1.Disease burden of chronic kidney disease attributable to high BMI in China and trend prediction in 1992-2021

Objective To analyze the disease burden of chronic kidney diseases (CKD) attributed to high body mass index (BMI) in China from 1992 to 2021 and predict the disease burden for the next decade, and to provide evidence for the prevention and treatment of CKD. Methods Using the Global Burden of Disease (GBD) database and the Joinpoint model, the average annual percentage rate change (AAPC) of the mortality rate and disability-adjusted life year (DALY) rate was calculated to describe and analyze the CKD disease burden attributed to high BMI in China from 1992 to 2021. The ARIMA model was employed to predict and analyze the change trend of the CKD disease burden. Results From 1992 to 2021, the mortality rate and DALY rate attributed to high BMI-induced chronic kidney disease showed an upward trend. Compared to 1992, the attributed number of deaths increased by 324.38%, and DALYs increased by 268.56%; the mortality rate increased by 64.00%, and the DALY rate grew by 51.62%. From 1992 to 2021, the mortality rate and DALY rate for males were lower than those for females, but the growth rate for males exceeded that of females. From 1992 to 2021, the mortality rate and DALY rate of chronic kidney disease attributed to high BMI in China increased with age. The average annual change rate of chronic kidney disease attributed to high BMI in China from 1992 to 2021 (mortality rate: 1.40 per 100,000 (95% CI: 1.04–1.76), DALY rate: 1.43 per 100 000 (95% CI: 1.17–1.70)) was higher than thHuaiyin Normal University, Huai'anher social demographic index (SDI) regions. The ARIMA model predicted that the age-standardized mortality rate increased from 2.91 per 100 000 in 2022 to 3.05 per 100 000 in 2026, and the age-standardized DALY rate increased from 69.65 per 100 000 in 2022 to 73.58 per 100 000 in 2026. Conclusion Chronic kidney disease attributed to high BMI in China is on the rise, and it will continue to grow in the future. The focus of CKD prevention and control should be on males and the elderly, while active measures should be taken to reduce the occurrence and progression of chronic kidney disease.
2.Early assessment of responsive neurostimulation for drug-resistant epilepsy in China: A multicenter, self-controlled study.

BACKGROUND:
To evaluate the efficacy and safety of the first cohort of people in China treated with a responsive neurostimulation system (Epilcure TM , GenLight MedTech, Hangzhou, China) for focal drug-resistant epilepsy in this study.
METHODS:
This multicenter, before-and-after self-controlled study was conducted across 8 centers from March 2022 to June 2023, involving patients with drug-resistant epilepsy who were undergoing responsive neurostimulation (RNS). The study was based on an ongoing multi-center, single-blind, randomized controlled study. Efficacy was assessed through metrics including median seizure count, seizure frequency reduction (SFR), and response rate. Multivariable linear regression analysis was conducted to explore the relationships of basic clinical factors and intracranial electrophysiological characteristics with SFR. The postoperative quality of life, cognitive function, depression, and anxiety were evaluated as well.
RESULTS:
The follow-up period for the 19 participants was 10.7 ± 3.4 months. Seizure counts decreased significantly 6 months after device activation, with median SFR of 48% at the 6th month (M6) and 58% at M12 ( P <0.05). The average response rate after 13 months of treatment was 42%, with 21% ( n = 4) of the participants achieving seizure freedom. Patients who have previously undergone resective surgery appear to achieve better therapeutic outcomes at M11, M12 and M13 ( β <0, P <0.05). No statistically significant differences were observed in patients' scores of quality of life, cognition, depression and anxiety following stimulation when compared to baseline measurements. No serious adverse events related to the devices were observed.
CONCLUSIONS:
The preliminary findings suggest that Epilcure TM exhibits promising therapeutic potential in reducing the frequency of epileptic seizures. However, to further validate its efficacy, larger-scale randomized controlled trials are required.

3.Functional chimeric perforator flap of medial femoral condyle for osteochondral and soft tissue reconstruction in hand and foot joints.

