1.Epidemiological characteristics and trends of postoperative pneumonia in 22 tertiary general hospitals in Jiangsu Province

Objective To understand the epidemiological characteristics and trends of postoperative pneumonia(POP)in tertiary general hospitals in Jiangsu Province,and provide theoretical basis for carrying out targeted pre-vention and control measures.Methods Surgery patients from 22 tertiary general hospitals in 12 cities in north,central,and south of Jiangsu Province from January 1,2022 to December 31,2023 were chosen as studied subjects,occurrence of POP was analyzed and compared.Results A total of 848 274 surgical procedures were performed in 22 hospitals,and 3 606 cases of POP occurred,with an incidence of 0.43%.The incidence in 2023 was 0.37%,which was lower than that in 2022(0.49%),with statistically significant difference(P<0.001).The top three de-partments with high incidence of POP were neurosurgery(6.71%),cardiothoracic surgery(2.91%),and general surgery(0.77%).Among hospitals of different grades,the incidence of POP in tertiary first-class hospitals was 0.44%,which was higher than that in other tertiary hospitals(0.37%).There was no statistically significant difference in the incidence of POP between municipal and district/county hospitals(P>0.05).The incidence of POP in hospitals with a bed:infection control full-time staff ratio<200∶1 was lower than that in hospitals with the ratio ≥200∶1(0.39%vs 0.47%,P<0.001),while the incidence of POP in hospitals with a proportion ≥30%of full-time staff being doctors was higher than that in hospitals with a proportion<30%(0.45%vs 0.36%,P<0.001).The incidence of POP in male patients was higher than that in female patients(0.62%vs 0.26%,P<0.001).The incidence of POP in elderly patients aged≥65 was higher than that in patients aged<65(0.73%vs 0.26%,P<0.001).A total of 2 667 strains of infectious pathogens were detected,with the top three being Acine-tobacter baumannii,Klebsiella pneumoniae,and Pseudomonas aeruginosa,accounting for 28.95%,22.72%,and 15.45%,respectively.The detection rates of carbapenem-resistant Acinetobacter baumannii(CRAB),carba-penem-resistant Klebsiella pneumoniae(CRKP),and carbapenem-resistant Pseudomonas aeruginosa(CRPA)were 60.75%,21.45%,and 32.28%,respectively.The detection rate of CRKP decreased in 2023 compared with 2022,with statistically significant difference(P<0.05).Conclusion The overall incidence of POP in tertiary general hos-pitals in Jiangsu Province is relatively low,but there are significant differences among different hospitals.There-fore,perioperative prevention and control measures should be carried out based on the epidemiological characteristics of patients.
2.Application of health education based on TRIZ mode in patients with senile degenerative heart valvular disease

Objective:To analyze the clinical effect of health education based on Theory of Inventive Problem Solving(TRIZ)mode in patients with senile degenerative heart valvular disease(SDHVD).Methods:This randomized con-trolled study enrolled 126 SDHVD patients admitted in Department of Cardiology,The First Affiliated Hospital of Xinjiang Medical University between May 2020 and October 2022.They were divided into control group(n=63,health education mode)and intervention group(n=63,health education based on TRIZ mode),and intervened for 3 months.Coagulation function,cardiac function indexes,mastery of health knowledge,incidence of adverse cardi-ovascular events,compliance of medication and review were compared between two groups.Results:After inter-vention,compared with patients in control group,those in intervention group had significant higher activated partial thromboplastin time(APTT)[(39.22±2.04)s vs.(32.82±1.73)s],prothrombin time(PT)[(14.32±0.39)s vs.(12.73±0.45)s],left ventricular ejection fraction(LVEF)[(58.58±1.87)%vs.(53.58±1.48)%],scores of health knowledge questionnaire[(78.95±2.27)points vs.(55.07±3.63)points]and Morisky Medication Ad-herence Scale-8[(8.27±0.52)points vs.(6.28±0.89)points]and proportion of review(93.65%vs.65.08%),and significant lower left ventricular end-diastolic diameter(LVEDd)[(51.70±2.12)mm vs.(57.61±1.56)mm]and total incidence of adverse cardiovascular events(15.87%vs.36.51%)(P<0.01 all).Conclu-sion:The health education mode based on TRIZ theory could effectively improve coagulation function and cardiac function,improve patients'understanding of the disease,medication and review compliance,and reduce incidence of adverse cardiovascular events in SDHVD patients.
3.Rational use of medical consumables based on difference analysis of consumption proportion of single diseases

