1.The predictive value of Naples prognostic score and prognostic nutritional index for postoperative recurrence and survival in endometrial cancer patients

Objective To investigate the impact of Naples prognostic score(NPS)and prognostic nutritional index(PNI)on postoperative recurrence and survival in patients with endometrial cancer(EC).Methods A total of 258 EC patients were selected for surgical treatment.Clinical data such as the patients' age at admission,pathological grade,depth of myometrial invasion,lymphovascular space invasion(LVSI),lymph node metastasis and clinical stage of the International Society of Gynecological Oncology(FIGO)were collected.One week before the operation,the laboratory indicators were detected in patients,and NPS and PNI were calculated.The patients were followed up and divided into the recurrence group(n=72)and the non-recurrence group(n=186)according to whether there was recurrence after the operation.The recurrence curves were plotted using Kaplan-Meier method,and the Log-rank test was used to compare the differences in recurrence curves between patients with different NPS and PNI.The Cox proportional hazards regression model was used to analyze the influencing factors of postoperative recurrence in patients with EC.The receiver operating characteristic(ROC)curve was used to evaluate the predictive efficacy of PNI and NPS scores for postoperative recurrence in patients with EC.Results The proportion of patients with high-risk NPS scores was significantly higher in the recurrence group than that in the non-recurrence group(41.7%vs.19.4%,P<0.05).The PNI was significantly lower in the recurrence group than that in the non-recurrence group(42.1±7.6 vs.47.2±8.9,P<0.01).Multivariate Cox regression analysis showed that advanced age,elevated serum carbohydrate antigen 125(CA125),pathological grades G2 and G3,myometrial invasion depth>1/2,positive LVSI,FIGO stage Ⅲ-Ⅳ and medium-high risk patients with NPS score were independent risk factors for recurrence.Postoperative radiotherapy and chemotherapy and elevated PNI were independent protective factors for recurrence in patients.ROC curve analysis showed that the combined predictive value of NPS score and PNI for postoperative recurrence in patients with EC was superior to that of individual predictions.Conclusion Both NPS and PNI can be used as potential indicators to predict the prognosis of patients with endometrial cancer.

3.LC-MS/MS determination of unsymmetrical dimethylhydrazine concentrations in rat plasma and toxicokinetic studies

Objective To establish a rapid and sensitive liquid chromatography-tandem mass spectrometry(LC-MS/MS)analysis method for determination of unsymmetrical dimethylhydrazine(UDMH)contents in rat plasma and investigate the toxicokinetic characteristics of UDMH in rats.Methods Twenty-two SD rats were divided into the intravenous injection of 10 mg/kg dose group(4 females)and intragastric administration groups(low,medium and high dose,with 6 rats in each group,half males and half females).The rats were given 10mg/kg by intravenous administration and 10 mg/kg,30 mg/kg,and 90 mg/kg single dose of UDMH by gavage.Blood samples were collected from the orbital venous plexus at 0 hour before administration and at different time points after administration.The plasma samples were extracted with protein precipitation and derivatization before being analyzed using the LC-MS/MS method.Separation was carried out on a ZORBAX column(4.6mm×75mm,3.5 μm),with a mobile phase composed of 0.3%acetonitrile/formic acid at a flow rate of 1 mL/min.Propranolol was used as the internal standard.An electrospray ionization(Turbo Ionspray)source was applied and the mass spectrometer was operated in a positive MRM mode.Quantitative analysis showed that the ionization source unsymmetrical dimethylhydrazine and propranolol was at m/z:192.0→148.1,m/z:260.2→116.1,respectively.The toxicokinetic parameters were analyzed with the DAS 2.1 software.Results Quantification of UDMH exhibited a good linearity within the concentration range of 50-50000 ng/mL,with a linear correlation coefficient greater than 0.9900 and a lower limit of quantification of 50 ng/mL.The average recovery rate of UDMH was 98.1%,compared with 100.5%for the internal standard propranolol hydrochloride.The inter batch precision of standard curve samples ranged from 0.7%to 6.3%,and the relative error was between-7.1%and 6.2%.The inter batch and intra batch precision of quality control samples ranged from1.8%to 19.8%,and the relative error from-9.8%to 0.2%.The main pharmacokinetic parameters of UDMH in rats exposed to 10 mg/kg,30 mg/kg,and 90 mg/kg gavage were UC(0-t):(7624.99±2569.31),(34284.04±6657.15),(84720.88±22354.80)μg/L·h,t1/2:(0.07±0.15),(2.24±1.45),(3.04±0.90)h,Tmax:(0.75±0.27),(0.51±0.29),(0.29±0.10)h,Cmax:(4454.14±1329.45),(19442.45±9121.07),(32334.35±9882.41)μg/L,F:(77.34±26.06)%,(115.92±22.51)%,(95.48±25.19)%.Conclusion The LC-MS/MS method is highly accurate and specific,and is suitable for the toxicokinetic study of UDMH in rats.Single gavage administration of UDMH results in absorption and elimination saturation at a high dose.This study provides data for toxicological studies related to UDMH.
4.Analysis on the utilization and satisfaction with clinical services for the elderly through hospital-at-home in Xuzhou City

