1.Rapid health technology assessment of avatrombopag for the treatment of primary immune thrombocytopenia in adults

Objective To evaluate the efficacy,safety and cost-effectiveness of avatrombopag for treating primary immune thrombocytopenia(ITP)by rapid health technology assessment(rHTA),to provide evidence-based basis for clinical decision-making.Methods PubMed,Cochrane Library,Web of Science,Embase,CNKI,WanFang Data and the official website of health technology assessment(HTA)agency were electronically searched to collect systematic reviews/Meta-analyses,pharmacoeconomic literature,and HTA reports of avatrombopag for the treatment of ITP from inception to May 31,2025.Two reviewers independently screened studies,extracted data,assessed the quality of included studies,and qualitatively analyzed the result.Results A total of 12 studies were included,including 10 systematic reviews/Meta-analyses,1 pharmacoeconomic study and 1 HTA report.In terms of effectiveness,compared with placebo,treatment with avatrombopag significantly elevated platelet response,durable platelet response,early platelet response,and platelet count,reduction in use of concomitant ITP medication(P<0.05),and there was no significant difference in the need for remedial measures during the treatment period(P>0.05).Avatrombopag was also superior to eltrombopag,romiplostim,rituximab alone or in combination with recombinant human thrombopoietin(rhTPO)for platelet response(P<0.05).In terms of safety,there was no significant difference in the incidence of adverse events,serious adverse events,bleeding events and thrombotic events when comparing avatrombopag to placebo,eltrombopag,romiplostim and rituximab for the treatment of ITP(P>0.05).Economic study showed that avatrombopag has an economic advantage compared to eltrombopag and romiplostim,but it does not have economic advantages compared with observation and rescue.Conclusion Compared with placebo or other second-line treatments for ITP,avatrombopag has good efficacy and no significant differences in safety.Regarding economics,avatrombopag demonstrates greater cost-effectiveness than eltrombopag and romiplostim.However,due to its higher price,it does not have economic advantages compared to observation and rescue.Future economic research should be conducted domestically.
2.Epidemic factors in foodborne parasitic diseases in ethnic minority areas of Guizhou Province from a One Health perspective

This study was aimed at understanding the prevalence and influencing factors of food-borne parasitic diseases in ethnic minority areas of Guizhou Province,to provide a scientific basis for the development of appropriate intervention measures based on the human-animal-environment One Health concept.In 2023,the infection status of the human population,reservoir hosts,intermediate hosts,food-borne parasitic diseases,and related social and environmental factors were investigated in Congjiang County in Qidongnan Miao and Dong Autonomous Prefecture;Luodian County in Qiannan Buyi and Miao Autonomous Prefecture;and Ceheng County in Qianxinan Buyi and Miao Autonomous Prefecture.At least 1 000 individuals were sampled from each county,along with at least 50 insect-protected host samples from each location.Food-borne parasite infections were detected with the modified Kato thick smear method.A questionnaire survey was administered to the population.Detection of food-borne parasitic metacercariae was performed in intermediate host fish through the flaking and digestion method,and in crabs through the pounding and sedimentation method.The chi-square test was used to compare rates,and logistic regression was applied for multivariate analysis.A total of 3 023 questionnaires and fecal samples were collected.Males accounted for 47.50%,females accounted for 52.50%,and members of ethnic minorities accounted for 96.06%.A total of 186 food-borne parasitic infections were identified,and the infection rate was 6.15%.Five insect species were detected,which showed an infection rate of 5.39%.The infection rate of Clonorchis sinensis was 0.33%,that of Taenia was 0.40%,that of Heteroceles was 0.17%,that of Acanthus was 0.17%,and that of Echinostoma was 0.03%.Human infections with Echinostomus colloides and Echinostomia transferoris had not previously been reported in China.Single-factor analysis revealed statistically significant differences in food-borne parasite infections according to various factors,including the consumption of untreated water,raw fish and shrimp,raw pig blood,raw cow gastric juice,and raw pork and beef,as well as raw pig and cow viscera(P<0.05).Multivariate analysis indicated that the risk factors for food-borne parasite infections among residents in minority areas of Guizhou Province included the consumption of raw pig blood(OR=2.841,95%CI:1.809-4.463),raw cow gastric juice(OR=2.122,95%CI:1.297-3.469),and raw fish and shrimp(OR=1.779,95%CI:1.049-3.018).A total of 173 fecal samples of the reservoir host were examined,which showed a rate of food-borne parasite infection of 5.2%.A total of 510 intermediate host fish were examined,which showed a 4.51%positivity rate of encysted metacercaria of Clonorchis sinensis.The crab,pig,and beef samples were not positive.In conclusion,food-borne parasitic infections were prevalent in ethnic minority regions of Guizhou Province,and consumption of raw food were influencing factors.A focus on populations with raw food consumption habits,including raw pig blood,cow gastric juice,fish and shrimp,is essential.Concurrently,monitoring of animal hosts must be strengthened to perform key interventions according to the One Health concept.

