1.Relationship between ventricular arterial coupling and haemodynamics in patients with sepsis and its predictive value for prognosis

Objective To investigate the relationship between ventricular arterial coupling(VAC)and haemodynamics of patients with sepsis and the predictive value for the 28-day prognosis.Methods A total of 164 patients with sepsis were selected and given fluid resuscitation treatment(infusion of 30 mL/kg of crystalloid over 3 h),and changes of VAC[effective aortic elasticity(Ea)/ventricular end-systolic elasticity(Ees)]values and hemodynamic parameters of patients resuscitated for 0 h,24 h and 72 h were observed.The difference between Ea/Ees for 24 h and 72 h of resuscitation and 0 h was calculated.Pearson method was used to analyze the correlation between Ea/Ees and Δcentral venous pressure(CVP),Δspiratory volume index(SVI)and Δcardiac index(CI).According to the 28-day prognosis,patients were divided into two groups:the survival group(123 cases)and the death group(41 cases).General information was collected in two groups of patients.The predictive values of 24 h ΔEa/Ees and 72 h ΔEa/Ees for 28-day death in sepsis patients were analyzed by multivariate Cox regression models and receiver operating characteristics(ROC)curve.Results The Ea/Ees of 164 patients resuscitated for 24 h and 72 h were lower than those of 0 h,and the Ea/Ees of 72 h was lower than that of 24 h(all P<0.05).CVP,SVI and CI of 164 patients resuscitated for 24 h and 72 h were higher than those of 0 h,and the CVP,SVI and CI of 24 h was higher than that of 72 h(all P<0.05).The 24 h ΔEa/Ees was significantly lower than that of 72 h ΔEa/Ees(P<0.05),and 24 h ΔCVP,SVI and CI were higher than those of 72 h(P<0.05).There was a negative correlation between 24 h ΔEa/Ees and 24 h ΔCVP,24 h ΔSVI and 24 h ΔCI(P<0.05).Acute Physiology and Chronic Health Status Score II(APACHE Ⅱ)≥20 points,Sequential Organ Failure Score(SOFA)≥10 points,24 h ΔEa/Ees and 72 h ΔEa/Ees were higher in the death group than those in the survival group(P<0.05).Elevated APACHE Ⅱ,SOFA,24 h ΔEa/Ees and 72 h ΔEa/Ees were risk factors affecting the 28-day prognosis of patients with sepsis(P<0.05).The 24-hour ΔEa/Ees and 72-hour ΔEa/Ees were more effective in predicting the 28 d prognosis of patients with sepsis(P<0.05).Conclusion Changes of Ea/Ees can better reflect the haemodynamic changes in sepsis patients in the early stages of treatment and help to predict the 28-day prognosis of sepsis patients.
2.Correlation between Serum IGF-1,IGFBP-3,BAP and Abundance of Intestinal Flora in Children with Idiopathic Short Stature

Objective To explore the correlation between serum insulin-like growth factor1(IGF-1),insulin-like growth factor binding protein3(IGFBP-3),bone-specific alkaline phosphatase(BAP)and abundance of intestinal flora in children with idiopathic short stature(ISS).Methods A total of 86 children with ISS who were treated with short-acting recombinant growth hormone for the first time in Department of Endocrinology of Baoding Hospital,Beijing Children's Hospital,Capital Medical University,were enrolled from July 2021 to June 2023.Before treatment and at 6 months after treatment,physical indexes(height,weight)and bone age indexes[bone age index(BAI),bone age difference(BAD)]were detected with the same instrument at the same place.The fasting venous blood was collected to detect levels of serum IGF-1,IGFBP-3 and BAP by enzyme-linked immunosorbent assay.The abundances of intestinal floras were monitored by 16s rDNA high-throughput sequencing technology combined with bioinformation analysis technology.A total of 40 healthy children during the same period were enrolled as control group,and differences in different indexes within the same groups or between the two groups were compared by independent sample t test or paired t test.The correlation between abundances of intestinal floras and levels of serum IGF-1,IGFBP-3 and BAP was analyzed by Pearson correlation analysis.Results Compared with the control group.the height,weight,BAD,BAI,levels of serum IGF,IGFBP-3 and BAP,Chaol,Shannon and Pielou e of ISS patients decreased before treatment,and the differences were statistically significant(t=7.119~44.143,all P<0.05).After treatment,height,weight,BAD,BAI,levels of serum IGF,IGFBP-3 and BAP,Chaol,Shannon and Pielou e were increased in ISS children(t=3.145~39.188),the above indexes were still lower than those in control group(t=2.389~69.893),the differences were statistically significant(all P<0.05),respectively.In intestinal floras,dominant bacterial groups at phylum level were Firmicutes,Bacteroidetes,Actinobacteria and Proteobacteria,while dominant bacterial groups at genus level were Blautia,Bifidobacterium,Faecalibacterium and Bacteroides.Compared with the control group,before treatment,the abundances of Actinobacteria and Proteobacteria increased in the ISS group,while the abundances of Bacteroidetes,Bifidobacteria and Bacteroides decreased,and the differences were statistically significant(t=25.224~46.345,all P<0.05).After treatment,abundances of Bacteroidetes,Bifidobacterium and Bacteroides in the ISS group increased compared to before treatment,while abundance of Proteobacteria decreased,and the differences were statistically significant(t=10.055~39.245,all P<0.05).The abundances of Bacteroidetes,Bifidobacterium and Bacteroides in ISS group were lower than those in control group,while abundance of Proteobacteria was higher than that in control group,and the differences were statistically significant(t=6.290~20.540,all P<0.05).The abundances of Bacteroidetes,Bifidobacterium and Bacteroides were positively correlated with levels of serum IGF-1,IGFBP-3 and BAP(r=0.300~0.759,all P<0.05).Conclusion The abundances of intestinal floras are decreased,and the structure is abnormal in children with ISS.The abundances of beneficial bacteria in intestinal floras are related to levels of serum IGF-1,IGFBP-3 and BAP.In clinical treatment of ISS,combined treatment with intestinal flora disorders may be conducted.

