1.Ancient and Modern Application and Key Information Analysis of Classic Formula Erchentang

Erchentang is a classic formula widely used by medical practitioners throughout history. In this paper,ancient and modern literature of Erchentang were collected, and bibliometrics was employed to analyze its historic evolution,prescription meaning,herbs origin, processing method,preparation methods, and clinical application. A total of 84 pieces of data were collected, and 58 pieces of data involving 53 ancient medical Chinese books were screened, sorted, and processed. Combined with research of modern scholars,the research has found that the Erchentang originated from the Taiping Huimin Huiye Shijie Fang compiled by the Imperial Medical Bureau of the Song Dynasty. The basic information about the origin of the drugs is quite clear. Pinelliae rhizoma in the formula is the dried tuber of Pinellia ternata. Citri exocarpium rubrum is the dried mature peel of Citrus reticulata and its cultivated varieties, with the inner white membrane removed. Poria is the whitest dry sclerotia of Poria cocos; Glycyrrhizae radix et rhizoma is the dried root and rhizome of the Glycyrrhiza uralensis. The dosage is 5.70 g Pinelliae rhizome and Citri exocarpium rubrum, 3.43 g Poria, and 1.69 g Glycyrrhizae radix et rhizoma praeparata cum melle. During the decoction process, the above-mentioned herbs should be chopped, with 300 mL water, 7 g ginger in thick slices, and 2 g Mume fructus added, and it was then simmered together to 180 mL. After removing the medicinal residue, it can be taken warmly. Erchentang has the effect of drying dampness and resolving phlegm, regulating Qi and harmonizing the middle. It can be used in treating the syndrome of phlegm and dampness,as well as symptoms such as frequent cough,white phlegm,fullness in chest and diaphragm,nausea and vomiting,limb drowsiness,anorexia,dizziness,palpitations,white and greasy tongue coating, and slippery pulse. The above results provide reference for future research and development of Erchentang.
2.Analysis of potential profiles and influencing factors of discharge readiness of patients after 125I seed implantation

Objective To analyze the potential profiles of discharge readiness in patients after 125I seed implantation and the influencing factors of different profiles.Methods A total of 221 patients who underwent 125I seed implantation in the Interventional Therapy Department,Zhejiang Cancer Hospital from October 2023 to October 2024 were selected as the research objects by convenient sampling method.The general information questionnaire,discharge readiness scale and social support rating scale were used for investigation.Mplus8.3 software was used to conduct latent profile analysis on the discharge readiness of patients after 125I seed implantation,and multivariate Logistic regression model was used to analyze the influencing factors of different discharge readiness profiles.Results The discharge readiness of patients after 125I seed implantation was divided into three potential profiles,"low discharge readiness-low self-care group"(10.41%),"medium discharge readiness-low others care group"(32.58%)and"high discharge readiness-high emotional support group"(57.01%).Logistic regression analysis showed that the number of short-term complications and the level of social support were the influencing factors of the potential profiles of discharge readiness in patients after 125I seed implantation(P<0.05).Conclusion There is heterogeneity in the discharge readiness of patients after 125I seed implantation,and personalized intervention strategies should be formulated according to different profiles.
3.Evaluation value of urinary 8-oxo-7, 8-dihydroguanosine in the short-term prognosis of sepsis in frail elderly patients

Objective:To investigate the significance of urinary 8-oxo-7, 8-dihydroguanosine(8-oxoGuo)in assessing the short-term prognosis of sepsis in frail elderly patients.Methods:We conducted a cross-sectional study involving 62 frail elderly patients diagnosed with sepsis who were admitted to the Emergency Intensive Care Unit(EICU)at Beijing Hospital between March 2021 and March 2022.Based on their 28-day prognosis, the patients were categorized into two groups: those who died and those who survived.Upon admission, we collected urine samples and clinical data from both groups.We employed isotope dilution high-performance liquid chromatography-mass spectrometry to measure the levels of the RNA oxidation marker 8-oxoGuo in the urine.Results:A total of 62 frail elderly patients[aged(85.1±6.3)years]diagnosed with sepsis were included in the study, comprising 36 patients in the 28-day mortality group and 26 patients in the survival group.Univariate analysis revealed that the survival group had significantly lower body temperature, blood calcitonin(PCT)levels, sequential organ failure assessment(SOFA)scores, and urinary 8-oxoGuo levels compared to the mortality group.Additionally, the survival group exhibited a higher mean arterial pressure(MAP)than the mortality group, with all differences reaching statistical significance(all P<0.05).Spearman correlation analysis indicated that urinary 8-oxoGuo levels were positively correlated with both PCT and SOFA scores in frail elderly sepsis patients( r=0.426, 0.768, both P<0.05).Furthermore, logistic regression analysis identified urinary 8-oxoGuo and SOFA as independent risk factors for 28-day mortality in this population( OR=1.936, 1.427; P=0.006, 0.002).The area under the receiver operating characteristic curve(AUC)for urinary 8-oxoGuo and SOFA in predicting the 28-day prognosis of frail elderly sepsis patients was 0.761 and 0.741, respectively, both demonstrating statistical significance(both P<0.001). Conclusions:Our findings suggest that urinary 8-oxoGuo possesses strong predictive value for the short-term prognosis of sepsis in this vulnerable population.
4.Current situation and prospect of the construction of an integrated digital and intelligent diagnosis and treatment center for foot and ankle surgery

