1.Three-dimensional finite element analysis of molar distalization with clear aligners with different thicknesses and edges

BACKGROUND:One of the advantages of clear aligner treatment is molar distalization.However,tooth tilting movement and loss of anterior anchorage may occur during treatment.There are few studies on whether these problems can be improved by selecting clear aligners with different thicknesses and edges to improve the clinical treatment effect.OBJECTIVE:To analyze the control ability of clear aligners with different thickness and edges on the central incisor,lateral incisor,and second molar when pushing the maxillary second molar distally by three-dimensional finite element analysis.METHODS:Three-dimensional finite element analysis models of bilateral maxillary second molar distalization with clear aligner,maxillary dentition,periodontal ligament,and alveolar bone with different thicknesses and margins were established by Mimics,Geomagic Wrap,3-matic and SolidWorks software,respectively.There were 16 combinations of four thicknesses(0.4,0.5,0.625,and 0.75 mm)and four margins(scallop,straight,straight extension 2 mm and straight extension 4 mm).The data were imported into Ansys Workbench software for design and solution.The mean value,peak value and distribution of the periodontal ligament equivalent stress of the second molar,the equivalent stress and the maximum initial displacement of the second molar,and the control ability of each appliance on the second molar,central incisor,and lateral incisor were analyzed.RESULTS AND CONCLUSION:(1)The mean equivalent stress of periodontal ligament of the second molar,the equivalent stress of the second molar and the maximum initial displacement of the second molar increased with the extension of the appliance edge and the increase of the thickness of the appliance in the 16 groups of models.(2)When the thickness of appliances was the same,the maximum equivalent stress of the second molar in the linear appliance group was the highest,and the maximum equivalent stress of the second molar in the linear extended appliance group was greater than that in the scallop appliance group.When the edge of the appliance was the same,the periodontal ligament equivalent stress peak of the second molar increased with the increase of the thickness of the appliance.The equivalent stress distribution in the periodontal ligament of the second molar in the linear extendable appliance group was more uniform than that in the scallop appliance group and the linear appliance group.(3)When the thickness of the appliance was the same,the scallop-shaped appliance had the worst control on the second molar.When the edge of the appliance was the same,with the increase of the thickness of the appliance,the control of the second molar by the linear extender appliance was gradually stronger than that by the linear appliance.The control of the central incisor was stronger and more stable with the linear extended 2 mm appliance,while the control of the lateral incisor was stronger and more stable with the linear appliance.(4)The results showed that when using clear aligners to push molars distally,extending the edge of the appliance could improve the control of the molars and reduce the tilting movement of the teeth.The design of a straight extension margin of 2 mm for the central incisor and a straight edge for the lateral incisor can enhance the control of the anchorage incisor and reduce the labial inclination of the anterior teeth.
2.Quality Evaluation of Naomaili Granules Based on Multi-component Content Determination and Fingerprint and Screening of Its Anti-neuroinflammatory Substance Basis

ObjectiveTo establish an ultra-performance liquid fingerprint and multi-components determination method for Naomaili granules. To evaluate the quality of different batches by chemometrics, and the anti-neuroinflammatory effects of water extract and main components of Naomaili granules were tested in vitro. MethodsThe similarity and common peaks of 27 batches of Naomaili granules were evaluated by using Ultra performance liquid chromatography (UPLC) fingerprint detection. Ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) technology was used to determine the content of the index components in Naomaili granules and to evaluate the quality of different batches of Naomaili granules by chemometrics. LPS-induced BV-2 cell inflammation model was used to investigate the anti-neuroinflammatory effects of the water extract and main components of Naomaili granules. ResultsThe similarity of fingerprints of 27 batches of samples was > 0.90. A total of 32 common peaks were calibrated, and 23 of them were identified and assigned. In 27 batches of Naomaili granules, the mass fractions of 14 components that were stachydrine hydrochloride, leonurine hydrochloride, calycosin-7-O-glucoside, calycosin,tanshinoneⅠ, cryptotanshinone, tanshinoneⅡA, ginsenoside Rb1, notoginsenoside R1, ginsenoside Rg1, paeoniflorin, albiflorin, lactiflorin, and salvianolic acid B were found to be 2.902-3.498, 0.233-0.343, 0.111-0.301, 0.07-0.152, 0.136-0.228, 0.195-0.390, 0.324-0.482, 1.056-1.435, 0.271-0.397, 1.318-1.649, 3.038-4.059, 2.263-3.455, 0.152-0.232, 2.931-3.991 mg∙g-1, respectively. Multivariate statistical analysis showed that paeoniflorin, ginsenoside Rg1, ginsenoside Rb1 and staphylline hydrochloride were quality difference markers to control the stability of the preparation. The results of bioactive experiment showed that the water extract of Naomaili granules and the eight main components with high content in the prescription had a dose-dependent inhibitory effect on the release of NO in the cell supernatant. Among them, salvianolic acid B and ginsenoside Rb1 had strong anti-inflammatory activity, with IC50 values of (36.11±0.15) mg∙L-1 and (27.24±0.54) mg∙L-1, respectively. ConclusionThe quality evaluation method of Naomaili granules established in this study was accurate and reproducible. Four quality difference markers were screened out, and eight key pharmacodynamic substances of Naomaili granules against neuroinflammation were screened out by in vitro cell experiments.

