1.Best evidence summary of weight management in patients undergoing endocrine therapy for prostate cancer

Objective·To retrieve and evaluate evidence related to weight management during endocrine therapy for prostate cancer patients,and summarize the best evidence available.Methods·The PIPOST model was adopted to construct evidence-based questions.Following the"6S"model of evidence resources,a systematic computer-based search was conducted to retrieve relevant evidence on weight management during prostate cancer endocrine therapy from various databases and websites,including Guidelines International Network,BMJ Best Practice,UpToDate,JBI Evidence-Based Healthcare Center Database,American Guidelines Network,Medlive,Cochrane Library,PubMed,CINAHL,Web of Science,Embase,CNKI,Chinese Biomedical Literature Database,Wanfang Database,and VIP Database,as well as the websites of the American Urological Association,European Urological Association,and Urological Branch of Chinese Medical Association.The evidence retrieved included guidelines,clinical decisions,recommended practices,expert consensuses,evidence summaries,systematic reviews,and original studies.The search period spanned from database establishment to December 2023.Two researchers independently evaluated the quality of the included literature.The included articles were read one by one and summarized according to themes.Priority was given to high-level,high-quality,and newly published authoritative literature during evidence screening and extraction.The included and extracted evidence was uniformly classified into levels 1?5 according to the JBI evidence pre-classification and evidence recommendation level system,and the evidence recommendations were divided into strongly recommended(Grade A)and weakly recommended(Grade B).Results·A total of 12 articles were retrieved,consisting of 4 guidelines,3 expert consensuses,2 evidence summaries,2 randomized controlled trials,and 1 quasi-experimental study.All 12 articles were rated as high quality and were included.The best evidence was summarized into 8 aspects:weight management assessment and monitoring,multidisciplinary collaboration,personalized support,family involvement strategies,nutrition management strategies,exercise management strategies,health education,and follow-up management.Conclusion·The best evidence for weight management during endocrine therapy for prostate cancer is summarized into eight aspects.In clinical application,specific clinical situations and patient characteristics should also be considered to improve the effectiveness of clinical interventions and the quality of care.
2.Expression Levels of KPNB1 and MERTK in NSCLS Tissues and Correlation with the Clinical Pathological Features and the Prognosis of Progression Free Survival

Objective To study the expression levels of karyopherin β1(KPNB1) and Mer receptor tyrosine kinase (MERTK) in non-small cell lung cancer (NSCLC) tissues and their correlation with clinical pathological features,as well as the prognosis of progression-free survival. Methods A total of 118 NSCLC patients who visited Ordnance Industry 521 Hospital from February 2018 to February 2020 were selected. Immunohistochemical methods were used to detect the expression of KPNB1 and MERTK in NSCLC cancer and paracancer tissue. Kaplan-Meier survival curve analysis was conducted to assess the impact of KPNB1 and MERTK on the progression-free survival prognosis of NSCLC patients. COX regression analysis was performed to identify factors affecting the prognosis of progression-free survival in NSCLC patients. Results The positive rates of KPNB1 (61.02%) and MERTK (62.71%) in cancer tissues were higher than those in adjacent tissues (8.48%,6.78%),and the differences were statistically significant (x2=69.945,81.408,all P<0.001). There was a significant positive correlation between KPNB1 and MERTK expression in the organization (r=0.744,P<0.001). The positive rates of KPNB1(84.21%,82.50%),MERTK (84.21%,85.00%)in NSCLC tissues with lymph node metastasis and TNM stage ⅢA were higher than those without lymph node metastasis(50.00%,52.50%) and TNM stage Ⅰ~Ⅱ(50.00%,51.28%),and the differences were statistically significant (x2=11.078~12.855,all P<0.001). The 3-year progression-free survival rates of KPNB1and MERTK positive and negative groups were 26.39% (19/72) and 56.52% (26/46),27.03% (20/74) and 56.82% (25/44),respectively,with statistically significant differences (Log Rankx2=0.980,8.463,P=0.001,0.004) . KPNB1 positive,MERTK positive,lymph node metastasis and TNM stage ⅢA were risk factors for progression free survival prognosis in NSCLC patients (waldx2=7.810~10.906,all P<0.001). Conclusion The expression of KPNB1 and MERTK is elevated in NSCLC,both of which are related to TNM staging and lymph node metastasis. They are new biomarkers for evaluating the progression-free survival prognosis of NSCLC.

