1.Exploring the Pathogenesis and Treatment of Adenoidal Hypertrophy in Children Based on the "Kenang (窠囊)" Theory

By analyzing the connotation of "kenang (窠囊)" and its relationship with adenoid hypertrophy (AH) in children, this paper suggests that kenang plays a significant role in the development process of pediatric AH, from the initial accumulation of phlegm-dampness in the early stage, to the binding of phlegm and stasis in the middle stage, and finally the healthy qi deficiency with phlegm stagnation in the late stage, and may also serve as a key entry point for the diagnosis and treatment. From the perspective of "kenang", and in combination with the core pathogenesis of pediatric AH characterized by lung heat and phlegm stasis, this paper proposes that the key therapeutic principles include clearing heat to resolve phlegm, activating blood to remove stasis, and reducing swelling to dissipate nodules. A staged treatment strategy based on the self-formulated Bizhi Formula (鼻窒方) with modifications is used. In the early stage of the disease, the treatment should be soothing the wind and relieving the exterior, resolving phlegm and dredging the orifices to prevent the initial knot of kenang, for which Bizhi Formula and Cangerzi Powder (苍耳子散) with modifications can be used. In the middle stage, the treatment should be dissolving phlegm and expelling stasis, dispersing knots and unblocking orifices to break down the establishment of kenang, and Bizhi Formula combined with Haizao Yuhu Decoction (海藻玉壶汤) with modifications is suggested. In the late stage, the treatment emphasizes reinforcing healthy qi and consolidating the root, resolving phlegm and dredging collaterals in order to prevent recurrence of kenang, and the suggested prescription is modified Bizhi Formula combined with Shenling Baizhu Powder (参苓白术散).

3.Application of preoperative pulmonary rehabilitation training in patients undergoing cardiac surgery:a scoping review

Objective A scoping review of studies on pulmonary prehabilitation in cardiac surgery patients was conducted to provide evidence support for the construction of a preoperative pulmonary rehabilitation program for cardiac surgery patients that suitable for China's national conditions.Methods In accordance with the scope review's research methodologies,databases including PubMed,Embase,Web of Science,CINAHL,CNKI and Wanfang were searched by the computer for relevant studies.The deadline for retrieval is from the establishment of databases to June,2024.The included literature was systematically analyzed.Results 26 articles were finally included.Among them,4 were quasi-experiment studies,while the other 22 were randomized controlled trials.Forms of the intervention included comprehensive breathing exercises,inspiratory muscle training,positive expiratory pressure,incentive spirometer training and balloon blowing training.The intervention initiation ranged from 10 weeks to 1 day preoperatively;the outcome measures included postoperative pulmonary complications,lung function metrics,the 6-Minute Walk Test,duration of mechanical ventilation,length of hospital stay,patient-reported outcomes and so on.Conclusion There remains a deficiency in standardized protocols for preoperative pulmonary rehabilitation training among patients undergoing cardiac surgery.High-quality studies should be conducted,and intervention strategies for pulmonary prehabilitation in cardiac surgery patients should be optimized and a unified evaluation standard system should be established.
4.Ideological and political teaching practice of Medical Immunology course based on mixed teaching model—Taking"Antibody"as an example

The integration of ideology and politics into the basic course of medical specialty is one of the important contents of the school teaching reform,and it is also an important way for the school to realize moral cultivation and"all-round education,whole-process education,all-dimensional education".Taking the Medical Immunology course of Xinjiang Second Medical College as an example,this paper explores the teaching practice cases of the ideological and political content of the Medical Immunology course by organically integrating the ideological and political elements of the course with the teaching knowledge points through the mixed teaching mode,which is conducive to cultivating students'strong social responsibility and excellent professional ethics,enhancing cultural and national self-confidence,and enhancing patriotism,so as to better carry out the basic task of moral cultivation.

