ObjectiveTranscription factor NFE2 was observed abnormal expression in myeloproliferative neoplasm (MPN) patients. However, how NFE2 is transcriptionally regulated remains ambiguous. This study aims to explore the elements and molecular mechanisms involved in the transcriptional regulation of NFE2. MethodsActive enhancers were predicted by public NGS data and conformed experimentally via dual luciferase reporter assay. After that, PRO-seq and GRO-seq data was used to detect enhancer RNAs transcribed from these enhancers. RACE was utilized to clone the full length enhancer RNA (eRNA) transcripts, and RT-qPCR was used to measure their expression in different leukemia cell lines as well as the transcript levels during induced differentiation. Finally, to investigate the molecular function of the eRNA, overexpression and knockdown of the eRNA via lentivirus system was performed in K562 cells. ResultsWe identified three enhancers regulating NFE2 transcription, which located at -3.6k, -6.2k and +6.3k from NFE2 transcription start site (TSS) respectively. At the -3.6k enhancer, we cloned an eRNA transcript and characterized that as a lncRNA which was expressed and located in the nucleus in three types of leukemia cell lines. When this lncRNA was overexpressed, expression of NFE2 was upregulated and decreases of K562 cell proliferation and migration ability were observed. While knocking down of this lncRNA, the level of NFE2 decreases correspondingly and the proliferation ability of K562 cells increases accordingly. ConclusionWe identified an enhancer lncRNA that regulates NFE2 transcription positively and suppresses K562 cell proliferation.

OBJECTIVE To evaluate the accuracy of three individualized drug delivery tools, i.e. JPKD, SmartDose and NextDose, in predicting tacrolimus dose and blood concentration after kidney transplantation, and analyze the influential factors of prediction accuracy. METHODS The clinical data of adult hospitalized patients treated with tacrolimus after kidney transplantation from January 2021 to June 2023 were retrospectively collected. Three individualized dosing tools, i.e. JPKD, SmartDose and NextDose, were used to predict the dose and plasma concentration of tacrolimus. The absolute prediction error (APE) and prediction error (PE) between the measured value and the predicted value, and prediction success rate were calculated (APE<30% indicating a good forecast). Pearson assay or Spearman assay was used to analyze the correlation between the predicted dosage and actual dosage, as well as the predicted and measured blood concentration values using three software; univariate analysis was used to investigate the influential factors for prediction accuracy of JPKD, SmartDose and NextDose. RESULTS A total of 110 hospitalized patients were included in this study, and tacrolimus doses and plasma concentrations were monitored. The predicted doses of JPKD, SmartDose and NextDose were (2.0±0.7), (2.7±1.9), (1.8±0.8) mg, their measured value was (1.9±0.6) mg, and the correlation coefficients between the predicted values and the measured value were 0.841, 0.450, 0.247 (P<0.001); the median APEs were 6.00%, 52.07% and 30.40%, and the median PEs were 5.00%, 18.50% and -3.50%; the prediction success rates were 98.45%, 30.05% and 49.22%. The predicted values of tacrolimus concentrations using JPKD, SmartDose, NextDose were (6.74±3.36), (6.93±5.02), 9.00(5.80±12.60) ng/mL, the measured value was 8.64(7.11,9.77) ng/mL, and the correlation coefficients between the predicted values and the measured value were 0.997 (P<0.001), -0.066 (P=0.360), 0.920 (P<0.001). The median APEs were 5.54%, 45.91% and 35.56%, and PEs were -4.94% (median), -17.050% (median) and 36.93% (average value); the prediction success rates were 97.93%, 32.64% and 37.31%. Univariate analysis showed that the dosage, blood concentration, body weight, transplantation time and others were related to the prediction accuracy (P<0.05). CONCLUSIONS The good prediction rates of tacrolimus dose and blood concentration in kidney transplant patients using three personalized drug delivery tools, from high to low, are JPKD, NextDose, and SmartDose, suggesting that JPKD can be prioritized in clinical use.

Objective To evaluate the characteristics of different antigen-specific T cell immune responses to severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)after inoculation with 2 doses of SARS-CoV-2 inactivated vaccine for 12 months. Methods Fifteen healthy adults were enrolled in this study and blood samples collected at 12 months after receiving two doses of SARS-CoV-2 inactivated vaccine. The level and phenotypic characteristics of SARS-CoV-2 antigen-specific T lymphocytes were detected by activation-induced markers(AIM)based on polychromatic flow cytometry. Results After 12 months of inoculation with 2 doses of SARS-CoV-2 inactivated vaccine, more than 90%of adults had detectable Spike and Non-spike antigen-specific CD4+ T cells immune responses(Spike:14/15,P=0.0001;Non-spike:15/15,P<0.0001).80%of adults had detectable Spike and Non-spike antigen-specific CD8+ T cells immune responses(Spike:12/15,P=0.0463;Non-spike:12/15,P=0.0806).Antigen-specific CD4+ T cells induced by SARS-CoV-2 inactivated vaccination after 12 months were composed of predominantly central memory(CM)and effector memory 1(EM1)cells.On the other hand,in terms of helper subsets,antigen-specific CD4+ T cells mainly showed T helper 1/17(Th1/17)and T helper 2(Th2)phenotypes.Conclusions SARS-CoV-2 inactivated vaccination generates durable and extensive antigen-specific CD4+ T cell memory responses, which may be the key factor for the low proportion of severe coronavirus disease 2019(COVID-19)infection in China.

Objective To observe the effectiveness of fluorescence labeling-based assay bundle intervention in the prevention and control of multidrug-resistant organism(MDRO)infection. Methods Patients who were detected MDRO in a hospital from January to December 2022 were selected as the research subjects.MDRO monitoring data and implementation status of prevention and control measures were collected.Fluorescence labeling assay was adopted to monitor the cleaning and disinfection effectiveness of the surrounding object surface of the bed units.Based on the bundled prevention and control measures as well as management mode of the pre-intervention group, the post-intervention group implemented enhanced rectification measures for the problems found by the pre-interven-tion group.Changes in relevant indicators between January-June 2022(before intervention)and July-December 2022(after intervention)were compared. Results There were 136 MDRO-infected patients in the pre-intervention group,208 MDRO strains were detected and 10 healthcare-associated infection(HAI)occurred. There were 128 MDRO-infected patients in the post-intervention group,198 MDRO strains were detected and 9 HAI occurred.Af-ter intervention, the total detection rates of methicillin-resistant Staphylococcus aureus(MRSA),carbapenem-re-sistant Acinetobacter baumannii(CRAB),and total MDRO from patients decreased significantly compared to before intervention(all P<0.05).After intervention, the detection rates of MRSA,carbapenem-resistant Pseudomonas aeruginosa(CRPA),CRAB,and total MDRO from the surrounding object surface were all lower than those before intervention(all P<0.05).The detection rate of MDRO from surrounding object surface before intervention was 34.52%,which showed a decreased trend after intervention(P<0.05).The clearance rate of fluorescent labeled markers before intervention was 41.84%,which showed an upward trend after implementing intervention measures(from July to December),and increased to 85.00%at the end of intervention(November-December).The comp-liance rates of issuing isolation medical orders, placing isolation labels, using medical supplies exclusively, and cor-rectly handling medical waste after intervention have all increased compared to before intervention(all P<0.05).Conclusion Adopting fluorescence labeling-based assay bundle intervention can effectively improve the effectiveness of MDRO infection prevention and control.
