1.Evolution of Medication Patterns in Traditional Chinese Medicine for Treating Inflammatory Bowel Disease

ObjectiveTo explore the evolution of medication patterns and syndrome-herb associations of traditional Chinese medicine (TCM) in treating inflammatory bowel disease (IBD), providing a theoretical foundation for precise syndrome differentiation and treatment in clinical practice. MethodsMedical case literature on TCM treatment of IBD from 1960 to 2024 was retrieved to establish a database. Frequency statistics, cluster analysis, change point detection, and association rule mining were employed to comprehensively analyze the syndrome distribution, therapeutic methods, medication patterns, and their temporal variations. ResultsA total of 685 medical cases were included. Common syndromes were dampness-heat (66.42%) and spleen deficiency (56.20%). Primary therapeutic methods included heat clearing (63.65%), spleen invigorating (47.45%), and dampness draining (36.79%). High-frequency herbs included Coptidis Rhizoma (354), Paeoniae Radix Alba (303), Aucklandiae Radix (292), Codonopsis Radix (253), and Glycyrrhizae Radix et Rhizoma (244). Initial prescription clustering revealed three core therapeutic method combinations: heat clearing and detoxifying (represented by Baitouweng Tang), spleen invigorating and Qi reinforcing (represented by Shenling Baizhusan), and cold-heat regulation (represented by Wumeiwan combined with Shaoyao tang). Temporal analysis identified 2008 as a key transition point in TCM treatment of IBD, with significantly increased usage frequency of heat-clearing and dampness-drying herbs such as Fraxini Cortex, Phellodendri Chinensis Cortex, Sophorae Flavescentis Radix, and Scutellariae Radix as well as hemostatic herbs such as carbonized Sanguisorbae Radix, Bletillae Rhizoma, Agrimoniae Herba, and Notoginseng Radix et Rhizoma. Follow-up efficacy analysis showed median improvement rates of 64.0% at the first follow-up, 76.0% at the second follow-up, and 78.7% at the third follow-up. Syndrome-drug association analysis revealed specific herb pairs with significant therapeutic advantages, including Notoginseng Radix et Rhizoma + Coicis Semen, Sanguisorbae Radix + Coptidis Rhizoma, and Codonopsis Radix + Aconii Lateralis Radix Praeparaia. ConclusionTCM medication patterns for treating IBD demonstrate distinct temporal evolution characteristics, with significantly increased usage frequency of herbs such as Fraxini Cortex, Notoginseng Radix et Rhizoma, and Agrimoniae Herba. Significant therapeutic method-herb associations and syndrome-herb association patterns exist, with the formation of specific herb pairs, providing evidence-based support for precise syndrome differentiation and treatment of IBD.
2.The effects of megestrol on related cytokines in patients with lung cancer complicated by sarcopenia

Objective:To detect the increase in skeletal muscle mass after oral megestrol administration and its influence on the levels of four cytokines,analyze the relationship between cytokine expression and sarcopenia,and provide new insights for the management,curat-ive effect prediction,and early screening of lung cancer patients with sarcopenia. Methods:A total of 60 patients with lung cancer complic-ated by sarcopenia were recruited from The Central Hospital of Shaoyang between June 2023 and June 2024. They were randomly assigned into an experimental group (30 cases) and a control group (30 cases). Patient data,including age,sex,body mass index,skeletal muscle mass index (SMI),blood glucose,serum albumin,triglyceride levels,and the levels of four cytokines (IL-1β,IL-6,IFN-γ,and TNF-α),were recorded. The correlation between the four cytokines and sarcopenia was investigated after the intervention. Results:There was no significant differ-ence between the experimental and control groups except for the IL-1β level (P>0.05). The levels of the four cytokines decreased signific-antly on the 10th day of medication compared to days 10-25 (P<0.05). A positive correlation between the levels of the four cytokines at dif-ferent time points (P<0.05) was observed. After megestrol intervention,SMI values increased by 11.8% from the baseline. Conclusions:Oral administration of megestrol can reduce the levels of cytokines IL-1β,IL-6,IFN-γ,and TNF-α while increasing the skeletal muscle mass to a certain extent. This suggests that megestrol may have positive therapeutic value in improving the prognosis and related clinical symptoms of patients with lung cancer and sarcopenia.
3.Construction and application of a family integrated care program for very low birth weight infants in China

