1.Longitudinal analysis of perioperative nutritional indicators and their correlation with prognosis in spontaneous ICH patients

Objective To longitudinally analyze the changes in perioperative nutritional indicators in elderly patients with spontaneous intracerebral hemorrhage(ICH)and their correlation with the score of modified Rankin scale(mRS)score.Methods A total of 295 elderly ICH patients ad-mitted to the neurosurgery intensive care unit of North China University of Science and Technolo-gy Affiliated Hospital from June 2022 to November 2024 were consecutively enrolled in this study.The levels of relevant nutritional indicators were measured before(T1),and in 1(T2),3(T3),5(T4)and 7 d(T5)after surgery.According to the mRS score in 90 d after onset of the disease,they were divided into a good prognosis group(mRS score≤ 2,114 cases)and a poor prognosis group(mRS score≥3,181 cases).The general data and levels of nutritional indicators at above time points were compared between the two groups.Binary logistic regression analysis was applied to study the relationship between the perioperative nutritional indicator levels and the prognosis in elderly patients with spontaneous ICH.Results The poor prognosis group had significantly higher SBP,larger proportions of stroke history,hemorrhage in the brainstem and midline dis-placement,and increased NIHSS score than the good prognosis group(P<0.05,P<0.01).Obvi-ously lower BMI(56.48±9.54 kg vs 66.62±8.12 kg),decreased serum levels of albumin[40.24(39.10,41.16)g/L vs 43.30(40.65,45.50)g/L],prealbumin[203.00(164.00,261.00)mg/L vs 229.00(180.00,282.00)mg/L]and TC[4.68(4.11,5.39)mmol/L vs 4.91(4.47,5.66)mmol/L],reduced lymphocyte count[1.58(0.95,2.03)× 109/L vs 1.77(1.99,1.91)× 109/L],and reduced GCS[7.00(5.00,10.00)vs 9.00(7.00,13.00)]and Barthel[55.00(43.00,64.00)vs 61.00(52.00,69.00)]scores at admission were observed in the poor prognosis group than the good prognosis group(all P<0.01).At all different time points,the poor prognosis group also got notably higher Controlling Nutritional Status(CONUT)scores(reaching the highest levels at 3 d after opera-tion),and lower Prognostic Nutritional Index(PNI)score and Geriatric Nutritional Risk Index(GNRI)score(both declining the lowest levels at same time)when compared with the poor prog-nosis group(all P<0.01).Binary logistic regression analysis showed that COUNT scores,PNI scores and GNRI scores before(T1)and at 1(T2),3(T3),5(T4)and 7 d(T5)after operation were independent risk factors for 90-day poor prognosis in elderly patients with spontaneous ICH.Conclusion The fluctuations of 3 nutritional indicators at different time points in the periopera-tive period are closely associated with the 90-day prognosis of elderly ICH patients.Monitoring the changes in the nutritional indicators can quickly and conveniently predict the prognosis of the pa-tients,which has high clinical application value.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
4.Analysis on the correlation between changes of waist circumference and blood lipids in military pilots and intervention suggestions

