1.Retrospective analysis of a tuberculosis outbreak among junior high school students in Chongqing

Objective:
To analyze changes in tuberculosis infection among junior high school students before and after tuberculosis exposure, so as to provide a reference for improving school tuberculosis prevention and control measures and policy formulation.
Methods:
Retrospectively collect data on a tuberculosis outbreak that occurred in a grade of a junior high school in Chongqing in 2025, including tuberculosis screening records of students in this grade upon their enrollment in 2022 (1 156 students) and after two tuberculosis outbreaks in 2023 (206 students) and 2025 (171 students). The Wilcoxon signed rank test for paired design was used to compare the induration diameters of the subjects, and the Chi square test was adopted to analyze the rate of tuberculosis infection among students.
Results:
In the tuberculosis outbreak in 2023, the rate of tuberculosis infection among close contacts ( 11.84 %) and the rate of tuberculosis infection among freshrman at school enrollment (12.89%) showed no statistically significant difference ( χ 2=0.25, P >0.05). The rate of tuberculosis infection of close contacts in the 2025 tuberculosis outbreak (55.56%) was higher than that in the 2023 outbreak (11.84%) ( χ 2=30.42, P <0.01). Among the 106 students included in the cohort analysis, the median induration diameter was 3.50 (1.50, 7.50) mm in 2023 and 8.75 (4.25, 11.50) mm in 2025, with a statistically significant difference ( Z=-5.76, P <0.01). There was no statistically significant difference between the infection rate in 2022 (16.98%) and that in 2023 (10.38%) ( χ 2=1.96, P =0.16). The infection rate in 2025 (43.40%) was higher than those in 2022 and 2023 ( χ 2=17.55, 29.39, both P <0.017). The seroconversion rate of students in the same class in 2025 ( 58.00 %) was higher than that of students in different classes (16.07%), with a statistically significant difference ( χ 2=20.19, P <0.01). All 72 individuals with latent tuberculosis infections identified during the pandemic in 2023 and 2025 refused to undergo prophylactic treatment.
Conclusions
The lack of preventive treatment may be the underlying cause of the successive outbreaks during the epidemic. Early detection of infection sources and standardized outbreak management are crucial to controlling the spread of the epidemic.
2.Application effect of narrow band imaging combined with magnifying endoscopy in early gastric cancer screening

Objective To analyze the application effect of narrow band imaging(NBI)combined with magnifying endoscopy(ME)in early gastric cancer(EGC)screening.Methods A total of 199 patients who found focal lesions by ordinary white light endoscopy(WLE)were selected from April 2022 to October 2023.NBI combined with ME examination was performed,and histopathological examination of the specimen obtained from the lesion area was conducted.The pathological diagnosis results were recorded,and the endoscopic image qualities of WLE and NBI combined with ME examination were observed.The consistency between WLE,NBI combined with ME and patho-logical diagnosis was analyzed.The diagnostic value of WLE and NBI combined with ME for EGC was evaluated.Results The image qualities of microvascular and glandular duct by NBI combined with ME were better than those by WLE(P<0.05).The consistency between WLE diagnosis and pathological diagnosis was comparatively strong(Kappa=0.687).The consistency between NBI combined with ME and pathological diagnosis was very strong(Kappa=0.947).The consistency between NBI combined with ME and pathological diagnosis was better than that of WLE.The sensitivity,specificity,positive predictive value,negative predictive value,accuracy,and area under the curve(AUC)of NBI combined with ME in diagnosing EGC were higher than those of WLE,with statistically significant differences(P<0.05).The AUC of NBI combined with ME for diagnosing EGC was higher than that of WLE,with statistically significant difference(Z=3.493,P=0.000).Conclusion NBI combined with ME can increase the diagnostic efficiency of EGC,along with good image quality.
3.The protective effect of Gualou Guizhi granules on neuronal injury induced by LPS-activated microglia based on Notch signaling pathway