Objective To develop a comprehensive family integrated care program for very low birth weight in-fants and to investigate the initial application effect of the program.Methods The family integrated care program for very low birth weight infants was developed using semi-structured interviews,a comprehensive literature review,and an expert consensus.A non-synchronous controlled trial was subsequently carried out.Between January and May 2023,35 participants accompanied by their parents for a minimum of 6 hours daily over a span of at least 3 days,were consecutively selected as an intervention group.Between January and May 2022,35 participants in the control group were treated with routine closed neonatal intensive care unit model.The application effect of the pro-gram on the clinical outcomes of very low birth weight infants was assessed,such as weight gain rate during hospi-talization,breastfeeding days,incidence of late-onset sepsis,incidence of extrauterine growth restriction at discharge,TIMP level at corrected gestational age of 37 weeks and hospitalization days.Results The authority coefficient of the 8 invited experts specializing in neonatal nursing and nursing management was 0.925.A comprehensive pro-gram was developed,consisting of 2 stages—preparation and implementation,and addressing 7 key themes:staff team formation and training,environmental support,assessment,parental training,family-integrated care implementation,peer support,and comprehensive communication.No shedding cases occurred during the application of the program.The outcomes indicated that the intervention group experienced a higher rate of weight gain and more days of breast-feeding compared to the control group,and the incidence of extrauterine growth retardation upon discharge was also lower,with statistically significant differences(all P<0.05).However,there were no significant differences in other clinical outcomes(all P>0.05).Conclusion The family integrated care program for very low birth weight infants de-veloped in this study is scientifically grounded,comprehensive,and practical.The implementation of this program can promote the weight gain and breastfeeding among very low birth weight infants,reduce the occurrence of extrauter-ine growth restriction upon discharge,and does not increase the incidence of late-onset sepsis.

6.Nomenclature standardization of radiotherapy in cervical cancer

Objective:To standardize the naming of organ at risk (OAR) and target area during cervical cancer radiotherapy based on AAPM TG-263.Methods:After self-programming of Matlab software to implement the reading and resolution of radiotherapy structure files, the naming of each substructure was automatically output, recorded and restored. After naming all substructures, the structure names were classified by keywords. According to TG-263, a standard naming conversion table of OAR and target area was developed, and the classified structure names were standardized through procedures. Finally, the standardized named radiotherapy structure files were output and imported into the treatment planning system (TPS).Results:The radiation structure of 144 patients with cervical cancer was successfully transformed and displayed correctly in TPS. Before the transformation, the naming of OAR and target area lacked of uniform norms and standards, and the naming of the same structure significantly differed. After the transformation, 43 naming methods of OAR and 74 naming methods of the target area were unified into 20 and 8 naming methods, which were more convenient for staff understanding and communication.Conclusion:The standardization of cervical cancer radiotherapy structure naming can reduce the inconsistency of naming and provide reference for the standardized naming of pelvic tumors.
7.Correlation analysis of the serum homocysteine, methionine and cysteine in patients with chronic heart failure