Objective To analyze the the differences in the use of medical consumables of single diseases,so as to provide ideas for the supervision of the rational use of medical consumables.Methods The case information of the patients discharged from some hospital in Hunan Province who underwent artificial femoral head replacement for femoral neck fracture,transurethral plasma electrosurgery for prostatic hyperplasia and percutaneous nephrolithotripsy for renal calculi from January 1,2022 to December 31,2023 were selected.Based on the results of the normality test,difference analyses were carried out over the consumption ratios of inpatients undergoing different procedures at different time periods and the consumption ratios treated by different physicians,and the percentile method was used to determine the recommended range for the consumption ratio based on the difference analysis results.SPSS 26.0 software was used for statistical analysis.Results The inpatients undergoing artificial femoral head replacement for femoral neck fracture at different periods had no significant difference in the consumption ratio(P>0.05);the inpatients going through transurethral plasma electrosurgery for prostatic hyperplasia had the consumption ratio in 2023 decreased by 5.22%when compared with that in 2022,with the difference being statistically significant(P<0.05);the inpatients undergoing percutaneous nephrolithotripsy for renal calculi had the consumption ratio at 2023 increased by 6.49%when compared with that at 2022,with the difference being statistically significant(P<0.05).There were no strong correlations between physician and inpatient consumption ratios for the three single-diseases(P<0.05),while the consumption ratios by different physicians for each single disease had differences.There were respectively 50%,40%and 50%of the physicians implementing artificial femoral head replacement for femoral neck fracture,transurethral plasma electrosurgery for prostatic hyperplasia and percutaneous nephrolithotripsy for renal calculi had the average consumption ratios higher than the average annual consumption ratios for the single diseases.The recommended ranges of the consumption ratio was[51.16%,63.89%]for artificial femoral head replacement for femoral neck fracture,[8.76%,10.77%]for transurethral plasma electrosurgery for prostatic hyperplasia and[26.40%,32.80%]for percutaneous nephrolithotripsy for renal calculi.Conclusion Under the premise of ensuring the quality of medical care,medical consumables can be scientifically and rationally supervised by setting a reasonable range,seizing the"vital few"and implementing joint management and control to reduce the consumption ratio.[Chinese Medical Equipment Journal,2025,46(7):87-91]
4.Successful treatment of a case of lethal dose of felodipine poisoning with V-A ECMO

A 36-year-old male developed unconsciousness and no response to voice stimuli after taking approximately 2 050 mg felodipine (the specific time was unknown). Two hours later, he was sent to the department of emergency by his family and admitted to the hospital. His vital signs showed body temperature 35.1 ℃, pulse 148 times/min, respiration 32 times/min, and blood pressure 65/34 mmHg. Acute drug poisoning, acute toxic cardiomyopathy, acute toxic shock, acute type Ⅱ respiratory failure, acute toxic encephalopathy, and acute renal failure were diagnosed based on the patient′s clinical manifestations combined with laboratory tests results, cardiac ultrasound, chest and abdominal CT scans. Endotracheal intubation connected to a ventilator for invasive assisted ventilation, pressure boosting, and fluid resuscitation were given. At the same time, repeated gastric lavage and enema were performed to remove toxins. Blood perfusion was intermittently and repeatedly administered, and continuous renal replacement therapy was used. The blood concentration of felodipine was 1 298 μg/L at 2 hours after admission, and cardiac arrest occurred at 4 hours. Venous-arterial extracorporeal membrane oxygenation (V-A ECMO) treatment was administered immediately. After 48 hours of ECMO operation, sedatives were discontinued and the patient′s consciousness was improved after 4 hours. On the 5th day of ECMO treatment, his heart rate was 72 beats per minute, and blood pressure was 127/65 mmHg. The blood concentration of felodipine decreased to 2 μg/L. The patient′s vital signs were significantly improved and ECMO supportive treatment was withdrawn. After 26 days of hospitalization, the patient recovered and was discharged.
5.Construction and Performance of CD44-targeted Teniposide Nano-delivery System for Anti-B-cell Lymphoma Activity in vitro

