1.Association study of pediatric obstructive sleep apnea hypopnea syndrome and otitis media with effusion based on clinical prediction model and Mendelian randomization

OBJECTIVE To explore the correlation between childhood obstructive sleep apnea hypopnea syndrome and otitis media with effusion,to provide evidence-based basis for clinical diagnosis and treatment.METHODS A total of 985 children were diagnosed in the Department of Otolaryngology,Heping Hospital Affiliated to Changzhi Medical College from September 2019 to March 2025.Combined or no OME patients for multivariate logistics regression analysis and construct related risk prediction model,obtain data of OSAHS(exposure factors)and OME(outcome)from the public database of genome-wide association studies(GWAS),two-sample Mendelian randomization(MR)analysis.RESULTS The risk prediction model for OME in children with OSAHS,Hosmer-Lemeshow goodness of fit test(χ2=12.541,P=0.157),C index of 0.749,Calibration calibration curve fitting,and correlation between children OSHAS and OME.Furthermore,the MR analysis using inverse variance weighting observed a positive causal relationship between OSAHS and OME in children(OR=1.05,95%CI=0.999-1.051,P=0.005).MR Egger test results(Q=42.358,P=0.716)and IVW(Q=43.271,P=0.759)showed no heterogeneity in the analysis,MR Egger intercept(intercept=-0.004,P=0.921)showed no horizontal pleiotropy,and the MR analysis results were stable.CONCLUSION There is a correlation between pediatric OSAHS and OME,and it increases the risk of OME.
2.Analysis of virtual labor medication rules and clinical experience based on data mining

Objective To analyze the medication rules and clinical experience of Qin Zujie in treating virtual labor based on data mining.Methods Outpatient medical records of Qin Zujie,diagnosed with deficiency syndrome at Ethnic Medicine Characteristic Diagnosis and Treatment Center,Guangxi International Zhuang Medical Hospital from January to December 2024 were selected.A standardized database was established and processed,yielding 148 clinical prescriptions containing 171 herbal ingredients.Frequency analysis,association rule analysis,and cluster analysis were employed to examine medication rules and clinical experience.Results The 20 Chinese herbal ingredients used more than 30 times each,with the first three being Zhigancao,Baizhu,Danggui.These herbs are primarily warm in nature,neutral in temperature,and slightly cold in effect,characterized by sweet,pungent,and bitter flavors.They mainly affect the spleen meridian and function to tonify deficiency,with Qi-tonifying herbs being the most common.Through association rule analysis,the most frequently combined pairs were Baizhu-Dangshen.Cluster analysis identified three core prescription clusters.Conclusion Qin Zujie's academic thought of treating virtual labor is to supplement deficiency as the main,detoxification as the auxiliary,and treat both the symptoms and the causes.

4.Flexible ultrasound array transducer technology for measuring intravascular diameter and blood flow velocity

Objective To explore the feasibility of prototype of flexible ultrasound array transducer for measuring intravascular diameter and blood flow velocity.Methods A prototype of flexible ultrasonic array transducer composed of a surface mount flexible ultrasonic array transducer(referred to as the transducer)and acquisition system was designed and fabricated.Doppler body model and vascular simulation control system was used to simulate human blood.Mindray M7 color multi-functional ultrasound diagnostic instrument(abbreviated as M7)and the prototype were adopted to detect pulse wave excitation mode,spectral morphology,echo signal time interval,blood flow velocity,etc.Taken the results of M7 as standards,the effectiveness of the prototype was evaluated.Then the inner diameter and flow velocity of left common carotid artery,left brachial artery and left radial artery of 20 healthy controls(HC)were measured respectively using M7 and the prototype,and the consistency of measurement results were analyzed to evaluate the accuracy of the prototype.The safety of the prototype was evaluated according to subjectively scoring the comfort level of HC applying the transducer and whether there was skin discomfort,injury,etc.after removing the transducer.Results The collected blood flow spectrum of the above vascular simulation control system acquire with the prototype were clear,and the spectral morphology,flow velocity,as well as echo signal time interval were basically consistent with those of M7.The consistency of the inner diameter of left common carotid artery,left brachial artery and left radial artery measured by the prototype and M7 in 20 HC were all very strong(ICC=0.919-0.992),while of flow velocities were moderate or very strong(ICC=0.524-0.831).The subjective score of comfort level of application of transducer was 3(2,3),and no skin discomfort occurred.Conclusion Flexible ultrasound array transducer technology could be used to measure intravascular diameter and blood flow velocity.