6.Observation of the Application Effect of Hyperbaric Oxygen Combined with Task OrientedTraining in Hemiplegic Patients after Cerebral Infarction

Objective:To observe the application effect of hyperbaric oxygen combined with task oriented training in hemiplegic patients after cerebral infarction.Methods:106 hemiplegic patients after cerebral infarction admitted to Xinglin Hospital of Xiamen from March 2022 to June 2024 were prospectively selected and randomly divided into control group(received task oriented training,n=53)and observation group(received hyperbaric oxygen combined with task oriented training,n=53).Berg balance scale(BBS),clinical spasticity index(CSI),national institute of health stroke Scale(NIHSS),motor function[fugl-meyer motor function assessment scale(FMA),motor assessment scale(MAS)],walking ability[functional ambulation category(FAC),6-minute walking distance(6MWD)],quality of life and self-care ability[quality of life scale for stroke(SS-QOL),activity of daily living(ADL)]before intervention and 8 weeks after intervention were compared between the two groups.Result:Compared with control group 8 weeks after intervention,the observation group had higher BBS,FMA,MAS,FAC,SS-QOL,ADL scores,and 6MWD,while had lower NIHSS and CSI scores(P<0.05).Conclusion:Hyperbaric oxygen combined with task oriented training in hemiplegic patients after cerebral infarction,can effectively improve patients' motor function,balance ability,and self-care ability,enhance their quality of life,and reduce neurological damage.
7.Effect of musculoskeletal injuries in operating room doctors on their mental health

Objective To understand the current status of work-related musculoskeletal disorders (WMSDs) among operating room doctors,thereby analyzing its effect on mental health. Methods A total of 1013 doctors in the operating rooms of 30 hospitals in 24 provinces from September to December 2022 were selected by cross-sectional survey method. The domestic musculoskeletal disease questionnaire was used to investigate the occurrence of WMSDs in operating room doctors,the symptom check list-90 (SCL-90) was used to evaluate the psychological status of operating room doctors,the relationship between WMSDs related items and SCL-90 scores was analyzed,the correla-tion between the rest time of each shift and SCL-90 factors was analyzed by Pearson method,and the influencing factors of mental health of operating room doctors were analyzed by multivariate Logistic regression.Results A total of 1100 questionnaires were distributed and 1013 valid questionnaires were recovered,with an effective rate of 92.10%. The incidence of WMSDs among operating room doctors in the past 1 year was 95.06%. The body parts with incidences of WMSDs from high to low were as follows:waist (36.92%),neck (35.54%),shoulder (33.56%),and upper back (32.77%). In the past 1 year,the SCL-90 scores and total scores of patients with musculoskeletal pain in various body parts,keeping the same back posture for a long time while work,bending the back at the same time of turning frequently while working,bowing the head for a long time while working,and bending the knee for a long time while working were significantly higher than those who were without the above conditions (P<0.05). Shorter rest time of each shift and longer periods of keeping the same back posture were the risk factors for the development of mental illness in operating room doctors. Conclusion The incidence of WMSDs in operating room doctors is high,which can influence the mental health of operating room doctors. Therefore,operating room doctors should avoid keeping the same back posture for a long time,bending the back at the same time of turning frequently,bowing the head for a long time,and bending the knee for a long time while working,and increase the rest time of each shift,in order to improve the mental condition of the operating room doctors.

9.Literature Analysis of Methimazole-induced Antineutrophil Cytoplasmic Antibody Associated Vasculitis

Objective To investigate the occurrence and characteristics of Antineutrophil cytoplasmic antibody(ANCA)associated vasculitis induced by methimazole,and to provide references for clinical safe drug use.Methods Case reports of ANCA associated vasculitis induced by methimazole published in Wanfang,CNKI,PubMed,and Web of Science were searched from the inception to October 31 st,2023.Demographic characteristics,drug use,complications,treatment and outcome were analyzed using descriptive statistical method.Results A total of 14 patients from 14literature were included.There were 3 males and 11 females with ages ranging from 8 to 79 years,with a mean age of(47.79±23.47)years.Four patients developed symptoms within 1 year,nine patients developed symptoms from 2 to 12 years,and 1 patient developed symptoms 24 years after medication.ANCA associated vasculitis affected kidney in 5 patients,lung and skin in 5patients,vision in 2 patients and heart in 2 patients.All patients discontinued methimazole,2 patients improved spontaneously without treatment,1 patient improved after anti-infection,and all others received hormonal or immunosuppressive therapy.1 patient developed death,and all others improved or were cured after treatment.Conclusions ANCA-associated vasculitis is a rare adverse reactionof methimazole.Most patients have a long latency period before the onset of disease,mainly involving multiple organs such asskin,kidney,lung,and eyesight.Clinicians should pay attention to differentiate it from primary vasculitis and discontinue the drug as soon as possible.When serious organ damage occurs,glucocorticoids and immunosuppressants should be adminstered promptly to avoid aggravation of the disease and endangerment of life.