5.Type 2 spondyloepimetaphyseal dysplasia with joint laxity caused by KIF22 gene mutation: a case report and literature review

A male patient aged 1 year and 8 months with type 2 spondyloepimetaphyseal dysplasia with joint laxity (SEMDJL2) was reported. The clinical characteristics included short stature, flat middle face, hypotonia, limb joint relaxation, hyperextension of metacarpophalangeal articulation, etc. In addition, the patient had a history of congenital laryngeal stridor. Thus, SEMDJL2 was determined according to the above symptoms and medical history. Sanger sequencing showed that the child carried a c.443C>T missense mutation in the KIF22 gene, which resulted in an amino acid variation namely p.Pro148Leu. This phenotype was preliminarily determined as a pathogenic mutation. Therefore, it is suggested that next-generation sequencing genetic testing could be helpful for genetic diagnosis in children with congenital laryngeal stridor, systemic joint relaxation, and excessive joint extension.
6.Therapeutic efficacy of extended therapy with interferon and a nucleoside analogue for treating patients with HBeAg-positive chronic hepatitis B

Objective To observe the therapeutic efficacy of extended therapy with interferon and a nucleoside analogue for treating patients with HBeAg?positive chronic hepatitis B( CHB)?Methods Sixty cases patients diagnosed with CHB in You′an Hospital of Beijing Affiliated to Capital Medical University from March 2012 to May 2015 were retrospectively analyzed?They were divided into group A with 29 cases and group B with 31 cases according to different treatment methods?Group A were treated with pegylated interferonα?2a( Peg?IFNα?2a) combined with adefovir dipivoxil for 96 weeks,group B were treated with Peg?IFN α?2a combined with adefovir and lamivudine for 96 weeks?The patients'recovery rate of ALT,hepatitis B virus( HBV) DNA response rate and HBeAg and HBsAg seroconversion rate and conversion rate of 12,24,48,96 weeks in the treatment were observed?Results There were no significant differences in recovery rate of ALT,hepatitis B virus( HBV) DNA response rate,HBeAg seroconversion rate and HBsAg seroconversion rate between the two groups at differenct time points( P>0?05)?HBeAg seroconversion rates in group A were 34?5% at 48 weeks,62?1% at 96 weeks, and the difference was significant( P=0?036)?HBeAg seroconversion rates in group B were 35?5% at 48 weeks, 61?3% at 96 weeks,and the difference was significant ( P=0?042)?Conclusion The treatment of peginterferonα?2a in combination with adefovir dipivoxil and lamivudine is not more efficacious than peginterferon α?2a in combination with adefovir dipivoxil?The extended treatment of Peg? IFNα?2a plus a nucleoside analogue can achieve high rates of HBeAg seroconversion in patients with HBeAg?positive CHB.
8.A Modified Extraction Method of Circulating Free DNA for Epidermal Growth Factor Receptor Mutation Analysis.

PURPOSE: Circulating free DNA (cfDNA) in plasma is promising to be a surrogate for tumor tissue DNA. However, not all epidermal growth factor receptor (EGFR) mutations in tumor tissue DNA has been detected in matched cfDNA, at least partly due to inefficient cfDNA extraction method. The purpose of this study was to establish an efficient plasma cfDNA extraction protocol. MATERIALS AND METHODS: The yield of plasma cfDNA extracted by our modified phenol-chloroform (MPC) method from non-small-cell lung cancer (NSCLC) patients was compared with that by QIAamp MinElute Virus Spin kit (Qiagen kit) as control, using the Wilcoxon rank-sum test. TaqMan quantitative polymerase chain reaction (qPCR) assays were used to quantify the plasma cfDNA extracted. Both Mutant-enriched PCR (ME-PCR) coupled sequencing and DxS EGFR mutation test kit were used to evaluate the impact of extraction method on EGFR mutation analysis. RESULTS: MPC method extracted more plasma cfDNA than Qiagen kit method (p=0.011). The proportion of longer fragment (> or =202 bp) in cfDNA extracted by MPC method was significantly higher than by Qiagen kit method (p=0.002). In the sequencing maps of ME-PCR products, a higher mutant peak was observed on plasma cfDNA extracted by MPC method than by Qiagen kit method. In DxS EGFR mutation test kit results, plasma cfDNA extracted by MPC method contained more tumor-origin DNA than by Qiagen kit method. CONCLUSION: An improved plasma cfDNA extraction method of MPC is provided, which will be beneficial for EGFR mutation analysis for patients with NSCLC.
