1.Molecular epidemiological characteristics of clinical isolates of CRKP resistant to ceftazidime/avibactam

OBJECTIVE To investigate the molecular epidemiological characteristics of clinical isolates of carbapen-em-resistant Klebsiella pneumoniae(CRKP)resistant to ceftazidime/avibactam(CZA).METHODS From Jan.2022 to Dec.2023,totally 63 strains of non-repetitive CZA-resistant CRKP that were isolated for the first time from hospitalized patients of the First Affiliated Hospital of Nanchang University were enrolled in the study,and 50 strains of CZA-sensitive CRKP were randomly chosen as the research subjects.The drug susceptibility rates of the strains were observed.The drug resistance genes,virulence genes and capsular serotypes of the strains were detected by means of polymerase chain reaction(PCR).The molecular epidemiological characteristics of the strains were observed by using molecular biological techniques such as pulse field gel electrophoresis(PFGE)and multilocus sequence typing(MLST).RESULTS Among the CZA-resistant CRKP strains,45(71.43%)were iso-lated from sputum.The result of drug susceptibility testing showed that the drug resistance rates of the CZA-re-sistant strains to amikacin,aztreonam and minocycline were lower than those of the CZA-sensitive strains(P<0.05),and the drug resistance rates of the CZA-resistant strains to tigecycline was higher than that of the CAZ-sensitive strains(P<0.05).The carrying rates of blaKPC,blaNDM,blaSHV-1,blaTEM-1,blaCTX-M,blaqnrS,blaacc(6')-Ib and blarmtB genes of the CZA-resistant strains were relatively high.Among the detected capsular serotypes,K64(n=18,28.57%)was dominant.ST11(n=25,39.68%)was predominant strain among the CZA-resistant CRKP strains.CONCLUSIONS ST11 is dominant among the CZA-resistant CRKP strains.The strains carry with multiple drug resistance genes and virulence genes.The drug resistance rate of the CZA-resistant strains to tigecycline is higher than that of the CZA-sensitive strains,and it is necessary to attach great importance during the clinical treatment.
2.Evaluation of “1+1+2M” healthcare management model by healthcare workers in a large public hospital and its influencing factors

Objective:To analyze the evaluation of the “1+1+2M” healthcare management model (outpatient core medical staff+ward core medical staff+multi interdisciplinary team+multi department coordination) by healthcare workers in a large public hospital and its influencing factors.Methods:It was a cross-sectional study, using the judgment sampling method to select 70 healthcare workers in the hospital healthcare department in November 2024. A self-designed public hospital healthcare management model evaluation questionnaire was used to investigate the “1+1+2M” healthcare management model. A total of 70 questionnaires were distributed, and 70 valid questionnaires were collected (100%). The basic information, team integration, human resource allocation, and evaluation of healthcare management models in the healthcare workers were collected. The factors affecting healthcare worker evaluation were identified with Kruskal Wallis test and multiple logistic regression analysis.Results:Among the 70 healthcare workers included in the analysis, there were 30 doctors, 30 nursing staff, and 10 management/technical personnel; 19 males and 51 females; more than 70% of the participants were aged 50 years or younger; the length of service was (19.11±9.44) years, and the length of service for healthcare was (9.79±6.06) years. Healthcare workers showed high satisfaction with the “1+1+2M” healthcare management model, with an overall satisfaction rate of 87.14% (61/70). The evaluation of work collaboration, business norms, and mode configuration by healthcare workers was positively correlated with the evaluation of healthcare management mode [very satisfied with health management mode: relatively satisfied with work collaboration ( OR=0.064, 95% CI: 0.004-0.965), satisfied with business norms ( OR=0.018, 95% CI: 0.001-0.312), relatively satisfied with business norms ( OR=0.061, 95% CI: 0.004-0.938), satisfied with mode configuration ( OR=0.006, 95% CI: 0.000-0.111); satisfied with health management mode: satisfied with business norms ( OR=0.049, 95% CI: 0.005-0.498), satisfied with mode configuration ( OR=0.074, 95% CI: 0.006-0.943)] (all P<0.05). Conclusions:The health care staff in a large public hospital have a good evaluation of the operation effect of the “1+1+2M”health management model. The higher the evaluation of work collaboration, business norms, and model configuration by the health care staff, the higher the overall satisfaction with the health management model.
3.Clinical characteristics and treatment outcomes of adult patients with phytosterolemia presenting with Thrombocytopenia