7.Causal relationship between circulating inflammatory factors and sensorineural hearing loss:A two-way Mendelian randomization study

OBJECTIVE To explore the causal relationship of circulating cytokines and sensorineural deafness by Mendelian randomization.METHODS Large-scale genome-wide association studies(GWAS)data sets based on UK Biobank were used to identify single nucleotide polymorphisms(SNP)associated with exposure and outcome.The SNP(F>10)significantly associated with exposure data(P<5e-6)and strongly associated with exposure in the F statistic was selected as the instrumental variable for a two-sample Mendelian randomization analysis.The causal effect was estimated using MR-Egger,regression and inverse variance weighting(IVW).Sensitivity analysis was performed to assess the robustness of the results.RESULTS This study confirmed that VEGF(OR=1.04,95%CI:1.01-1.07,P=0.00),SDF-1α(OR=1.10,95%CI:1.02-1.18,P=0.01)and IL-13(OR=1.04,95%CI:1.01-1.06,P=0.01),IL-10(OR=1.06,95%CI:1.02-1.11,P=0.01),IL-7(OR=1.05,95%CI:1.01-1.08,P=0.00)increased the risk of sensorineural deafness.The backward analysis did not find that sensorineural deafness significantly affected the levels of these cytokines.No significant heterogeneity or horizontal pleiotropy was observed among the instrumental variables.CONCLUSION This study reveals a positive correlation between elevated levels of VEGF,SDF-1α,IL-13,IL-10,and IL-7 with sensorineural hearing loss,providing novel targets for further development of biomarkers for the prediction,treatment,and prevention of sensorineural hearing loss.
8.Study on the correlation between opioid-induced constipation and gene polymorphism

OBJECTIVE To investigate the effect of gene polymorphism on opioid-induced constipation. METHODS The target genes related to opioid-induced constipation were screened out through searching guidelines, databases and evidence-based medical data, and then 100 cancer pain patients who received opioid drugs for analgesia were included as the study subjects. According to whether there were adverse effects of constipation after medication or not, they were divided into test group and control group, with 50 cases in each group. The target gene was detected by PCR or fluorescence in situ hybridization. The SNPStats program was used to carry out Hardy-Weinberg balance test and correlation analysis between gene polymorphism and opioid-induced constipation. The multivariate Logistic regression analysis was used to explore the relevant predictive factors of opioid-induced constipation, and receiver operating characteristic (ROC) curve of subjects was drawn to analyze the effectiveness of each predictive factor in predicting opioid-induced constipation. RESULTS CYP2D6, CYP3A5*3, ABCB1 and OPRM1 were selected as target genes for detection. The results of genotype detection showed that the frequency distribution of CYP2D6 (rs1065852, rs1135822, rs16947, rs28371725, rs28371735), CYP3A5*3 (058rs776746), ABCB1 (062rs1045642), OPRM1 (047rs1799971) alleles were consistent with Hardy-Weinbergbalance test. The correlation analysis results showed that the proportion of genotype GG and AG in CYP3A5*3 (058rs776746, 163.com A>G) and genotype AA and AG in OPRM1 (047rs1799971, A>G) of patients was significantly higher in test group than that in the control group (P<0.05). Multivariate Logistic regression analysis showed that medication duration, CYP3A5*3 and OPRM1 gene polymorphism could be used as predictors of opioid- induced constipation in patients (P<0.05). The ROC curve analysis results showed that the areas under the ROC curves for medication duration and CYP3A5*3, OPRM1 gene polymorphism were 0.648, 0.640 and 0.670, respectively, with the optimal cutoff values of 124.0, 0.5 and 0.5, respectively. CONCLUSIONS Genotype GG and AG in CYP3A5*3 (058rs776746,A>G) and genotype AA and AG in OPRM1 (047rs1799971,A>G) are associated with opioid-induced constipation, which are expected to become clinical predictors of opioid-induced constipation, and more attention should be paid to the occurrence of constipation in patients who have been taking opioids for a long time.

Objective:To explore the functional changes of brain network node information integration and separation in patients with obsessive-compulsive disorder (OCD) and its correlation with clinical symptoms.Methods:Fifty-six patients with obsessive-compulsive disorder (OCD group) and fifty-six healthy controls (control group) matched in gender, age and years of education were enrolled. All participants accepted resting-state functional magnetic resonance imaging scans of the whole brain. Yale-Brown Obsessive Compulsive Scale(Y-BOCS) was used to assess patients′ clinical symptoms. Based on graph theory and independent-sample t-test, the differences in functional network topological properties of nodes between the two groups were analyzed. Partial correlation analysis was used to discuss the relationship between the values of these properties and clinical symptoms. Results:Compared with the control group, the OCD group showed decreased global efficiency and increased shortest path length of the left superior temporal gyrus rostral area and right posterior central gyrus trunk region(both P<0.001, FDR corrected); decreased local efficiency( P=0.002, uncorrected) and clustering coefficient( P<0.001, FDR corrected) of the left inferior frontal gyrus dorsal region. Further analysis showed that the score of the global efficiency of the left superior temporal gyrus rostral area was positively correlated with the score of obsessive thoughts sub-scale ( r=0.390, P=0.005). The score of the shortest path length of the left superior temporal gyrus rostral area was negatively correlated with the obsessive thoughts sub-scale ( r=-0.359, P=0.010) in the OCD group. Conclusion:OCD patients have abnormal information integration and separation functions in the frontoparietal network(inferior frontal gyrus, posterior central gyrus) and abnormal information integration functions in the superior temporal gyrus rostral area. Higher global efficiency and shorter the shortest path length of the superior temporal gyrus rostral area suggest more obsessive thoughts in OCD patients.
