1.Segmented Time Study and Optimization Strategy for Clinical Application of Ethos Online Adaptive Radiotherapy.

OBJECTIVE:
To analyze the time characteristics of the Ethos online adaptive radiotherapy (OART) process in clinical practice and provide guidance for the comprehensive optimization of each stage of adaptive radiotherapy.
METHODS:
The study involved 61 patients with cervical, rectal, gastric, lung, esophageal, and breast cancers who underwent Ethos OART. The mean ± standard deviation of segmental time, total time, and target volume for these patients were tracked. The time characteristics for different cancer types were evaluated, and the average time for target and organ at risk (OAR) modifications was compared with the average target volume for each cancer type.
RESULTS:
Cervical cancer born the longest total treatment time, while breast cancer had the shortest. For all cancer types except breast cancer, the modification time for target and OAR was the most time-consuming segment. The average time for target and OAR modifications aligned with the trend of the average target volume.
CONCLUSION
The total treatment time for various cancers ranges from 15 to 35 minutes, indicating room for improvement.

Severe open tibiofibular fractures account for approximately 28.1% of all open fractures. Among them, Gustilo-Anderson type IIIB/C fractures present significant clinical challenges due to associated bone and soft tissue defects, high infection rates, and risk of amputation. Inadequate preoperative assessment may lead to suboptimal emergency surgical planning or intraoperative complications. Historically, external fixation was often preferred, but this approach has been associated with limitations such as restricted joint mobility, delayed bone union, joint stiffness, and disuse osteoporosis, resulting in poor functional recovery. With advancements of debridement techniques, standardization of antibiotic use, and popularization of early soft tissue coverage, early internal fixation has gained broader acceptance. Nevertheless, controversies persist regarding the choice of fixation method, timing of definitive fixation, use of reamed versus unreamed intramedullary nailing, and necessity of fibular fixation. To standardize the diagnosis and early management of severe open tibiofibular fractures, reduce complication rates, and improve functional recovery, the Society of Microsurgery of the Chinese Medical Association organized a panel of domestic experts to develop the Evidence-based guideline for the diagnosis and early fixation of severe open tibiofibular fractures ( version 2025), using evidence-based methodology. The guidelines provided 12 recommendations covering diagnostic and early fixation strategies of severe open tibiofibular fractures, aiming to provide clinicians with scientifically grounded and standardized guidance.
3.Home-based phototherapy implementation and management needs in patients with vitiligo: a questionnaire survey analysis

Objective:To investigate the current status of the implementation of home-based phototherapy (HBPT) in patients with vitiligo, and to analyze management needs among patients receiving HBPT.Methods:A questionnaire survey was conducted on the application of HBPT among patients with vitiligo who visited the outpatient clinic of the Hospital for Skin Diseases, Chinese Academy of Medical Sciences from December 2021 to November 2022. Additionally, the popularization and usage of HBPT as well as needs of patient management were investigated and analyzed in these vitiligo patients.Results:A total of 496 valid questionnaires were collected from 496 patients with vitiligo (241 males [48.5%] and 255 females [51.5%]) . Their ages at visit ranged from 2 to 67 (30.87 ± 12.36) years. The most commonly affected sites were the head and face (52.2%) , followed by hair (32.1%) , hands and feet (31.4%) , trunk (30.2%) , limbs (24.3%) , neck (19.5%) , and perineum (9.9%) . Among the participants, 320 (64.5%) were currently using or had used HBPT, and 352 (70.8%) expressed a willingness to learn more about HBPT usage guidelines and health education. Regarding the repigmentation outcomes after HBPT: among 312 patients, 54 (17.3%) reported complete recovery, 64 (20.5%) were markedly improved, 142 (45.5%) experienced improvement, and 52 (16.7%) showed no response. Adverse reactions occurred in 223 patients (71.5%) , of whom 28 (9.0%) experienced severe adverse reactions. The most desired guiding information was the adjustment method for phototherapy dosage and treatment duration (184/352, 52.3%) ; the most effective way to receive health education information was through verbal education by medical staff (177/352, 50.3%) .Conclusion:For vitiligo patients who were willing to accept and use HBPT, the most desired guiding information was the adjustment method for phototherapy dosage and treatment duration, and verbal health education by medical staff appeared to be the main way to obtain health education information.
4.Expression of miR-574 in placental tissue of preeclampsia and its significance in the pathogenesis of preeclampsia

