1.High expression of IGFBP1 in lung adenocarcinoma tissues promotes the proliferation and invasion of lung adenocarcinoma cells

Objective To investigate the association between butyrate metabolism-related genes(BMRGs)and the prognosis of lung adenocarcinoma(LUAD),construct a prognostic model,and evaluate its predictive value.Methods Differentially expressed BMRGs(BMR-DEGs)were screened from the TCGA-LUAD dataset.Cox and LASSO regression analyses were employed to establish a risk scoring model,stratifying patients into low-and high-risk groups.The model's performance was assessed using Kaplan-Meier(K-M)and receiver operating characteristic(ROC)curves,with external validation conducted using the GSE68465 dataset.In vitro experiments included RT-PCR and Western blotting to measure insulin-like growth factor binding protein 1(IGFBP1)expression.IGFBP1 was knocked down using siRNA,and its effects on cell proliferation,migration,and invasion were evaluated via CCK-8,colony formation,Transwell,and wound healing assays.Gene Set Enrichment Analysis(GSEA)was performed to explore IGFBP1-related regulatory mechanisms.Results We identified a total of 51 BMR-DEGs,primarily involved in biological processes such as cell proliferation,migration,and invasion,and associated with the PI3K-Akt and MAPK signaling pathways.The constructed 5-gene risk scoring model demonstrated that high-risk patients had significantly worse overall survival(OS)than low-risk patients(P<0.05).IGFBP1 was highly expressed in LUAD tissues and correlated with poor prognosis.Knockdown of IGFBP1 significantly suppressed cancer cell proliferation,migration,and invasion(P<0.05).GSEA revealed that IGFBP1 was linked to lipid metabolism-related pathways.Conclusion BMRGs are associated with LUAD prognosis,and the established risk scoring model exhibits strong predictive performance.High IGFBP1 expression correlates with unfavorable outcomes,and its knockdown inhibits cancer cell proliferation,migration,and invasion.

3.Clinical effect analysis of endoscopic thyroid surgery for thyroid cancer through different approaches

Objective:To compare the clinical effects of endoscopic thyroidectomy with different approaches on patients with thyroid cancer.Methods:A prospective study was conducted on 96 patients with thyroid cancer who underwent complete endoscopic thyroidectomy (CET) by the same experienced surgical team in the Head and Neck Surgery Department of Shanxi Cancer Hospital from Jan. 2020 to Jan. 2023. The patients were randomly divided into a trans-subclavian approach group of 48 cases and a transoral vestibular access group of 48 cases using a random number table method. The trans-subclavian approach group underwent CET via the subclavian approach, while the transoral vestibular access group underwent CET via the oral vestibular approach. The surgical and postoperative rehabilitation conditions, complications, and cosmetic satisfaction scores were compared between the two groups. The immune indicators (T lymphocyte subsets (CD3 +, CD4 +), CD4 +/CD8 +), serum soluble interleukin 2 receptor (SIL-2R), tissue kallikrein 11 (KLK11), midkine (MK) levels were measured before surgery. At 1 d, 3 d, and 7 d postoperatively, neck function was assessed using the visual analog scale (VAS), neck injury index, and dysphagia index. Results:The operation time of the transoral vestibular access group and the Trans-subclavian approach group were (117.58±10.87) min and (101.84±11.35) min, respectively ( P<0.001), and the hospitalization time was (4.31±0.86) d and (5.12±0.91) d, respectively ( P<0.001). The drainage volume was (65.13±12.49) mL and (78.65±15.32) mL, respectively ( P<0.001). At 1, 3, and 7 days after surgery, the CD3 + levels in the transoral vestibular access group and the trans-subclavian approach group were (41.53±3.86) % and (38.29±3.51) %, respectively ( P<0.001), (46.21±4.35) % and (42.81±4.06) %, respectively ( P=0.001), and (48.23±4.47) % and (45.10±4.23) %, respectively ( P<0.001). The CD4 + levels were (33.27±3.90) % and (30.18±3.45) %, respectively ( P<0.001), (36.28±4.15) % and (33.46±3.87) %, respectively ( P=0.001), and (38.69±4.22) % and (35.17±4.10) %, respectively ( P<0.001). The CD4 +/CD8 + levels were (1.31±0.22) and (1.16±0.21), respectively ( P=0.001), (1.40±0.23) and (1.20±0.22), respectively ( P<0.001), and (1.58±0.24) and (1.45±0.25), respectively ( P=0.011). There was no significant difference in the levels of serum SIL-2R, KLK11, MK, VAS scores, cervical injury index, or swallowing dysfunction index between the transoral vestibular access group and the trans-subclavian approach group on postoperative day 1, 3, and 7 ( P>0.05). The incidence of complications in the transoral vestibular access group and the trans-subclavian approach group was 8.33% (4/48) and 22.92% (11/48), respectively ( P=0.049), and the cosmetic satisfaction rate was 95.83% (46/48) and 81.25% (39/48), respectively ( P=0.025) . Conclusions:Both the transoral vestibular approach and the subclavian approach for CET treatment of thyroid cancer can cause damage to cervical function. The former can reduce immune function damage, help with early postoperative recovery, and improve safety and patient satisfaction with cosmetic appearance, but it can prolong the operation time.
4.Advances in the complications related to transcatheter closure of patent ductus arteriosus in children

