Objective:To investigate and analyze the service needs of family members of Neurosurgical Intensive Care Unit patients and provide references for improving Neurosurgical Intensive Care Unit nursing services.Methods:Totally 270 family members of neurosurgical patients hospitalized between March and May 2024 at Intensive Care Unit of Beijing Tiantan Hospital, Capital Medical University were selected by convenience sampling. Kano Model-based Questionnaire on the Needs of Family Members of Neurosurgical Intensive Care Unit Patients designed based on literature review and expert consultation, was distributed to investigate their families service needs. The Kano model analysis was applied to categorize service needs and prioritize them, followed by constructing a satisfaction-importance quadrant diagram.Results:A total of 270 questionnaires were distributed, with 248 valid responses collected, yielding an effective response rate of 91.85% (248/270) . The Kano attribute analysis revealed that all service needs were classified as "expected attributes". The satisfaction-importance quadrant diagram indicated that 10 items were located in the "advantage zone", nine items in the "retention zone", four items in the "observation zone", and seven items in the "improvement zone".Conclusions:The Kano model-based analysis of service needs helps healthcare providers identify the diverse needs of family members of Neurosurgical Intensive Care Unit patients. It is recommended that while maintaining the provision of services in the "advantage zone", healthcare providers integrate hospital resources to continuously improve services in the "improvement zone" to enhance family satisfaction and strengthen hospital competitiveness.

Objective:To systematically evaluate the incidence of psychological dysfunction in critically ill patients with post-intensive care syndrome (PICS) .Methods:Computer search was conducted on CNKI, Wanfang Database, VIP, SinoMed, PubMed, Embase, Cochrane Library, Web of Science, CINAHL and Scopus for literature related to the incidence of psychological dysfunction in critically ill PICS patients, and the search period was from establishment of the databases to April 2023. Data extraction and quality evaluation were carried out for the included literatures and Stata 17.0 software was used for meta-analysis.Results:A total of 32 articles were included, with a total of 496 399 patients. Meta-analysis results showed that the incidence of psychological dysfunction in critically ill PICS patients was 31.0% (95% CI: 26%-35%). Subgroup analysis results showed that the incidence of psychological dysfunction in PICS patients was higher in foreign literature, literature on virus-infected ICU patients, and literature published from 2017 to 2023, with incidence of 32.5% (95% CI: 25%-40%), 34.0% (95% CI: 29%-39%), and 32.8% (95% CI: 28%-38%), respectively. Conclusions:The incidence of psychological dysfunction in critically ill patients with post-intensive care syndrome is relatively high. Medical staff should pay more attention to the psychological health of post-ICU patients, strengthen screening of psychological dysfunction in critically ill patients with post-intensive care syndrome and give timely targeted intervention measures.

Objective:To summarize the evidence of non-drug management of postoperative delirium in neurosurgery patients at home and abroad, so as to provide reference for medical and nursing staff to formulate intervention measures.Methods:This study is a best evidence summary. In view of the problem of non-drug management of postoperative delirium in neurosurgery patients, according to the "6S" pyramid model, the evidence on non-drug management of postoperative delirium in neurosurgery patients was retrieved through computer in domestic and foreign databases, guide websites and professional association websites, including clinical practice guidelines, expert consensus, clinical decision-making, systematic review, evidence summary and so on. The retrieval period was from January 1, 2016 to December 31, 2021. Two researchers independently evaluated the quality of article and extracted evidence, and used the Joanna Briggs Institute (JBI) evidence pre-grading and evidence recommendation grading system (2014 version) to grade and recommend the included evidence.Results:A total of 17 articles were included, including 6 guidelines, 3 expert consensus, 3 systematic reviews, 3 evidence summaries, and 2 clinical decision-makings. Finally, 39 best evidences were summarized in 7 aspects, including risk factor assessment, screening, non-drug prevention, non-drug treatment, information support and health education, follow-up and evaluation, and medical worker training.Conclusions:The best evidence for non-drug management of postoperative delirium in neurosurgical patients is comprehensive. Medical and nursing staff can select evidence according to the clinical situation and the actual situation of patients, and carefully and reasonably transform and apply evidence.