OBJECTIVE:
To evaluate the effectiveness of free medial femoral condyle (MFC) functional chimeric perforator flap (FCPF) transplantation in reconstructing joint function by repairing concomitant osteochondral defects and soft tissue defect in hand and foot joints.
METHODS:
A retrospective analysis was performed on 6 patients (5 males, 1 female; mean age of 33.4 years, range 21-56 years) with traumatic osteochondral joint defects and associated tendon, nerve, and soft tissue defects treated between January 2019 and November 2024. Defect locations included metacarpal heads (n=2), metacarpophalangeal joint (n=1), first metatarsal head (n=1), base of first proximal phalanx (n=1), and talar head (n=1), with soft tissue defects in all cases. Osteochondral defect sizes ranged from 1.5 cm×1.2 cm×0.7 cm to 4.0 cm×0.6 cm×0.6 cm, and skin defects ranged from 4 cm×3 cm to 13 cm×4 cm. The stage Ⅰ treatment included debridement, antibiotic-loaded bone cement filling of bone-cartilage defects, fracture internal fixation, and coverage with vacuum sealing drainage. Stage Ⅱ involved harvesting a free MFC- FCPF included an osteochondral flap (range of 1.5 cm×1.2 cm×0.7 cm to 4.0 cm×0.6 cm×0.6 cm), gracilis and/or semitendinosus tendon grafts (length of 4-13 cm), saphenous nerve graft (length of 3.5-4.0 cm), and a perforator skin flap (range of 6 cm×4 cm to 14 cm×6 cm), each with independent vascular supply. The flap was transplanted to reconstruct joint function. Donor sites were closed primarily or with skin grafting. Flap survival was monitored postoperatively. Radiographic assessment was used to evaluate bone/joint healing. At last follow-up, the joint function recovery was assessed.
RESULTS:
All 6 MFC-FCPF survived completely, with primary healing of wounds and donor sites. All patients were followed up 6-44 months (mean, 23.5 months). The flaps at metacarpophalangeal joint in 1 case and at ankle joint in 1 case were treated with degreasing repair because of their bulky appearance, while the other flaps had good appearance and texture. At 3 months after operation, the visual analogue scale (VAS) score for pain during joint movement of recipient site was 0-2, with an average of 0.7; at last follow-up, the VAS score of the donor site was 0-1, with an average of 0.3. According to the Paley fracture healing scoring system, the osteochondral healing of all the 6 patients was excellent. The range of motion of the metacarpophalangeal joint in 3 cases was 75%, 90%, and 100% of contralateral side respectively, the range of motion of the metatarsophalangeal joint in 2 cases was 65% and 95% of contralateral side respectively, and the range of motion of the ankle joint in 1 case was 90% of contralateral side. The hand function was evaluated as excellent in 2 cases and good in 1 case according to the upper limb function evaluation standard of the Chinese Medical Association Hand Surgery Society, and the foot function was evaluated as excellent in 2 cases and fair in 1 case according to the Maryland foot function score of 93, 91, and 69, respectively. The International Knee Documentation Committee (IKDC) score of 6 knees was 91-99, with an average of 95.2.
CONCLUSION
The free MFC-FCPF enables precise anatomical joint reconstruction with three-dimensional restoration of tendon, nerve, capsule, and soft tissue defects, effectively restoring joint function and improving quality of life.

4.Nose-to-brain delivery of targeted lipid nanoparticles as two-pronged β-amyloid nanoscavenger for Alzheimer's disease therapy.

Alzheimer's disease (AD), characterized by β-amyloid (Aβ) aggregation and neuroinflammation, remains a formidable clinical challenge. Herein, we present an innovative nose-to-brain delivery platform utilizing lactoferrin (Lf)-functionalized lipid nanoparticles (LNPs) co-encapsulating α-mangostin (α-M) and β-site APP cleaving enzyme 1 (BACE1) siRNA (siB). This dual-modal therapeutic system synergistically combines the neuroprotective and microglia-reprogramming capabilities of α-M with the transcriptional silencing of BACE1 via siB, thereby simultaneously inhibiting Aβ production and enhancing its clearance. Fabricated via a microfluidic approach, the LNPs exhibited uniform particle size distribution, great encapsulation efficiency, and robust colloidal stability. Upon intranasal administration, Lf-functionalization enabled superior brain-targeting efficacy through receptor-mediated transcytosis. In vitro studies demonstrated that α-M reversed Aβ-induced low-density lipoprotein receptor downregulation, promoting microglial phagocytosis and autophagic degradation of Aβ, while siB effectively suppressed BACE1 expression, abrogating Aβ synthesis. In vivo investigations in APP/PS1 transgenic mice revealed remarkable cognitive recovery, substantial Aβ plaque reduction, and alleviation of neuroinflammation and oxidative stress. This intricately designed LNP system, exploiting a non-invasive and efficient nose-to-brain delivery route, provides a biocompatible, synergistic, and transformative therapeutic strategy for the multifaceted management of AD.
5.Tougu Xiaotong Capsule promotes repair of osteoarthritis cartilage damage in mice by activating the CXCL12/GDF5 pathway.

