1.Interaction Frequency, Interaction Satisfaction, and Physical Frailty by Living Arrangement among Community-Dwelling Older Adults

Background:
Living alone and limited social interaction are associated with frailty, while low satisfaction with interactions is associated with negative health outcomes. However, the effects of living arrangements, interaction frequency, and interaction satisfaction on physical frailty remain unclear. This study examined the associations among living arrangements, the frequency and satisfaction of social interactions, and physical frailty among community-dwelling older adults.
Methods:
A cross-sectional study was conducted among 5,538 community-dwelling adults aged 60 or older living in Aichi Prefecture, Japan. The revised Japanese version of the Cardiovascular Health Study criteria evaluated physical frailty. Frequency and satisfaction of social interactions were evaluated using the National Center for Geriatrics and Gerontology–Social Network Scale (SNS). Logistic regression analysis was conducted by living arrangement, with physical frailty as the dependent variable and the SNS as the explanatory variable. In the sensitivity analysis, SNS was categorized into two groups based on the median, then combined with living arrangements into four groups to examine the association with physical frailty.
Results:
A total of 338 (6.3%) participants were classified as physically frail. Participants with high SNS scores showed significantly lower odds of physical frailty regardless of living arrangement (OR: 0.36, 95% confidence interval [CI]: 0.16-0.80; OR: 0.66, 95% CI: 0.45-0.96). Sensitivity analysis similarly showed the high SNS group had lower ORs for physical frailty, regardless of living arrangements.
Conclusion
High frequency and satisfaction with social interactions were negatively associated with physical frailty, suggesting that enhancing both aspects of social engagement may be beneficial.
2.A Systematic Review and Meta-Analysis of Systemic Corticosteroids for the Palliation of Dyspnea in Patients with Cancer

Objective: Almost half of all patients with cancer experience dyspnea, which can have various causes. Although systemic corticosteroids are administered to relieve symptoms, their efficacy has not been established. This systematic review aims to determine the efficacy of systemic corticosteroids for dyspnea in patients with cancer. Methods: CENTRAL, MEDLINE, EMBASE, and Ichushi-Web databases were searched for articles published from their inception to September 23, 2019, on studies of systemic corticosteroid administration for dyspnea in patients with cancer. The primary outcome measure was dyspnea intensity, as assessed by patient-reported outcomes. Secondary outcome measures were quality of life, delirium, and severe adverse events. Results: Two RCTs were included in the meta-analysis. With regard to alleviating dyspnea, the systematic corticosteroid group was associated with significantly greater dyspnea relief than the placebo group (mean difference: −0.71 [95% CI: −1.4 to −0.03]). However, a meta-analysis of quality of life and delirium could not be performed due to insufficient data. Analysis of severe adverse events showed no significant difference in their incidence between the corticosteroid and control groups (relative rate: 0.96 [95% CI: 0.19-4.93]). Conclusions: Systemic corticosteroids may be effective in treating dyspnea in patients with cancer, particularly those with lung involvement. Limiting the conditions for which corticosteroids are approved is expected to promote their appropriate use and minimize their adverse effects. However, further investigation is needed to determine the appropriate dosage, and the conditions in which corticosteroids are effective.
3.Screening of nontoxic dyes for improved visualization and success rate in mouse embryo transfer

Background:
Advancements in genome editing technology and the globalization of life science research have highlighted the need for refined techniques in individualizing fertilized mouse eggs. Traditional embryo transfer surgery, which demands precise and objective success assessment, presents challenges for technical mastery and animal welfare. To address these issues, we developed a novel culture medium incorporating a dye library that enhances the visibility of ultrafine capillaries.
Results:
We screened 32 compounds based on their color properties and excluded those with clear cytotoxicity through colony formation inhibition tests. Five compounds demonstrated developmental outcomes comparable to those of the control group during embryo culture. Gene expression analysis revealed high correlation with control embryos, indicating no adverse developmental impact. No maternal toxicity was observed during and after embryo transfer using the dyed medium, which also produced normal offspring. A randomized survey confirmed improved visibility compared to conventional methods.
Conclusion
This study successfully identified nontoxic dyes that enhance the visibility of embryo transfer culture media, potentially improving precision and animal welfare in embryological research. Based on our findings, adding Brilliant Blue FCF or Fast Green FCF to the medium is recommended.
4.Reproducing Gradient Field Application to Localize Magnetic Resonance Signals Using MRI Simulation