Objective:To analyze the correlation and diagnostic value of serum homocysteine (Hcy), methionine (Met) and cysteine (Cys) in patients with chronic heart failure (CHF).Methods:One hundred and seventy-eight patients with acute decompensation CHF (CHF group) and 70 healthy persons (healthy control group) from October 2018 to September 2019 in Affiliated Zhongshan Hospital of Dalian University were continuously enrolled. In CHF group, heart failure with reduced ejection fraction (HFrEF) was in 53 cases, heart failure with mid-range ejection fraction (HFmrEF) was in 50 cases, and heart failure with preserved ejection fraction (HFpEF) was in 75 cases. Serum levels of Hcy, Met and Cys were detected by tandem mass spectrometry. Serum level of N-terminal brain natriuretic peptide precursor (NT-proBNP) was detected by electrochemical luminescence immunity. The left ventricular end-diastolic diameter (LVEDd), left ventricular end-systolic diameter (LVESd) and early diastolic peak blood flow velocity of mitral valve annulus/early diastolic peak velocity of mitral annulus (E/e′) were detected by echocardiography, then left ventricular eject fraction (LVEF) was calculated. Correlation was analyzed by Pearson correlation analysis. The receiver operator characteristic (ROC) curve was drawn, and the area under curve (AUC) was used to evaluate the efficacy of serum Hcy, Met, Cys, NT-proBNP and LVEF in the diagnosis of CHF.Results:The Hcy, Met, Cys, NT-proBNP, LVEDd and E/e′ in CHF group were significantly higher than those in healthy control group: (12.64 ± 5.02) μmol/L vs. (8.71 ± 3.47) μmol/L, (23.38 ± 5.75) μmol/L vs. (20.52 ± 4.18) μmol/L, (343.45 ± 44.49) μmol/L vs. (290.53 ± 48.38) μmol/L, (5 759.43 ± 3 806.22) pg/L vs. (40.24 ± 31.91) pg/L, (52.67 ± 12.27) mm vs. (46.41 ± 12.27) mm and (17.32 ± 5.61)% vs. (9.54 ± 2.64)%, the LVEF was significantly lower than that in healthy control group: (45.27 ± 4.93)% vs. (62.37 ± 5.41)%, and there were statistical differences ( P<0.01 or <0.05). The Hcy and Cys in patients with HFmrEF and HFrEF were significantly higher than those in patients with HFpEF: (16.29 ± 8.18) and (18.68 ± 8.99) μmol/L vs. (13.75 ± 6.48) μmol/L, (346.64 ± 51.85) and (361.40 ± 52.34) μmol/L vs. (329.35 ± 55.16) μmol/L, and there were statistical differences ( P<0.05); there were no statistical differences between patients with HFmrEF and patients with HFrEF ( P>0.05). The serum Met in patients with HFrEF was significantly higher than that in patients with HFpEF and HFmrEF: (28.74 ± 8.22) μmol/L vs. (24.76 ± 7.60) and (25.15 ± 6.96) μmol/L, and there was statistical difference ( P<0.05); there was no statistical difference between patients with HFpEF and patients with HFmrEF ( P>0.05). Pearson correlation analysis result showed that serum Hcy, Met and Cys were positively correlated with NT-proBNP ( r = 0.632, 0.206 and 0.455; P<0.01), positively correlated with E/e′( r = 0.463, 0.198 and 0.346; P<0.01), and negatively correlated with LVEF ( r = -0.491, -0.152 and -0.330; P<0.05 or <0.01). ROC curve analysis result showed that ROC the cut-off value for the diagnosis of CHF with serum NT-proBNP based on the maximum Youden index (0.994) was 120 pg/L, and AUC was 0.994 (95% CI was 0.997 to 1.000); the cut-off value for the diagnosis of CHF with serum Hcy based on the maximum Youden index (0.646) was 10.56 μmol/L, and AUC was 0.899 (95% CI 0.859 to 0.939); the cut-off value for the diagnosis of CHF with serum Met based on the maximum Youden index (0.218) was 25.58 μmol/L, and AUC was 0.637 (95% CI 0.563 to 0.711); the cut-off value for the diagnosis of CHF with serum Cys based on the maximum Youden index (0.391) was 298.05 μmol/L, and AUC was 0.765 (95% CI 0.700 to 0.830); the AUC of LVEF less than 0.5. Conclusions:Serum Hcy, Met and Cys levels in patient with CHF are significantly increased, which are positively correlated with NT-proBNP and E/e′, negatively correlated with LVEF. Moreover, serum Hcy has certain application value in the diagnosis of CHF.
8.Comparative study of non-invasive hemodynamics and echocardiography on heart failure with preserved ejection fraction patients:one year follow-up

Objective:A comparative study of non-invasive hemodynamics and echocardiography in 139 cases of heart failure patients with preserved ejection fraction (HFpEF) at baseline and one year follow-up to explore its value on diagnosis, monitoring and prognosis in patients with HFpEF.Methods:The baseline and one year follow-up data of 139 patients with HFpEF in Affiliated Zhongshan Hospital of Dalian University patients who had been enrolled in the China PEACE 5P-HF from June 2016 to May 2018 were analyzed retrospectively. The general data were collected which contented age of the study subjects is (30 - 80) y, average age (64.0 ± 12.3) y, and 63.31% male, (88/139) and 36.69% female (51/139), 56.8% smokers (79/139). t-test way was used to analyze the baseline and one year follw-up data, The indexs included blood pressure (BP), estimated glomerular filtration rate (eGFR) and 6-munites walk test (6MWT). Non-invasive hemodynamic indicators included stroke volume (SV), ejection fractions (EF), cardiac index (CI), index of contratility (IC), pulmonary artery wedge pressure (PCWP), maximum angiectatic velocity(AMPC), left ventricular ejection time (LVET), left ventricular isovolumetric relaxation time (LVLVIVRT), pre-ejection period/left ventricular ejection fractions (PEP/LVET), left ventricular end diastolic pressure (LVEDP) and left cardiac work index (LCWI). Hemodynamic indicators included left ventricular end diastolic dimension(LVEDd), left ventricular end systolic dimension (LVEDs), interventricular septal thickness at diastole (IVSD), left ventricular ejection fractions (LVEF) and E/e′.Results:There was no significant difference between the baseline and one year follow-up data in SBP, DBP, NT-proBNP, eGFR, 6MWT ( P>0.05). There were significant increase in SV, EF, CI, IC in one year′ follow-up compared with that in baselinee [(73.39 ± 29.47) ml vs. (63.39 ± 30.08) ml, (64.87 ± 9.16)% vs. (61.81 ± 9.02)%, (3.06 ± 1.10) ml/(min·m 2) vs. (2.62 ± 1.06) ml/(min·m 2), (0.039 ± 0.037) L/s vs. (0.028 ± 0.015) L/s] ( P<0.05). PCWP in one year′ follow-up was significantly decreased compared with that in baselin [(9.21 ± 3.34) mmHg (1 mmHg = 0.133 kPa) vs. (9.87 ± 3.13) mmHg]( P<0.05), However, AMPC, LVE, LVLVIVRT, PEP/LVET, LVEDP, LCWI in baseline and one year′ follow-up showed no significant difference ( P>0.05). The Hemodynamic indicators in baseline and one year′s follow-up were as followed: LVEF in one year′ follow-up was significantly elevated compared with that in the baseline [(63.53 ± 8.39)% vs. (61.02 ± 7.16)%]; E/e′ in one year′s follow-up was significantly decresed compared with that in the baseline [12.89 ± 5.86 vs. 14.32 ± 6.61]( P<0.05); there were no significant differences in LVEDd, LVEDs and IVSD in baseline compared with those in one year′s followed-up ( P>0.05). Conclusions:Hemodynamic indicators including SV, EF, CI, IC and PCWP could be new reflections of early diagnosis, monitoring and prognosis on HFpEF. The combination of non-invasive hemodynamics and echocardiography on HFpEF can be more significant in reflecting the changes of myocardial remodeling and cardiac function.
9.Diagnostic value of contrast-enhanced ultrasound for dysplastic nodule with a focus of hepatocellular carcinoma