OBJECTIVES:
To explore the mechanism by which Tougu Xiaotong Capsule (TXC) promotes chondrogenic differentiation and cartilage repair in mice with osteoarthritis (OA).
METHODS:
Fifty 8-week-old male C57BL mice were randomly divided into normal control group, cartilage damage (induced by subchondral ring-shaped drilling) model group and TXC treatment groups at low, moderate and high doses (184, 368 and 736 mg/kg, respectively). Saline (in normal control and model groups) and TXC were administered after modeling by daily gavage for 6 consecutive weeks. The changes of cartilage damage in the mice were assessed by measuring thermal withdrawal latency (TWL) and mechanical withdrawal threshold (MWT) and using micro-CT, modified safranine O and fast green staining, HE staining, and qPCR. Primary cultures of mouse synovial mesenchymal stem cells (SMSCs) with lentivirus vector transfection for interfering CXCL12, TXC treatment, or both for 24 h were examined for chondrogenic differentiation using immunofluorescence staining, scratch assay, immunocytochemistry, and Western blotting.
RESULTS:
In mouse models with cartilage damage, TXC treatment at the moderate dose significantly alleviated joint pain, promoted cartilage repair, and upregulated the mRNA expression levels of CXCL12, GDF5, collagen II, aggrecan, Comp and Sox9 in the cartilage tissue. In primary mouse SMSCs, CXCL12 knockdown resulted in significant reduction of GDF5 protein expression, migration ability and Sox9 protein expression, and these changes were obviously reversed by TXC treatment.
CONCLUSIONS
TXC promotes chondrogenic differentiation of mouse SMSCs to promote repair of cartilage damage in mice by activating the CXCL12/GDF5 pathway.

6.Clinical observation of esmolol for acute anterior ST-segment elevation myocardial infarction in patients undergoing PCI

OBJECTIVE To explore the effects of esmolol on cardiac function, inflammatory factors and serum microRNA (miR)-29a and miR-129-5p in patients with acute anterior ST-segment elevation myocardial infarction after percutaneous coronary intervention (PCI). METHODS A total of 120 patients with acute anterior ST-segment elevation myocardial infarction undergoing PCI in our hospital from April 2021 to June 2023 were selected and divided into control group (60 cases) and study group (60 cases) according to the random number table method. The control group was given conventional treatment, and the study group was additionally given Esmolol hydrochloride injection based on the control group for one week. The levels of cardiac function indexes (left ventricular ejection fraction, left ventricular end-systolic volume index, left ventricular end-diastolic diameter, peak ejection fraction, cardiac output), inflammatory factors (C-reactive protein, myeloperoxidase, interleukin-6, brain natriuretic peptide, homocysteine) myocardial enzyme indexes (creatine kinase isoenzyme, β2-microglobulin, cardiac troponin Ⅰ), and serum expression of miR-29a and miR-129-5p were observed in two groups, and the occurrence of ADR was recorded. RESULTS After one week of treatment, left ventricular ejection fraction, peak ejection fraction, cardiac output, and the expression of miR-129-5p in two groups was significantly higher than before treatment (P<0.05), while left ventricular end-systolic volume index, left ventricular end-diastolic diameter, inflammatory factors and myocardial enzyme index levels, and the expression of miR-29a were significantly lower than before treatment (P< 0.05). The study group was significantly better than the control group (except for the creatine kinase-MB) (P<0.05). There was no statistically significant difference in the incidence of symptomatic hypotension, symptomatic bradycardia, cardiogenic shock, and arrhythmia between two groups (P>0.05). CONCLUSIONS Esmolol can improve cardiac function, reduce inflammatory factors, lessen myocardial injury, and regulate serum expressions of miR-29a and miR-129-5p in patients with acute anterior ST- segment elevation myocardial infarction after PCI, with good safety.
7.Clinical first aid of acute laryngeal obstruction in adults and etiological analysis

Objective To summarize and analyze the clinical first aid characteristics and etiology classi-fication of adult acute laryngeal obstruction so as to guide the targeted treatment in clinic.Methods The clini-cal data of the patients with the main symptoms such as suffocation and inspiratory dyspnea visiting in this hospital from January 2015 to December 2021 were collected.They conducted emergency tracheotomy for first aid,and their etiological distribution and characteristics were analyzed.Results A total of 94 patients with a-cute laryngeal obstruction and emergency tracheotomy caused by various causes were collected,73 males and 21 females,aged (65.44±12.23)years old.The proportions of disease causes from high to low were in turn 60 cases of tumor (63.83%),12 cases of bilateral vocal cord paralysis (12.77%),11 cases of inflammation (11.70%),5 cases of trauma (5.32%),2 cases of laryngeal edema (2.13%),2 cases of giant foreign matter (2.13%) and 2 cases of tube taking off after tracheotomy (2.13%).The age had the statistical difference a-mong different etiologies (F=4.463,P=0.001).The age in the patients with tumor,infection and bilateral vocal cord paralysis was greater than that of the other groups with statistical difference[(67.21±10.45)years old vs.(52.00±16.42)years old,t=2.993,P=0.012].The male patients with different severities of acute la-ryngeal obstruction were more than female patients,but the difference in the sex composition was not statisti-cally significant (P=0.469).Among the patients with emergency tracheotomy,there were 6 cases of compli-cating incision bleeding and 5 cases of subcutaneous emphysema;1 case of right pneumothorax complicating apposite side atelectasis,and all were improved by symptomatic treatment.Conclusion Acute laryngeal ob-struction in adults is critical.The common causes include tumor,bilateral vocal cord paralysis and inflamma-tion.The risk of emergency tracheotomy and complications occurrence risk are relatively high.
8.Study on relationship between risk perception and nursing working environment with mental resilience in emergency department nurses