Objective:To provide basis for personalized health interventions for military pilots by analyzing the longitudinal variation trend of waist circumference and its correlation with BMI and lipid metabolism indicators among military pilots.Methods:A retrospective analysis was used on the base of collecting the physical examination data of 323 pilots who had been consecutively recuperated for 3 times and completed 3 health indicator examinations in Air Force Healthcare Center for Special Services Hangzhou from January of 2017 to January of 2022. The pilots were divided into waist circumference <85 cm group, ≥85-90 cm group, and ≥90 cm group. According to their waist circumference change trend, they were divided into increased group (difference of waist circumference >0 cm) and decreased group (difference of waist circumference ≤0 cm). According to the 5-year change rate of waist circumference (Δ5), they were divided into waist circumference relative increased group (Δ5>3%), relative stable group (-3%≤Δ5≤3%), and relative decreased group (Δ5<-3%). The stratification of different waist circumference, the 5-year change rate of waist circumference and BMI, blood lipid indexes and abnormal rate of hyperlipidemia were compared. Waist circumference was measured and determined according to the relevant standards for flying personnel and adults.Results:①The mean waist circumference of 323 pilots was close to the value of pre-central obesity. The mean BMI had reached the overweight standard, and the mean TG and TC were at high end of normal values.②During the 5-year period, the values of blood lipid metabolism indicators were repeatedly measured 3 times, and the mean values of TG, TC, and LDL-C showed an increasing trend, with significant differences ( F=7.39, 9.49, 3.67, P=0.001,<0.001, =0.026) and the mean values of TG and TC of the third measurement had approached to high limit. ③The results of BMI, TG in 3 measurements, LDL-C in the first and third measurements, TC in the first measurement and the incidence of hyperlipidemia were all increased with the increase of waist circumference and the differences were significant ( F=3.06-133.39, χ2=9.08-22.99, all P<0.05); HDL-C decreased with the increase of waist circumference, with significant differences ( F=6.32, 9.78, 10.86, P=0.002, <0.001, <0.001). The mean BMI of the ≥85-90 cm group reached the overweight level in the 3 measurements. The mean values of TG and TC in the first 2 measurements were at high end of normal value, but the mean values in the third measurement were higher than the normal range. The mean values of BMI, TG, and TC in ≥90 cm group were all within the abnormal range. ④ The 5-year change rate of BMI and LDL-C in the waist circumference increased group were higher than those in the waist circumference decreased group ( F=26.02, 14.95, both P<0.001). ⑤The waist circumference relative stable and relative increased group had a higher incidence of new hyperlipidemia, with 27 cases (21.77%) and 24 cases (20.00%) respectively. The waist circumference relative decreased group had the highest improvement rate of hyperlipidemia in 5 cases (6.33%), but the difference was not significant (all P>0.05). Conclusions:The abnormal rates of waist circumference, BMI, and blood lipids in military pilots are relatively high and show an increasing trend. The changes in waist circumference are closely related to BMI and blood lipids, and maintaining waist circumference within a normal range helps to maintain stability of blood lipid metabolism.
5.Advances in the complications related to transcatheter closure of patent ductus arteriosus in children

Patent ductus arteriosus(PDA)is one of the most common congenital heart diseases in childhood.The prevalence of PDA is about 0.5% and PDA accounts for 5%-10% of congenital heart diseases. Continuous opening of the ductus arteriosus can affect children's growth and development. Meanwhile,PDA increases the possibility of pneumonia,infective endocarditis,neonatal necrotizing enterocolitis,and other diseases.Compared with traditional surgery,percutaneous interventional closure of PDA in children has a higher surgical success rate and shorter hospital stay. Clinical practice has proved that percutaneous interventional closure is the preferred method for children with PDA. However,transcatheter closure also has complications,including displacement,valvular regurgitation,hemolysis and thrombocytopenia. The review aims to illustrate the mechanism and treatment of the complications after PDA interventional closure,in order to increase the physician's awareness of those complications,reduce the incidence of the complications and improve the prognosis of the patients.

7.Case report of PERCHING syndrome and literature review

The clinical data, laboratory testing, genetic testing results, diagnosis and treatment process of a child with PERCHING syndrome diagnosed and treated in the Department of Neonatology, the Third Affiliated Hospital of Zhengzhou University in June 2022 were retrospectively analyzed, and the relevant literatures were reviewed.The proband mainly presented with dyspnea and feeding difficulties after delivery, facial nevus flammeus, protrusion of eyes, small fissure of eyes, wide nasal root, limited opening of mouth, slightly high palatal arch, special posture, cryptorchid, hypospadias, and high muscle tone of limbs.Magnetic resonance imaging of the brain suggested possible agenesis of corpus callosum.Genetic testing showed complex heterozygous variations in the KLHL7 gene, and the two mutation sites have not been previously reported.A case of PERCHING syndrome caused by the KLHL7 gene mutation in China was reported for the first time, which provided new ideas for the diagnosis and treatment of children with PERCHING syndrome and reliable genetic evidence for family reproduction.
8.Clinical analysis of 10 cases of Bacillus cereus sepsis in premature infants