Objective:To understand the utilization of clinical services provided through hospital-at-home to the elderly in Xuzhou City, and analyze patient satisfaction.Methods:The stratified cluster sampling method was used to select community elderly people aged 60 and above in Xuzhou city to conduct a face-to-face questionnaire survey.Descriptive analysis was used to examine the utilization and evaluation of home-based clinical services for the elderly.Multiple linear regression analysis was used to analyze associated factors for the satisfaction of the elderly in this care setting.Results:A total of 203 elderly people who had received this type of services were investigated.The overall satisfaction of the elderly in Xuzhou City with home-based clinical services was 79.3%(161/203), and the average satisfaction score of the comprehensive evaluation was(4.11±1.03)points.Age( β=-0.011), marital status( β=0.164), informed consent for service( β=0.162), doctors' bed-side services( β=0.146), service fee( β=0.346), and the home hospital bed assembling process( β=0.257)were the influencing factors for satisfaction with family hospital bed services, according to the patients involved in the comprehensive evaluation(all P<0.05). Conclusions:The utilization and evaluation with clinical services by elderly people in the hospital-at-home setting in downtown Xuzhou is relatively good, with relatively high satisfaction.Comprehensive measures should be taken in response to factors related to satisfaction to promote sustainable development of clinical services through hospital-at-home in China.
5.Effects of lipopolysaccharide on mice iron metabolism related genes mRNA expression

ObjectiveTo investigate the effects of lipopolysaccharide (LPS) on mRNA expression of iron metabolism related genes. Methods Ten male mice (2 months) were injected intraperitoneally with lipopolysaccharide(0.5 μg/g). After 6 hours, mice were sacrificed and then sera, liver and spleen were collected. The mice blood routine was measured. The serum iron and total iron binding capacity (TIBC) were determined with reagent kit. The quasi-quantitative reverse transcription polymerase chain reaction (RT-PCR) was performed for mRNA of hepatic hepcidin(HP), ferroportin1(Fpn1), transferrin receptor 1(TfR1) and spleenic HP, Fpn1 and interleukin-6(IL-6). Results The serum iron and TIBC were reduced in mice injected LPS, which exhibited mild anemia(P<0.05) . LPS can increase the expression of hepatic hepcidin and decrease Fpn1 and TfR1 in liver after LPS administration 6 hours(P<0.05). In spleen, IL-6 was upregulated and Fpn1 downregulated(P<0.05). Conclusion LPS can influence serum iron through regulating the mRNA expression of hepatic and spleenic iron metabolism related genes, such as HP, Fpn1 and TfR1.