Objective:To analyze the clinical characteristics of adult patients with phytosterolemia presenting with thrombocytopenia as the initial manifestation.Methods:A retrospective analysis was conducted on eight adult patients with phytosterolemia who visited Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, from December 2020 to December 2023.Results:① The participants consisted of 2 (25%) male and 6 (75%) female patients, with a median age at diagnosis of 55 years (range: 29-66 years). The median duration from the discovery of thrombocytopenia to diagnosis was 10 years (range: 0.2-50 years). ② Compared with the normal control group (30 healthy adult volunteers) and the immune thrombocytopenia (ITP) control group (20 patients with ITP), patients with phytosterolemia exhibited significantly higher mean platelet volume and large platelet ratio. Peripheral blood smears revealed that the mean platelet diameter and the proportion of large platelets (diameter> 4 μm) were significantly higher in patients with phytosterolemia than those in the normal and ITP control groups ( P<0.01). ③ After a low-plant-sterol diet and ezetimibe treatment, five patients demonstrated decreased serum sitosterol and campesterol levels, increased hemoglobin concentration and platelet counts, and reduced platelet volume. Conclusion:Adult-onset phytosterolemia presenting with thrombocytopenia as the initial manifestation is prone to misdiagnosis. The presence of hemolytic anemia, splenomegaly, increased large platelets and schistocytes on peripheral blood smears, and xanthomas are crucial diagnostic indicators. Restricting dietary plant sterol intake and using ezetimibe to inhibit sterol absorption effectively lowers serum plant sterol levels and improves hematological abnormalities.
4.Astrocyte FGF7/FGFR2 autocrine signaling mediates neuroinflammation and promotes MPTP-induced degeneration of dopaminergic neurons.

Reactive astrocytes, which exhibit a correlation with the degeneration of dopaminergic neurons, are present in a considerable number during the progression of Parkinson's disease (PD). However, the underlying factors shaping astrocyte reactivity and neuroinflammation in PD remain inadequately elucidated. Here, we demonstrate that fibroblast growth factor 7 (FGF7)/FGF receptor 2 (FGFR2) autocrine signaling intensifies astrocyte reactivity and inflammation. Genetic deletion of Arrb2, β-Arrestin2 encoding gene, led to escalated astrocyte reactivity in MPTP-treated mice, which was further substantiated in astrocyte-specific Arrb2 knockdown mice. RNA sequencing profiling of Arrb2 knockout astrocytes identified Fgf7 as a critical effector of astrocyte reactivity. Subsequently, conditional knockdown of Fgf7 and its receptor Fgfr2 in astrocytes elicited advantageous effects for MPTP-treated mice by restraining the inflammatory phenotypic transition of reactive astrocytes. Furthermore, deletion of astrocytic Fgf7 mitigated MPTP-induced pathology in Arrb2 knockout mice. Mechanistically, STAT1 was distinguished as the transcription factor suppressing Fgf7 expression, while β-Arrestin2 counteracted the proteasomal degradation of STAT1 by binding to RNF220, an E3 ubiquitin ligase for STAT1. More importantly, selectively engaging dopamine D2 receptor (Drd2)/β-Arrestin2-biased signaling using the agonist UNC9995 exhibited therapeutic potential in MPTP-treated mice via moderation of astrocytic FGF7 production, thereby restoring balance in astrocyte reactivity. Collectively, our study bridges a crucial knowledge gap by elucidating the novel functions of FGF family members within the central nervous system, particularly within the context of PD. The autocrine signaling of FGF7/FGFR2 represents a novel mechanism and a potential druggable target for modulating astrocyte-derived inflammation.
5.Comparative analysis between circumareolar small incision and three-port endoscopic surgery for the treatment of Simon Ⅱ gynecomastia

Objective To compare the clinical efficacy between liposuction combined with circumareolar small incision and three-port endoscopic surgery for the treatment of Simon Ⅱ gynecomastia (GYN). Methods Comparative case data of 120 patients with GYN were retrospectively analyzed, 61 patients in the open group underwent circumareolar small incision mastectomy after liposuction, and 59 patients in the endoscopic group underwent three-port endoscopic mastectomy after liposuction. The two groups were compared in terms of surgery-related indexes, occurrence of postoperative complications and patient satisfaction. Results The unilateral operation time of the open group was shorter than that of the endoscopic group, the unilateral gland resection weight in the open group was more than that in the endoscopic group, the hospitalization cost of the open group was less than that of the endoscopic group (all P<0.01). There was no significant difference in unilateral liposuction volume, drainage volume on the first postoperative day, and time to drain removal between the two groups (P>0.05). The incidence of complications in the open group and the endoscopic group were 8.2% and 13.6% respectively, and there was no significant difference between the two groups (P>0.05). The difference in the overall satisfaction scores between the two groups was not statistically significant (P>0.05). Conclusions Liposuction combined with circumareolar small incision or three-port endoscopic surgery both has good cosmetic effects in the treatment of Simon Ⅱ GYN. The operation with circumareolar small incision is simple, has a shorter operation time, costs less, and does not require special equipment, which is suitable for promotion and application in medical institutions.
6.Research progress and nursing inspiration on the relationship between social activities and cognitive function in older adults

With the increasing aging of the population in China,the problem of cognitive decline with age is becoming more prominent,and how to move forward to maintain and improve cognitive function in older adults has become an important research topic.Active participation in social activities has a protective effect on cognitive function in older adults.This paper summarizes the concepts of social activities,relationships with cognitive function,and strategies for promoting interventions.The corresponding nursing inspirations were proposed,aiming to provide new perspectives for improving cognitive function in older adults.
7.Analysis of influencing factors on the trajectories of psychological symptom clusters in pregnant women with assisted reproductive technology and nursing revelation

