Objective:To analyze the genetic characteristics of the VWF gene c.7332G>A nonsense mutation and explore its molecular pathogenesis. Methods:Phenotypic diagnosis of the proband was performed using VWF:Ag,VWF:RCo,FⅧ:C and multimeric analysis. The probands were genotyped by NGS whole-exome sequencing, and the sequencing results were validated by sanger sequencing. The family members were genotyped by Sanger sequencing. The VWF gene c.7332G>A nonsense mutant plasmid was constructed. After transfection, the function of VWF gene c.7332G>A mutant plasmid was verified at cell level in vitro. The mRNA level was detected by qRT-PCR,and the expression level of protein was detected by Western blot, the function of multimerization was verified by the multimeric analysis. Results:VWF:Ag and VWF:RCo were all less than 3%in the proband, and the multimeric analysis showed multimer deficiency. The proband was diagnosed as type 3 VWD.The homozygous nonsense mutation of VWF gene c.7332G>A was detected by gene sequencing. The VWF mRNA level of the mutant plasmid was decreased, and the VWF protein expression in the cell supernatant was decreased, the mutant protein was truncated and the function of VWF multimerization was impaired. Conclusion:A homozygous mutation in exon 43 of VWF gene,c.7332G>A,was responsible for the probands type 3 VWD in the proband. The mutation caused a decrease in the relative level of VWF mRNA and protein, and impaired the function of VWF multimerization.

Following extensive interdisciplinary research and development over several years, mechanical circulatory support devices(MCSD),including ventricular assist device(VAD)and total artificial heart(TAH),are now established as vital treatment options for patients with advanced heart failure. These devices have proven to be crucial in assisting or replacing a failing heart, offering patients a new lease of life and improving their quality of life.Currently, mechanical circulatory support(MCS)has become a well-recognised,long-term treatment option for patients who are unable to undergo heart transplantation due to donor organ shortages or contraindications.Given their continuous availability independent of donor organ limitations, these devices are poised to play an increasingly vital role in the future of medicine. This article aims to summarize the evolution, clinical applications, categorization, and potential complications of MCSD.

OBJECTIVE:
This study aimed to explore the association between body mass index (BMI) and mortality based on the 10-year population-based multicenter prospective study.
METHODS:
A general population-based multicenter prospective study was conducted at four sites in rural China between 2013 and 2023. Multivariate Cox proportional hazards models and restricted cubic spline analyses were used to assess the association between BMI and mortality. Stratified analyses were performed based on the individual characteristics of the participants.
RESULTS:
Overall, 19,107 participants with a sum of 163,095 person-years were included and 1,910 participants died. The underweight (< 18.5 kg/m 2) presented an increase in all-cause mortality (adjusted hazards ratio [ aHR] = 2.00, 95% confidence interval [ CI]: 1.66-2.41), while overweight (≥ 24.0 to < 28.0 kg/m 2) and obesity (≥ 28.0 kg/m 2) presented a decrease with an aHR of 0.61 (95% CI: 0.52-0.73) and 0.51 (95% CI: 0.37-0.70), respectively. Overweight ( aHR = 0.76, 95% CI: 0.67-0.86) and mild obesity ( aHR = 0.72, 95% CI: 0.59-0.87) had a positive impact on mortality in people older than 60 years. All-cause mortality decreased rapidly until reaching a BMI of 25.7 kg/m 2 ( aHR = 0.95, 95% CI: 0.92-0.98) and increased slightly above that value, indicating a U-shaped association. The beneficial impact of being overweight on mortality was robust in most subgroups and sensitivity analyses.
CONCLUSION
This study provides additional evidence that overweight and mild obesity may be inversely related to the risk of death in individuals older than 60 years. Therefore, it is essential to consider age differences when formulating health and weight management strategies.

Ventricular assist devices(VADs)have emerged as a pivotal therapeutic option for end-stage heart failure, undergoing significant technological advancements from pneumatic membrane pumps to continuous-flow centrifugal pumps. This review provides a detailed overview of several VAD models, including HeartMate Ⅲ,EVAHEART 2,and Heart Con.The initial generation of VADs established a foundation for subsequent developments, despite the occurrence of frequent complications. The second generation of VADs demonstrated a notable improvement in patient prognosis but carried the risk of ventricular collapse. The third generation of VADs represents a further reduction in size, incorporating magnetic levitation technology to enhance durability and blood compatibility. In the future, the development of VADs will focus on better emulating the physiological function of the native heart, improving pulsatility, and extending device longevity. As technology advances,VADs are expected to offer heart failure patients with a greater range of treatment options and a higher quality of life.

Pancreatic cancer is characterized by significant drug resistance, and despite continuous advancements in treatment regimens, the 5-year survival rate of patients remains low.The nuclear factor-κB(NF-κB)signaling pathway, frequently mutated in tumors, has been identified as a critical factor in triggering drug resistance.Multiple studies have demonstrated that strategies targeting NF-κB signaling transduction exhibit promising outcomes in pancreatic cancer treatment. Therefore, exploring the relationship between the NF-κB signaling pathway and drug resistance in pancreatic cancer has become a research hotspot in pancreatic cancer treatment. This review summarizes recent advances in the relationship between NF-κB signaling pathway and tumor drug resistance,as well as its role in pancreatic cancer treatment.Specifically, the mechanisms by which the NF-κB signaling pathway mediates drug resistance in pancreatic cancer are elaborated from two perspectives:chemotherapy and immunotherapy, aiming to provide insights for pancreatic cancer treatment and future research.

AIM To study the effects and possible mechanism of Fuke Qianjin Formula and its modifications on sequelae of pelvic inflammatory disease(PID)in rats.METHODS The SD rats were randomly divided into the normal group, the sham operation group, the model group,Kangfuyan group(FYK,0.756 g/kg),Fuke Qianjin Formula group(FKQ,2.7 g/kg),Yijun Formula group(YJ,2.7 g/kg),Kangyan Formula group(KY,2.7 g/kg)and Buyi Formula group(BY,2.7 g/kg).After 28 days of corresponding administration by gavage, the rats were put to death to procure their uterus, spleen, thymus and liver for the calculation of the organs indices levels;the observation of the general morphology of uterus and oviduct and histopathological examination of uterus and oviduct;the measurement of.the serum levels of IL-1β,IL-18 and IL-6 by ELISA;the detection of the uterine expressions of TGF-β1,caspase-1,NLRP3,IL-18 and CTGF by Western blot;and the detection of the expressions of CD4+and CD8+in spleen lymphocytes by flow cytometry.RESULTS Compared with the model group,FKQ and YJ group,KY group and BY group displayed decreased indices levels of the uterus, spleen and thymus(P<0.01);FKQ group and KY group displayed reduced uterine swelling, increased endometrial glands, and reduced inflammatory exudation(P<0.01),and lower serum levels of IL-1β,IL-18 and IL-6(P<0.01);FKQ group,KYgroup and BY group displayed decreased protein expressions of TGF-β1 and CTGF(P<0.05,P<0.01);FKQ group and KY group displayed decreased protein expressions of NLRP3,caspase-1 and IL-18(P<0.05,P<0.01);and FKQ group and BY group displayed increased CD4+/CD8+ratio(P<0.01).CONCLUSION Being protective to sequelae of PID rat models,Fuke Qianjin Formula and its modifications may contribute their anti-inflammatory efficacy by inhibiting the activation of NLRP3 body to hinder the activation of caspase-1,reduce the secretion and release of pro-inflammatory factor IL-18,and thus inhibit pyroptosis.