Patent ductus arteriosus(PDA)is one of the most common congenital heart diseases in childhood.The prevalence of PDA is about 0.5% and PDA accounts for 5%-10% of congenital heart diseases. Continuous opening of the ductus arteriosus can affect children's growth and development. Meanwhile,PDA increases the possibility of pneumonia,infective endocarditis,neonatal necrotizing enterocolitis,and other diseases.Compared with traditional surgery,percutaneous interventional closure of PDA in children has a higher surgical success rate and shorter hospital stay. Clinical practice has proved that percutaneous interventional closure is the preferred method for children with PDA. However,transcatheter closure also has complications,including displacement,valvular regurgitation,hemolysis and thrombocytopenia. The review aims to illustrate the mechanism and treatment of the complications after PDA interventional closure,in order to increase the physician's awareness of those complications,reduce the incidence of the complications and improve the prognosis of the patients.
5.Inhibitory effects of magnesium citrate on oxidative stress in chronic renal failure

Objective To investigate the inhibitory effects of magnesium citrate(MgCit)on oxidative stress in chronic renal failure(CRF).Methods SD rats were divided into CRF model group,MgCit groups(375 and 750 mg/kg),normal control group,and MgCit control group(750 mg/kg).The morphology of mitochondria in thoracic artery vascular smooth muscle cells(VSMCs)was observed by transmission electron microscopy.The content of superoxide dismutase(SOD)and malonaldehyde(MDA)in rat aorta and plasma was detected by the kit.The VSMCs were divided into normal control group,CRF model group,and MgCit groups(1.5 and 3 mmol/L).The levels of superoxide anion(DHE)and apoptosis were quantitatively detected by flow cytometry.Results Compared with the control groups,the mitochondria were swollen and the cristae fractured or disappeared in the model group;MgCit intervention could reduce mitochondrial swelling,but not cristae fracture.In the model group,SOD level in aorta and plasma decreased(P<0.05)while MDA level increased(P<0.05).MgCit intervention could increase SOD in aorta and plasma,but decrease MDA level(P<0.05).In the CRF environment,the DHE content of VSMCs and apoptosis in CRF model group increased(P<0.05).MgCit intervention could decrease DHE content and inhibit apoptosis(P<0.05).Conclusion MgCit inhibits oxidative stress levels in vivo and in vitro in CRF.
6.Influence of aspartyl-tRNA synthetase 2 on the proliferation and migration of lung adenocarcinoma cells

