Hepatitis virus is the main pathogen causing liver inflammation and damage. Early detection is crucial for the effective treatment of hepatitis. The detection technology of hepatitis virus has gone through multiple stages, including immunological detection technology and nucleic acid detection technology. The emergence of emerging molecular detection technologies makes its detection more sensitive and convenient, including nanotechnology, Raman spectroscopy technology, microfluidic technology and biosensor technology. The development of these technologies has promoted the early diagnosis of hepatitis, but their clinic applications are still facing challenges. In the future, the development of hepatitis virus detection technology is expected to transform in the form of multidimensional and interdisciplinary innovation process, with its core objectives being the realization of more precise, convenient, and accessible detection methods, thereby comprehensively advancing the progress of hepatitis prevention and control efforts.

The clustered regularly interspaced short palindromic repeat-CRISPR-associated protein (CRISPR-Cas) system is renowned for its exceptional gene-editing capabilities. In recent years, the discovery of the trans-cleavage activity of Cas12 and Cas13 proteins has shown great potential in the field of molecular diagnostics. The trans-cleavage activity of the CRISPR-Cas system has been widely applied in point-of-care testing (POCT). This article will provide a detailed discussion on the principles, advantages, and challenges of combining the CRISPR-Cas system with lateral flow assays, colorimetric methods, microfluidic technologies, smartphone applications, and portable detection sets. The CRISPR-Cas system demonstrates versatility in the POCT field with its ability to detect nucleic acids, pathogenic microorganisms, and non-nucleic acid targets. As CRISPR-Cas-based POCT continues to advance, these developments provide strong support for the formulation of personalized medical and public health strategies, further promoting the realization of precision medicine.

5.Robot-assisted screw fixation for unstable pelvic ring fractures

Objective:To investigate the clinical efficacy of robot-assisted screw fixation for unstable pelvic ring multifocal fractures.Methods:A retrospective analysis was performed on 76 patients with unstable pelvic fractures treated with orthopaedic robot-assisted screw fixation in the Trauma Center of the Affiliated Central Hospital of Shandong First Medical University from January 2015 to June 2022, including 43 males and 33 females, aged 52.53±13.68 years (range, 16-87 years). There were 43 cases of falling injuries from high places, 22 cases of traffic accidents, 11 cases of crushing injuries and heavy objects. Fifty-five patients were employed before the injury, while 21 were not. Fractures were classified according to the Tile classification, with 72 cases classified as type B and 4 cases as type C. Robot-assisted fixation techniques included internal fixator (INFIX), anterior ring screws, sacroiliac screws, and LC-II screws. Intraoperative blood loss, fluoroscopy frequency, surgical time, and the success rate of initial guidewire placement were recorded. Fracture reduction quality was evaluated using the Matta criteria, and postoperative pelvic function recovery was assessed using the Majeed criteria.Results:A total of 150 surgical procedures were performed on 76 patients, including 34 cases of INFIX fixation, 48 cases of anterior ring screws, 61 cases of sacroiliac joint screws, and 7 cases of LC-II screws. The mean intraoperative fluoroscopy frequency was 46.63±17.50 times (range, 15-93 times). Intraoperative fluoroscopy frequency varied among different fixation techniques, with INFIX group at 16.44±4.32 times, LC-II group at 21.59±5.80 times, anterior ring screws group at 29.44±11.65 times, and sacroiliac screws group at 23.10±11.87 times. The intraoperative blood loss was 20 (10, 47.5) ml (range, 5-300 ml), and the surgical time was 105 (86, 150) min (range, 30-290 min). The mean surgical time varied among different fixation techniques. All patients were followed up for an average of 6.46±2.26 months (range, 3-16 months). Clinical healing was achieved in all patients within 6 months, with an average time of 3.14±0.50 months. At the last follow-up, fracture reduction quality assessed by the Matta score was excellent in 21 cases and good in 43 cases, with an excellent/good rate of 84% (64/76). The Majeed score was 81.82±9.14 points (range, 50-92 points). For patients who were employed before the injury, the Majeed score was 86.55±4.85 points (range, 60-92 points), with 49 cases rated as excellent, 5 cases as good, and 1 case as fair. For patients who were not employed before the injury, the Majeed score was 69.43±5.34 points (range, 50-73 points), with 18 cases rated as excellent, 2 cases as good, and 1 case as fair. The overall excellent and good rate was 97% (74/76). Among patients who underwent INFIX internal fixation, 8 cases experienced lateral femoral cutaneous nerve injury postoperatively, all of which recovered sensation after 3 months; 1 case using LC-II screws experienced screw loosening postoperatively and was advised to reduce activity, then the screw was removed after fracture healing at 6 months postoperatively; 1 case using anterior ring channel screws experienced surgical site infection postoperatively, which was controlled after debridement.Conclusion:In the treatment of unstable pelvic ring multiple fractures, robot-assisted screw fixation has less blood loss, less fluoroscopy times, high success rate of planning guide needle, satisfactory reduction quality and postoperative function.
6.Analyses of DXA in diagnosing osteoporosis of postmenopausal rheumatoid arthritis patients in Qinghai region and the risk factors of them

