Opioid-free anesthesia(OFA)is a multimodal analgesic strategy that combines multiple non-opioid drugs and/or techniques to obtain high-quality anesthesia.With the popularization of the concept of enhanced recovery after surgery(ERAS),OFA has become a common concern hot spot for perioperative phy-sicians.OFA is in line with the ERAS concept,and under multimodal anesthesia and pain management,it can reduce surgical stress,inflammation reaction and postoperative complications,significantly improve the prog-nosis of patients,promote postoperative rapid recovery and reduce the burden of medical and health resources.This article reviews the adverse reactions of opioids,OFA concept and indications,the clinical application of OFA in recent years,the existing controversies and problems to be solved.
2."Four Common Characteristics" of Liver and Eyes and Research Ideas Inspired by "Liver Opens at Eyes"

The theory of "liver opens at the eyes" was first seen in Yellow Emperor's Internal Canon of Medicine, which is the ancient people's summary of the connection between the liver and the eyes. The theory of "liver opens at the eyes" suggests the characteristic of "co-damage and co-recover of liver and eyes". It has been found in clinical practice that liver diseases and eye diseases often occur together, and "liver and eyes co-recover" is an ideal choice. The key to achieving "liver and eyes co-recover" is to analyze its pharmacological material basis and mechanism. With the development of modern medicine, more and more evidence indicates that the liver and eyes have complex and close relationships in physiological and pathological aspects. In a pathological state, there is a phenomenon of "liver and eyes co-damage", and after the intervention of traditional Chinese medicine, "liver and eyes co-recover" occurs. "Liver and eyes co-damage and co-recover" can be explained through the "co-material basis and co-action mechanism". On this basis, the research group tentatively proposed that the liver and eyes had "four common characteristics" (4CCs), namely "co-damage, co-recover, co-material basis, and co-action mechanism" from the theoretical connotation of traditional Chinese medicine, clinical practice, and molecular biology. Additionally, the group also took the intervention of Prunella vulgaris, traditional Chinese medicine, for removing liver fire and improving eyesight on immune liver injury (ILI) and allergic conjunctivitis (AC) as examples to analyze 4CCs. This project aims to deeply analyze the scientific connotation of the theory of "liver opens at the eyes", reveal the common characteristics and biological essence of liver and eyes, explore a new research paradigm of "liver and eyes co-recover", and provide a reference for the study of common problems of multi-organ associated diseases.
3.Hematopoietic stem cell transplantation of non-cryopreserved sibling umbilical cord blood for major thalassaemia in children: a report of 9 cases

Objective:To explore the safety and advantages of non-cryopreserved sibling umbilical cord blood hematopoietic stem cell transplantation for major thalassaemia in children.Methods:From October 2016 to June 2021, 9 patients with major beta thalassaemia received non-cryopreserved hematopoietic stem cell transplantation of sibling umbilical cord blood at Zhongshan Hospital of Xiamen University. The pretreatment scheme, the process of stem cell implantation and follow-up were analyzed and summarized.Results:Among the 9 cases, there were 5 males and 4 females with a median age of 4(2~11)years. Median level of ferritin was 2 997(1 936~5 512)μg/L. At gestational weeks 12~16, each patient's mother underwent villi testing to confirm that the donor without thalassaemia major was complete HLA-matched with the patient. All of them received an intensive conditioning regimen made up of cyclophosphamide(CTX), fludarabine and busulfan(Bu). Graft-versus-host disease(GVHD) was prevented by cyclosporine A(CSA)and mycophenolate mofetil(MMF)with or without methotrexate(MTX). Except for one failed implant, 8 cases were successfully engrafted. Median time of neutrophil implantation was 19.5(15~26)days, median time of platelet implantation 32(22~34)days and median time of erythrocyte implantation 30.5(18~37)days. Up until September 1, 2021, the median follow-up period was 27(3~59)months and the rate of successful engraftment 88.89%. There was no transplant-related mortality. Overall survival was 100% and thalassaemia-free survival 88.89%. Two patients developed grades Ⅱ skin acute GVHD(22.2%). No grade Ⅲ-Ⅳ GVHD or chronic GVHD occurred. Epstein-Barr virus infection occurred in 1 case.No infection of cytomegalovirus occurred.Conclusions:For major thalassaemia in children, stem cell transplantation of non-cryopreserved sibling cord blood is both safe and feasible with a high implantation rate and a low incidence of GVHD.
4.Mediating role of social support between mindfulness and perceived pressure in young and middle-aged patients with inflammatory bowel disease

