1.Comparative Study on the Quality of Robotic and Manual Dispensing in Intravenous Admixture Service

Objective To compare the quality of liquids dispensed by intelligent dispensing robot and traditional manual in PIVAS,to ensure the safety of clinical infusion.Methods Using omeprazole and ambroxol as the main drugs and 5%glucose injection and 0.9%sodium chloride solution as the solvent,a number of quality indexes including drug residues and the number of insoluble particles were evaluated in the two modes of manual and robotic dispensing,respectively.Results Under the robot dispensing mode and manual dispensing mode,the residual amount of drugs was less than 5%,the insoluble particles were far less than the limit,the pH value was in line with the human infusion range,and the endotoxin and aseptic culture were negative results,with no statistical significance(P>0.05).Conclusions The quality of liquid dispensed under the robotic dispensing mode is guaranteed,and there is no significant difference in the quality of liquid dispensed with the traditional manual dispensing mode,which provides a reliable guarantee for the large-scale application of robotic dispensing in intravenous admixture service.

3.Anti-fatigue activity of selenium nanoparticles functionalized by polysaccharides from Pleurotus tuber-regium sclerotium

Objective To investigate the anti-fatigue effect of PTR-SeNPs in vivo by measuring the muscle relative length of hindlimb,load-bearing swimming time as well as serum and liver indexes of mice.Methods 48 male C57/BL6 mice were randomly assigned into 4 groups with 12 mice in each group,including vehicle control group(control group),swimming training exercise group(EC group)with vehicle treatment,swimming training exercise with low dose of PTR-SeNPs group(LPTR-SeNPs)and high dose of PTR-SeNPs group(HPTR-SeNPs).The mice were intragastrically administrated with normal saline in both control group and EC group,as well as 2.5 and 10 μmol/(kg·bw)PTR-SeNPs in LHPTR-SeNPs group,respectively,once per day for consecutively 21 days.After swimming training exercise,the muscle structures in the hind limb of mice were examined by magnetic resonance imaging.Furthermore,the burdened swimming time was measured,the serum content of blood lactic acid(BLA),urea nitrogen(BUN),alanine aminotransferase(ALT),glutamic oxalate aminotransferase(AST)and lactate dehydrogenase(LDH),as well as the hepatic level of glycogen(HG),malondialdehyde(MDA)and activity of catalase(CAT)and superoxide dismutase(SOD)were determined.Results Compared with the control group,the serum contents of BLA,BUN,ALT,AST and LDH in EC group(P<0.05 or P<0.01)and hepatic CAT in HPTR-SeNPs group(P<0.01)were significantly increased.The muscle relative length of hind limbs and the burdened swimming time were extended by HPTR-SeNPs markedly(P<0.05).There was no significant difference in MDA level in LHPTR-SeNPs group.Compared with EC group,the burdened swimming time of mice was significantly prolonged(P<0.01),the contents of BLA and BUN were obviously decreased in the HPTR-SeNPs group(P<0.05 or P<0.01),the level of HG was significantly increased in the LHPTR-SeNPs groups(P<0.05 or P<0.01),the serum content of ALT,AST and LDH were markedly decreased in the HPTR-SeNPs group(P<0.05 or P<0.01).Hepatic SOD activity was remarkably increased in LPTR-SeNPs group(P<0.05),the level of CAT was evidently increased(P<0.01)and AST was decreased(P<0.05)in the HPTR-SeNPs group.Conclusion PTR-SeNPs could improve the liver physiological function,increase glycogen storage,reduce the accumulation of metabolites and enhance the body's antioxidant capacity to ameliorate fatigue significantly,which could present the potential to be developed into health care products or drugs.
4.Construction of an acute toxoplasma encephalitis mouse model by the stereotaxic surgery with RH toxoplasma trophozoites

【Objective】 To construct an acute toxoplasma encephalitis mouse model by observing the pathological changes in the hippocampus of mice infected with Toxoplasma gondii strain RH. 【Methods】 The quantitative RH Toxoplasma gondii (100, 500, and 1 000 trophozoites) were injected into the hippocampal CA1 region of mice by the stereotaxic surgery; the survival status of mice was observed. Giemsa staining was used to observe the changes of toxoplasma in mouse ascites and brain tissue homogenates. Nissl staining and HE staining were used to observe the pathological changes of hippocampal nerve tissue. The distribution of Toxoplasma gondii in brain tissue was observed by immunohistochemical ABC method. 【Results】 The RH Toxoplasma gondii infected mice showed obvious symptoms such as arched back, bristling hair, abdominal distension, subtle tremor and hemiplegia on the fourth day of infection. The survival of mice in 100 trophozoites group was longer, no trophozoites of Toxoplasma gondii were found in ascites, a few pseudocysts were found in brain tissue homogenates after infected for 96 hours, and more trophozoites were found after death. Nysl staining and HE staining showed more tissue necrosis foci and loss of nerve cells in CA1 area after infected 144 h. The injury aggravated with the prolongation of infection time. Toxoplasma trophozoites were found in ascites and brain homogenates of mice in 500 and 1000 trophozoites groups. Nissl staining revealed neuronal loss and massive necrosis in the hippocampus. HE staining showed necrosis and inflammatory cell infiltration. The brain tissue injury significantly aggravated compared with 100 trophozoites group. The distribution of Toxoplasma gondii in the necrotic foci was confirmed by immunohistochemistry. 【Conclusion】 The survival of 100 trophozoite mice infected with Toxoplasma gondii strain RH was longer, and the pathological changes of brain tissue gradually aggravated. The damage was relatively confined to the brain tissue, and the mice showed typical symptoms of toxoplasma encephalitis. Therefore, the mouse model of acute toxoplasma encephalitis can be constructed by localized infection of 100 toxoplasma trophozoites, which can lay a foundation for future research on the mechanism of toxoplasma injury to cranial nerves.
5.Efficacy of low-frequency neuromuscular electrical stimulation in the treatment of penile hypersensitive premature ejaculation