This study aimed to reproduce, through MRI simulation, the process by which gradient fields are applied during signal acquisition to obtain spatial information in MR signals. A digital phantom containing three cylinders filled with the simulated material was used to acquire MR signals by gradient echo using MRI simulations, performed using a pulsesequence diagram. MR signals were acquired under three different conditions: a) without a gradient field applied, b) with only a gradient field applied, and c) with a dephasing lobe and gradient field applied. The MR signal acquired under (a) was time-domain data that, after Fourier transformation (FT), yielded a single spectral data corresponding to the resonance frequency. However, when gradient field is applied to obtain spatial information, phase dispersion (dephasing) inevitably occurs over time; furthermore, this dephasing is superimposed. Consequently, the MR signals acquired in b) and c) were confirmed to be frequency-domain data after FT, as they were obtained under gradient field application. Furthermore, the necessity of the dephasing lobe was confirmed, as data reflecting the nature of the k-space could not be obtained without its inclusion during gradient field application during signal acquisition.
5.Trial of “Nationwide Kampo Web Test” Targeting Medical Professionals Nationwide

A 25-question “Nationwide Kampo Web Test” was conducted to evaluate the basic knowledge and diagnostic ability of Kampo among medical professionals nationwide. We analyzed the scores of all 608 examinees (241 doctors, 257 pharmacists, 77 others, 12 acupuncturists, 10 nurses, and 11 dentists) and evaluated the test quality. The performance of all examinees was 67.7 ± 16.9 points (mean score ± standard deviation). Among the doctors, the mean score of Kampo medicine specialists (83.8 ± 8.7 points) was significantly higher than that of non-specialists (65.3 ± 16.3 points) (p<0.0001). The performance of doctors who prescribed 20 or more Kampo medicines (78.4 ± 11.5 points) was significantly higher than that of other doctors (p<0.0001). Among the 25 questions, the difficulty levels of 13, 8, and 4 were easy, moderate, and hard (correct answer rates: 70% or more, 40-70%, and less than 40%), respectively. The ability of 13 questions (52%) to distinguish between good and poor performers was extremely high (discrimination index of 0.5 or higher). Collectively, these results supported the good performance of our test, suggesting that it is a highly practical learning achievement evaluation system that could utilize information and communication technology and is expected to be used in future lifelong learning in the field of Kampo medicine.
6.The Effect of High-flow Nasal Cannula Oxygen for Dyspnea in Patients with Advanced Disease: Systematic Review

Objective: To evaluate the efficacy of high-flow nasal cannula oxygen (HFNC) for dyspnea in patients with advanced disease. Methods: A literature search was conducted using MEDLINE, Cochrane Library, EMBASE, and Ichu-shi Web. Inclusion criteria were: 1) randomized controlled trials evaluating the effect of HFNC on dyspnea; 2) aged 18 years or older with advanced disease with hypoxemia; 3) control group was conventional oxygen therapy or noninvasive positive pressure ventilation. Exclusion criteria were: 1) patients in intensive care unit, 2) weaning from ventilator. Results: Six studies (4 from database searches, and 2 from hand searches) were included. In the 2 studies evaluating short-term intervention, one showed HFNC was more efficacious, and the other conventional oxygen was more efficacious. In the 2 studies evaluating long-term interventions: one showed HFNC was more efficacious, and the other showed no significant difference. In the 2 studies evaluating the intervention during exercise, one showed HFNC was more efficacious, and the other showed no significant difference. Conclusion: HFNC may be effective for dyspnea in patients with advanced disease associated with hypoxemia.
7.System Construction, Practice, and Explanation of the Answer for the “Common Kampo Test”

At the 71st Annual Meeting of the Japan Society for Oriental Medicine held in August 2021, we performed a special program focusing on education in Kampo medicine, “Pre-graduation and post-graduation education in Kampo medicine for the next generation.” As a part of this project, we constructed the “Common Kampo test” based on the “Standard lectures on Kampo” as a web-based test using Moodle, and then the test was conducted in the form of a web test with 163 participants. The commentary of the “Common Kampo test” was live-streamed and was viewed by many participants, including students, educators and academics from all over Japan. In this paper, we report on the series covering the construction of the web system, creation of questions, answer results, and commentary on answers. With the recent popularization of tablet devices and smartphones, this web-based test system has the potential to become a scientifically based and optimal learning system not only for students but also for medical professionals such as doctors, pharmacists, and nurses.
8.The Effect of Fan Therapy for Dyspnea in Patients with Chronic Progressive Disease: Systematic Review and Meta-analysis

