To report two cases of psychiatric symptoms associated with zonisamide, an antiepileptic drug, and raise clinical awareness of this potential adverse effect. Two male patients with epilepsy treated with zonisamide were retrospectively analyzed. Case 1 (25 years old) developed acute emotional and behavioral abnormalities (e.g., insomnia, aggression, incoherent speech) after switching from sodium valproate to zonisamide (200 mg/day). Case 2 (48 years old) had long-term zonisamide use (≥5 years) with persistent treatment-resistant psychotic symptoms (e.g., delusions, command hallucinations). Clinical courses, medication adjustments, and symptom responses were documented. In Case 1, psychiatric symptoms resolved after discontinuing zonisamide and switching to sodium valproate, with improved mood stability and reduced impulsivity. In Case 2, despite escalating antipsychotic medications (risperidone, clozapine), psychotic symptoms persisted, likely due to ongoing zonisamide use. Both cases highlighted zonisamide’s potential to exacerbate or induce psychiatric manifestations, possibly via mechanisms involving sodium/calcium channel inhibition and neurotransmitter dysregulation (e.g., dopamine, serotonin). Zonisamide can cause or worsen psychiatric symptoms, particularly in vulnerable individuals. Clinicians should monitor for mental health changes during zonisamide treatment and consider drug discontinuation or substitution with alternative antiepileptics (e.g., sodium valproate) if psychiatric adverse effects emerge. Awareness of this association is crucial to avoid misdiagnosis and optimize epilepsy management.

ObjectiveTo investigate the role of interleukin （IL）-8 secreted by tumor-associated macrophages （TAMs） in colorectal cancer （CRC） metastasis and its potential therapeutic value. MethodsThe TAMs model was constructed by phorbol ester （PMA） combined with IL-4/IL-13， and the model phenotype was verified by flow cytometry， RT-qPCR and ELISA. Human CRC HCT116 cells were treated with conditioned medium （CM） from M0 macrophages and TAMs， and the metastatic capacity of HCT116 cells was evaluated by wound-healing assay and Transwell assay. The expression levels of IL-8 in M0 macrophages and TAMs were detected by Western blot， RT-qPCR and ELISA. The expression level of epithelial-mesenchymal transition （EMT） markers （E-cadherin， N-cadherin） in HCT116 cells treated with CM-M0 and CM-TAMs were detected by Western blot and RT-qPCR. HCT116 cells were stimulated with CM-TAMs supplemented with IL-8 receptor inhibitor Reparixin. The HCT116 cells metastasis ability was evaluated by wound-healing assay and Transwell assay， and the expression levels of E-cadherin and N-cadherin were detected by Western blot and RT-qPCR. Finally， a splenic injection liver metastasis model was established using murine CRC cells CT26-LUC， and the effect of Reparixin on this animal model was observed by in vivo imaging. ResultsFlow cytometry showed that the proportion of M2 macrophages （CD11b⁺CD206⁺ cells） was elevated in the TAMs model. RT-qPCR and ELISA assays revealed that the levels of M2 macrophage markers IL-10 and TGF-βsignificantly increased. Wound-healing assay and Transwell assay demonstrated that CM-TAMs significantly enhanced the migration and invasion ability of HCT116 cells. Western blot， RT-qPCR and ELISA assays showed that TAMs highly expressed IL-8 and simultaneously induced downregulation of E-cadherin and upregulation of N-cadherin in HCT116 cells. Reparixin not only reversed the pro-metastatic effects of CM-TAMs， but also upregulated the expression level of E-cadherin and downregulated that of N-cadherin in HCT116 cells. Furthermore， it reduced the bioluminescence signal intensity of liver metastatic lesions in the splenic injection liver metastasis model. ConclusionThe TAMs promote CRC metastasis by secreting IL-8 to induce EMT. Inhibiting IL-8 secretion in TAMs or blocking the binding of IL-8 to its receptor may provide a new strategy for CRC treatment.