Objective:To analyze the current status, hotspots, and trends of community integration (CI) at home and abroad, providing directions for subsequent research.Methods:The China National Knowledge Infrastructure and the Web of Science Core Collection data were used, and CiteSpace software was employed to analyze the publication, countries, disciplines, and keywords of CI.Results:A total of 932 Chinese and 2 319 English hits were included. The publication of CI had shown an increasing trend both domestically and internationally. Sociology and rehabilitation were the main disciplines in CI domestically and internationally, respectively. Domestically, CI research primarily focused on the community integration of migrant workers, floating populations, and accompanying family members. Internationally, the emphasis was on the integration of individuals with disabilities and other special groups. Future research directions in China were likely to focus on group work, the elderly, and relocated populations. Internationally, CI research would continue to concentrate on CI experiences and intervention methods for individuals with disabilities and special populations.Conclusion:CI is increasingly gaining attention both domestically and internationally. However, domestic research remains insufficient and needs to be expanded through more empirical studies in various vertical subfields. Future domestic scholars should continue to focus on CI of the elderly and engage in interdisciplinary research.

9.Experience of the Treatment of Tubal Infertility Using Three-Stage Advanced Method

In traditional Chinese medicine, it is believed that the disease of tubal infertility is located in the uterus vessels, with stasis blocking uterus vessels as the core mechanism, and according to the characteristics of pathological changes in the course of treatment, the "three-stage advanced method" is proposed as the treatment plan. In the first stage of eliminating evil, the disease mechanism is characterized by externally-contracted heat toxin combined with endogenous dampness, stagnation and stasis in the uterus vessels, and the treatment is to clear heat and promote dampness, move qi and activate blood, and the self-prescribed Penyan Xiao Formula (盆炎消方). In the second stage of dissolving fixed abdominal mass, the disease mechanism is characterized by qi and blood stagnation and uterus vessels obstruction, and the treatment is to break up the stagnation of blood and move qi, drive away blood stasis and clear the channels, with self-prescribed Tongguan Formula (通管方). In the third stage of reinforcing healthy qi to support pregnancy, the disease mechanism is characterized by stasis of uterus vessels for a long period, and loss of kidney essence, therefore the treatment is to warm up the kidneys and eliminate the stasis, boost qi and nourish yin, with self-prescribed Yulin Zhuyun Formula (毓麟助孕方). At the same time, attention should be paid to the synergistic diagnosis and treatment of traditional Chinese medicine and Western medicines, and combination of syndrome differentiation with the identification of diseases.
10.Infection status and molecular types of rhinovirus among cases of acute respiratory tract infections in Luohe City, Henan Province, from 2017 to 2022

Objective:To understand the infection status and molecular types of rhinovirus (RV) among cases of Acute Respiratory Infections (ARIs) in Luohe City, Henan Province, from 2017 to 2022.Methods:From October 2017 to June 2022, clinical and epidemiological data were collected from 2 270 cases of ARIs at Luohe Central Hospital in Henan Province. Throat swab specimens were obtained from these cases. Real-time quantitative polymerase chain reaction (qPCR) was used to screen for RV-positive specimens. Subsequently, the positive samples were subjected to nested reverse transcription polymerase chain reaction (nested RT-PCR) to amplify the full-length VP1 region. Using the MEGA software, along with 169 RV reference strains recommended by the International Committee on Taxonomy of Viruses, a phylogenetic tree was constructed to determine RV types.Results:Among the 2 270 cases of ARIs, there were 1 283 male cases (56.52%). The median age ( Q 1, Q 3) was 3 (1, 6) years, with the population under 5 years old accounting for 68.59% (1 557/2 270). RV was detected in 137 cases (6.04%), of which 68 cases (49.64%) showed co-detection with other viruses, with the most common being co-detection with enterovirus, accounting for 14.60% (20/137). The RV detection rates in the age groups of 0-4 years, 5-14 years, 15-59 years, and≥60 years were 6.42% (100/1 557), 4.69% (21/448), 3.80% (6/158), and 9.35% (10/107), respectively, with no statistically significant differences ( χ2=5.310, P=0.150). The overall detection rates of RV before (2017-2019) and during (2020-2022) the COVID-19 pandemic showed no statistically significant difference ( χ2=1.823, P=0.177). A total of 109 VP1 sequences were obtained, including 62 types. Among them, RV-A, RV-B, and RV-C had 42, 3, and 17 types respectively. Conclusion:RV is one of the predominant pathogens in ARIs cases in Luohe City, Henan Province, from 2017 to 2022. Multiple types of RV co-circulate without any apparent dominant type.