Objective:To investigate the diagnosis of dual-energy X-ray absorptiometry(DXA)for osteoporosis(OP)of postmenopausal patients with rheumatoid arthritis(RA)in Qinghai region and the risk factors of them.Methods:A total of 200 postmenopausal female RA patients who admitted to Qinghai Hospital of Traditional Chinese Medicine from May 2022 to April 2023 were selected.All patients were tested for bone mineral density(BMD)after admission,and lumbar spines L1-L4,whole lumbar,large trochanter,Ward's triangle area,whole body and whole forearm were measured by DXA.According to the results of BMD test,patients whose BMD T values of all body parts-2.5 SD were less or equal to-2.5 were included in the OP group(121 cases),and patients whose BMD T value of all body parts were larger than-2.5 SD were included in the non-OP group(79 cases).The BMD T value of different body parts between two groups of RA patients were compared and analyzed.The area under curve(AUC)of receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency of BMD T value for OP.The logistic regression method was adopted to analyze the risk factors that postmenopausal RA patients of Qinghai region occurred OP.Results:The BMD T values of L1,L2,L3,L4,whole lumbar,large trochanter,Ward's triangular area,whole body and whole forearm of OP group were obviously lower than those of the non-OP group.In analysis of ROC curve,the sensitivities of BMD T values of L1,L2,L3,L4,whole lumbar,large trochanter,Ward's triangle area,whole body and forearm were respectively 96.20%,95.22%,90.16%,96.03%,92.01%,89.36%,99.26%,90.02% and 96.03% in diagnosing OP,and the specificities of them were respectively 81.00%,82.19%,85.22%,83.06%,83.06%,90.22%,80.06%,86.23%,83.09%,and the AUC values of them were respectively 0.908,0.905,0.896,0.906,0.903,0.879,0.918,0.901 and 0.906.The results of the logistic-regression analysis showed that advanced age,long disease course,rheumatic activity scores of 28 joints,erythrocyte sedimentation rate and Calcium supplementation were the risk factors of occurring OP in postmenopausal RA patients in Qinghai region.Conclusion:The DXA method that detects BMD of RA patients who occur OP can be used as gold standard to assess OP,and there are many risk factors that affect the occurrence of OP in postmenopausal RA patients of Qinghai region.The clinical work should combine with relative factors to formulate reasonable measure so as to reduce the incidence of OP.
7.Effect of autologous platelet plasmapheresis and reinfusion on acute kidney injury after total aortic arch replacement

Objective:To evaluate the effect of autologous platelet plasmapheresis (APP) and reinfusion on acute kidney injury (AKI) after total aortic arch replacement.Methods:Sixty American Society of Anesthesiologists physical status Ⅲ or Ⅳ patients of both sexes, aged 18-64 yr, with body mass index of 19-34 kg/m 2, scheduled for total aortic arch replacement under general anesthesia, were divided into 3 groups( n=20 each)using a random number table method: APP-reinfusion group (group A), acute normovolemic hemodilution (ANH) group (group N) and autologous blood withdrawal-reinfusion group (group C). In group A, APP was completed before the operation, concentrated red blood cells were transfused timely according to the intraoperative conditions, and platelet-poor and platelet-rich plasma were transfused after protamine neutralization.In group N, ANH was completed before the operation, and the whole blood released was transfused after protamine neutralization.Autologous blood withdrawal and washed red blood cell (RBC) transfusion were only performed in group C, and autologous blood withdrawal was performed in group A and group N. Cervical venous blood and urine samples were collected at 10 min after anesthesia induction (T 1), at 5min after cardiopulmonary bypass (T 2), immediately after the end of surgery (T 3), at 24h and 48h after surgery (T 4、5) for determination of the concentrations of hematocrit (Hct), plasma free hemoglobin (fHb), urinary neutrophil gelatinase-associated lipocalin (NGAL), tissue metalloproteinase-inhibitor -2 (TIMP-2) and insulin-like growth factor binding protein-7 (IGFBP-7) and for calculation of [TIMP-2]×[IGFBP-7]. Glomerular filtration rate (GFR) and concentrations of reatinine (Scr), urea, uric acid and cystatin C were measured before surgery, on the right postoperative day, and at 1, 2 and 3 days after surgery.The intraoperative requirements for allogeneic blood product transfusion, postoperative mechanical ventilation time, duration of intensive care unit (ICU) stay and hemodialysis were recorded. Results:Compared with group C, the postoperative requirement for allogeneic RBC transfusion in group A and group N and the allogeneic platelet transfusion in group A were significantly reduced, Hct at T 2 in group N was decreased, fHb at T 3 and T 4 in groups N and group A was decreased, Scr concentration on the right postoperative day and at 1 day after operation in group A was decreased, and cystatin C concentration on the right postoperative day and at 2 and 3 days after operation in group A was decreased ( P<0.05). Compared with group N, the postoperative requirement for the allogeneic platelet transfusion, concentrations of NGAL and TIMP-2 at T 3-5, IGFBP-7 concentration and [TIMP-2]×[IGFBP-7] at T 3 and T 4, Scr concentration on the right postoperative day and at 1 day after operation and cystatin C concentration on the right postoperative day and at 2 and 3 days after operation were significantly decreased in group A ( P<0.05). Conclusion:APP-reinfusion provides better efficacy in alleviating postoperative AKI than ANH in the patients undergoing total aortic arch replacement.
9.Clinical phenotype and genotype of early-onset facioscapulohumeral muscular dystrophy type 1