4.Effect of Preoperative Frailty on Prognosis of Pancreatic Cancer Patients after Pancreaticoduodenectomy

ObjectiveTo investigate the impact of preoperative frailty on the prognosis of patients with pancreatic cancer who underwent pancreaticoduodenectomy. MethodsA retrospective analysis was conducted on the clinical data of 435 pancreatic cancer patients who underwent pancreaticoduodenectomy at Sun Yat-sen Memorial Hospital of Sun Yat-sen University. Preoperative frailty was assessed using the FRAIL questionnaire. Binary logistics regression analysis was employed to identify factors influencing frailty, and Cox regression analysis was used to evaluate the effect of frailty on survival. According to the demographic characteristics, subgroup analyses were performed on the effect of frailty on prognosis of patients with pancreatic cancer with pancreaticoduodenectomy. ResultsAmong the 435 patients enrolled, 119 (27.4%) exhibited frailty, while 316 (72.6%) did not. Significant differences were observed between the two groups in various clinical parameters, including age, body mass index, American Society of Anesthesiologists (ASA) score, postoperative red blood cell transfusion, postoperative abdominal abscess, serum levels of glycoantigens 199, glycoantigens 125, and alpha fetoprotein, leukocyte count, neutrophil, high density lipoprotein (HDL) level, and pain intensity (P<0.05). Advanced age and an ASA score of Ⅲ were identified as risk factors for frailty, whereas HDL level was a protective factor. Non-frail patients had better postoperative survival times than frail patients. HDL was determined to be an independent protective factor for prognosis, while LDL was an independent risk factor. ConclusionsThis study demonstrates that preoperative frailty is a significant predictor of poor prognosis in pancreatic cancer patients who underwent pancreaticoduodenectomy.These findings suggest that preoperative frailty assessment and targeted interventions to improve nutritional and metabolic status could potentially enhance postoperative survival and quality of life in pancreatic cancer patients.
5.Comparative analysis of ion-selective electrode method and high-throughput rapid determination method for determination of fluoride level in drinking water

Objective:To analyze the differences in determination of fluoride level in drinking water by ion-selective electrode method and high-throughput rapid determination method.Methods:The precision test was carried out by using the two methods to measure two kinds of fluoride standard substances, water samples of external quality control assessment from 2021 to 2023 (two kinds each year) and the fluoride level in three drinking water samples (for 5 times/each sample). Accuracy testing was conducted by measuring the external quality control assessment water samples and the spiked recovery rates drinking water, and water samples were grouped (water fluoride ≤1.00, > 1.00 mg/L) and analyzed according to the "Hygienic Standards for Drinking Water" (GB 5749-85). SPSS 23.0 software was used for statistical analysis of the measurement results.Results:(1) The correlation coefficients ( r) of the working curves of the two methods were both > 0.990, meeting the quality control requirements. (2) In the precision test, when comparing the results of the two methods for detecting two kinds of fluoride standard substances, there was no statistically significant difference ( F = 0.36, 0.15, P = 0.564, 0.707), and the coefficients of variation ( CV) were all < 5%. The CV of the detection results of the external quality control assessment water samples and drinking water samples were < 5%. (3) In the accuracy test, when the fluoride concentration in water was ≤1.00 mg/L, there was no statistically significant difference in the spiked recovery rates between the two methods ( F = 0.49, P = 0.504). When the fluoride concentration in water was > 1.00 mg/L, there was a statistically significant difference in the spiked recovery rates between the two methods ( F = 24.75, P = 0.003). Conclusions:The ion-selective electrode method has the advantages of wide detection range and wide adaptability, while the high-throughput rapid determination method has high accuracy. Testing personnel can weigh and choose the appropriate determination method based on the actual laboratory conditions and sample concentration range.
6.Effect of clear and fixed aligners on smooth surface of teeth analyzed by Universal Visual Scoring System