Objective:To explore the mediating effect of psychological resilience between the emotional labor strategy and coping style in clinical nurses.Methods:This study was a cross-sectional study. From March to April 2020, convenience sampling method was adopted to select 460 clinical nurses from a Class Ⅲ Grade A hospital in Beijing as the survey subject. Nurses were investigated with the General Information Questionnaire, Connor-Davidson Resilience Scale (CD-RISC) , Simplified Coping Style Questionnaire (SCSQ) and Emotional Labor Strategy Scale. Pearson correlation analysis and structural equation model were used to test the correlation among variables and the mediating effect. In this study, a total of 460 electronic questionnaires were distributed, and 451 valid questionnaires were returned, with the effective response rate of 98.04%.Results:Among 451 nurses, the total CD-RISC score was (67.51±19.86) , and the surface-playing dimension in the Emotional Labor Strategy Scale was (21.86±6.40) , and the scores of positive coping dimension and negative coping dimension in SCSQ were (20.77±8.68) and (8.22±4.81) respectively. The total score and all dimension scores of CD-RISC, the dimension scores of the Emotional Labor Strategy Scale were positively correlated with the positive coping style score with a statistical difference ( r=0.119-0.492, P<0.05) . Psychological resilience played a partial mediating effect between the emotional labor strategy and positive coping style also with a statistical difference (β=0.295, P<0.01) , and the mediating effect accounted for 70.91% of the total effect. Conclusions:Emotional labor strategies can indirectly affect nurses' coping styles through their psychological resilience. Nursing managers should pay attention to the emotional labor and psychological resilience management of clinical nurses to help them actively respond to clinical challenges and ensure the quality of care.

Objective:To preliminarily construct a structured education program suitable for children with asthma and their parents, so as to provide a reference and guidance for the education and management of children with asthma.Methods:The intervention literature in the field of childhood asthma health education publishing from January 2009 to January 2019 was systematically retrieved in the Cochrane Library, Embase, Web of Science, PubMed, CNKI, Wanfang, and China Biomedical Literature Service System (SinoMed) . Three researchers conducted quality evaluation and content analysis of the literature, and constructed the first draft of a structured education program for children with asthma and their parents based on the patient's health education program. The program was finally formed after revision by a meeting of 9 experts.Results:The expert's personal authority coefficient was from 0.70 to 1.00, and the group authority coefficient was 0.88. The program included two parts, namely structured education implementation framework and process, and structured education content. The structured education content covered 8 topics including supportive self-management education, non-drug management, use of inhalation devices, management of severe uncontrolled asthma, management of adolescent asthma, acupoint application, acupoint massage and precautions, totaling 50 items.Conclusions:The structured education program for children with asthma and their parents is scientific and rigorous, which can provide a reference for the clinical practice of the education and management of children with asthma.

Objective:To explore effects of clinical nursing pathway based on enhanced recovery after surgery (ERAS) in patients with adrenal tumor.Methods:Totally 60 patients with adrenal tumor admitted in the Department of Urinary Surgery at Beijing Tiantan Hospital, Capital Medical University from April to September 2019 were selected by convenient sampling and divided into the control group ( n=30) and the research group ( n=30) according to the random number table. Patients in the control group received routine care, while patients in the research group underwent clinical nursing pathways based on ERAS. The postoperative recovery status (time of first drinking after operation, time to get out of bed, time of exhaust, length of hospital stay, and duration of drainage tube indwelling) , postoperative pain and patient satisfaction were compared between the two groups. Results:The time of first drinking, time to get out of bed, time of exhaust, length of hospital stay, and duration of drainage tube indwelling in the research group were shorter than those in the control group, and the difference between the two groups was statistically significant ( P<0.05) ; the pain score on postoperative days 1 and 3 in the research group was lower than that in the control group, and the difference between the two groups was statistically significant ( P<0.05) ; the satisfaction score of the research group was higher than that of the control group, and the difference between the two groups was statistically significant ( P<0.05) . Conclusions:The clinical nursing pathways based on ERAS, when applied to patients with adrenal tumor, can promote the recovery of patients from surgery, shorten the postoperative hospital stay, reduce patient pain, and improve patient satisfaction.