Objective:To study the clinical features, diagnosis and treatment of Bacillus cereus sepsis in premature infants.Methods:From February 2011 to February 2021, 10 cases of Bacillus cereus sepsis in premature infants admitted to the Department of Neonatology of our hospital were retrospectively analyzed.Results:The 10 cases of premature infants with Bacillus cereus sepsis included 5 males and 5 females, with gestational age 27 +2~35 +2 weeks, birth weight 940~2 430 g and the age of onset 7~35 days. At the beginning, all the infants showed lethargy and recurrent apnea as the onset symptoms. 8 cases had gray color skin, 7 cases had fever, 7 cases experienced septic shock, 5 cases had neonatal seizures and 4 cases showed abdominal distension. 7 cases were complicated with purulent meningitis and 3 cases with necrotizing enterocolitis. 9 cases had significantly decreased white blood cells and platelets and significantly increased C-reactive protein and procalcitonin at the onset. Among the 7 cases of purulent meningitis, 5 cases had multiple encephalomalacia. During follow-up of all the infants, 4 cases died, 3 cases cured and 3 cases survived with severe neurological sequelae. Conclusions:Bacillus cereus sepsis is a serious infectious disease for premature infants with acute onset, rapid progress and high mortality. Complication of purulent meningitis and serious neurological sequelae are common.
9.Effects and mechanisms of allogeneic epidermal stem cells on the survival of allogeneic full-thickness skin grafts in nude mice with full-thickness skin defect wounds

Objective:To investigate the effects and mechanisms of allogeneic epidermal stem cells (ESCs) on the survival of allogeneic full-thickness skin grafts in nude mice with full-thickness skin defect wounds.Methods:Experimental research methods were applied. Primary ESCs that appeared paving stone-like after being cultured for 7 d were obtained by enzymatic digestion method from one 4-week-old male BALB/c-NU nude mouse (the same strain, age, and sex below). The cells of third passage were identified by flow cytometry to positively express ESC marker CD44 and negatively express CD45, meanwhile, the positive expression of ESC markers of p63 and integrin 6α, and negative expression of CD71 were identified by immunofluorescence method. The ESCs of third passage in the logarithmic growth phase were used for the following experiments. Twenty-six nude mice were equally divided into phosphate buffered saline (PBS) group and ESCs group according to the random number table. A full-thickness skin defect wound was made on the back of each nude mouse, and then the wounds of the two groups were sprayed with equal volumes of PBS and ESCs, respectively. The wounds were transplanted with full-thickness skin grafts cut from the backs of 4 other nude mice. Each ten nude mice from the two groups were selected, the wound healing and skin survival on post surgery day (PSD) 0 (immediately), 3, 7, 14, and 21 were observed, and the survival ratio and shrinkage rate of skin grafts on PSD 3, 7, 14, and 21 were calculated (the number of sample was the number of surviving skin grafts at each time point); the blood perfusion in the skin grafts on PSD 3, 7, and 14 was detected by the laser speckle blood flow imager, and the blood flow ratio of nude mice skin grafts in ESCs group to PBS group at each time point was calculated (the number of sample was the pair number of surviving skin grafts in group pairing at each time point). The skin graft tissue of each 3 nude mice remained in the two groups were collected on PSD 7, and the mRNA expressions and protein expressions of tumor necrosis factor α (TNF-α), interleukin 8 (IL-8), IL-10, type Ⅰ collagen, type Ⅲ collagen, and matrix metalloproteinase 9 (MMP-9) in the tissue were detected by real-time fluorescent quantitative reverse transcription polymerase chain reaction and Western blotting, respectively. Data were statistically analyzed with Log-rank test, analysis of variance for repeated measurement, one-way analysis of variance, independent sample t test, and Bonferroni correction. Results:Taking the condition on PSD 0 as a reference, the wounds of nude mice in the two groups healed gradually on PSD 3, 7, 14, and 21, and the shrinkage of skin grafts was gradually obvious. Among them, the shrinkage healing of wound of nude mice in PBS group was more significant than that in ESCs group. On PSD 3, the skin graft of 1 nude mouse failed in ESCs group, while the skin graft of 3 nude mice failed in PBS group. On PSD 7, the skin graft of another nude mouse failed in PBS group. The survival ratio of skin grafts of nude mice in the two groups was similar on PSD 3, 7, 14, and 21 ( P>0.05). On PSD 3, 7, 14, and 21, the shrinkage rates of skin grafts of nude mice in ESCs group were (9.2±0.4)%, (19.7±1.2)%, (53.6±3.5)%, and (62.2±5.1)%, respectively, which was significantly lower than (11.0±0.9)%, (47.8±2.8)%, (86.1±7.1)%, and (89.7±9.0)% in PBS group ( t=5.719, 26.650, 11.940, 7.617, P<0.01). On PSD 3, 7, and 14, blood perfusion signals were observed in the skin grafts of nude mice in the two groups. The average blood perfusion ratios of the skin grafts of nude mice in ESCs group to PBS group were greater than 1, and there was no statistically significant difference in the overall comparison of 3 time points ( P>0.05). On PSD 7, compared with those of PBS group, the mRNA and protein expressions of TNF-α, IL-8, type Ⅰ collagen, and type Ⅲ collagen in the skin graft tissue of nude mice in ESCs group were significantly reduced, while the mRNA and protein expressions of IL-10 and MMP-9 in the skin graft tissue of nude mice in ESCs group were significantly increased (in mRNA comparison, t=2.823, 2.934, 2.845, 2.860, 3.877, 2.916, P<0.05). Conclusions:Allogeneic ESCs can reduce the shrinkage of allogeneic full-thickness skin grafts transplanted on full-thickness skin defect wounds in nude mice, promote the formation of new blood vessels between the skin graft and the wound, reduce inflammation and collagen protein expression, and promote the expression of MMP-9, thus improving the survival quality of skin grafts.
10.Investigation and analysis of the disease spectrum of military divers