Objective:To explore the mediating role of social support between mindfulness and perceived pressure in young and middle-aged patients with inflammatory bowel disease (IBD) .Methods:From January to September 2021, the convenient sampling method was adopted to select 350 patients with inflammatory bowel disease who were treated in 5 ClassⅢ Grade A hospitals in Shanghai. The general information questionnaire, Mindful Attention Awareness Scale (MAAS) , Social Support Ratings Scale (SSRS) and Chinese Perceived Stress Scale (CPSS) were used for survey. Pearson correlation analysis was used to test the correlation between variables. AMOS 22.0 was used to establish the structural equation model, the maximum likelihood method was used to fit and modify the data and the Bootstrap method was used to test the mediation effect. A total of 350 questionnaires were distributed in this study and 324 valid questionnaires were recovered, with an effective recovery rate of 92.57%.Results:The total score of perceived stress in young and middle-aged IBD patients was (27.81±3.86) . Social support, mindfulness and perceived stress were negatively correlated ( r=-0.306, -0.220, P<0.01) . Social support played a partial mediating role between mindfulness and perceived stress, accounting for 55.13% of the total effect. Conclusions:The perceived stress level of young and middle-aged IBD patients is relatively high, which is not optimistic. Mindfulness affects perceived stress in young and middle-aged IBD patients through social support. Clinical workers need to pay attention to condition of mindfulness and social support of patients and take targeted psychological interventions to reduce their perceived stress.
5.Current status and influencing factors of cancer-related fatigue in patients receiving chemotherapy for prostate cancer

Objective:To explore the current status and influencing factors of cancer related fatigue (CRF) in patients receiving chemotherapy for prostate cancer.Methods:Totally 165 patients with prostate cancer who received chemotherapy in the Department of Urinary Surgery at Tenth People's Hospital of Tongji University from March 2018 to November 2019 were selected by convenient sampling and investigated with the general information questionnaire, Brief Fatigue Inventory (BFI) , Social Support Revalued Scale (SSRS) and International Prostate Symptom Scores (IPSS) . The influencing factors of severe CRF in patients receiving chemotherapy for prostate cancer were analyzed.Results:Among the 165 patients, 139 had fatigue, including 77 mild fatigue and 62 moderate to severe fatigue cases. Univariate analysis showed that there were statistically significant differences in the degree of CRF among patients receiving chemotherapy for prostate cancer with different appetite, duration of disease, duration of chemotherapy, level of social support, and urinary system symptoms (χ 2=7.45, 14.37, 9.93, 6.87, 25.65; P< 0.05) . Multivariate regression analysis revealed that duration of the disease, duration of chemotherapy, level of social support and urinary system symptoms were the influencing factors of moderate to severe CRF in patients receiving chemotherapy for prostate cancer ( P< 0.05) . Conclusions:CRF is a common problem in patients undergoing chemotherapy for prostate cancer, which is affected by many factors. Clinical medical staff should formulate more targeted individualized treatment and nursing plans to alleviate the symptoms of CRF and further improve the quality of life of the patients based on their actual situation.
6.Qualitative research on the quality evaluation of hospital-community linked transitional care for patients with heart failure

Objective:To understand the experience and suggestions of medical and nursing staff on the quality evaluation of hospital-community linked transitional care for patients with heart failure and to provide a reference for establishing the quality evaluation index system of hospital-community linked transitional care for patients with heart failure.Methods:From October to November 2019, a total of 10 nursing managers, doctors working in chest pain centers of a ClassⅢ hospital and nursing managers in a community hospital that had carried out hospital-community linkage transitional care in Shanghai were selected as interviewees by the objective sampling method. Qualitative research method was adopted to collect data through face-to-face and semi-structured in-depth interviews, and Colaizzi 7-step analysis method was adopted for analysis.Results:A total of 5 themes were extracted, namely, outcome index of patients with heart failure was the focus of quality evaluation, process quality evaluation was the key point, structural quality should be paid enough attention, index characteristics determined the evaluation time, and quality evaluation was still faced with many difficulties.Conclusions:Topics related to hospital-community linked transitional care quality assessment extracted from the perspective of medical and nursing staff can provide a reference basis for building a scientific and widely applicable index system and promote the continuous improvement of hospital-community linked transitional care for patients with heart failure.
7.Research progress of continuous care quality evaluation indexes among heart failure patients

Heart failure is a serious threat to human health and a huge financial burden on their families.Continuous care plays an important role in the rehabilitation of heart failure. Many hospitals are actively exploring and implementing the continuous care for heart failure patients, and standardizing the quality indexes of heart failure care has become an urgent need. This article reviews the related researches on the evaluation indexes of continuous care quality for patients with heart failure from three aspects, structure, process and results and presents the research deficiencies and future research directions so as to provide a reference for the construction of targeted and standardized evaluation indexes of continuous care for patients with heart failure in the future.
8. Visual analysis of the research literature related to narrative nursing in China

