BACKGROUND:The relationship between rheumatoid arthritis and coronary atherosclerosis has received extensive attention.Inflammation is related to rheumatoid arthritis and coronary atherosclerosis, indicating that there may be a common pathophysiological pathway between the two diseases. However, observational studies have not yet clarified the causal relationship.OBJECTIVE:To explore whether there is a causal relationship between rheumatoid arthritis and coronary atherosclerosis,as well as the potential causal relationship with 1 400 serum metabolites and 91 inflammatory factors through a Mendelian randomization analysis.METHODS:Coronary atherosclerosis data are from Finngen database, rheumatoid arthritis data are from IEU OpenGWAS database, serum metabolites data are from Canadian Longitudinal Study on Aging,Augsburg Cooperative Health Research and British Twin Project Research, and data of 91 inflammatory proteins are from research published in Nature Immunology in 2023.Mendelian randomization analysis was performed using data from genome-wide association studies, and causal effects were evaluated using inverse variance weighting,MR-Egger regression, weighted median, weighted model, and simple model methods, with inverse variance weighting being the primary analysis method. To enhance robustness,Cochran's Q-test MR-Egger intercept was used for sensitivity analysis.RESULTS AND CONCLUSION:(1)Inverse variance weighting results showed that rheumatoid arthritis was positively correlated with the increased relative risk of coronary atherosclerosis(odds ratio=1.002,95%confidence interval=1.001-1.003,P=0.003).There was no reverse causal relationship between coronary atherosclerosis and rheumatoid arthritis. In addition,96 serum metabolites and 9 inflammatory factors were found to have causal relationships with coronary atherosclerosis. There was a causal relationship between 51 serum metabolites and 7 inflammatory factors and rheumatoid arthritis.(2)This study provided epidemiological evidence between rheumatoid arthritis and coronary atherosclerosis, and emphasized the potential role of serum metabolites and inflammatory factors in the pathogenesis of these diseases. These findings may contribute to the development of new treatment strategies.Due to the limited inclusion of data from Asian populations, most contemporary studies used international databases and European population analyses.By collecting and analyzing the health data of European populations,it is conducive to a better understanding of the effects and potential role of Chinese medicine in Europe, and to further promote the practice of modern integration of Western and Chinese medicine.Meanwhile, through the comparative study with the European databases,it is possible to reveal the genetic differences and susceptibility to diseases among different populations, providing more dimensions and perspectives for global health research.

Objective To explore the mechanism through which dexmedetomidine(Dex)alleviates doxorubicin(Adr)-induced myo-cardial injury via regulating nuclear factor κB(NF-κB)expression.Methods Sprague-Dawley rats were divided into control,Adr,and Adr+Dex groups.Theirs hearts were harvested for hematoxylin and eosin(HE)staining, immunohistochemical staining,real-time polymerase chain reaction(PCR),and Western blotting anlyses. The rat primary cardiomyocytes, breast cancer cell line MDA-MB-23,lung cancer cell line H226,gastric cancer cell line AGS,and bladder cancer cell line 5637 were cultured and divided into control,Adr,Adr+Dex,Dex,and Adr+Dex+NF-κBi groups.CCK-8 and immunofluorescence staining were performed to detect the reactive oxygen species(ROS)contents. Results The myocardial arrangement of the rats in the Adr group was disordered, myocardial cell activity was lower, the mitochondrial membrane potential was lower,ROS production was higher, and NF-κB mRNA and protein contents were sub-stantially lower than those in the control group. The cardiomyocyte morphology was improved, cell activity was higher, mitochondrial mem-brane potential was increased,ROS production decreased, and NF-κB expression significantly increased in the Adr+Dex group compared with those in the control group. The mitochondrial membrane potential in the Adr+Dex+NF-κBi group was lower, and ROS generation was increased compared with the control group. The activity of the tumor cells in the Adr group was lower, and no statistically significant diffe-rences were found compared with that in the Adr+Dex group. Conclusion Treatment with Dex may not affect the chemotherapeutic effects of Adr.Dex administration may increase the myocardial mitochondrial membrane potential and reduce ROS generation by regu-lating NF-κB levels, thereby reducing Adr-induced myocardial damage.

Gastric cancer remains one of the most prevalent and lethal malignancies of the digestive tract worldwide, underscoring the urgent need for more effective targeted therapeutic strategies.Poly(ADP-ri-bose)polymerase(PARP)inhibitors have demonstrated remarkable efficacy in tumors with homologous recombination repair(HRR)deficiency;however, their clinical application in gastric cancer remains limited.Clinical evidence suggests that patients harboring Helicobacter pylori infection in combination with HRR gene mutations exhibit a significantly elevated risk of developing gastric cancer, thereby supporting the potential benefit of PARP inhibition in this setting. In this study,a kinase inhibitor library was screened in combination with the PARP inhibitor olaparib in gastric cancer cells. And we identify the cy-clin-dependent kinase 8/19(CDK8/19)inhibitor Senexin A as a compound that synergistically enhances the cytotoxic effect of PARP inhibition(P<0.05).Phenotypic validation using CCK-8 and colony for-mation assays demonstrated that the combination treatment significantly suppressed cellular proliferation and clonogenic potential compared to either monotherapy(P<0.0001).Mechanistically,alkaline comet assays revealed a significant increase in DNA damage in the combination treatment group relative to either single-agent group(P<0.0001),suggesting that the synergistic effect results from the exacerbation of DNA damage via impaired DNA repair mechanisms. In addition, treatment with CDK8/19 inhibitors a-lone markedly increased the formation of γH2AX and 53BP1 foci in irradiated gastric cancer cells(P<0.0001),indicating inhibition of DNA damage repair pathways.Transcriptome sequencing further re-vealed that CDK8/19 inhibition impacts critical cellular pathways, including DNA repair, cell cycle reg-ulation,and RNA splicing.Co-immunoprecipitation assays confirmed that inhibition of CDK8/19 kinase activity significantly reduces the phosphorylation level of PARP1,suggesting a potential regulatory inter-action.Immunohistochemical analysis of tumor and adjacent non-tumor tissues from gastric cancer pa-tients demonstrated that CDK8 is significantly overexpressed in tumor tissues, supporting its potential as both a prognostic biomarker and a therapeutic target.Collectively, this study elucidates a mechanistic ba-sis by which CDK8/19 inhibition enhances the sensitivity of gastric cancer cells to PARP inhibitors. These findings provide a strong rationale for the combined use of CDK8/19 and PARP inhibitors as a tar-geted therapeutic strategy and offer promising translational implications for advancing personalized medi-cine in gastric cancer treatment.
