The scientific research and innovation capabilities of medical students are intrinsically linked to the sustained and high-quality development of national healthcare initiatives.Cultivating outstanding medi-cal students with independent scientific capabilities and innovative consciousness is a critical component in the education and training of high-level medical professionals. Our investigation revealed that within the imperfections of the cultivating model, some faculty and students at medical schools have an insufficient understanding of scientific research and innovation and lack motivation for engaging in such activities, which hinder the progression of scientific research activities.Consequently,we initiated a teaching practice and exploratory study on the"tutorial system"aimed at fostering medical students'scientific research and innovation abilities.Based on the principle of"research informing teaching, teaching and research advan-cing together,"this study implements a"tutorial system"coordinated by tutors, supplemented by graduate and undergraduate student mentors,to cultivate innovative thinking, stimulate interest in scientific re-search,and enhance practical and research skills among medical students.Through collaborative efforts within"scientific research innovation teams,"various educational methods—including preliminary re-search,in-class and extracurricular activities,intra-group and inter-group interactions, and theoretical and practical applications—are employed to improve and strengthen the cultivation of medical students'scientif-ic research and innovation abilities. This study aims to provide valuable references for optimizing medical education management systems and enhancing the quality of medical student training.

Objective:To describe the prevalence and distributions of mood disorder in Inner Mongolia Au-tonomous Region, and analyze the related risk factors. Methods:The multistage stratified sampling method with un-equal probability was used to select permanent residents aged 18 years and over in Inner Mongolia Autonomous Re-gion.The Composite International Diagnostic Interview 3.0 was used as a diagnostic tool.Mood disorders were di-agnosed according to the Diagnostic and Statistical Manual of Mental Disorders Fourth Edition(DSM-Ⅳ).Single and multivariate analyses were used to investigate the related factors of mood disorders.Results:Totally,12 315 community residents were interviewed in the survey. The weighted 12-month prevalence and lifetime prevalence of mood disorder were 5.4%and 8.7%,respectively.Weighted 12-month prevalence of depressive disorder was 4.9%,and that of bipolar disorder was 0.3%.Among all subtypes of mood disorder, the 12-month prevalence rate of major depressive disorder(3.1%)was the highest.Multivariate logistic regression analysis showed that female, unmarried, separated or divorced, unemployment, family history, other mental disorders, sleep disorders and chronic diseases(OR=1.56,2.80,2.07,1.42,13.46,7.97,3.23,2.13)were risk factors of mood disorder, while aged 65 years and over(OR=0.44)was protective factor of mood disorders. The lifetime consultation rate in patients with mood disorders was 15.5%,the rate of psychiatric consultation was 3.7%,the rate of medication was 1.8%.Con-clusion:It indicates that female residents and people who are unmarried, separated and divorced, unemployed, with family history, suffering from other mental disorders, suffering from sleep disorders, and suffering from chronic dis-eases may be high risk groups of mood disorders, and the utilization rate of health services is rather low in Inner Mongolia Autonomous Region.

Objective To evaluate the safety and efficacy of valve-in-valve transcatheter mitral valve replacement(ViV-TMVR)in patients with bioprosthetic valve degeneration who are at high surgical risk. Methods This study is a multi-center,retrospective cohort analysis of 20 consecutive patients who underwent transseptal ViV-TMVR using the Edwards SAPIEN 3 transcatheter heart valve(THV).The primary endpoints include technical success and procedural success, both defined according to the Mitral Valve Academic Research Consortium(MVARC)criteria,as well as mortality and functional change assessed based on New York Heart Association(NYHA)classification at 30-days and six months post-procedure.Clinical follow-up assessments are conducted at 30-days and six months. Results From February 2021 to October 2022,a total of 20 patients with symptoms of bioprosthetic valve degeneration were enrolled across nine sites in China. The patients had a mean age of(73.5±5.5)years,with 85.0%being females and 70.0%classified as NYHA class Ⅲ/Ⅳ.The study achieved a 100.0%technical success rate and a 90.0%procedural success rate finally. All patients remained alive during the 30-day follow-up period. However, six months post-intervention,two patients(10.0%)were re-hospitalized due to heart failure, and sadly, one of them(5.0%)died.None of the patients reported any adverse events related to ViV-TMVR during the follow-up period.Notably, there was a significant improvement in NYHA class compared to baseline(P=0.0004)at six-month follow-ups.Conclusions The transseptal ViV-TMVR technique proved to be highly successful and was associated with significant improvement in NYHA class function. These findings strongly suggest that it serves as a safe and efficient treatment alternative for high-risk patients suffering from bioprosthetic valve degeneration.

Objective To analyze the correlation between lesion volume, lesion mass, and maximum lesion diameter in the assessment of advanced hepatocarcinoma with lung metastasis, and to evaluate the application value of total volume response and total mass response of lung metastatic lesions in efficacy assessment. Methods A retrospective analysis was conducted on the CT imaging data of 20 patients clinically confirmed with hepatocarcinoma and lung metastases, followed by subsequent follow-up to monitor their survival outcomes.Volume measurement software was used to measure the volume of lesions before and after treatment. We recored lesion diameter, volume measurements and CT values, calculated the mass of the lesions. The correlation between lesion volume, mass and diameter was analyzed,as well as the correlation between the change rates of volume, mass and lesion diameter.Additionally, the total volume and total mass of all lesions were calculated. The correlation between the change rates of total volume/total mass and the change rate of pulmonary lesion diameter under the RECIST 1.1 criteria,as well as the correlation with changes in patients'tumor markers, were analyzed.Furthermore, the overall volume response and overall mass response of lesions were evaluated based on changes in total volume and total mass, and their consistencies with the RECIST 1.1 criteria for efficacy evaluation were analyzed. Finally, univariate Cox regression analysis was performed to explore the association between these variables and patient survival outcomes. Results There was strong correlation between lesion volume, mass and tumor diameter(r=0.771,0.775),between the rate of change in mass and the rate of change in lesion diameter(r=0.846),and between the rates of change in total volume/total mass and the rate of change in pulmonary lesion diameter under the RECIST 1.1 criteria(r=0.800,0.896).The correlation between the rates of change in total volume/total mass and patients'tumor markers was not statistically significant. There was moderate correlation between the rate of change in volume and the rate of change in lesion diameter(r=0.692).The evaluation results of total volume response and total mass response for pulmonary lesions in advanced hepatocarcinoma with lung metastasis were generally consistent with the RECIST 1.1 criteria(Kappa=0.486,0.426).Univariate Cox regression analysis revealed that total lesion volume(P=0.047)and total lesion mass(P=0.049)were independent prognostic factors for survival outcomes. Conclusion Lesion volume, mass, and diameter,as well as their respective change rates, were found to be interrelated.Furthermore, total lesion volume and total lesion mass were identified as independent prognostic factors for survival outcomes. The total volume response and total mass response are promising evaluation methods in evaluating the efficacy of lung metastasis of hepatocarcinoma, which are different from the RECIST 1.1 evaluation criteria.