Objective:To explore the clinical, pathological and genetic characteristics of early-onset facioscapulohumeral muscular dystrophy type 1 (FSHD1), in order to increase awareness of the disease.Methods:In this retrospective study, the history of 3 patients, who were diagnosed with early-onset FSHD1 by molecular genetic test in Pediatric Outpatient Department of Peking University First Hospital from 4 th June 2012 to 4 th June 2018, were collected. Their clinical data, genotypes, phenotypes and pathological features of muscle biopsy were analyzed. Results:All the three patients were males at the age of 14 years, 11 years and 9 years 11 months, respectively, whose onset age was between infancy and early childhood and they got confirmed diagnosis within 4 to 10 years after the onset of illness. Their molecular genetic testing indicated that the number of D4Z4 repeat arrays located in 4qA were 2, 3 and 4, which was consistent with the characteristics of early-onset FSHD1. Their common clinical manifestations were facial, scapular and proximal lower limb muscle progressively and asymmetrically weakness. All patients had different severity of spine deformity and high-frequency dominant sensorineural hearing loss, however, the phenotype of the third patient with 4 D4Z4 repeats was significantly the most severe.Conclusions:Early-onset FSHD1 usually concealed onset and is difficult to diagnose. Its precise diagnosis depends on molecular genetic techniques, but the genotypes of 3 patients here are not corresponding to phenotypes strictly and it is necessary to accumulate more cases for further analysis in order to provide a more reliable basis for the relationship of genotype-phenotype and prognosis evaluation of the disease.
10.Epidemiological investigation of adult single fractures of cuneiform in the east and west areas in China from 2010 through 2011

Objective To compare the epidemiological characteristics of adult single fractures of the cuneiform between the east and west areas in China from 2010 through 2011.Methods Thirty-five hospitals in the east coast area (group A) and 28 hospitals in the west inland area (group B) were selected for this investigation. The data of adult single fracture of the cuneiform treated between January 2010 and De-cember 2011 at the 63 hospitals were collected through the PACS system and medical records inquiry system. The epidemiological characteristics concerning age, gender and fracture classification were compared between the 2 groups. Results A total of 457 adult single fractures of the cuneiform were collected, involving 384 cases in group A and 73 cases in group B. They accounted respectively for 4.38% (384/8,772) and 3.53% (73/2, 068) of the adult tarsal bone fractures in the same period, showing no significant difference (P >0.05), accounted respectively for 1.71% (384/22, 455) and 1.15% (73/6, 354) of the adult foot frac-tures, showing a significant difference (P <0.05), and accounted respectively for 0.20% (384/192,991) and 0.09% (73/81, 143) of all the adult fractures, showing a significant difference (P <0.05). The median age was 39 years in group A (29, 50) and 36 years in group B (25, 45), showing a significant difference (P <0.05). The male to female ratio was 1.61:1 (237/147) for group A and 1.52:1 (44/29) for group B. The peak age range of the fracture for males was from 21 to 30 years old in both groups, and its proportion was 28.69% (68/237) in group A and 38.64% (17/44) in group B. The peak age range of the fracture for females was from 41 to 50 years old in both groups, and its proportion was 26.53% (39/147)in group A and 27.59% (8/29) in group B. All the above comparisons were statistically insignificant (P> 0.05). The proportion of the age group of≥61 years was significantly larger in group A than in group B (P <0.05). The high risk type of the fracture was type 85-A1 in both groups, and its proportion was 61.98% (238/384) in group A and 52.05% (38/73) in group B. The proportion of type 85-B1 was significantly larger in group B than in group A (P <0.05). Conclusions Adult single fractures of the cuneiform were more common in young and male patients. The proportion of elderly patients (≥61 years old) was significantly larger in the east area. The high risk type of the fracture was simple fracture involving the medial cuneiform bone (type 85-A) in both areas. The proportion of the comminuted fracture of the medial cuneiform (type 85-B1) was significantly larger in the west area.
