Cardiovascular disease has become the main cause of death worldwide, seriously threatening human life and health. Some studies have shown that cluster of differentiation 74(CD74)is involved in the pathology and inflammatory pathways of cardiovascular diseases, which has attracted wide attention from scholars.CD74 is a major histocompatibility complex(MHC)Ⅱ chaperone and macrophage migration inhibitory fac-tor(MIF)receptor.It activates a variety of signaling pathways and plays a biological role in the occurrence and progression of cardiovascular diseases. This article summarizes the relevant lit-erature advances of CD74 in cardiovascular diseases,to explore the pathogenesis from the perspective of CD74 and seek disease protection and more effective therapeutic targets.

Objective This study aimed to investigate the efficacy of modified endoscopic autologous cartilage eustachian tube pharyngeal orifice tuboplasty(MEACETT)in patients with patulous eustachian tube(PET).Meth-ods A retrospective analysis was conducted on the clinical data of 27 patients(30 ears)diagnosed with PET who underwent MEACETT.Autologous cartilage was used through the incision at the posterior end of the inferior turbi-nate and filled into the lateral wall of the pharyngeal orifice of the eustachian tube.Without affecting the movement function of the eustachian tube during swallowing, the collapse of the pharyngeal orifice was fully filled. Before and after the surgery, the visual analogue scale(VAS),the eustachian tube dysfunction questionnaire-7(ETDQ-7)and hospital anxiety and depression scale(HADS)was used for assessment to evaluate the surgical efficacy. Results There was no significant difference in depression scores before and after surgery(P>0.05).However,postopera-tive anxiety scores,ETDQ-7 scores, and VAS scores were significantly lower than preoperative scores(P<0.05).Among the 27 patients,9 showed significant symptom relief,13 exhibited partial relief, and 5 had no significant change compared to preoperative symptoms. The overall response rate of the treatment(significant relief and partial relief)was 81.48%(22/27).All surgeries were successfully performed.Except for secretory otitis media occurring in 2 cases,no major complications were observed. Conclusion MEACETT demonstrates significant symptom relief in PET patients, with high surgical safety and low complication rates, making it worthy of clinical promotion.

Objective To evaluate the diagnostic efficacy of the R value in tubomanometry(TMM)for the di-agnosis of patulous eustachian tube(PET).Methods The clinical data of 58 patients with PET and 65 controls were retrospectively analyzed.TMM was performed on both groups under nasopharyngeal pressures of 30,40,and 50 mbar respectively. The diagnostic efficacy of the R value for PET was evaluated through receiver operating char-acteristic(ROC)curves.Results In the control group, the average R values under nasopharyngeal pressures of 30,40,and 50 mbar were 0.86±0.50,0.76±0.41,and 0.68±0.34 respectively. In contrast, the corresponding R values in the PET group were significantly lower, which were 0.56±0.38,0.50±0.36,and 0.46±0.38 respec-tively.According to the ROC curve analysis, the areas under the curve(AUC)at these pressures were 0.62,0.74,and 0.74 respectively. The specificity and sensitivity of the R value under nasopharyngeal pressures of 30,40,and 50 mbar were 76.90%and 54.30%,74.60%and 68.10%,86.90%and 54.30%,respectively.Under pressures of 30,40,and 50 mbar, the incidence rates of R>1 in the control group and the PET group were 29.23%(38/130)and 12.77%(12/94)(x2=8.69,P=0.003),20.00%(26/130)and 6.38%(6/94)(x2=7.20,P=0.007),10.00%(13/130)and 3.19%(3/94)(x2=2.87,P=0.09)respectively.Conclusion Although the low R value in TMM reflects the presence of PET to some extent,it does not provide adequate sensitivity and specificity to serve as an independent diagnostic criterion for PET.

Objective To investigate the value of the volume percentage of solid component in differential diagnosis of stage Ⅰ lung adenocarcinoma subtypes, and to predict the optimal critical CT value of solid component. Methods A total of 962 nodules with sur-gical pathological findings confirmed as adenocarcinoma in situ(AIS)or pulmonary adenocarcinoma and manifested as subsolid nod-ules on thin-section CT were analyzed retrospectively, and divided the lesions into:(1)AIS(n=350)or pulmonary adenocarcinoma(n=612)groups;and(2)minimally invasive adenocarcinoma(MIA)(n=213)or invasive adenocarcinoma(IAC)(n=399)groups based on pathological findings. The volume percentage of solid components within pulmonary nodules was measured via CT density histogram analysis at different thresholds, the diagnostic efficacy of different thresholds was analyzed and the optimal critical CT val-ues of solid component was found. Results In the AIS versus pulmonary adenocarcinoma groups, age, mass, and volume percentage of solid component were statistically significant(P<0.001);the CT threshold of-350 HU had the highest diagnostic efficacy[area under the curve(AUC)0.859,accuracy 81.69%,sensitivity 85.60%,and specificity 74.86%,respetively],and the critical value of volume percentage of solid component determined under this threshold was 8.3%.In the MIA versus IAC groups,age(P=0.017),mass(P<0.001),and volume percentage of solid component(P<0.001)were statistically significant;the CT threshold of-350 HU had the highest diagnostic efficacy(AUC 0.857,accuracy 78.27%,sensitivity 82.91%,and specificity 69.48%,respectively),and the critical value of volume percentage of solid component determined under this threshold was 16.9%.Conclusion The volume percentage of solid component measured based on CT density histogram analysis may be helpful in the differential diagnosis of early stage lung ade-nocarcinoma subtypes.

Objective:To evaluate the effect of ionizing radiation on chromosome aberration of peripheral blood lymphocytes in children with vascular interventional procedure. Methods:The chromosome aberration of peripheral blood lymphocytes was analyzed before and after vascular interventional procedure to assess the effect of low-dose ionizing radiation on chromosomes. Peripheral blood (PB) was collected using heparin as an anticoagulant from 26 patients before and after (within 10 minutes) vascular interventional surgery. Informed consent was obtained from the patients′ parents. Results:Dicentric + centric ring (dic+ r) formation increased significantly ( U=647, P<0.01) in peripheral blood lymphocytes of children after interventional operation. No correlation was observed between the rate of dic+ r and the cumulative dose (CD), dose area product (DAP), age, body weight, gender and the estimated effective dose E ( P>0.05). There was no significant difference in dic+ r rate of children between different genders, ages and lesion sites ( P>0.05). The dic+ r rate after interventional operation exceeded 40×10 -3 in 4 of 26 cases with the highest of 95.83×10 -3. Conclusions:The ionizing radiation during children′s vascular interventional procedure might lead to the increase of chromosome aberration of peripheral blood lymphocytes and some children are abnormally sensitive. Great attention should be paid to the justification and radiation protection during interventional procedure for children.

N-methyl-D-aspartate receptor (NMDAR),an important ionic glutamate receptor and a ligand and voltage-gated ion channel characterized by complex composition and functions and wide distribution, plays a key role in the pathological and physiological process of diseases or stress states.NMDAR can mediate apoptosis through different pathways such as mitochondrial and endoplasmic reticulum damage, production of reactive oxygen species and peroxynitrite, and activation of mitogen-activated protein kinase and calpain. This paper reviews the structure, distribution, and biological characteristics of NMDAR and the mechanisms of NMDAR-mediated apoptosis.