Objective To summarize the changing prevalence of carbapenem resistance in Enterobacterales based on the data of CHINET Antimicrobial Resistance Surveillance Program from 2015 to 2021 for improving antimicrobial treatment in clinical practice.Methods Antimicrobial susceptibility testing was performed using a commercial automated susceptibility testing system according to the unified CHINET protocol.The results were interpreted according to the breakpoints of the Clinical & Laboratory Standards Institute(CLSI)M100 31st ed in 2021.Results Over the seven-year period(2015-2021),the overall prevalence of carbapenem-resistant Enterobacterales(CRE)was 9.43%(62 342/661 235).The prevalence of CRE strains in Klebsiella pneumoniae,Citrobacter freundii,and Enterobacter cloacae was 22.38%,9.73%,and 8.47%,respectively.The prevalence of CRE strains in Escherichia coli was 1.99%.A few CRE strains were also identified in Salmonella and Shigella.The CRE strains were mainly isolated from respiratory specimens(44.23±2.80)%,followed by blood(20.88±3.40)%and urine(18.40±3.45)%.Intensive care units(ICUs)were the major source of the CRE strains(27.43±5.20)%.CRE strains were resistant to all the β-lactam antibiotics tested and most non-β-lactam antimicrobial agents.The CRE strains were relatively susceptible to tigecycline and polymyxins with low resistance rates.Conclusions The prevalence of CRE strains was increasing from 2015 to 2021.CRE strains were highly resistant to most of the antibacterial drugs used in clinical practice.Clinicians should prescribe antimicrobial agents rationally.Hospitals should strengthen antibiotic stewardship in key clinical settings such as ICUs,and take effective infection control measures to curb CRE outbreak and epidemic in hospitals.

Objective To characterize the changing species distribution and antibiotic resistance profiles of respiratory isolates in hospitals participating in the CHINET Antimicrobial Resistance Surveillance Program from 2015 to 2021.Methods Commercial automated antimicrobial susceptibility testing systems and disk diffusion method were used to test the susceptibility of respiratory bacterial isolates to antimicrobial agents following the standardized technical protocol established by the CHINET program.Results A total of 589 746 respiratory isolates were collected from 2015 to 2021.Overall,82.6%of the isolates were Gram-negative bacteria and 17.4%were Gram-positive bacteria.The bacterial isolates from outpatients and inpatients accounted for(6.0±0.9)%and(94.0±0.1)%,respectively.The top microorganisms were Klebsiella spp.,Acinetobacter spp.,Pseudomonas aeruginosa,Staphylococcus aureus,Haemophilus spp.,Stenotrophomonas maltophilia,Escherichia coli,and Streptococcus pneumoniae.Each microorganism was isolated from significantly more males than from females(P<0.05).The overall prevalence of methicillin-resistant S.aureus(MRSA)was 39.9%.The prevalence of penicillin-resistant S.pneumoniae was 1.4%.The prevalence of extended-spectrum β-lactamase(ESBL)-producing E.coli and K.pneumoniae was 67.8%and 41.3%,respectively.The overall prevalence of carbapenem-resistant E.coli,K.pneumoniae,Enterobacter cloacae,Pseudomonas aeruginosa,and Acinetobacter baumannii was 3.7%,20.8%,9.4%,29.8%,and 73.3%,respectively.The prevalence of β-lactamase was 96.1%in Moraxella catarrhalis and 60.0%in Haemophilus influenzae.The H.influenzae isolates from children(<18 years)showed significantly higher resistance rates to β-lactam antibiotics than the isolates from adults(P<0.05).Conclusions Gram-negative bacteria are still predominant in respiratory isolates associated with serious antibiotic resistance.Antimicrobial resistance surveillance should be strengthened in clinical practice to support accurate etiological diagnosis and appropriate antimicrobial therapy based on antimicrobial susceptibility testing results.
5.Comprehensive Evaluation of the Medical Service Capacity of the First Batch of County-Level Medical Subcenters in Guizhou Province

