Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Objective To evaluate the efficacy and safety of transcatheter aortic valve implantation(TAVI)for the treatment of primary aortic valve regurgitation(NAVR)and to compare the difference in the choice of prosthetic valve size and the difference in complications with aortic stenosis(AS).Methods According to the definition of Valve Academic Research Consortium(VARC-3),143 patients with NAVR/AS treated with TAVI and patients with NAVR treated with surgical aortic valve replacement(SAVR)at Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology,China, from March 2019 to September 2024 were selected, and clinical data on baseline, perioperative, and primary endpoint events were were retrospectively collected and compared. Results Forty-three patients with NAVR were treated with TAVI,with a device success rate of 86.0%and a surgical success rate of 95.3%.Subgroup comparisons:(1)NAVR-TAVI group than NAVR-SAVR group:patients in the TAVI group had a significantly shorter operative time than those in the SAVR group(P<0.001);complete left bundle branch block was more likely to occur after TAVI(P=0.042),and complete right bundle branch block was more likely to occur after SAVR(P=0.044).SAVR postoperatively The incidence of congestive heart failure was higher(P=0.013),and the mortality rate was significantly higher in the SAVR group than in the TAVI group(P=0.019).(2)NAVR-TAVI group than AS-TAVI group:the differences in access selection,THV size[28(22,34)mm vs.24(22,32)mm,P=0.044]and proportion of THV overdiameter[14%(7%,20%)vs.7%(3%,11%),P<0.001]were statistically significant.patients in AS and NAVR groups had 1 case of permanent pacing after TAVI treatment. In the AS and NAVR groups, there was 1 case of permanent pacemaker implantation after TAVI.2 patients in the AS group were converted to surgical treatment, and 6 patients died. Conclusions The use of"off-label"(transfemoral)and"on-label"(transapical)TAVI devices(both from domestic sources)is safer than SAVR for the treatment of NAVR,especially in elderly and high-risk patients.Compared with patients with AS treated with TAVI,larger diameter annulas are usually selected for NAVR,with higher rates of valve migration, but overall safety and efficacy are comparable to AS.

Aging is characterized by a gradual deterioration of the physiological integrity of cells, tissues, and or-gans,resulting in a decrease in the body's physiological functions and an acceleration of the onset of age-related diseases, ultimately leading to death. The aging of the liver, which is a critical metabolic organ,is closely linked to various chronic liver diseases, such as hepatitis, liver fibrosis, and cirrhosis, and it ex-acerbates their prognosis and is a primary risk factor for their development at all stages. Therefore,a comprehensive understanding of the causes, mechanisms, and potential therapeutic targets associated with liver aging holds significant clinical importance for delaying or potentially reversing liver aging and for treating chronic liver diseases.Stem cells, which are potential anti-aging agents, present a promising and effective alternative for managing liver aging. In this review,we systematically assess the driving factors, characteristics, and underlying mechanisms of liver aging. We then discuss the current status of the use of stem cells to mitigate liver senescence and address related liver diseases. The review reveals that a stem cell-based approach represents a promising therapeutic strategy for combating liver aging and associated diseases.

Objective:To explore the effect of electric acupuncture combined with Tianding acupoint on the efficacy, syndrome score, and quality of life of post-stroke intractable hiccup(IH).Methods:A total of 102 post-stroke IH patients admitted to our hospital from October 2022 to October 2023 were retrospectively included, and were divided into control group and observation group, with 51 cases in each group. The matched group patients were given conventional acupuncture treatment, while the observation group patients were given conventional electric acupuncture combined with Tianding acupoint treatment. The clinical efficacy, effectiveness, hiccup symptom scores before and Post-treatment,and quality of life score between two groups of patients. Results:The proportion of patients in the observation group who took immediate effect(27.45%vs.5.88%)and the proportion of patients who took effect within 24 hours of the first treatment(60.78%vs.17.65%)were higher than matched group. The average onset time[(32.14±9.72)h vs.(70.34±23.51)h]was significantly shorter than matched group(P<0.05).The treatment recovery rate of observation group was higher than matched group(80.39%vs.50.98%,P<0.05),and the total effective rate of treatment was not different from matched group(96.08%vs.86.27%,P>0.05).Post-treatment,both groups of patients showed a significant decrease in hiccup symptom scores, and there were significant differences at different time points(P<0.05).The hiccup symptom scores of the observation group patients at 1 d,3 d,and 5 d post-treatment were lower than matched group(P<0.05).Post-treatment,the overall scores of appetite, mental state, sleep, and quality of life in both groups increased, and the scores in the observation group were higher than matched group(P<0.05).Conclusion:The combination of electric acupuncture and Tianding acupoint has a significant therapeutic effect on post-stroke IH,which can quickly take effect, improve patients'clinical symptoms, and enhance their quality of life.

Objective:
This study aims to investigate the anatomical relationship among the nerve roots, intervertebral space, pedicles, and intradural rootlets of the cervical spine for improving operative outcomes and exploring neuroventral decompression approach in posterior endoscopic cervical discectomy (PECD).
Methods:
Cervical computed tomography myelography imaging data from January 2021 to May 2023 were collected, and the RadiAnt DICOM Viewer Software was employed to conduct multiplane reconstruction. The following parameters were recorded: width of nerve root (WN), nerve root-superior pedicle distance (NSPD), nerve root-inferior pedicle distance (NIPD), and the relationship between the intervertebral space and the nerve root (shoulder, anterior, and axillary). Additionally, the descending angles between the spinal cord and the ventral (VRA) and dorsal (DRA) rootlets were measured.
Results:
The WN showed a gradual increase from C4 to C7, with measurements notably larger in men compared to women. The NSPD decreased gradually from the C2–3 to the C5–6 levels. However, the NIPD showed an opposite level-related change, notably larger than the NSPD at the C4–5, C5–6, and C7–T1 levels. Furthermore, significant differences in NIPD were observed between different age groups and genders. The incidence of the anterior type exhibited a gradual decrease from the C2–3 to the C5–6 levels. Conversely, the axillary type exhibited an opposite level-related change. Additionally, the VRA and DRA decreased as the level descended, with measurements significantly larger in females.
Conclusion
A prediction of the positional relationship between the intervertebral space and the nerve root is essential for the direct neuroventral decompression in PECD to avoid damaging the neural structures. The axillary route of the nerve root offers a safer and more effective pathway for performing direct neuroventral decompression compared to the shoulder approach.