Objective:
To analyze the research hotspots of domestic narrative nursing and explore its related research trends which provide reference and guidance for its application and development in China.
Methods:
Wanfang, CNKI, VIP, CBM and other databases were retrieved from inception until October 2018. Word frequency analysis and graph cluster analysis were performed on the main information using Excel, Bicomb, Ucinet, Netdraw and gCLUTO software.
Results:
A total of 649 articles were included. The result of visual analysis showed that the annual amount of publications showed an upward trend. The articles distributed abroad in many different journals which educational journals account mostly. The study regions were distributed in 34 provinces except Taiwan of China, Hainan, Tibet, and Macao Special Administrative Regions where mainly concentrated on East China and North China. The cooperation among authors and institutions both were not close; A total of 25 high-frequency key words were intercepted,and three hotspots of narrative nursing research in China were obtained,which respectively were: (1) The application of narrative medicine and the cultivation of narrative ability; (2) The application research of narrative therapy in humanistic care; (3) The application of narrative nursing in nursing practice.
Conclusions
Narrative nursing has attracted the attention of domestic scholars, but the depth and breadth of research still need to be improved. It is necessary to strengthen cooperation between inter-regional institutions, individuals and scientific research groups and pay attention to research hotspots in this field. Also,there is a need to conduct more high quality studies to verify the significance of narrative nursing.
9.Effect of tacrolimus on short chain fatty acids in rats

Objective To investigate the changes of short chain fatty acids (SCFA) induced by tacrolimus (FK506) in rats and evaluate its effect on blood glucose levels. Methods Ten SD rats were divided into the FK506 group and control group (n=5 in each group). In the FK506 group, the rats were received a subcutaneous injection of FK506 (3 mg/kg) +sunflower oil solution containing 10% ethanol daily for consecutive 4 weeks. In the control group, the rats were received a subcutaneous injection of an equivalent amount of sunflower oil solution containing 10% ethanol for consecutive 4 weeks. During the drug injection period, the body mass of rats was measured every week in two groups. After the drug injection period, blood glucose level, SCFA content in the blood and feces samples were measured in two groups. Results Compared with the control group, the relative body mass of rats in the FK506 group was significantly lower at the 2nd, 3rd and 4thweeks (all P<0.01). Compared with the control group, the blood glucose levels of rats in the FK506 group were significantly increased at 0, 30, and 60 min after giving glucose (P<0.01-0.05). Compared with the control group, the contents of acetic acid, propionic acid, isobutyric acid, butyric acid, isovaleric acid and valeric acid in the feces sample were significantly lower in the FK506 group (P<0.01-0.05). Conclusions FK506 can upregulate the blood glucose level in rats, which is probably induced by the decrease of SCFA content in rat feces.
10.Allogeneic hematopoietic stem cell transplantation for β-thalassemia major patients using a reduced-intensity conditioning regimen

Objective To observe the efficacy and safety of reduced-intensity conditioning regimen used in allogeneic hematopoietic stem cell transplantation (HSCT) for children with β-thalassemia major.Methods We retrospectively analyzed the clinical data of 15 children with β-thalassemia major undergoing allogeneic HSCT with a reduced-intensity conditioning regimen from March 2013 to March 2017.Fifteen patients were diagnosed definitely,and the median age at transplantation was 5 years (range:3-6 years),including 11 with HSCT from unrelated donors (UDs),3 of HLA 8/10 matched and 8 of HLA10/10 matched.The remaining 4 patients out of 15 with HSCT were from related donors with HLA matched,3 donors were siblings and 1 was mother.All patients used a reduced-intensity conditioning regimen.The median mononuclear cell (MNC) dose and CD34 positive cell dose were 11.4 × 108/kg (range:4.8-20 × 108/kg)and 9.8 × 106/kg (range:5.9-27.2 × 106/kg),respectively.Graft-versus-host disease (GVHD) was prevented by cyclosporine A,methotrexate,MMF and ATGf.Results All 15 patients had successful engraftment.Median time to neutrophil and platelet engraftment was 12 days (range:9-21 days) and 15 days (range:10-25 days) respectively.Two patients developed grades Ⅱ acute GVHD and 4 patients developed chronic GVHD from unrelated donors,while there was no acute GVHD and 1 patient developed chronic GVHD from related donors.No patients suffered from serious transplantation-related complications,such as hepatic veno-occlusive disease (VOD),hemorrhagic cystitis,EB virus reactivation,CMV reactivation and hepatitis C,etc.The median follow-up time was 24 months (range:2-48 months).All patients were healthy and became transfusion-independent.Conclusion The reduced-intensity conditioning regimen proved to be safe and effective for children with β-thalassemia major given allogeneic HSCT.