BACKGROUND:Leukoplakia is a common adverse reaction to traditional orthodontic treatment.There are still some controversies about the difference between light-transmitting clear and fixed aligners in the periodontal health status,incidence of leukoplakia,saliva and related bacteria in patients undergoing orthodontic treatment.OBJECTIVE:To compare the effects of clear and fixed aligners on the smooth surface of orthodontic teeth using Universal Visual Scoring System.METHODS:A total of 80 patients admitted to Department of Orthodontics,Second Hospital of Hebei Medical University from January 2018 to December 2022 were selected and randomly divided into a control group(n=40)and an observation group(n=40)using a random number table method.The control group received orthodontic treatment with fixed aligners,and the observation group received orthodontic treatment with clear aligners.Before treatment and after 6,12,and 24 months of treatment,the labial or buccal sides of teeth 13,33,16,and 36 were selected for each patient as the research subjects,and the visual scoring system was used to evaluate the white spot lesions.RESULTS AND CONCLUSION:(1)Before treatment and after 6,12,and 24 months of treatment,there was no significant difference in the color of tooth white spots and the severity of caries between the two groups of patients(P>0.05).(2)For tooth 13,after 12 months of treatment,the color grade and caries severity of white spots were both higher than before treatment(P<0.05).For tooth 33,the color grade and caries severity after 12 months of treatment were both higher than before treatment(P<0.05).The severity of caries after 12 months of treatment was higher than that after 6 months of treatment(P<0.05).For tooth 16,the color grade of white spots and the severity of caries after 12 months of treatment were both higher than those after 6 months of treatment(P<0.05).(3)Multi-level ordered multi-classification Logistic regression model analysis showed that the color and severity of white spots on different teeth had little correlation with the type of appliance and were greatly affected by the correction time factor.Among them,the color change and severity of the initial white spots on teeth 33 and 13 were statistically significant at 12 and 24 months of treatment.(4)It is not possible to conclude that there is a difference in the effect of fixed and clear aligners on the white spot lesions of smooth surfaces of the teeth,but an increase in the risk of dental caries with the increase in the duration of treatment can be detected.
7.Risk prediction models for readmission in patients after percutaneous coronary intervention: a systematic review and critical appraisal

Objective:To systematically evaluate the risk prediction model for readmission in patients after percutaneous coronary intervention, and to provide reference for medical staff to apply or optimize relevant risk assessment tools.Methods:China National Knowledge Infrastructure, VIP, Wanfang Data, China Biomedical Literature Database, and Cochrane Library, PubMed, Embase, CINAHL, Scopus and Web of Science were searched for the related studies on readmission risk prediction models in patients with percutaneous coronary intervention from the establishment of the databases to April 25, 2024. Two researchers independently screened the literature, extracted data, and evaluated the risk of bias in the included studies.Results:A total of eleven articles were included, involving sixteen readmission risk prediction models, with readmission rates ranging from 0.70% to 31.44% and the areas under subjects′working characteristic curves ranging from 0.604 to 0.899. Calibration methods were reported in ten models, five studies reported processing methods of missing data, and external validation was used in three studies. The overall risk of bias was higher. The top six predictors of repeated reports in the readmitted model were age, renal insufficiency, sex, congestive heart failure, diabetes and health insurance.Conclusions:The readmission risk prediction models had good predictive performance. However, the quality of the model methodology was limited. It is necessary to improve the research quality in data sources, measurement and definition of predictive factors, processing of missing data and model evaluation. In the future, data mining can be used to apply the readmission prediction model in the early stage of admission, so as to identify high-risk patients as early as possible and effectively prevent the occurrence of readmission.
8.Preliminary study on the role and mechanism of ISG15 in exosomes in platinum resistance in ovarian cancer

