1.Research progress on the intervention of human umbilical cord mesenchymal stem cell in neurodegenerative disease

Human umbilical cord mesenchymal stem cell (hUC-MSC) as a cell-based therapeutic strategy have demonstrated significant application potential in the field of intervention for neurodegenerative disease (NDD) due to their advantages such as self-renewal, multi-directional differentiation, and low immunogenicity. hUC-MSC effectively intervenes in the pathological features and neurological functions of various disease models such as Alzheimer disease, Parkinson’s disease, amyotrophic lateral sclerosis, and multiple sclerosis primarily through multiple mechanisms such as homing and differentiation, mediating paracrine actions and releasing exosomes, as well as immune regulation and anti-inflammation. Some clinical studies have also preliminarily verified their safety and effectiveness. Currently, its research still faces challenges such as immune rejection reactions requiring further observation, long-term safety needing evaluation, mechanisms of action not being fully elucidated, and slow progress in clinical trials. Future research needs to establish pharmaceutical standards for hUC-MSC, deepen their pharmacological mechanisms and clinical trials, ultimately providing new and effective drug treatment options for patients with NDD.
2.Role and clinical significance of CST1 in digestive system neoplasms

Cysteine protease inhibitor SN(Cystatin SN , CST1), a member of the type 2 cystatins superfamily, regulates tumor biological activities by modulating cysteine protease activity. CST1 is abnormally expressed in digestive system neoplasms, such as esophageal cancer, gastric cancer, hepatocellular carcinoma, colorectal cancer, and pancreatic cancer. CST1 influences tumor cell proliferation, migration, and epithelial-mesenchymal transition (EMT) through signaling pathways including Wnt, STAT3, and PI3K-AKT. Its expression levels significantly correlated with tumor stage, metastasis, and patient prognosis, suggesting its potiential as a novel biomarker for early diagnosis, therapeutic evaluation, and prognosis in digestive system neoplasms. This article systematically reviews the molecular characteristics of CST1, its mechanisms of action in digestive system neoplasms, clinical significance, and explores its potential as a therapeutic target.
3.Study on the in vivo intestinal absorption and tissue distribution of silybin nanocrystals prepared by two methods

OBJECTIVE To investigate the absorption characteristics and tissue distribution of silybin (Sy) nanocrystals prepared by two methods in different intestinal segments of rats. METHODS Sy nanocrystals (i.e. Sy-NS-G and Sy-NS-F) with comparable particle sizes were prepared using high-pressure homogenization and anti-solvent precipitation methods, respectively. Rats were randomly divided into three groups: Sy raw drug group, Sy-NS-G group, and Sy-NS-F group. Each group was further divided into three subgroups with low, medium, and high (60, 120, 180 μg/mL) mass concentrations (calculated based on Sy), with 3 rats in each subgroup. The absorption rate constant (Ka) and apparent absorption coefficient (Papp) of Sy raw drug, Sy-NS-G and Sy-NS-F in different intestinal segments were investigated by using the in vivo one-way intestinal perfusion experiment. Additionally, the rats were divided into three groups: Sy raw drug group, Sy-NS-G group, and Sy-NS-F group, with 20 rats in each group. Rats in each group were administered a single intragastric dose of 50 mg/kg (calculated based on Sy). They were sacrificed at 0.3, 1, 4, 10, and 24 hours post-administration respectively, to investigate the tissue distribution of Sy raw drug, Sy- NS-G, and Sy-NS-F in the heart, liver, spleen, lungs, kidneys, brain and intestines. RESULTS In duodenum and jejunum, the Ka and Papp of the nanocrystals prepared by the two methods remained unchanged with the increase of Sy concentration, and there was no significant difference (P>0.05); the absorption of Sy-NS-F in the duodenum was greater than that of Sy-NS-G; the absorption sites of Sy-NS-G and Sy raw drug were mainly in the ileum, while those of Sy-NS-F were mainly in the duodenum and ileum. The concentrations of Sy-NS-G and Sy-NS-F in different tissues of rats were different; Sy-NS-G peaked in most tissues at 1 h, and the distribution concentration was as follows: intestine>spleen>heart>lungs>liver≈brain>kidneys. Sy-NS-F reached its peak at 1 h, and the distribution concentration was in the order of intestine>spleen>kidney>lung>heart≈liver>brain. CONCLUSIONS The absorption mode of Sy nanocrystals in the duodenum and ileum is mainly passive diffusion. In the duodenum, the absorption of Sy-NS-F is greater than that of Sy-NS-G; there are significant differences in the tissue distribution of Sy-NS-G and Sy-NS-F in rats.
4.Research progress on the pharmacological mechanisms of Acorus tatarinowii-Polygala tenuifolia herb pair in ameliorating cognitive impairment

Cognitive impairment is a major public health challenge facing global aging societies, and currently lacks effective treatment measures. Herb pair, characterized by their rigorous compatibility and synergistic effects, demonstrate unique advantages in clinical practice. Acorus tatarinowii-Polygala tenuifolia is a classic herbal pair for treating cognitive impairment, widely utilized in various traditional Chinese medicine formulations, such as Kaixin san, Shenghui tang, and Yuanzhi san. This article summarizes the pharmacological mechanisms of A. tatarinowii, P. tenuifolia and their compatible compound prescriptions in ameliorating cognitive impairment. It is found that they can exert effects in ameliorating cognitive impairment through mechanisms such as reducing amyloid β-protein deposition and inhibiting excessive phosphorylation of Tau protein, suppressing inflammatory responses, alleviating oxidative stress, protecting neurons and regulating neurotransmitters, modulating the structure and function of the blood- brain barrier, and regulating autophagy. Subsequently, in-depth analysis can be conducted on the active ingredients of A. tatarinowii- P. tenuifolia herb pair that ameliorate cognitive impairment, along with the addition of relevant clinical trials for verification. This will provide theoretical foundations and research approaches for the treatment of cognitive impairment using traditional Chinese medicine.
5.Angiographic manifestations and interventional treatment outcomes of inferior vena cava occlusion associated with dangerous collateral vessels in Budd-Chiari syndrome

