. The glenopolar angle (GPA) of fractured scapula was increased from preoperative (25±12) degrees to postoperative (41±5) degrees (P<0.01). The healing time of fractures healed was 3-8 months, with an average time of (4.4±1.3) months. CONCLUSIONS The lateral-medial combined fixation through minimally invasive surgical approach for the scapula body fractures allows visualization of fracture reduction without extensive muscular or subcutaneous flaps, and is associated with successful fracture healing and high functional scores of the shoulder.
5.Effect and elimination methods of neonatal hyperbilirubinemia on the chemiluminescent detection of HBsAg

Objective Investigate the effect of neonatal hyperbilirubinemia on the detection of HBsAg by chemiluminescence and its elimination methods.Methods Case control study.The HBsAg in human serum was detected in 200 cases of hyperbilirubinemia neonates who were hospitalized in Beijing Children's Hospital,Capital Medical University from July 2015 to May 2016 and whose serum total bilirubin level exceeded 200 μmol/L.The positive serum was further detected by 16 200×g high-spoed centrifugation or blue light irradiation for 8 hours,and the results of re-assay of HBsAg were recorded.The retest positive serum wastested for HBV DNA load and checked the results of their mother's examination in HBV.136 adult serum samples with total bilirubin levels exceeding 200 μmol/L in the Peking University First Hospital,were taken as reference to compare the influence of hyperbilirubinemia between adults and newborns on the determination of HBsAg.Results The median level of serum total bilirubin in neonates was 259.0 μ mol/L (226.5,312.5);median level of indirect bilirubin 244.1 μmol / L(212.5,295.8).Median level of serum total bilirubin in adults 356.4 μmol/L(295.9,435.1);median level ofindirect bilirubin 137.1 μmol/L (107.8,172.7).The HBsAg test was negative in adults,11 cases (5.5%) were positive in newborns,their" HBV DNA load was less than<100 IU/ml.Among them,9 have inoculated hepatitis B vaccine and 2 were unknown.10 of 11 mothers of infants were healthy and 1 was positive for HBsAg,HBeAb,HBcAb.2 of the 11 positive specimens turned negative of HBsAg after high-speed centrifugation.In addition to high speed centrifugation,4 cases turned negative after blue light irradiation.5 cases remained positive after high speed centrifugation and blue light irradiation.Conclusions Neonatal hyperbilirubinemia,which is different from that of adults,is mainly caused by indirectly bilirubin increased,which is one of the main reasons for false positive detection of HBsAg by chemiluminescence in neonates.High-speed centrifugation and blue light irradiation can eliminate the influence of serum indirect bilirubin on the detection of HBsAg to the greatest extent.

Regulating phase Ⅰ and Ⅱ enzyme,drug transporters and constitutive androstane receptor (CAR) play an important role in dug metabolism and keeping balance on the levels of glucose,lipids and hormone in serum.As a hub of material metabolism within body,CAR can affect the efficacy of various drugs and be involved in many common metabolic diseases.Studying its polymorphism is referential for explaining the individual difference genetically and predicting the occurrence and development of diseases.This review focuses on the correlation between the CAR polymorphism and metabolic,which provides evidence for predicting the development of disease and choosing clinical drug dosage.
7.Early diagnosis of Parkinson's disease by PET/CT apoptosis imaging with tracer of 18F-ML-10

Objective The 18F-ML-10 PET/CT apoptosis imaging was taken in the early Parkinson's disease (PD) rat models.The feasibility of diagnosis of PD with 18F-ML-10 PET/CT apoptosis imaging is explored.Methods Twenty adult healthy male Sprague-Dawley rats were randomly divided into control group and PD model group (n=l 0).Intrastriat administration of 6-hydroxy dopamine (6-OHDA) was performed to induce progressive and retrograde degenerative changes in the substantial nigra of neurons (PD models).One week after apomorphine inducement,the rotational behaviors of the rats were evaluated.~C-CFT PET/CT imaging and 18F-ML-10 PET/CT were performed to observe the dopamine transport protein expression in the corpus striatum and apoptosis of dopaminergic neurons in the substantia nigra.Immunofluorescence staining of anti-tyrosine hydroxylase (TH) antibody was performed to evaluate the survival of dopaminergic cells in the compact part of substantia nigra.TUNEL was performed to evaluate the apoptosis of dopaminergic cells in the compact part of substantia nigra.Nissl staining was performed to detect the cellular morphology.Results One week after PD modeling,the rats in the experimental group obviously rotated to the contralateral,and the average rotation speed was (4.52 ±1.03) r/min.The ratio of 11C-CFT between the right and left striatum of rats in the experimental group was 0.556±0.017,which was significantly lower than that of the control group (0.998±0.013,P<0.05).The radioactivity ratio of'8F-ML-10 between the right and left substantia nigra of rats was 1.722±0.083,which was significantly higher than that of the control group (1.024±0.056,P<0.05).Immunofluorescence showed that the ratio of TH-positive neurons between the right and left compact part of substantia nigra in rats of the experimental group was 0.528 ±0.012,which was significantly lower than that of the control group (1.036±0.030,P<0.05).TUNEL showed that the number of dopaminergic neuronal apoptosis in the experimental group was 43.200±2.507,which was significantly larger than that of the control group (1.400±0.427,P<0.05).Conclusion PD is associated with apoptosis of dopaminergic neurons;18F-ML-10 PET/CT imaging can be used to diagnose PD in the early-stage.
8.Expressions of Toll-like receptor 2, Toll-like receptor 4 and myeloid differentiation factor 88 in skin lesions of patients with sporotrichosis