【Objective】 To investigate the efficacy of low-frequency neuromuscular electrical stimulation in the treatment of penile hypersensitive premature ejaculation. 【Methods】 A total of 66 patients treated during Nov.2021 and Aug.2022 were randomly divided into electrical stimulation group (n=22), local anesthesia group (n=21), and combined therapy group (n=23). The electrical stimulation group received low-frequency neuromuscular electrical stimulation, 5 times a week;the local anesthesia group used compound lidocaine cream 30 minutes before sexual intercourse;the combined therapy group received both treatments. After 3-month treatment, the latency of dorsal nerve somatosensory evoked potential (DNSEP), glans penis somatosensory evoked potential (GPSEP), intravaginal ejaculation latency time (IELT), premature ejaculation diagnostic tool score (PEDT), and spouse sexual satisfaction score were collected. 【Results】 After treatment, IELT, PEDT, spouse’s sexual life satisfaction score, DNSEP and GPSEP of the three groups were significantly improved (P<0.05). The combined therapy group outperformed the other two groups in all parameters (P<0.05). There were no statistically significant differences between the electrical stimulation group and local anesthesia group (P>0.05). 【Conclusion】 Low-frequency neuromuscular electrical stimulation is effective in the treatment of penile hypersensitive premature ejaculation, and the combination of local anesthetics is more effective, which is worthy of clinical application and promotion.
6.Investigation of time limit errors of preparation and delivery for cytotoxic antineoplastic drugs and exploration of improvement strategies

Objective:To explore a method to shorten the time for preparation and delivery of cytotoxic antineoplastic drugs in pharmacy intravenous admixture services (PIVAS), so that patients can use drugs in time according to the drug labels.Methods:The circumstances of prolonged preparation and delivery time of cytotoxic antineoplastic drugs prepared in PIVAS in Beijing Tiantan Hospital from October 8 to December 31, 2018 were investigated by the project team, the causes were analyzed, and the improvement strategies were made. The drug preparation and delivery time after the improvement strategies was counted and compared with that before improvement to evaluate the improvement effect.Results:From 8 October to 31 December 2018, a total of 4 156 bags of cytotoxic antineoplastic drugs were prepared and delivered in PIVAS in our hospital and the delivery time of 951 bags need to be paid attention to. Though the recording time was not accurate, it could be determined that the delivery time of at least 4.7% (45/951) of those bags had been extended. After taking the improvement measures of priority review for prescription of cytotoxic antineoplastic drugs, priority preparation, and delivery by special person from 20 January 2020, the delivery time of drugs was shortened from 2.5-4.5 hours to 0.5-4.0 hours, and the incidence of delivery time prolongation decreased to 0.9% (12/1 281).Conclusions:In hospital PIVAS, there was a phenomenon of delivery time prolongation of cytotoxic antineoplastic drugs when the drugs were delivered, which might lead to the storage time of the liquid exceeding the requirements of the drug labels. Priority prescription review and delivery of cytotoxic antineoplastic drugs could effectively shorten the delivery time.

The particle size and zeta potential of microemulsions were evaluated upon dilution with aqueous medium. In vitro release was detected by a dialysis method in reverse. The effects of SMEDDS at different dilutions and surfactants at different concentrations on the metabolism of MDZ were investigated in murine hepatocytes. The cytotoxicity of SMEDDS at different dilutions was measured by LDH release and MTT technique. The effects of SMEDDS on the CYP3A enzymes activity were determined by Western blotting. Our results showed that dilution had less effect on the particle size and zeta potential in the range from 1:25 to 1:500. The MDZ was completely released in 10 h. A significant decrease in the formation of 1'-OH-MDZ in rat hepatocytes was observed after treatment with both SMEDDS at dilutions ranging from 1:50 to 1:250 and Cremophor RH 40 or Tween 80 at concentrations ranging from 0.1% to 1% (w/v), with no cytotoxicity observed. A significant decrease in CYP3A protein expression was observed in cells by Western blotting in the presence of either Cremophor RH40 or Tween 80-based SMEDDS at the dilutions ranging from 1:50 to 1:250. This study suggested that the excipient inhibitor-based formulation is a potential protective platform for decreasing metabolism of sensitive drugs that are CYP3A substrates.

10.Improved left ventricular endocardial border echo resolution by perfluoropropane-albumin microsphere injection: a multiple center stage Ⅲ clinical study

0.05 ). After once injection both observers considered the number of clearly recognized endocardial border segments increased significantly. The number evaluated by observers A increased from 2.68 ? 0.95 to 5.99 ? 0.10 while from 2.82 ? 1.03 to 5.99 ? 0.11 by observers B( P 0.05 ). The average contrast enhancement rate of LV endocardial border was 99.7 %. Perfluoropropane-albumin microsphere injection had no significant effection on vital signs such as blood prssure, heart rate and respiration. Electrocardiogram didn′t change markedly and the variance of the laboratory findings like blood and urine routine examination, hepatic and renal function was in normal range. Only one case( 0.33 %) had slight side-effects who suffered from mild nausea and diarrhea, which suggested the clinical safety of this contrast agent. Conclusions Perfluoropropane-albumin microsphere injection could enhance the resolution of LV endocardial borders and make the judgement of regional myocardial movement easier. It has little side-effects and will be appropriate for clinical use.