Background and purpose:Epithelial ovarian cancer(EOC)has a poor prognosis and is prone to developing resistance to platinum-based chemotherapy.Exosomes are currently believed to be involved in platinum resistance in ovarian cancer.This study explored the role and mechanism of exosomes on platinum resistance of ovarian cancer.Methods:Through ultracentrifugation,exosomes were isolated and analyzed using electron microscopy,particle size studies,and Western blot for detailed exosome analysis.We detected the expression levels of exosomal proteins and related signaling pathways.Exosomal protein expression profile was analyzed by proteomics.Key differential proteins were screened by intersecting with existing drug resistance datasets.Furthermore,patients diagnosed with EOC via pathological analysis at the Fudan University Shanghai Cancer Center from August 2023 to August 2024,who underwent surgery and fulfilled the eligibility requirements,were gathered to examine the link between the expression of interferon-stimulated gene 15(ISG15)in serum exosomes and resistance to platinum medication.The expression levels of related proteins in serum exosomes were quantitatively detected by enzyme-linked immunosorbent assay(ELISA).Receiver operating characteristic(ROC)curve was plotted to explore the predictive value of serum exosomal protein in ovarian cancer resistance.Results:Exosomes derived from platinum-sensitive and drug-resistant ovarian cancer cells were extracted and proteomic analysis was further performed.We identified 9 differential proteins associated with platinum-resistance and found the key molecule interferon-stimulated gene 15(ISG15).Compared with sensitive cells,the expression of ISG15 in exosomes of drug-resistant ovarian cancer cells was significantly upregulated.Exosome tracer tests showed that exosomes were successfully taken up by ovarian cancer receptor cells.After coculture with drug-resistant exosomes,the expression level of ISG15 in ovarian cancer receptor cells was increased.Knockdown ISG15 in EOC cells decreased the expression of ISG15 in exosomes,and incubation of ISG15-knockdown exosomes decreased the cell viability on the condition of platinum drugs,indicating that ISG15 in exosomes regulated platinum resistance of ovarian cancer cells.Moreover,ISG15 could regulate the expression of multidrug resistance protein 1(MDR1)through phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)/nuclear factor-kappa-light-chain-enchancer of activated B cells(NF-κB)signaling pathway.This study included a total of 87 patients.Clinical serum samples also showed that the expression of ISG15 in exosomes was higher in platinum-resistant ovarian cancer patients than in sensitive ovarian cancer patients.Using serum exosomal ISG15 as an indicator to distinguish between sensitivity and resistance,the area under ROC curve was 0.779(P<0.05),cutoffvalue was 27.35ng/mL,sensitivity was 70.2%,and specificity was 76.4%.Conclusion:ISG15 in exosomes can regulate the expression of MDR1 in ovarian cancer cells through PI3K/AKT/NF-κB signaling pathway,and promote platinum resistance in ovarian cancer cells.

Objective:To study the characteristics of oral microbiota in patients with periodontitis and its effects on Toll-like recep-tors(TLRs)signaling pathway and levels of inflammatory factors.Methods:Fifty patients with periodontal disease who received treatment in our hospital from January 2021 to September 2023 were selected as the trial group,and 50 healthy physical examination people during the same period were selected as the control group.Subgingival microbiota was cultured and gingival crevicular fluid samples were collected.Subgingival bacterial diversity was detected by 16S rRNA,interleukin-6(IL-6),IL-1β and tumor necrosis factor-α(TNF-α)levels in gingival crevicular fluid were detected by enzyme-linked immunosorbent assay,and TLRs signaling path-way was detected by real-time quantitative polymerase chain reaction.Results:The subgingival Simpson index in the trial group was higher than that in the control group.Subgingival microbiota was significantly separated between the two groups,and the relative abundance of Prevotella,Hemophilus and Veillonella were significantly different(P<0.05).The number of microbiota in the trial group was higher than that in the control group(P<0.05).The levels of IL-6,IL-1β,TNF-α and mRNA relative expression of TLRs signaling pathway in gingival crevicular fluid in trial group were higher than those in control group(P<0.05).The levels of IL-6,IL-1β and TNF-α in gingival crevicular fluid of patients with periodontal disease were positively correlated with the number of microbiota of Parvobacteria,Tenella forsythiae,Prevotella intermedia and Porphyromonas gingivalis.Logistic regression analysis showed that the number of microbiota of Parvobacteria,Tenneella forsythiae,Prevotella intermedia and Porphyromonas gingivalis were independent factors influencing the high expression of IL-6,IL-1β and TNF-α.Conclusion:In periodontitis patients,the subgingi-val microbiota was changed,the mRNA expression of TLRs signaling pathway and the expression of inflammatory factors were in-creased,and the number of subgingival microbiota was significantly correlated with the expression of inflammatory factors.