Objective:To comprehensively evaluate the medical service capacity of 50 first batch county medical sub-centers in Guizhou Province,it provides the basis and references for further research on the construction of county medical sub-centers in Guizhou province and improvement of the medical service capacity of township health centers(community health service centers)in Guizhou Province.Methods:Through TOPSIS method,RSR method and the fuzzy combination method,the medical service capacity of the primary medical institutions in the county medical sub-center was evaluated.Results:Among the first batch of 50 county-level secondary medical centers,the top three in terms of medical service capabilities were G18,G6,and G36,while the last three were G27,G15,and G22.Conclusion:There were significant differences in the medical and health service capacity among the 50 sub-centers.It is suggested to continue to increase the support for sub-centers,formulate relevant policies according to local conditions and time conditions,and improve the medical service capacity of sub-centers.

7.Mechanosensory activation of Piezo1 via cupping therapy: Harnessing neural networks to modulate AMPK pathway for metabolic restoration in a mouse model of psoriasis.

OBJECTIVE:
Psoriasis, a common chronic inflammatory skin condition with genetic underpinnings, is traditionally managed with cupping therapy. Although used historically, the precise mechanical effects and therapeutic mechanisms of cupping in psoriasis remain largely unexamined. This study aimed to evaluate cupping therapy's efficacy for psoriasis and investigate its role in modulating inflammatory responses and cellular metabolism.
METHODS:
Psoriasis was induced in mice using topical imiquimod (IMQ). The effects of cupping on psoriatic lesions were assessed using the Psoriasis Area and Severity Index score, histology, immunohistochemistry, and immunofluorescence staining. polymerase chain reaction sequencing (RNA-seq) and Western blotting were conducted to examine changes in mRNA expression and the AMP-activated protein kinase (AMPK) signaling pathway.
RESULTS:
Cupping therapy significantly reduced inflammation, epidermal thickness, and inflammatory cell infiltration in mice with IMQ-induced psoriasis. Immunohistochemistry and immunofluorescence showed lower expression of inflammatory markers and a shift in T-cell populations. RNA-seq and Western blotting indicated that cupping upregulated Piezo1 and activated the AMPK pathway, improving energy metabolism in psoriatic skin.
CONCLUSION
Cupping therapy reduces epidermal hyperproliferation and inflammation in psoriasis, rebalancing the local immune microenvironment. Mechanistically, cupping promotes calcium influx via Piezo1, activates AMPK signaling, and supports metabolic homeostasis, suggesting therapeutic potential for psoriasis. OBJECTIVE:
This study aimed to explore the association between body mass index (BMI) and mortality based on the 10-year population-based multicenter prospective study.
METHODS:
A general population-based multicenter prospective study was conducted at four sites in rural China between 2013 and 2023. Multivariate Cox proportional hazards models and restricted cubic spline analyses were used to assess the association between BMI and mortality. Stratified analyses were performed based on the individual characteristics of the participants.
RESULTS:
Overall, 19,107 participants with a sum of 163,095 person-years were included and 1,910 participants died. The underweight (< 18.5 kg/m 2) presented an increase in all-cause mortality (adjusted hazards ratio [ aHR] = 2.00, 95% confidence interval [ CI]: 1.66-2.41), while overweight (≥ 24.0 to < 28.0 kg/m 2) and obesity (≥ 28.0 kg/m 2) presented a decrease with an aHR of 0.61 (95% CI: 0.52-0.73) and 0.51 (95% CI: 0.37-0.70), respectively. Overweight ( aHR = 0.76, 95% CI: 0.67-0.86) and mild obesity ( aHR = 0.72, 95% CI: 0.59-0.87) had a positive impact on mortality in people older than 60 years. All-cause mortality decreased rapidly until reaching a BMI of 25.7 kg/m 2 ( aHR = 0.95, 95% CI: 0.92-0.98) and increased slightly above that value, indicating a U-shaped association. The beneficial impact of being overweight on mortality was robust in most subgroups and sensitivity analyses.
CONCLUSION
This study provides additional evidence that overweight and mild obesity may be inversely related to the risk of death in individuals older than 60 years. Therefore, it is essential to consider age differences when formulating health and weight management strategies.