Objective To analyze the disease spectrum and spectrum features of military divers, so as to provide solid evidence for the health management profile of divers and realize the goal of early prevention and health promotion, thus enhancing their fighting capacity. Methods The health data of 1864 military divers who had convalescence and health examination in the sanatorium from 2008 to 2016 were collected for the study, and SPSS 21. 0 software was used for statistical analysis of the obtained results. Results In the 1864 divers, 1594 man times were affected by various diseases, accounting for 85. 51%, and the same person who suffered from several diseases accounted a higher percentage. There were 1145 man times with 2 or more than 2 diseases, accounting for 71. 83%. Comparison of different service years revealed that those in the 9-12 service years group with 2 and 3 diseases had the highest percentage of morbidity, accounting for 24. 58% and 30. 82%respectively, and those in the≥17 service years group with 4 diseases had the highest percentage of morbidity, accounting for 28. 36%. Statistically significance could be noted, when comparisons were made between them (χ2 =101. 273, P=0. 000). In accordance with the examinations by different departments, the diseases could be classified into 12 categories. The total number of diseases in this population was 3156, of which the top 5 diseases were optical diseases, oral diseases, metabolic diseases, ENT diseases and surgical diseases. The number of cases was 895 (28. 36%), 845 (26. 77%), 568 (18. 00%), 231 (7. 32%) and 194 (6. 15%) respectively. Furthermore, comparison of different disease categories for those with different service years revealed that there was statistical significance, when comparisons were made between optical, metabolic, ENT, surgical and digestive diseases (χ2 =19. 874, χ2 =73. 005,χ2 =17. 147, χ2 =23. 210, χ2 =18. 697, P <0. 05). For the above-mentioned diseases, those divers in the 9-12 service years group had the highest morbidity, and the number of patients was respectively 245 (27. 37%), 152 (26. 76%), 52 (22. 51%), 49 (25. 26%) and 48 (27. 27%). When classified with different service years, the analysis of the top 10 disease spectrum showed that gingivitis and ametropia were prominent disorders, ranking the top two. The rates of such metabolic disease as fatty liver, hypertriglyceridemia and hyperuricemia increased with the increase of service years, and the rates of morbidity also increased with the progress of service years, with the order of disease spectrum being the top 5 in the years from 2014 to 2016. Lumbar disc herniation, allergic rhinitis and hyperbilirubinemia were the top 10 new diseases in the years from 2014 to 2016. Conclusion The health status of military divers was not optimistic, especially for those in the 9-12 service years group. In accordance with the disease spectrum, urgent measures should be taken for better health management of divers, so that their health level could be maintained, the capacity of sustained diving operations could be ensured, thus enabling them to handle those urgent and dangerous tasks at any time, and prolong ing their professional life.