Aim To investigate the protective effect of Gualou Guizhi granules(GLGZG)on neuronal injury induced by LPS-activated microglia based on Notch signaling pathway.Methods LPS-activated microglia were co-cultured with neurons to construct neuron inju-ry models,and the cells were divided into the control group,model group,Notch inhibitor(DAPT)group,GLGZG(50,100,200 mg·L-1)group,DAPT+100 mg·L-1GLGZG group.After intervention,the activity of HT22 cells was detected by CCK-8 method,and rel-ative mRNA expression was detected by real-time PCR.The relative protein expression was detected by Western blot.Results Compared with the model group,after GLGZG intervention,the cell activity was significantly improved,GLGZG decreased IL-6,IL-12,Bax,Notch 1,caspase-3,Delta-1,NICD,RBPSUH,HES1 expression,and increased Bcl-2 expression(P<0.05).Compared with the model group,the NICD,RBPSUH and HES1 mRNA and protein expressions significantly decreased after DAPT treatment(P<0.05),and there was no superposition effect with GLG-ZG.Conclusion GLGZG may play a neuroprotective role by inhibiting inflammatory factors and apoptosis,and inhibiting Notch signaling pathway.
4.Significance of basophil levels in prognostic evaluation of intra-abdominal infection

Objective:To assess the relationship between basophil levels and mortality in patients with intra-abdominal infection.Methods:Information on patients with intraperitoneal infection admitted to the intensive care unit were extracted from the MIMIC database.A time-dependent Cox regression model was used to adjust for confounders associated with 28-day mortality.Propensity score matching(PSM)was used to balance the baseline differences be-tween groups with different basophil levels,and a restricted cube chart(RCS)was used to show the relationship between basophil count and 28-day mortality in patients with intra-abdominal infection.Results:A total of 4403 patients with intra-abdominal infection were enrolled in the MIMIC database.Patients with high basophil levels have lower mortality than those with low basophil levels.There was an L-shaped curve between basophil level and 28-day mortality,with a cut-off value of 0.47×109/L.Cox regression analysis showed that basophil levels were an independent protective factor for mortal-ity in patients with intra-abdominal infection after adjusting for potential confounders(HR=0.586,95%CI:0.443-0.769).Protective factors for death at basophil levels remained after PSM adjusted for potential confounders(HR=0.628,95%CI:0.470-0.832).Conclusion:Basophil level is an independent protective factor for mortality in patients with intra-abdominal infection,and basophil levels should be dynamically monitored to better evaluate the prognosis of patients.
5.Effects of Sitting Baduanjin Combined with Sacubitril/Valsartan on Cardiopulmonary Function,Exercise Capacity and Vascular Endothelial Function in Patients with Coronary Heart Disease after PCI

Objective:To observe the effects of Sitting Baduanjin combined with Sacubitril/Valsartan on cardiopulmonary function,exercise capacity and vascular endothelial function in patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods:A retrospective analysis was conducted on the clinical data of 86 patients with coronary heart disease who underwent percutaneous coronary intervention(PCI)at Zhangzhou Third Hospital from April 2023 to August 2024,among these patients,43 were treated with a combination of Sacubitril/Valsartan and Sitting Baduanjin,forming the study group,the other 43 patients,treated with sacubitril/valsartan alone,were randomly assigned in a 1∶1 ratio to form the control group.The cardiopulmonary function[cardiac index(CI),left ventricular ejection fraction(LVEF),The ratio of forced expiratory volume to forced vital capacity(FEV/FVC),maximum ventilation volume(MVV)].Exercise capacity[6-minute walking test distance(6MWD),maximum motion load(MWL),peak oxygen uptake(VO2peak)],vascular endothelial function[endothelin-1(ET-1),vascular endothelium-dependent diastolic function(FMD),vascular pseudohematologic factor(vWF)]and the occurrence of adverse cardiovascular events between the two groups were compared.Results:CI,LVEF,FEV/FVC and MVV of the study group at 2 months after intervention were higher than those of the control group(P<0.05).6MWD,MWL,VO2peak of the study group at 2 months after intervention were higher than those of the control group(P<0.05).ET-1,vWF of the study group at 2 months after intervention were lower than those of the control group,FMD was higher than that of the control group(P<0.05).There was no significant difference in the total incidence of adverse cardiovascular events between the two groups(P>0.05).Conclusion:Sitting Baduanjin combined with Sacubitril/Valsartan can effectively improve the cardiopulmonary function,exercise capacity and vascular endothelial function of patients with coronary heart disease after PCI.
6.A Systematic Review of Replacing Tyrosine Kinase Inhibitors with Similar Drugs After Liver Injury