Objective: To evaluate the efficacy of fan therapy for the relief of dyspnea in patients with chronic progressive disease. Methods: A systematic electronic database search of all available articles published before October 23, 2019 was conducted using Ichushi-Web of the Japan Medical Abstract Society databases, CENTRAL, EMBASE, and MEDLINE. In addition, a hand-search for updates was performed using PubMed on June 30, 2020 and December 7, 2021. The inclusion criteria were: 1) any RCTs comparing the effect of fan therapy with any other intervention, and 2) patients aged ≥18 years. Exclusion criteria were: 1) duplicate references, and 2) conference presentations. Results: We identified 110 studies, of which 10 met our criteria for inclusion. Finally, five studies were used in the meta-analysis. Fan therapy significantly improved dyspnea in patients with chronic progressive disease compared to control groups with a standardized mean difference of −1.43 (95% confidence interval: −2.70 to −0.17, I2=94%, p<0.0001). Conclusion: Fan therapy was found to be effective in reducing dyspnea in chronic progressive disease.
9.Risk of Hyperkalemia due to the Administration of Angiotensin Ⅱ Type 1 Receptor Blocker and Calcium Channel Blocker: Retrospective Cohort Study Based on Japanese Medical Information Database.

Objective:To assess the risk for hyperkalemia caused by treatment with angiotensin Ⅱ Type 1 receptor blockers (ARB) in clinical practice with Japanese medical database.Design:A cohort study in patients treated with ARB alone and those treated with calcium channel blockers (CCB) alone as control.Methods:The Diagnosis Procedure Combination (DPC) database provided by Medical Data Vision Co., Ltd. was used to identify patients who received a diagnosis of hypertension (ICD-10 codes, I10 to I15) and were treated with ARB or CCB from April 2008 to June 2017. A logistic regression model was applied to estimate adjusted odds ratios (OR) and their 95% confidence intervals (CI) in these patients. The outcome in the logistic model was hyperkalemia (serum potassium≧5.5 mEq/L) and the covariates were sex, age, renal insufficiency, hepatic insufficiency, and baseline serum potassium levels. And, subgroup analysis was also performed in patients with and without renal insufficiency.Results:The incidence of hyperkalemia (per 1000 person-years) with ARB was 39.4 and that with CCB was 32.6. And, median periods from the index date to the date of occurrence of hyperkalemia for both exposure and control groups were 36 days (Min-Max:12-85) and 51.5 days(Min-Max:8-88)respectively. However, treatment with ARB was not associated with occurrence of hyperkalemia (OR 1.26, 95%CI: 0.58-2.75). The risk for hyperkalemia among those with renal insufficiency was higher (OR 3.31, 95%CI: 1.39-7.88)and as baseline serum potassium increased, the risk increased as well (OR 9.20, 95%CI: 3.52-24.10). And, the subgroup analysis also showed that rare occurrence of hyperkalemia by ARB and elevation risk for hyperkalemia by baseline serum potassium.Conclusion:The clinical data showed rare occurrence of hyperkalemia caused by ARB, indicating that renal insufficiency and baseline serum potassium levels affected the onset of the disease in clinical practice. Previous studies also reported the effects of renal insufficiency and other factors on the onset of hyperkalemia. ARB should be prescribed carefully in patients with these factors, as is conventionally done.
10.Allergy to carminic acid: in vitro evidence of involvement of protein-binding hapten

We previously described a rare case of anaphylaxis presumably induced by carminic acid in cochineal dye used as a food additive. In this study, highly pure carminic acid was added to an albumin-containing buffer at various concentrations, followed by serial dilution. Varying the mixing ratio of carminic acid and albumin affected the extent of histamine release from passively sensitized basophils. Similar basophil histamine release occurred with carminic acid-globulin solutions. These results provide experimental evidence indicating that basophil activation is dependent on hapten (carminic acid) and carrier (protein) interaction.