Hepatocellular carcinoma(HCC)is the third leading cause of cancer-related deaths,and al-though considerable progress has been made in the clinical diagnosis and treatment of HCC,the prognosis for patients remains poor,with a 5-year survival rate of only about 18％.The development of HCC is driven by the occurrence of mutations in driver genes that can directly promote cell proliferation,survival and metastasis.With the development of molecular biology and genomics technologies,researchers have elucidated that driver mutations that give HCC cells a selective growth advantage,the ability of the cells to resist apoptosis,maintain proliferative signals,initiate invasion and metastasis,induce angiogenesis,enable metabolic remodeling and immune escape.Exploring the key drivers of HCC development to fur-ther elucidate the pathogenesis of HCC may provide new directions for the diagnosis and treatment of HCC as well as improving the prognosis.In this paper,we summarize the driver gene mutations in HCC from various biological pathways such as telomere maintenance,cell cycle,the Wnt signaling pathway,oxida-tive stress,and epigenetic modifications.We also summarize the application prospects of driver gene mu-tations in the diagnosis and treatment of HCC.We aim to provide a reference for the diagnosis,treatment and research of HCC.

Toxoplasma gondii is an intracellular protozoan pathogen with a global distribution,and dogs are considered a potential risk factor for human toxoplasmosis.This study was aimed at systematically analyzing the epidemiological characteris-tics of canine T.gondii infection in China from 1987 to 2023,to provide a scientific basis for the prevention and control of T.gondii in the country.Epidemiological data on canine T.gondii infections in China from 1987 to 2023 were retrieved from PubMed,China National Knowledge Infrastructure(CNKI),Wanfang Database,and Baidu Scholar.A database was established with Excel,and the data were visualized with ArcGIS 10.2 software.Statistical analysis was performed in SPSS 26.0 software,and group differences were analyzed with the X2 test.A P-value of＜0.05 was considered statistically significant.From 1987 to 2023,the overall seroprevalence of T.gondii antibodies in dogs in China remained stable,and the overall sero-prevalence rate was 13.97％.Yunnan Province had the highest seroprevalence,at 27.65％,whereas Shaanxi Province had the lowest seroprevalence,at 0.56％.Significant differences were observed among provinces(P＜0.05).Epidemiological data on canine T.gondii infections were not available for some regions.The seroprevalence in southwestern China was significantly higher than that in other regions(P＜0.05).A comparison of the seroprevalence between 1987-2004 and 2005-2023 revealed significant differences(P＜0.05).Canine T.gondii infection is widespread in China and shows a stable epidemic cycle.Appro-priate prevention and control measures should be implemented,along with strengthened surveillance of T.gondii outbreaks.Public education on the prevention and control of toxoplasmosis should be enhanced to decrease transmission risk and safeguard public health.

Objective:To investigate the effects of TP53 genetic status(wild-type/mutated/null)on the drug resistance of decitabine(DAC)in myelodysplastic syndromes(MDS)and identify key resistance-associated genes.Methods:Two myeloid cell lines with distinct TP53 status(M-07e:wild-type;SKM-1:mutated;)were treated with gradient DAC concentrations(0-10 μmol/L)for 0-72 h.Cell viability was detected by CCK-8 assay.RNA-Seq transcriptomics,and methylation profiling were integrated to analyze differentially expressed genes.Results:Decitabine(DAC)treatment induced time-and dose-dependent inhibition of cell viability in CCK-8 assays,with SKM-1 cells exhibiting the highest resistance(IC50=5 μmol/L vs M-07e=0.5 μmol/L,P＜0.01).Transcriptomic analysis revealed 662 upregulated and 452 downregulated genes in DAC-treated M-07e cells,while SKM-1 cells showed 515 upregulated and 73 downregulated genes.By proteomic profiling,117 upregulated and 136 downregulated proteins were identified in M-07e cells,while 91 upregulated and 46 downregulated proteins were identified in SKM-1 cells following DAC exposure.Through integrated analysis of upregulated genes and proteins expression profiles,181 candidate genes were screened out,while methylation studies identified 884 hypomethylated genes with high-sensitivity loci and CpG density.Notably,31 genes overlapped between these datasets,and functional annotation indicated these drug-resistance-associated genes are primarily involved in positive regulation of cell differentiation,negative regulation of binding processes,and negative regulation of cellular component organization.Conclusion:TP53 mutations drive DAC resistance via epigenetic reprogramming.Targeting these genes may improve outcomes in TP53-mutated MDS.