Although teniposide(VM26)is widely used in the treatment of lymphoma,its poor water sol-ubility,low bioavailability and systemic toxicities still limit its clinical application.Nano-delivery systems are effective in increasing the bioavailability and reducing the toxicity of VM26,but there is an urgent need to overcome the problem of its non-specific targeting.Therefore,in this paper,we designed and constructed a hyaluronic acid-modified teniposide-targeted nano-delivery system(VM26-TNDS),and characterised its drug encapsulation rate,particle size and zeta potential.We also investigated the effects of VM26-TNDS on B-cell lymphoma cells with different expression of CD44 receptor,in terms of cellular targeting,inhibitory effect of proliferation,and induction of apoptosis and necrosis.The results showed that the drug encapsulation efficiency of VM26-TNDS exceeded 85%,and its liquid formulation could be stably stored at 4 ℃ for more than 6 months without precipitation.Based on CD44 receptor expression,Granta-519(high expression),Raji(medium-low expression)and SU-DHL-4(almost no expression)were screened for cellular experiments.Compared with VM26-NDS,the targeted modification could effec-tively reduce the uptake of VM26-TNDS by RAW264.7 and increase the uptake of VM26-TNDS by CD44 receptor-expressing lymphoma cells.The inhibitory proliferative effect and apoptotic necrosis-inducing a-bility of VM26-TNDS were stronger than those of VM26-NDS for Granta-519 and Raji cells,whereas there was no significant difference in the inhibitory effect on proliferation and ability to induce apoptosis and necrosis between VM26-NDS and VM26-TNDS in SU-DHL-4 cells,reflecting the targeting advantage for VM26-TNDS,as expected.However,its toxic effect on B-cell lymphoma cells only reflected the targeting advantage at some concentrations(0.25 μmol/L and 0.5 μmol/L),which met the expectation.The a-bove results indicate that a teniposide-targeted nano-delivery system,VM26-TNDS,has been successfully prepared in this study.VM26-TNDS improves the delivery efficiency of VM26 by targeting human B-cell lymphoma cells expressing the CD44 receptor,thus killing human B-cell lymphoma cells more effectively and overcoming the problem of non-specific targeting in drug delivery to improve the therapeutic effect.Its biological therapeutic effects and mechanisms still need to be proved by more in vitro and in vivo ex-perimental evidence.
6.Research progress on effects of orexin and its receptor antagonists on epilepsy

Epilepsy is a chronic neurological disorder caused by an imbalance between excitation and inhibition in the central nervous system.Recently,the role of the orexin system in the pathogenesis of epilepsy has garnered significant attention.Orex-in primarily regulates arousal states and enhances neuronal excit-ability through activation of OX1-R/OX2-R receptors.Studies have shown that elevated orexin levels lower the seizure thresh-old,while orexin receptor antagonists(ORAs)exhibit potential antiepileptic effects.ORAs suppress epileptiform discharges by reducing glutamate release,enhancing GABAergic inhibitory effects,and modulating the thalamocortical circuit.Animal ex-periments and preliminary clinical studies demonstrate that OX1R antagonists decrease excitatory synaptic transmission,whereas OX2 R antagonists primarily strengthen GABA-mediated inhibition.With the widespread application of ORAs in sleep disorders,exploring their clinical value as antiepileptic drugs will become a key focus for future research.This review summa-rizes the role of the orexin system in epileptogenesis and discus-ses the research progress and future directions of ORAs as poten-tial antiepileptic agents.

As early mobilization of patients in the intensive care unit(ICU)is increasingly recognized as a key measure for improving patient prognosis and reducing complications,related research is increasing.However,conventional early mobilization methods are often limited by factors such as patient condition,nursing resources,and environmental factors,making it difficult to achieve optimal results.In recent years,virtual reality(VR)technology,owing to its unique immersion and interactive features,has emerged as a focus of research in the field of ICU patient rehabilitation.This review summarizes the advances in the application of VR technology in the early mobilization of ICU patients,focusing on its advantages over traditional methods,specific implementation approaches,clinical outcomes,and challenges encountered during application.By synthesizing existing evidence,this review aims to provide valuable references for clinical practice and to explore future directions for VR technology in ICU rehabilitation,with the goal of promoting its widespread application in the early mobilization of critically ill patients.
10.(Meta)transcriptomic Insights into the Role of Ticks in Poxvirus Evolution and Transmission: A Multicontinental Analysis.

OBJECTIVE:
Poxviruses are zoonotic pathogens that infect humans, mammals, vertebrates, and arthropods. However, the specific role of ticks in transmission and evolution of these viruses remains unclear.
METHODS:
Transcriptomic and metatranscriptomic raw data from 329 sampling pools of seven tick species across five continents were mined to assess the diversity and abundance of poxviruses. Chordopoxviral sequences were assembled and subjected to phylogenetic analysis to trace the origins of the unblasted fragments within these sequences.
RESULTS:
Fifty-eight poxvirus species, representing two subfamilies and 20 genera, were identified, with 212 poxviral sequences assembled. A substantial proportion of AT-rich fragments were detected in the assembled poxviral genomes. These genomic sequences contained fragments originating from rodents, archaea, and arthropods.
CONCLUSION
Our findings indicate that ticks play a significant role in the transmission and evolution of poxviruses. These viruses demonstrate the capacity to modulate virulence and adaptability through horizontal gene transfer, gene recombination, and gene mutations, thereby promoting co-existence and co-evolution with their hosts. This study advances understanding of the ecological dynamics of poxvirus transmission and evolution and highlights the potential role of ticks as vectors and vessels in these processes.
