1.Construction of device-related pressure injury prevention program in orthopedics patients based on evidence-based and SSKINS clinical management model

Objective:To construct a device-related pressure injury prevention program for orthopedic patients based on evidence-based and SSKINS (S: skin, S: surface, K: keep moving, I: incontinence, N: nutrition, S: self-care) clinical management model, and to provide reference for clinical nursing practice.Methods:In June 2023, a research team was set up to retrieve the literature related to device-related pressure injury in orthopedic patients, and the evidence was extracted and screened with reference to the SSKINS clinical management model, and the first draft of the protocol was formulated based on clinical practice. The final plan was determined through two rounds of expert consultations from October to December 2023.Results:The response rate of the two rounds of expert correspondence was 100%(20/20). The expert authority coefficients were 0.897 and 0.907, the overall Kendall coordination coefficients of the two rounds were 0.116 and 0.136 respectively, and the Kendall coordination coefficients of all levels were 0.104-0.213 (all P<0.001). The final protocol contains 6 first-level items (assessment and examination, the use and adjustment of device, prophylactic dressings, skin protection, nutritional support, health education) and 31 second-level items. Conclusions:The proposed orthopedic device-related pressure injury prevention program is scientific and practical, and can provide a reference for the prevention and management of clinical medical staff.
2.Qualitative study on neurosurgical nurses′ experience of lack of perioperative nursing for intracranial tumors patients

Objective:To deeply understand the experience of neurosurgery nurses on the lack of perioperative nursing for intracranial tumors patients, so as to provide reference for reducing the lack of nursing and improving the quality of nursing and patients′ medical experience.Methods:From December 2023 to February 2024, the neurosurgical nurses of Union Hospital, Tongji Medical College, Huazhong University of Science and Technology were selected by the purpose sampling method with maximum differentiation. Semi-structured interviews were conducted with phenomenological research methods. The interview data were transcribed, coded, categorized and described.Results:A total of 17 neurosurgical nurses were enrolled, including 6 males and 11 females, aged 24-45(33.29 ± 6.74) years. Three themes of perioperative nursing lack experience of neurosurgery nurses were obtained, including nursing lack experience (lack of safety nursing, lack of timely nursing, lack of high efficiency nursing, lack of effectiveness nursing, lack of fair nursing, and lack of patient-centered), the physical and mental effects of nursing lack on nurses (impacts of lack of nursing on nurses′ physiology and psychology), and reasons for lack of nursing (personnel, equipment, materials, organization management, environment).Conclusions:Neurosurgical nurses lack of perioperative nursing for patients with intracranial tumor, which has a certain impact on the physical and mental health of nurses, suggesting that neurosurgical nurses should attach great importance to the lack of nursing for patients with intracranial tumor and formulate a scientific and effective management system.
3.Scoliosis among middle school students and the correlation between axial trunk rotation angle and Cobb angle

Objective:To investigate the prevalence of scoliosis among middle school students through school-based screening, and to analyze the correlations between axial trunk rotation (ATR) angle and Cobb angle among those with scoliosis considering gender, age, body mass index (BMI) and curve type, so as to provide references for early prevention and treatment.Methods:Random cluster sampling was used to enroll 9501 middle school students from 11 schools in the Hanyang District of Wuhan. They underwent a six-step scoliosis screening with a forward bending test, scoliometer measurements, and confirmatory X-ray examinations. The students exhibiting scoliosis were stratified by gender, age, BMI, and curve type. The correlation between ATR angle and Cobb angle was subsequently analyzed within each subgroup.Results:Scoliosis was observed in 358 of the students (150 boys and 208 girls), a positive screening rate of 3.8%. Moreover, the positive screening rate was significantly higher among the girls (4.8%) than among the boys (2.9%). There was a moderate Spearman correlation ( r=0.69) between the ATR and Cobb angles in 358 of the students with scoliosis, both the boys ( r=0.69) and the girls ( r=0.70). When it came to age, there was a positive correlation between the ATR and Cobb angles for students aged 13 to 16 irrespective of their BMI classification, except that there was no significant correlation between the two angles for those classified as obese. In terms of Peking Union Medical College (PUMC) classification, there was a positive correlation for those in types Ia, Ib, Ic, IIa, IIb, IIc and IId, but there was no significant correlation between the two angles for those in type III. Conclusions:Scoliosis is prevalent among the group of adolescents studied. Society, schools and parents need to prioritize prevention, heightened awareness, timely diagnosis and early treatment, particularly for girls. The ATR angle may serve as a practical surrogate for estimating Cobb angle severity in scoliosis screening for single-curve or double-curve cases. However, ATR measurements may yield false-negative results among those with triple-curve deformities or the obese, underscoring the need for supplementary diagnostic methods among such populations.

7.CircPUM1 regulates malignant biological behaviors of endometrial cancer Ishikawa cells through the miR-337-3p/NPM1 axis