Objective To evaluate the roles of Toll-like receptor 2 (TLR2),TLR4 and myeloid differentiation factor 88 (MyD88) in immune recognition and immune mediation in sporotrichosis by detecting their expressions in skin lesions of sporotrichosis.Methods Biopsy specimens were obtained from the skin lesions of 19 patients with sporotrichosis and normal skin of 12 healthy human controls.Immunohistochemistry was performed to observe the expressions of TLR4 and MyD88,and real-time fluorescence-based quantitative PCR to quantify the expressions of TLR2 and MyD88 mRNAs.Data were expressed as mean ± standard error.Statistical analysis was done with the SPSS17.0 software.Independent samples t-test was conducted to compare the parameters between the lesional and control specimens.A p-value of less than 0.05 was considered to be statistically significant.Results TLR4 and MyD88 were mainly observed in the whole epidermis except the stratum corneum as well as plasma cells and lymphocytes in the dermis and subcutaneous tissue in lesional skin.However,TLR4 was nearly absent and MyD88 was weakly expressed in the dermis and subcutaneous tissue in normal skin.The expression levels of TLR4 and MyD88 were significantly higher in the lesional skin than in the control skin (TLR4,63.767 ± 3.829 vs.5.167 ± 3.246,t =4.82,P < 0.05; MyD88,57.236 ± 4.744 vs.10.588 ± 1.640,t =3.30,P < 0.05).Similarly,the lesional skin showed significantly stronger expressions of TLR2 and MyD88 mRNAs compared with the normal skin (TLR2,1.974 ± 1.452 vs.1.430 ± 1.073,P < 0.05; MyD88,2.028 ± 2.061 vs.0.688 ± 0.422,P < 0.05).Conclusion Sporothrix may induce the development of sporotrichosis by interacting with host immune system via TLR signaling pathways.
9.Improved Quality Standard for Kangshiming Mixtures

Objective:To improve the quality standard for Kangshiming mixtures. Methods:The identification of Angelicae Sinen-sis Radix, Astragali Radix and Phellodendri Chinensis Cortex was carried out by TLC. The contents of puerarin and paeoniflorin in the preparations were determined by HPLC. Results:The spots displayed in TLC were clear without interference from the negative control. The linear range for puerarin was 1. 64-49. 20μg·ml-1(r=0. 999 9) and 4. 22-63. 30μg·ml-1(r=0. 999 6) for paeoniflorin. The average recovery was 99. 63%(RSD=2. 14%,n=9) and 99. 05%(RSD=2. 70%,n=9) for puerarin and paeoniflorin, respectively. Conclusion:The method is accurate, reliable and specific, and can be used in the quality control of Kangshiming mixtures.
10.Incidence and risk factors of cognitive impairment 3 months after first-ever stroke: a cross-sectional study of 5 geographic areas of China.

This study examined the incidence, neuropsychological characteristics and risk factors of cognitive impairment 3 months after stroke in China. Five regions that differed in geography and economy in China were selected. Patients from the hospitals located in the five regions were prescreened at admission, and the demographic data, vascular risk factors and clinical characteristics of stroke were obtained. A battery of cognitive-specific domain tests was performed in the patients who failed to pass cognitive screening 3 months post stroke. Patients were diagnosed as having post-stroke cognitive impairment (PSCI) or no cognitive impairment (NCI) based on the results of the neuropsychological tests. Univariate analysis was performed for suspect risk factors, and significant variables were entered in multivariable logistic regression analysis. Our results showed that a total of 633 patients were recruited 3 months after stroke; complete cognitive tests were performed in 577 of the stroke patients. The incidence of PSCI in these Chinese patients was 30.7%. There were 129 (22.4%) patients with visuospatial impairment, 67 (11.6%) with executive impairment, 60 (10.4%) with memory impairment and 18 (3.1%) with attention impairment. The risk factors associated with PSCI were older age (odds ratio [OR] 1.76, 95% confidence interval [CI] 1.20-2.58), low education level (OR 2.45, 95% CI 1.65-3.64), depressive symptom (OR 1.69, 95% CI 1.09-2.61), obesity (OR 2.57, 95% CI 1.41-4.71), stroke severity 3 months post stroke (OR 1.62, 95%CI 1.10-2.37) and cortex lesion (OR 1.55, 95% CI 1.04-2.31). It was concluded that PSCI occurs commonly 3 months after first-ever stroke in Chinese patients. Visuospatial ability may be the most frequently impaired cognitive domain for the patients with stroke. The critical risk factors of PSCI are older age, low education level, depressive symptom, obesity, stroke severity 3 months post stroke and cortex lesion.
