Objective To investigate the distribution patterns of TCM syndrome elements and syndrome types in Klebsiella pneumoniae pneumonia. Methods The study adopted a retrospective research method to collect the basic demographic data, pathogenic characteristics,TCM syndrome patterns, and four diagnostic examination information(including tongue and pulse manifestations)of hospitalized patients with Klebsiella pneumoniae pneumonia at Yueyang Hospital of Integrated Traditional Chinese and Western Medicine,Shanghai University of Traditional Chinese Medicine, from September 2022 to June 2024.Based on clustering analysis and factor analysis, the study explored the characteristics of TCM syndrome elements and the distribution patterns of TCM syndromes in Klebsiella pneumoniae pneumonia. Results A total of 365 patients(236 males,129 females)aged 23-100 years were included.Statistical analysis identified 33 core syndrome items, with cough being the most frequent(66.0%),followed by viscous phlegm(42.2%);the most common tongue characteristics were red tongue(50.4%)and greasy coating(45.8%),with slippery pulse being predominant(54.0%).Clustering analysis revealed four syndrome types:lung-kidney yin deficiency,phlegm-dampness with qi deficiency, yang deficiency with water overflow, and phlegm-heat obstructing the lung.Phlegm-heat obstructing the lung was the most prevalent(38.1%),followed by yang deficiency with water overflow(22.5%)and lung-kidney yin deficiency(22.2%).Factor analysis identified four common factors encompassing 21 variables, with a cumulative variance contribution rate of 74.888%.The results of syndrome elements were divided into phlegm, heat,qi stagnation, water retention, dampness, yang deficiency, yin deficiency,qi deficiency, and lung, kidney, heart, spleen based on pathological syndrome elements and pathological location syndrome elements. Conclusion Klebsiella pneumoniae pneumonia primarily affects the lungs, kidneys, heart, and spleen, with the lungs being the key organ. The most common TCM syndrome types are phlegm-heat obstructing the lung,lung-kidney yin deficiency, yang deficiency with water overflow, and phlegm-dampness with qi deficiency, among which phlegm-heat obstructing the lung is the most prevalent.

Persistent atrial fibrillation and other factors can cause mitral and tricuspid annular dilation and leaflet regurgitation, leading to severe functional mitral and tricuspid regurgitation.Patients often experience significant heart failure symptoms and poor prognosis. For patients with severe mitral or tricuspid regurgitation who are at high risk or contraindicated for surgical procedures, transbronchial repair(TEER)is an important alternative therapy that can effectively reduce valve regurgitation and improve cardiac function;Although there is a lack of large-scale data on atrial functional reflux, existing experience still shows that TEER can significantly reduce reflux and improve patients'quality of life. However, double valve intervention therapy poses challenges, especially when combined with TEER repair, which is technically more complex,time-consuming,and carries higher risks.Foreign data shows that simultaneous or staged double valve intervention can safely improve cardiac function and increase survival rates, but the optimal intervention strategy still needs further research.Due to the fact that tricuspid TEER devices have not yet been launched in China, only staged treatment can be adopted at present. This case report shows a patient with severe atrial functional mitral and tricuspid regurgitation who underwent staged transcatheter edge to edge repair surgery successfully.During a 1-year follow-up,bilateral valve regurgitation continued to improve, indicating that staged repair of bilateral atrioventricular valve regurgitation through the catheter margin is a feasible and effective treatment option.

Objective To explore the effect of a multicomponent exercise and cognitive stimulation program in elderly patients with mental disorders and sarcopenia,so as to provide references for reducing the risk of falls, preventing and improving sarcopenia, and enhancing cognitive function in patients. Methods The multi-component exercise and cognitive stimulation program was formulated through literature review and expert meeting. In this quasi-experimental study,76 elderly patients with mental disorders and sarcopenia who were hospitalized in a tertiary mental health center in Shanghai from January 2023 to February 2024 were selected as research subjects. They were randomly divided into an experimental group and a control group according to their hospitalization building number(38 cases in each group). The experimental group was treated with multicomponent exercise combined with cognitive stimulation program based on routine nursing, and the control group was treated with routine nursing. The risk of falls, skeletal muscle mass, muscle strength, physical function, cognitive function, and incidence of adverse events were compared between 2 groups after 12 weeks of intervention. Results A total of 75 patients with 37 in the control group and 38 in the experimental group completed the study. The TUG time,6M walking speed and the score of Short Physical Performance Bettery of the experimental group were all lower than those of the control group(P<0.05),and the scores of skeletal muscle mass, muscle strength, calf circumference, physical function and cognitive function of the experimental group were all higher than those of the control group(P<0.05).Neither group experienced any adverse events. Conclusion The application of this multicomponent exercise combined cognitive stimulation program developed for elderly patients with mental disorders and sarcopenia can effectively reduce the risk of falls, enhance the skeletal muscle mass and muscle strength and improve the cognitive function in elderly patients with mental disorders and sarcopenia.

This study analyzed the epidemiological characteristics of imported malaria in Yunnan Province from 2020 to 2023,to provide scientific evidence for formulating measures to decrease imported malaria and prevent re-establishment of malaria transmission.Malaria data reported by the China Disease Prevention and Control Information System were analyzed to determine parasite species;sources of infection;temporal, spatial, and population distributions;and importation routes.A total of 828 malaria cases were reported in the province.Plasmodium vivax and Plasmodium falciparum accounted for 89.98%and 8.33%of cases, respectively.A total of 47.58%of cases were imported from Myanmar, and all P.falciparum malaria ca-ses were from Africa.Thirteen(81.25%)prefectures or municipalities reported malaria, among which Dehong,Baoshan,Kunming, and Lincang reported 94.32%of cases.A total of 52.54%of cases were in young men.The proportion of cross-bor-der personnel flow, land input, and aircraft input were 88.89%and 11.11%respectively.A total of 98.19%of patients sought medical care within 7 days after fever onset, and 82.85%initiated diagnosis for malaria, and 84.90%of diagnoses were con-firmed by health facilities at or below the county level.Imported malaria is a major challenge in preventing re-establishment of transmission in Yunnan. Most imported cases involved cross-border malaria transmission of mainly Plasmodium vivax between China and Myanmar. To achieve malaria elimination, vigilance of health staff in malaria diagnosis and treatment should be pro-moted,and intensive malaria health education should be provided to people traveling to malaria endemic territories,to enable individual protection, and timely diagnosis and treatment after return from endemic countries.

Gastric cancer is a malignant tumor of the digestive system with strong invasiveness and a high metastasis rate. Its morbidity and mortality rank among the top five in the world and the prognosis is closely related to the disease stage.Multidisciplinary comprehensive treatment based on systemic antitumor drugs is generally adopted in patients with advanced or metastatic gastric can-cer,but the prognosis is typically poor. With the in-depth research on the tumor microenvironment, the development of multi-omics technology, and the application of immunotherapy and neoadjuvant chemoradiotherapy, the therapeutic effect of advanced gastric can-cer has been initially improved, and immunotherapy has become the most potential of the treatment strategies. Many studies have found that Helicobacter pylori(Hp)infection status is closely related to the efficacy of immunotherapy for gastric cancer, especially im-mune checkpoint inhibitors(ICIs),but there is not clear whether it has advantages or disadvantages. This article reviews the current research on the efficacy of Hp on ICIs in advanced gastric cancer to provide ideas for further research on the interaction between Hp in-fection and tumor immunotherapy.