With the specialization of orthopedics and the increasing complexity of foot and ankle disease profiles, traditional fragmented diagnosis and treatment models face challenges such as low efficiency and poor patient experience. By integrating a multidisciplinary collaboration model with digital-intelligent technologies such as artificial intelligence, big data, and 3D printing, the integrated diagnosis and treatment center emerges as an innovative, modern healthcare service model centered on the patient. It achieves a "one-stop" closed-loop management spanning specialized outpatient care, precision surgery, and remote rehabilitation. Intelligent surgical planning, robot-assisted therapy, and digital rehabilitation systems have significantly enhanced the precision and continuity of care. This article details the current construction status and development trends of integrated digital-intelligent diagnosis and treatment centers in the field of foot and ankle surgery. As medical models and innovative technologies continue to advance, these centers will play an even greater role in the future, bringing improved health outcomes to patients.
5.Serum growth hormone of children with infectious pneumonia and its association with blood biochemistry and body immunity

OBJECTIVE To explore the serum growth hormone(GH)of the children with infectious pneumonia and analyze its association with blood biochemistry and body immunity.METHODS A total of 211 children with infec-tious pneumonia who were treated in Huaihe Hospital,Henan University from Jan.2023 to Jan.2024 were recrui-ted as the research subjects and were divided into the mild group with 156 cases and the severe group with 55 cases according to the severity of disease.The change of serum GH and the levels of blood biochemical indexes and im-munity indexes were observed and compared between the two groups.Multivariate logistic regression analysis was performed for the influencing factors,receiver operating characteristic(ROC)curves were drawn to analyze the predictive efficiencies.Linear regression analysis was performed to observe the relationship between the infectious pneumonia and the blood biochemistry and body immunity.RESULTS The GH level of the severe group was(18.34±4.33)μg/L,higher than(10.16±2.84)μg/L of the mild group(t=15.853,P<0.001).There were sig-nificant differences in the blood biochemistry indexes,cellular and humoral immunity indexes between the severe group and the mild group(P<0.05).The difference values of changes of C-reactive protein(CRP),CD4+and immunoglobulin(Ig)G were the most(t=16.023,87.639,111.957,all P<0.001).ROC curve analysis showed that the area under the curve(AUC)of CD23+was the highest,with the sensitivity 100.00%,the specificity 98.88%.The result of the linear regression analysis showed that the GH was only positively correlated with the cellular immunity index CD23+(P<0.05).CONCLUSION The GH level of the children with infectious pneumonia is reduced;the chang of GH level is closely associated with the blood biochemical indexes CRP and the body im-munity of the children.

9.Optimization of oral fat tolerance test

Objective:To compare the effects of different test meals on postprandial triglycerides and to optimize the standard meal composition and the blood sampling protocol for the oral fat tolerance test.Methods:This study is a prospective, open-label, randomized, cross-over trial. In March 2023, 36 volunteers were recruited in Hebei General Hospital. They underwent a health examination and oral glucose tolerance test. Twenty-six healthy volunteers(11 males and 15 females) were included in this study, with an average age of(39.08±4.56) years. Each volunteer received 75 g protein meal, 75 g fat meal, 700 kcal fixed-calorie high-fat mixed meal, and a high-fat mixed meal with energy adjusted based on 10 kcal/kg body weight. A one-week washout period of regular diet was applied before each trial. Blood was collected at fasting status and 1, 2, 3, 4, 5, and 6 hours after a meal to detect serum triglycerides, total cholesterol, low density lipoprotein-cholesterol(LDL-C), high density lipoprotein-cholesterol(HDL-C), glucose, and insulin. The variations of postprandial metabolic indicators over time following the consumption of different test meals were analyzed. The disparities in postprandial metabolic responses between the two types of mixed meals were compared.Results:The protein meal, fat meal, fixed-calorie high-fat mixed meal, and adjusted-calorie high-fat mixed meal resulted in postprandial triglyceride increases of 22.45%, 115.40%, 77.14%, and 63.63%, and insulin increase of 560.43%, 85.69%, 554.18%, and 598.97%, respectively, and with reductions in total cholesterol, LDL-C, and HDL-C ranging from 5.64%-21.81%, respectively. The blood glucose changed slightly. Changes in metabolic indicators mainly occured within 4 hours. The comparison of the characteristics of postprandial triglycerides between the two high-fat mixed meals showed no statistically significant differences( P>0.05). Conclusion:A standardize protocol with a 700 kcal fixed-calorie high-fat mixed meal as test meal, and blood lipid levels measured at fasting and at 1, 2, 3, and 4 hours after consumption, can serve as an optimized approach for oral fat tolerance test.
10.Discussion on Disease Mechanism of Autism Spectrum Disorders from the Concepts of “Vital Activity” and “Qi Configuration”

This paper explored the disease mechanism of autism spectrum disorders (ASD) from the perspectives of “vital activity” and “qi configuration”, and it is believed that “vital activity” represents the internal regulatory mechanisms of the human body, while “qi configuration” represents the ability of the body to communicate and adapt to the external environment. Abnormal genetic factors lead to the extinction of vital activity in children with ASD, resulting in increased susceptibility to ASD. Environmental instability leads to the solitary qi configuration in ASD, triggering and exacerbating the manifestations of ASD on the basis of genetic susceptibility. In addition, epigenetic mechanisms also play an important role in the pathogenesis of ASD. Imbalances in vital activity and disruptions in qi configuration result in failure in qi transformation of zang-fu organs, with abnormal symptoms manifested through the five orifices. It is proposed that the treatment of ASD should aim to achieve a harmonious interaction between “vital activity” and “qi configuration” to accelerate the recovery of affected children.