Objective:Based on the analysis of The Cancer Genome Atlas (TCGA) database,the expression levels and clinical significance of aspartyl-tRNA synthetase 2 (DARS2) in lung adenocarcinoma (LUAD) tissues were assessed. This study also explored the influence of DARS2 on proliferation,migration,and invasion of LUAD cells. Methods:We analyzed the expression profiles of DARS2 mRNA in LUAD samples from both the TCGA and Gene Expression Omnibus (GEO) databases,evaluating the differences in DARS2 mRNA expression between LUAD and normal lung tissues,and its potential association with clinicopathological characteristics. Quantitative reverse transcription polymerase chain reaction (qRT-PCR) was used to determine the level of DARS2 mRNA,while Western blot verified the protein expression. Immunohis-tochemistry was used to determine the tissue location and to provide a comprehensive evaluation of DARS2 expression in LUAD tissues and cell lines. Using siRNA technology to transfer DARS2 specific siRNA and its negative control plasmid into A549 and H1299 cells. Cell prolifera-tion were measured by CCK-8,plate cloning was used to verify proliferation ability,while scratch and Transwell assays were used to detect migration and invasion abilities. The biological function of DARS2 in LUAD was examined through GO function and KEGG pathway enrich-ment analysis. A single-sample gene set enrichment analysis (ssGSEA) was used to explore the relationship between the expression level of DARS2 and tumor immune infiltration. Results:Analysis of TCGA and GEO databases showed that DARS2 mRNA was highly expressed in LU-AD tissues compared to adjacent tissues (all P<0.01). The expression level of DARS2 was significantly correlated with the pathological stage,T stage,N stage,M stage,sex,and smoking status of LUAD patients with LUAD (all P<0.05). High DARS2 expression predicted poor patient pro-gnosis (P<0.05). DARS2 was also highly expressed in LUAD and LUAD tissues (all P<0.05). DARS2 knockdown significantly reduced the prolif-eration,migration,and invasion abilities of A549 and H1299 cells (all P<0.05). DARS2-related genes were enriched in the cell cycle and Myc/Foxm1 pathways,which regulate key biological processes,such as cell proliferation. The expression level of DARS2 was significantly pos-itively correlated with the degree of Th2 cell infiltration into LUAD tissues. In contrast,it was negatively correlated with the infiltration of B,T,CD8+T,mast,and cytotoxic cells (all P<0.05). Conclusions:High DARS2 expression in LUAD tissues and cells indicates poor patient prognosis. Knocking down DARS2 expression inhibited the proliferation,migration,and invasion of LUAD cells. Therefore,DARS2 may be a therapeutic target and prognostic biomarker for LUAD.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
8.Research on the application of Traditional Chinese Medicine's five-element music therapy in patients with autoimmune liver disease of liver depression and spleen deficiency type

Objective To explore the effect of Traditional Chinese Medicine's five-element music therapy com-bined with individualized emotional counseling intervention in patients with autoimmune liver disease of liver de-pression and spleen deficiency type.Methods 74 patients with autoimmune liver disease of liver depression and spleen deficiency type who were admitted to the hepatology department of a tertiary hospital from January 2021 to February 2022 were selected as research subjects,and they were divided into an experimental group and a control group by a random number table with 37 cases in each group.The control group was given routine treatment and nursing care,as well as Traditional Chinese Medicine's emotional nursing.The experimental group received Tradi-tional Chinese Medicine's five-element music therapy combined with individualized emotional counseling intervention on the basis of routine treatment and nursing care,with an intervention period of 4 weeks.The Generalized Anxiety Disorder Scale(GAD-7),Patient Health Questionnaire Depression Scale(PHQ-9)and Pittsburgh Sleep Quality Index(PSQI)were used to evaluate patients within 24 hours after admission and after 4 weeks of intervention.Results A total of 72 patients completed the study,with 36 in the experimental group and 36 in the control group.Before and after intervention,the scores of GAD-7,PHQ-9,and PSQI in the experimental group were compared,and the dif-ferences were statistically significant(P<0.001).After the intervention,the GAD-7 score in the experimental group was(7.89±3.15),which was lower than(10.14±4.41)in the control group.The PHQ-9 score was(12.33±4.08)in the ex-perimental group,which was lower than(14.64±5.34)in the control group.The PSQI score was(12.39±3.06)points,which was lower than(14.22±3.37)in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of Traditional Chinese Medicine's five-element music therapy combined with individual-ized emotional guidance intervention can effectively improve the sleep quality and help alleviate negative emotions in patients with liver depression and spleen deficiency type of autoimmune liver disease.

The primary cause of injury and death in the elderly has been reflected in fall the elderly, so the application of reasonable and effective prevention strategies has great significance in reducing the risk of fall in the elderly. The research progress of virtual reality technology applied in preventing fall in the elderly at home and abroad over the years was systematically reviewed in this study. The mechanism of the technology in preventing fall in the elderly was mainly elaborated from five aspects of improving balance ability, gait disturbance, cognitive impairment, muscle strength and the fear psychology of falling. The purpose of this thesis is to broaden the research ideas of medical personnel on the prevention of fall of the elderly, provide more effective clinical practice plans, reduce the occurrence of fall, and guarantee the safety of the elderly.