Objective To investigate the expression of microRNA-574(miR-574)in placental tissue of pregnant women with preeclampsia(PE)and its significance in the pathogenesis of preeclampsia.Methods The placental tissue samples of 35 PE pregnant women(the PE group)and 20 normal pregnant women(the normal group)admitted to the Department of Obstetrics in Chengde Maternal and Child Health Care Hospital from March 2021 to June 2023 were collected,and the expression of miR-574 in the different placental tissues was detected by qRT-PCR.The trophoblast cells HTR8/Svneo were cultured in vitro and divided into the overexpression group,inhibition group and control group,which were transfected with miR-574 mimics,miR-574 inhibitor and miR-NC,respectively.The cell proliferation ability of each group was detected by CCK-8 assay;The cell apoptosis rate of each group was detected by flow cytometry;The cell invasion ability of each group was detected by transwell chamber experiment;The expression of EMT related proteins of cells in each group was detected by western blot.Results Compared with the normal group,the miR-574 expression of placental tissue in the PE group was significantly increased(P<0.05),and the expression of miR-574 in pregnant women with severe PE was significantly higher than that in pregnant women with mild PE(P<0.05).Compared with the control group,the expression of miR-574 of cells increased(P<0.05),the cell proliferation ability decreased(P<0.05),the cell apoptosis rate increased(P<0.05),the number of invasive cells reduced(P<0.05),the expression of E-cadherin protein increased(P<0.05),and the expression of MMP9 and N-cadherin proteins decreased in the overexpression group(P<0.05).Compared with the control group,the expression of miR-574 of cells decreased(P<0.05),the cell proliferation ability increased(P<0.05),the cell apoptosis rate reduced(P<0.05),the number of invasive cells increased(P<0.05),the expression of E-cadherin protein decreased(P<0.05),and the expression of MMP9 and N-cadherin proteins increased in the inhibition group(P<0.05).Conclusion The expression of miR-574 in the placental tissue of PE patients is increased,and it can participate in the pathogenesis of PE by influencing proliferation,apoptosis,invasion and EMT of trophoblast cell.

7.PD-1 Inhibitor Combined with Azacitidine and HAG Regimen for the Treatment of Relapsed/Refractory Acute Myeloid Leukemia:A Prospective,Single-Arm,Phase Ⅱ Clinical Study

Objective:To evaluate the efficacy and safety of PD-1 inhibitor combined with azacitidine and HAG regimen in the treatment of relapsed/refractory acute myeloid leukemia(R/R AML).Methods:This study is a prospective,single-arm,phase Ⅱ clinical trial that included R/R AML patients who met the inclusion criteria and were treated at The First Affiliated Hospital of Soochow University from December 2020 to August 2023.Patients could undergo allogeneic hematopoietic stem cell transplantation(allo-HSCT)after salvage therapy.The efficacy and safety were evaluated.Results:Twenty patients were enrolled,including 14 males and 6 females,with an average age of(50.7±15.3)years.The overall response rate(ORR)after one cycle of the treatment was 75.0%(15/20),and 35.0%(7/20)of the patients achieved complete remission(CR)or complete remission with incomplete hematologic recovery(CRi)after two cycles of the treatment.Eight patients received allo-HSCT.The main adverse events were hematologic toxicities,and no grade 5 adverse events occurred.Conclusion:The combination of PD-1 inhibitor,azacitidine,and the HAG regimen is a feasible and relatively safe treatment option for R/R AML,thus,to be worth further study.

9.Mechanism of Kechuanting granules in suppressing IL-33/ILC2s and pathogenic T cells to intervene in allergic airway inflammation

Aim To investigate the mechanisms of Ke-chuanting granules(KCT)inhibiting the IL-33/ILC2s pathway and pathogenic T cells to intervene in allergic airway inflammation.Methods Network pharmacolo-gy was utilized to analyze the potential targets and mechanisms of KCT-treated asthma.Allergic asthma models were induced in mice using OVA.Lung histo-pathology was conducted to observe injury changes.ELISA and quantitative PCR were utilized to measure key inflammatory factors and their mRNA expression levels in Th2-type asthma.Western blot was used to detect the phosphorylation levels of relevant proteins in the MAPK pathway.Flow cytometry was performed to evaluate the proportions of ILC2s,Th1,Th 17,Th2 and Treg cells.Results Network pharmacology iden-tified 227 main active components and 143 key targets of KCT in treating asthma,primarily enriched in signa-ling pathways such as MAPK and IL-17.Further vali-dation experiments demonstrated that KCT significantly alleviated lung inflammatory injury in asthmatic mice,reduced the number of B cells,production of I L-4,TNF-α and TGF-β,downregulated JNK phosphoryla-tion levels in lung tissue,as well as mRNA levels of Il-33,Bcl11b,Rorα,Tcf-7,Jun,Mapk3 and Mapk14.KCT intervention reduced the numbers of ILC2s and Th 17 cells in lungs and spleens of mice,and inhibited Th2 cell infiltration in lungs.Conclusions KCT ex-hibits therapeutic effects on allergic airway inflamma-tion in asthma,closely associated with the inhibition of the IL-33/ILC2s pathway,pathogenic T cell subsets,and JNK-MAPK signaling pathway.

As an important synthetic antibacterial drug, sulfonamides play an important role in the anti-infection field. Based on the research and development status of sulfonamides, this paper broke the classical structure of sulfanilamide, and designed and synthesized a series of acetyl-contained sulfanilamide tertiary amine thiol azole compounds and sulfanilamide tertiary amine amino azole compounds.