Objective To systematically evaluate the safety and efficacy of replacing tyrosine kinase inhibitors(TKIs)with similar drugs after they cause liver injury,based on case reports and case series studies.Methods PubMed,Embase,Cochrane,Web of Science,CNKI,Wanfang,and VIP databases were searched up to October 2023.Two reviewers independently screened the literature,extracted data,and assessed the quality of the studies.Descriptive and statistical analyses were performed.Results Twenty-six studies(22 case reports and 4 case series)were included,involving 75 patients who switched to similar drugs for TKI-induced liver injury,primarily targeting the epidermal growth factor receptor(EGFR),anaplastic lymphoma kinase(ALK),and multiple targets.The results indicate that majority of patients have favorable safety profiles after switching drugs,with normal liver function or no severe liver injury.Only one patient reported a serious liver adverse reaction,characterized by(grade 3 total bilirubin elevation).In terms of clinical efficacy,most patients responded well to the new TKIs,maintaining stable disease,or showing no progression during follow-up.Disease progression occurred in only two patients who switched from gefitinib to erlotinib after dose reduction due to non-hepatic adverse effects.Additionally,one patient who switched from erlotinib to afatinib experienced worsening tumor symptoms.Conclusion Evidence from published case reports and case series suggests that continuing treatment with similar drugs after TKI-induced liver injury,targeting EGFR,ALK,and multiple targets,demonstrates a certain degree of safety,efficacy,and clinical practicability.This can be a strategy after TKI discontinuation due to liver injury.However,there are no clear guidelines on drug selection,timing,and dosage,and further research is urgently needed.
7.A longitudinal case study on the dynamic evolution of rural family doctor contract service mode

Objective:To explore the development context and evolution mechanism of family doctor contract service mode in rural areas,and provide reference for relevant departments to continuously optimize the family doctor contract service policy.Methods:Based on the principle of theoretical sampling,J City in S Province,which is listed as one of the national primary health comprehensive pilot areas,was selected as the case study object,and the data were collected through interviews and literature review,and then grounded and coded with NVivo11 software.Results:74 initial concepts,24 sub categories and 9 main categories were sorted out,and the core categories were formed through selective coding.The signing service of family doctors in rural areas has gone through three stages:individual signing of rural doctors,team signing and team signing under the county medical community.Its evolution conditions have gone through the formalization of signing,service capability and efficiency synergy in turn.The three elements of the system adaptation action successively presents differential execution under the system de embedding,collaborative block under the structure embedding and collaborative execution under the system coupling,so as to gradually realize the system coupling from responsibility construction to ability improvement,and then to responsibility,ability and interest.Conclusion:the evolution mechanism of the contracted service mode of family doctors in rural areas reflects the dynamic adaptation under the conditions of political situation,social situation and policy situation,and gradually realizes the closed-loop of the three elements of responsibility,ability and interest based on the differentiation of the three elements of the system.
8.Isolation,identification,and biological characterization of enterotoxigenic Escherichia coli from a South China tiger

This study was aimed at identifying the pathogenic bacteria responsible for the death of a young tiger at the Fujian Meihua Mountain South China Tiger Breeding Research Institute.Tissue samples from the lungs,liver,and intestines of the deceased tiger were collected,and the bacteria were cultured inasterile environment.The bacterial strains were characterized according to their morphological and molecular biological properties,including assessment of virulence genes and antibiotic resistance genes,mouse lethality tests,and antibiotic susceptibility evaluations.A predominant bacterial strain isolated from the liver of the deceased tiger was identified as enterotoxigenic Escherichia coli(ETEC)strain Tiger22513F.Phylogenetic analysis of the 16S rRNA gene revealed that the Tiger22513F strain exhibited close genetic similarity to the reference strain ETEC(MF919609.1),with 99.9%nucleotide similarity,and resided on the same evolutionary branch.The Tiger22513F strain contained 11 antibiotic resistance genes(tetA,sul1,sul3,cmlA,floR,blaTEM,blaSHV,blaCMY-2,qnrA,qnrS,and qnrD)along with five virulence genes(VT1,fyuA,tsh,iucD,and ST).Mouse lethality tests indicated significant pathogenicity toward mice,affecting primarily the lungs,liver,and intestines.Antibiotic susceptibility testing demonstrated that this strain exhibited resistance to various classes of beta-lactam antibiotics,as well as quinolones and aminoglycosides.This investigation successfully isolated a multi-drug resistant enterotoxigenic Escherichia coli strain with pronounced pathogenicity from the liver of a deceased tiger;thus providing valuable scientific insights for clinical diagnosis,as well as prevention and control measures,against ETEC infections in South China tigers.