Under the background of forensic medicine becoming a first-level discipline,the opportuni-ties and challenges of discipline development coexist.Starting from the actual situation and characteris-tics of local medical colleges and universities,this paper discusses the problems and solutions for the development of forensic medicine discipline from the perspective of building a regional high-level medical university.Combined with the experiences of carrying out forensic medicine education in our college,this paper supplies our thoughts and practices on improving the discipline system,enhancing the ability to serve society,perfecting the talent cultivation model and promoting forensic culture,to provide reference and inspiration for the development of forensic medicine in other universities,jointly promote the advancement of forensic medicine in China to a new stage,and contribute the wisdom and strength of forensic medical experts to the construction of a law-based China,a safe China and a healthy China.

Objective To explore the correlation between fasting blood glucose(FBG)level and fractional flow reserve(FFR)in patients with borderline coronary artery disease,and to clarify its potential influence on FFR measurement.Methods From August 2020 to August 2023,the data of 135 patients with coronary atherosclerotic heart disease who received coronary angiography and FFR evaluation in the Fourth Affiliated Hospital of Harbin Medical University were retrospectively collected.According to the exclusion and inclusion criteria,85 cases of borderline diseased vessels of single coronary artery with stenosis degree of 50％-80％were screened out,and they were divided into FBG≥6.1 mmol/L group(47 cases)and FBG＜6.1 mmol/L group(38 cases).The baseline data,angiographic and functional indexes of the two groups were compared,and the correlation between FBG and FFR was analyzed.Results Compared with the FBG＜6.1 mmol/L group,the FBG≥6.1 mmol/L group had a higher proportion of FFR negative results(72.3％vs.23.7％,P＜0.001),and the FFR measurement values were generally increased[0.84(0.80,0.90)vs.0.75(0.68,0.80),P＜0.001],with statistically significant differences.Pearson correlation analysis was performed on all lesions,and FFR＞0.80(negative result)was positively correlated with FBG≥6.1 mmol/L(r=0.484,P＜0.001).Conclusions Among the patients with borderline coronary artery disease(50％-80％stenosis)included in this study,FBG≥6.1 mmol/L is significantly correlated with FFR＞0.80.For patients with borderline coronary lesions with elevated FBG,the influence of blood glucose factors should be carefully considered in clinical interpretation of FFR results.

To investigate the expression and distribution of follicular dendritic cell(FDC)and follic-ular helper T cells(Tfh)in mouse spleen and lymph nodes induced by foot-and-mouth virus-like particles(FMDV-VLPs),the recombinant pCMV-HA-HBcAg-VP1-VP4 plasmid was transfected into CHO-K1 cells to prepare FMDV-VLPs,and the recombinant FMDV VP1-VP4 protein was expressed and purified through a prokaryotic expression system.BALB/c mice were subcutaneous-ly immunized with FMDV-VLPs(VLP group)and recombinant FMDV VP1-VP4 protein(VP1-VP4 group),and PBS control group(PBS group)was set up.At 21 d post primary immunization(dpi),booster immunization was carried out.Spleen,shoulder lymph nodes,and submandibular lymph nodes were collected at 7,28,42,72 and 102 dpi,and frozen sections were prepared.Immu-nohistochemical staining was used to observe the expression and distribution of FDC and Tfh.The results showed that the number of FDC and Tfh cells in the VLP group in the spleen and lymph nodes showed a gradual increase from 7 to 42 dpi,and gradually decreased to non-immune levels starting from 72 dpi.The results indicate that FMDV-VLPs can induce the formation of GC in the spleen and lymph nodes,and the number of FDC and Tfh cells in GC can last for at least 42 d.This study provides a theoretical basis for the study of the immune memory response mechanism of B cells induced by FMDV virus-like particles.