Objective To investigate the angiographic manifestations and interventional treatment outcomes of inferior vena cava occlusion associated with dangerous collateral vessels in Budd-Chiari syndrome(BCS).Methods The data of 43 BCS patients with inferior vena cava occlusion and dangerous collateral vessels were retrospectively analyzed.All 43 patients underwent digital subtraction angiography(DSA)of the inferior vena cava and recanalization treatment of the occluded segment of the inferior vena cava.Results DSA in 43 patients showed that the inferior vena cava was occluded,and a total of 70 dangerous collateral vessels originated from the occluded end.All patients were successfully treated.DSA showed that the blood flow in the inferior vena cava was unobstructed and the dangerous collateral vessels disappeared.No complications,such as rupture or bleeding of the inferior vena cava,occurred during the interventional treatment.The 43 patients were followed up for 6-75 months after interventional treatment,and re-occlusion occurred in 6 cases.All patients made it through.Conclusion DSA can clearly show the dangerous collateral vessels originating from the occluded end of the inferior vena cava in BCS,and interventional treatment is safe and effective.

Objective Itproposes a WISN staffing demand model for clinical physicians to address the challenge that current physicians staffing standards fail to meet the operational needs of general hospitals.Methods Collect the 2023 annual operation data of a tertiary general hospital,using inpatient visits and surgeries as benchmarks to measure the workload demands of physicians in internal medicine and surgery,respectively.Additionally,integrate non-clinical workloads of physicians and the workload of resident physicians in standardized training to address the limitation of traditional models that focus solely on clinical tasks.Results The good applicability and robustness of the model in practical applications are verified through model comparison and sensitivity analysis,accurately reflecting the demand for physicians.Conclusion The model accurately reflects the workload of physicians in general hospitals,providing a quantitative estimation formula for physicians staffing.It has broad applicability and significant potential for wider adoption.

Cognitive impairment(CI)is a clinical syndrome characterized by progressive decline in advanced cognitive functions such as memory,thinking,and judgment.Its etiology and pathogenesis are complex,and there is currently a lack of specific drug interventions.Metformin,as a first-line hypoglycemic drug for type 2 diabetes,not only lowers blood glucose levels but also improves CI.This article reviews and summarizes the pharmacological effects and mechanisms of metformin in improving Alzheimer's disease,diabetes cognitive impairment,cognitive impairment after chemotherapy,in order to provide novel insights and approaches for the treatment of CI-related diseases.Studies have shown that the mechanism by which MET intervenes in CI mainly includes regulating β-amyloid protein and tau protein metabolism,reducing insulin resistance,inhibiting neuroinflammation,improving synaptic plasticity,improving mitochondrial dysfunction,regulating gut microbiota and lipid metabolism,etc.Future research needs to be conducted through interdisciplinary collaboration,fully integrating multiple omics data,and combining advanced technologies to further reveal their mechanisms of effect.

10.Role of single cell metacluster in the spatial microenvironment of intrahepatic cholangiocarcinoma in disease prognosis and early recurrence

Objective:To study the effects of different single cell phenotypes on the prognosis of patients with intrahepatic cholangiocarcinoma by using spatial analysis, providing clues for obtaining potential immunotherapeutic targets.Methods:This study was a retrospective cohort study. A total of 41 intrahepatic cholangiocarcinoma (ICC) patients who underwent surgery in Beijing Youan Hospital Affiliated to Capital Medical University from February 2013 to June 2019 were enrolled. According to the 5-year survival situation, the patients were divided into survival group ( n=10) and death group ( n=31). A metal label-based tissue imaging mass panel containing 36 related markers was designed and constructed for staining different components in tumor samples. Through the analysis of the type and quantity of different metacluster and spatial location information and combined with the clinical outcomes of patients with information, certain metaclusters were found related to the prognosis of patients. Measurement data with normal distribution were expressed as mean±standard deviation ( ± s), paired t-test was used for comparison between groups. Measurement data with skewed distribution were expressed as median, and rank sum test was used for comparison between groups. The chi-square test was used to compare the counting data between groups. Results:36 biomarkers of 41 ICC patients were located and quantified to generate 1 476 single-cell resolution histological images. The expression information of various markers was analyzed by t-distributed Stochastic Neighbor Embedding (tSNE), and subgroups annotations (1-29) were added. It revealed that the density of metacluster 7(CD8 + T cells) was lower in survival group. The density of metacluster 16(Bcl-2 + CK7 + cancer cells) within tumors, as well as the density of metacluster 3(Vista + GB + CD11b + neutrophils) within stroma were higher in death group. Conclusion:The density of metacluster 7(Activated CD8 + T cells), metacluster 16(Bcl-2 + CK7 + tumor cells) and a novel neutrophil metacluster 3(Vista + GB + CD11b + neutrophils) correlated with ICC patients prognosis.