Objective To investigate the relationship between the risk perception and nursing working environment with the mental resilience in the emergency department nurses.Methods The convenience sam-pling method was adopted.A total of 577 emergency department nurses in 45 hospitals of Guangdong Province from October to November 2023 were selected as the respondents.The CD-RISC scale,nursing staff risk per-ception questionnaire and nursing work environment scale were used to conduct the survey.The multiple line-ar regression was adopted to analyze the influencing factors of mental resilience in the emergency department nurses.The Pearson correlation was adopted to analyze the correlation between the risk perception and nursing working environment with the mental resilience.Results The gender,age,specialist nurse or not,post,hospi-tal type and hospital level had the effect on the mental resilience in the emergency department nurses (P<0.05).The scores of mental resilience scale in 577 nurses were (86.07±15.95) points and the scoring rate was 68.86%;the scores of nursing staff risk perception questionnaire were (79.38±16.32) points and the scoring rate was 56.70%;the scores of the nursing work environment scale were (93.51±13.25) points and the scoring rate was 75.41%.The multiple linear regression analysis showed that the post was the influencing factor of mental resilience in the emergency department nurses.The results of Pearson correlation analysis showed that the nursing work environment was positively correlated with the mental resilience (r=0.476),and negatively correlated with the risk perception (r=-0.252).Conclusion The hospital nursing managers should pay attention to the influencing factors of mental resilience in the emergency department nurses and fo-cus on three major types of risks affecting physical function,time and occupational exposure of the emergency department nurses to improve the working environment of emergency nursing.
9.Clinical study of neonatal varicella under the single-dose varicella vaccine strategy

Objective:To understand the epidemiological and clinical characteristics, and treatment outcomes of neonatal varicella during the era of the single-dose varicella vaccine (VarV) strategy in China, and to provide relevant evidence for the prevention of neonatal varicella.Methods:The clinical data of neonatal varicella cases from 2010 to 2020 at the Children′s Hospital of Fudan University were retrospectively collected. The clinical data included epidemiological exposure, vaccination history of family members, complications, treatment, outcomes, hospital stay and hospital cost. Independent sample t test and Spearman rank correlation coefficient were used for linear correlation analysis. Results:A total of 125 neonatal varicella cases were treated. Among them, 90.4%(113/125) had a clear history of exposure to a family member with varicella or herpes zoster before onset, with 94 cases involving the mother. Of the 94 mothers, 35(37.2%) had received one dose of VarV during childhood, four siblings had also received one dose of VarV, and four grandparents and one nanny had not been vaccinated with either VarV or shingles vaccine. All neonatal varicella cases were treated with intravenous acyclovir upon admission. Neonatal varicella cases with complications had a longer hospital stay ((4.9±2.0) days) and incurred higher hospital cost ((5 739.9±3 868.2) yuan) compared to those without complications ((4.1±1.2) days and (4 047.0±1 556.0) yuan, respectively). These differences were both statistically significant ( t=2.18, 2.34, respectively; both P<0.05). All neonatal varicella cases survived. There was a positive correlation between the hospital cost and the days from disease onset to hospitalization ( r=0.183, P=0.041). The hospital cost in the group whose mothers had received one dose of VarV was lower compared to that in the group whose mother had not been vaccinated ((3 857.9±899.0) yuan vs (4 673.8±2 228.6) yuan), with the difference statistically significant ( t=2.46, P=0.016). Conclusions:To prevent neonatal varicella, it is recommended that children of appropriate age complete two doses of VarV schedule, susceptible parents of reproductive age receive two doses of VarV, and individuals aged 50 years and older receive shingles vaccine.
10.Application progress of speckle tracking imaging in the diagnosis and evaluation of congenital heart disease

The early diagnosis of congenital heart disease is very important. Ultrasound is widely used in the early diagnosis and evaluation of congenital heart disease because of its advantages of non-radiation, non-invasive property and portability.Speckle tracking imaging has the advantages of being free from angle, efficient and accurate, which makes up for some shortcomings of traditional echocardiography. It greatly improves the diagnosis accuracy of fetal congenital heart disease, especially congenital heart disease with outflow tract obstruction.Myocardial strain index and left ventricular torsion can more accurately evaluate myocardial function,help to judge prognosis, choose the time of operation and assist perioperative management. Blood speck tracking helps identify different blood flow patterns and may become an important diagnostic tool for the diagnosis and treatment of congenital heart disease.