Objective To investigate the imaging features of dysplastic nodules with a focus of hepatocellular carcinoma ( DN-HCC ) on contrast-enhanced ultrasound ( CEUS ) and to improve the diagnostic accuracy . Methods The clinical data of 60 patients and CEUS imaging of 62 hepatic nodules [DN-HCCs , n =54 ;dysplastic nodules (DN) , n =8] pathologically proved were reviewed retrospectively . According to Contrast Enhanced Ultrasound Liver Imaging Reporting and Data System (CEUS LI-RADS) , the lesions were categorized . Results Significantly different CEUS patterns between DN-HCCs and DNs were observed ( P < 0 .05) . During the arterial phase ,54 DN-HCC lesions showed various enhancement patterns [ hypervascular ,59 .3% ( 32/54 ) ;nodule-in-nodule ,9 .3% ( 5/54 ) ;isovascular ,13 .0% ( 7/54 ) and hypovascular ,18 .5% (10/54)] . Of the 54 DN-HCC lesions ,44 .4% (24/54) showed washout during the late phase .Of the 8 DN lesions ,62 .5% (5/8) showed iso-enhancement during the arterial phase ,25% (2/8) showed hypo-enhancement ,and 12 .5% (1/8) showed hyper-enhancement . No DN lesion showed washout during the late phase .According to CEUS LI-RADS (LR) algorithm ,27 .8% (15/54) DN-HCCs were LR-5 ,46 .3% (25/54) DN-HCCs were LR-4 ,25 .9% (14/54) DN-HCCs and 100% (8/8) DNs were LR-3 . Regarding hyper-enhancement ( including local hyper-enhancement ) during the arterial phase or hypo-enhancement (including local hypo-enhancement) during the late phase as the diagnostic standard of DN-HCC , the diagnostic sensitivity , specificity and accuracy value were 83 .3% , 87 .5% and 83 .9% , respectively . Conclusions The imaging features of hyper-enhancement during the arterial phase or hypo-enhancement during the late phase on CEUS are useful to diagnose DN-HCCs .
10.Prevalence and Risk Factors of Mild Cognitive Impairment in Old People in Pension Facilities in Chongqing, China

Objective To investigate the prevalence status and risk factors of mild cognitive impairment (MCI) in old people in pension facilities in Chongqing, China. Methods From October to November, 2015, the people above 60 years old in four pension facilities in Chongqing were cluster sampled. They were interviewed with Mini-Mental State Examination, Instrumental Activity of Daily Living Scale, self-made general situation questionnaire for the elderly and the Geriatric Depression Scale. Results There were 25 MCI patients identified in 84 old people selected, with the prevalence of 29.76%. MCI prevalence was different with the factors such as genders, educational levels, economical status, smoking, eating freshwater fish frequently, participating in outdoor activities frequently, playing Mahjong/chess/cards fre-quently, watching TV/listening to broadcast/reading newspaper frequently, participating in social activities frequently, having religious be-liefs, suffering high blood pressure (χ2>4.271, P<0.05). Based on multivariate Logistic regression analysis, male was the protective factor (OR=0.055, 95%CI:0.005~0.618), while high blood pressure was the risk factor (OR=8.281, 95%CI:1.278~53.646) to the MCI. Conclu-sion MCI is prevalent in old people in pension facilities in Chongqing, which needs intervention targeted to the risk foctors.