Objective:To investigate the effect of circular RNA(cireRNA)pumilio RNA binding family member 1(PUM1)on the proliferation,migration,invasion and apoptosis of endometrial cancer(EC)Ishikawa cells by regulating the miR-337-3p/nucleophosmin 1(NPM1)axis.Methods:Ishikawa cells were selected and plasmid sh-circPUM1 and its negative control(sh-NC),anti-miR-337-3p and its negative control(anti-NC)were transfected into Ishikawa cells by RNA interference.The experiment cells were divided into the control group(non-transfected cells),sh-NC group,sh-circPUM1 group,sh-circPUM1+anti-NC group and sh-circPUM1+anti-miR-337-3p group.The qPCR method was applied to detect the expressions of circPUM1,miR-337-3p,and NPM1 mRNA in Ishikawa cells in each group.CCK-8 method,EdU staining method,Transwell assay,and flow cytometry were applied respectively to detect the effects of knocking down circPUM1 on the proliferation,migration,invasion and apoptosis of Ishikawa cells.Western blot was applied to detect the changes in the expressions of PCNA,NPM1,MMP-9,SNAIL,E-cadherin,BAX and C-caspase-3 proteins in Ishikawa cells.Dual luciferase reporter gene experiment was applied to verify the targeting relationship between circPUM1 and miR-337-3p,and between miR-337-3p and NPM1.Results:Cell proliferation ability,EdU positive cell rate,migration and invasion numbers,circPUM1,NPM1 mRNA and protein,the expressions of PCNA,NPM1,MMP-9 and SNAIL protein in Ishikawa cells in the sh-circPUM1 group were significantly lower than those in the sh-NC group and Control group(all P<0.05);the apoptosis rate,the expressions of miR-337-3p,E-cadherin,BAX,and C-caspase-3 proteins were significantly higher than those in the sh-NC group and the Control group(all P<0.05).Compared with those in the sh-circPUM1 group and the sh-circPUM1+anti-NC group,the apoptosis rate,miR-337-3p,the expressions of E-cadherin,BAX,and C-caspase-3 proteins in the sh-circPUM1+anti-miR-337-3p group were significantly lower(all P<0.05);Cell proliferation ability,EdU positive cell rate,migration and invasion numbers,NPM1 mRNA and protein,the expressions of PCNA,NPM1,MMP-9,and SNAIL proteins were significantly higher(all P<0.05).CircPUM1 might target and negatively regulate miR-337-3p,and miR-337-3p might target and negatively regulate NPM1.Conclusion:Knocking down circPUM1 can inhibit the malignant biological behavior of Ishikawa cells,which might be achieved by targeting the miR-337-3p/NPM1 axis.
8.Application Progress of Resting-State Functional Magnetic Resonance Imaging in Study of Default Mode Network in Patients with Vascular Cognitive Impairment

Vascular cognitive impairment(VCI)is a group of syndromes ranging from mild cognitive impairment to dementia caused by cerebrovascular disease,due to the lack of sensitivity and specific biomarkers,it is difficult to identify and diagnose early.Abnormal connectivity is observed in brain regions of patients with vascular cognitive disorders,locates mainly in the default mode network(DMN),and changes in their abnormal functional connectivity correlated with the degree of patients' cognitive impairment.Resting-state functional magnetic resonance imaging(rs-fMRI)is a commonly used method to detect the internal activity of the brain at resting state.The use of various rs-fMRI to study abnormal changes in the DMN in patients with VCI is useful to further investigate the pathogenesis of VCI and provide an objective basis for imaging.This article mainly reviews the application of rs-fMRI in the DMN in patients with VCI,bringing new perspectives for the correct diagnosis and assessment of VCI.
9.Bowel Sounds Detection Method Based on ResNet-BiLSTM and Attention Mechanism

Bowel sounds can reflect the movement and health status of the gastrointestinal tract.However,the traditional manual auscultation method has subjective deviation and is time-consuming and labor-intensive.In order to better assist doctors in diagnosing bowel sounds and improve the reliability and efficiency of bowel sound detection,this study proposed a deep neural network model that combines a residual neural network(ResNet),a bidirectional long short-term memory network(BiLSTM),and an attention mechanism.Firstly,a large number of labeled clinical data was collected using the self-developed multi-channel bowel sound acquisition system,and the multi-scale wavelet decomposition and reconstruction method was used to preprocess the bowel sounds.Then,log Mel spectrogram features were extracted and sent to the network for training.Finally,the performance and effectiveness of the model were evaluated and verified by 10-fold cross-validation and an ablation experiment.The experimental results showed that the precision,recall,and F1 score of the model reached 83%,76%,and 79%,respectively,and it could effectively detect bowel sound segments and locate their start and end times,performing better than previous algorithms.This algorithm can not only provide auxiliary information for doctors in clinical practice but also offer technical support for further analysis and research of bowel sounds.
10.Development and application of a transitional care practice protocol for neurosurgical ICU patients based on the SEC model

Objective:To develop a transitional care practice protocol for neurosurgical intensive care unit (ICU) patients based on the Secure-Encourage-Collaborate (SEC) model and to explore its effectiveness, providing a reference for clinical practice.Methods:Through literature analysis, expert verification in our hospital, core group discussions, and the Delphi expert consultation method, a transitional care practice protocol for neurosurgical ICU patients based on the SEC model was developed. A convenience sampling method was used to select 160 patients admitted to the Neurosurgical ICU of Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, from March 2022 to October 2023. Eighty patients admitted from March to December 2022 were designated as the control group, and eighty patients admitted from January to October 2023 were designated as the experimental group. The control group received the routine post-discharge follow-up care protocol, while the experimental group received the transitional care practice protocol based on the SEC model. The ICU quality control indicators, rehabilitation outcomes, adverse events were compared between the two groups.Results:The experimental group had significantly shorter ICU stay and total hospitalization days compared to the control group, and fewer cases of readmission within 48 hours after ICU discharge ( P<0.05). The experimental group showed lower pain scores, higher scores for self-care in daily activities, shorter time to first ambulation after ICU discharge, and longer daily ambulation time within the first week post-discharge compared to the control group, with statistically significant differences ( P<0.05). No statistically significant differences were observed between the two groups in the incidence of stage II or higher pressure injuries, falls/bed falls, unplanned extubations, aspiration, and lower limb deep vein thrombosis ( P>0.05) . Conclusions:The transitional care practice protocol for neurosurgical ICU patients based on the SEC model can effectively improve the quality of care.
