The Journal of Practical Medicine 2025;41(8):1149-1154
Objective Investigation into the impact of modified Chevron osteotomy on asymptomatic flatfoot following hallux valgus correction. Methods A retrospective analysis was performed on 118 patients(120 feet)who underwent modified Chevron osteotomy for hallux valgus between January 2018 and December 2022.Based on the Meary angle, the patients were categorized into two groups:an asymptomatic flatfoot group(49 cases,50 feet)and a non-flatfoot group(69 cases,70 feet).Preoperative general data and radiographic parameters were compared between the two groups at baseline and during the last follow-up.These parameters included the talona-vicular coverage angle(TNCA),talus-second metatarsal angle(T2MT)on weight-bearing anteroposterior foot radiographs, and the Meary angle on lateral radiographs. Results There were no significant differences in preop-erative general data between the two groups(P>0.05).In the asymptomatic flatfoot group, the Meary angle dem-onstrated a improvement(P<0.001)when comparing radiographic parameters intra-group between preoperation and the last follow-up.In the non-flatfoot group,TNCA and T2MT increased(P<0.05).Inter-group comparisons revealed that preoperatively,TNCA,T2MT,and the Meary angle were significantly worse in the asymptomatic flatfoot group compared to the non-flatfoot group(P<0.05).At the last follow-up,there were no significant differ-ences in TNCA and T2MT between the two groups(P>0.05);however,the Meary angle remained significantly lower in the asymptomatic flatfoot group than in the non-flatfoot group(P<0.05).Conclusions The modified Chevron osteotomy significantly enhances the medial longitudinal arch in patients with hallux valgus and asymptom-atic flatfoot, although its effect on correcting abduction is relatively limited. In patients with hallux valgus but without flatfoot, this procedure does not substantially alter the medial longitudinal arch;however,it can induce abduction, which aids in the correction of hallux valgus. This study confirms that the modified Chevron osteotomy effectively improves foot arch structure in asymptomatic flatfoot cases while addressing hallux valgus, without worsening the flatfoot condition.

OBJECTIVE:
To compare clinical efficacy of transaxillary posterior margin approach and translateral margin approach for the treatment of Miller typeⅡand Ⅳ scapular fractures.
METHODS:
From June 2020 to June 2024, 28 patients with fresh scapular fractures (Miller typeⅡand Ⅳ) who were treated with open reduction and locked plate internal fixation were retrospectively analyzed and divided into two groups. There were 13 patients in posterior axillary margin group, including 8 males and 5 females, aged from 26 to 71 years old with an average of (39.2±6.5) years old;5 patients with typeⅡand 8 patients with type Ⅳ according to Miller classification;9 patients caused by falling down, 4 patients caused by car accident;7 patients on the right side, 6 patients on the left side;the time from injury to operation ranged from 3 to 5 days with an average of (4.07±0.86) days. There were 15 patients in lateral scapula margin group, including 10 males and 5 females, aged from 27 to 63 years old with an average of (43.6±8.5) years old;7 patients with typeⅡand 8 patients with type Ⅳ according to Miller classification;10 patients caused by falling down, 5 patients caused by car accident;6 patients on the right side, 9 patients on the left side;the time from injury to operation ranged from 3 to 5 days with an average of (4.20±0.67) days. Operative time, intraoperative blood loss, fracture healing time, glenopolar angle (GPA) immediately after operation were compared between two groups. Visual analogue scale (VAS) and Constant-Murley shoulder joint function score were used to evaluate clinical effect at 3 and 6 months after operation, respectively, and the joint motions were evaluated at 6 months after operation.
RESULTS:
None of patients occurred wound infection, vascular and nerve damage, loosening or rupture of internal fixation. Both groups were followed up for 8 to 12 months with an average of (9.6±1.5) months. Operative time and intraoperative blood loss of posterior axillary margin group were (76.92±5.60) min and (84.86±10.08) ml, respectively, which were better than those of lateral scapula margin group(84.67±6.93) min and(115.00±12.39) ml(P<0.05). Immediate GPA in posterior axillary margin group (36.62±0.87) °and lateral scapula margin group (36.20±0.82)°were significantly improved compared with preoperative (16.10±1.14) °, (16.20±1.20) °(P<0.05), while there was no statistical significance between two groups (P>0.05). There were no significant difference in fracture healing time and postoperative VAS at 3 and 6 months between two groups (P>0.05). Postoperative Constant-Murley score of posterior axillary margin group (75.00±4.02) was higher than that of lateral scapular margin group (70.86±4.24) at 3 months (P<0.05), while there was no significant difference between two groups at 6 months after operation (P>0.05). At 6 months after operation, joint motion of posterior axillary margin group in flexion (160.38±13.61)°, abduction (154.61±13.91)°, rotation (83.08±2.53)°, rotation (62.69±2.59) ° and extension (51.54±3.15) °, respectively, which were higher than that of lateral scapular margin group in (148.00±15.44)°, (144.00±11.05)°, (79.67±3.99)°, (57.33±4.95)°, (47.33±4.16)°(P<0.05).
CONCLUSION
Compared with lateral margin of scapula intermuscular space approach, Buttress approach through the posterior axillary margin for the treatment of Miller typeⅡand Ⅳ scapular fractures provides extensive intraoperative visual field exposure, stable fracture support and less surgical trauma, which could shorten operative time and reduce intraoperative blood loss, providing a new reference for the treatment of complex scapular fractures.

Objective To achieve intelligent recognition and segmentation of common craniocerebral inju-ries(hereinafter referred to as"segmentation")by training convolutional neural network DeepLabV3+model based on CT images of blunt craniocerebral injury(BCI),and to explore the value of deep learning in automated diagnosis of BCI in forensic medicine. Methods A total of 5 486 CT images of BCI from living persons were collected as the training set, validation set and test set for model training and performance evaluation.Another 255 CT images of BCI and 156 normal craniocerebral CT images from living persons were collected as the blind test set to evaluate the ability of the model to seg-ment the five types of craniocerebral injuries including scalp hematoma, skull fracture, epidural hema-toma,subdural hematoma, and brain contusion.Another 340 BCI and 120 normal craniocerebral CT images from cadavers were collected as the new blind test set to explore the application value of the model trained by living CT images in the segmentation of BCI in cadavers. The five types CT images of all BCI except the blind test set were manually labeled;then, each dataset was inputted into the model to train the model. The performance of the model was evaluated and optimized based on the loss function and accuracy curves of the training set and validation set,and the generalization ability was evaluated based on the Dice value of the test set.According to the accuracy, precision and F1 value of the blind test set,the segmentation performance of the model for five types of BCI was evaluated. Results After training and optimizing the model, the average Dice values of the final optimal model to scalp hematoma, skull fracture, epidural hematoma, subdural hematoma and brain contusion segmen-tation were 0.766 4,0.812 3,0.938 7,0.782 7 and 0.858 1,respectively,all greater than 0.75,meeting the expected requirements.External validation showed that the F1 values were 93.02%,89.80%,87.80%,92.93%and 86.57%in living CT images,respectively;83.92%,44.90%,76.47%,64.29%and 48.89%in cadaveric CT images, respectively. The above suggested that the model was able to accu-rately segment various types of craniocerebral injury on living CT images, while its segmentation ability was relatively poor on cadaveric CT images, but still able to accurately segment scalp hematoma,epidu-ral hematoma and subdural hematoma. Conclusion Deep learning model trained on CT images can be used for BCI segmentation. However, the direct use of living persons'BCI models for the identifica-tion of cadaveric BCI has some limitations. This study provides a new approach for intelligent segmen-tation of virtual anatomical data for BCI.