Objective To explore the trajectory of psychological symptom clusters in pregnant women with assisted reproductive technology(ART),and analyze the influencing factors of each trajectory subgroups,in order to provide a theoretical basis for the management of psychological health during pregnancy in pregnant women with ART.Methods A total of 205 pregnant women who had conceived using ART were sampled from the obstetrics clinic of a tertiary hospital in Nanjing from August 2023 to April 2024 using a convenient sampling method.The baseline data were assessed by general information questionnaire,Symptom Checklist-90,Distress Disclosure Index and Positive Psychological Capital Questionnaire at 10-14 weeks gestation,and the follow-up information was assessed by Symptom Checklist-90 at 22-26 weeks of gestation and 34-38 weeks of gestation.Exploratory factor analysis was used to extract symptom clusters;the latent class growth mixture model was used to identify the track categories;the multiple logistic regression analysis was used to analyze the influencing factors of the track.Results 180 cases were finally included.By exploratory factor analysis,5,4 and 5 factors were extracted at 3 time points respectively.Trajectories of psychological symptom clusters in pregnant women with ART is divided into 3 potential classes:low level-slow relieving group(28.89%),high level-significant increasing group(6.11%),medium level-slow increasing group(65.00%).Logistic regression analyses showed that duration of infertility,number of ART,literacy,pain self-expression and positive psychological capital were influential factors in the potential categories of psychological symptom clusters in pregnant women conceived with ART(all P<0.05).Conclusion The trajectory of psychological symptom clusters in pregnant women with ART was divided into 3 potential classes.Medical workers could develop corresponding interventions based on the influencing factors and implement comprehensive and efficient symptom management.
8.Scoping review of financial toxicity assessment tools

Objective:To conduct a scoping review of financial toxicity assessment tools both domestically and internationally,and to summarize their scoring methods,evaluation content,development approaches,reliability/validity,and application status,aiming to provide references for selecting or developing such tools.Methods:Following scoping review methodology,databases including CNKI,VIP,Wanfang,PubMed,Web of Science,Embase,and CINAHL were searched for studies on financial toxicity assessment tools,with the search period spanning from database inception to Oct 8,2024.Results:A total of 34 eligible studies were included,covering 14 financial toxicity assessment tools.These tools primarily focused on economic status,psychosocial responses,or coping behaviors.Tools developed by domestic scholars exhibited strong specificity but lacked generalizability.Conclusions:Existing financial toxicity assessment tools are diverse,yet their reliability and validity in Chinese patient populations require further validation.Most scales lack comprehensive dimensions,and domestically developed tools are overly specific.A universal assessment tool suitable for China's cultural and policy environment needs to be developed.
9.Research progress on sentinel symptoms within symptom clusters in lung cancer patients

Lung cancer patients frequently experience multiple co-occurring symptoms that manifest as symptom clusters during treatment,severely compromising patients'quality of life.Sentinel symptoms,coexisting with symptom clusters,serve as predictive indicators for the emergence of these clusters.This study focuses on sentinel symptoms within symptom clusters in lung cancer patients,reviewing the current research status,identification methods,related intervention measures,and exist-ing research shortcomings.It summarizes the problems in current studies,with the goal of providing clinical nursing staff to as-sess and manage symptom clusters,as well as to conduct related nursing research.
10.Current status and factors influencing physical activity among pre-frail and frail older adults in the community

Objective:To explore the current status and factors influencing physical activity among pre-frail and frail older adults in the community.Methods:Convenience sampling was used to select 207 pre-frail and frail older adults from Donghuashi community and Fangzhuang community in Beijing from April to June 2024 as study subjects. Older adults were surveyed for their general information, lifestyle behaviors, nutritional status, and physical activity. Binary Logistic regression was used to explore the factors influencing the level of physical activity among pre-frail and frail older adults in the community.Results:A total of 207 questionnaires were distributed and 204 valid questionnaires were recovered, with a valid recovery rate of 98.55%. The Physical Activity Scale for the Elderly score of the 204 community-based pre-frail and frail older adults was 86.87 (52.14, 125.00), and the form of activity was predominantly walking (98.5%, 201/204) and light domestic physical activity (85.8%, 175/204). Binary Logistic regression showed that taking a nap ( OR=3.614), abnormal nighttime sleep duration ( OR=4.077), fear of falling before or during exercise ( OR=7.895), and risk of malnutrition ( OR=9.263) were risk factors for levels of physical activity in pre-frail and frail older adults in the community ( P<0.05), and good exercise cognition ( OR=0.055) was a protective factor for physical activity levels ( P<0.05) . Conclusions:Pre-frail and frail older adults in the community have low levels of physical activity, which is dominated by walking and household activities. Community healthcare workers should strengthen the management of physical activity for pre-frail and frail older adults, cultivate their good living habits, improve their sleep quality, ensure sufficient night sleep, help them overcome the fear of falling before or during exercise, set up the correct concept of exercise, form a good cognition of exercise, and guide their family members to pay attention to the nutritional status of older adults, beware of the risk of malnutrition, and improve the level of physical activity to delay or even reverse the frail state.
