1.Application study of auricular Gua Sha in patients with chronic renal failure accompanied by nausea and vomiting

Objective To explore the effectiveness of auricular Gua Sha based on auricular vagus nerve stimulation in patients with chronic renal failure accompanied by nausea and vomiting.Methods Using a convenience sampling method,100 patients with chronic renal failure and nausea and vomiting admitted to the Nephrology Department of a tertiary hospital in Zhejiang Province between January 2022 and December 2023 were selected.They were randomly divided into a test group and a control group,with 50 patients in each group.The test group received the conventional treatment and care for chronic renal failure with nausea and vomiting,along with auricular Gua Sha,with a 3-day intervention.The control group received only conventional treatment and care.The Traditional Chinese Medicine(TCM)symptom scores for nausea and vomiting,total effective rate of the efficacy index,symptom improvement time,total number of bowel movements,and adverse events were compared between the 2 groups.Results A total of 100 patients completed the study.After intervention,the scores for nausea and vomiting symptoms were lower in both groups compared to the scores before intervention,with the experimental group having lower scores than the control group(P<0.001);the total effective rate of the experimental group was 98.00%,higher than 88.00%in the control group(P<0.001).Following intervention,the time for improvement of nausea and vomiting symptoms in the experimental group was(7.50±1.74)minutes,shorter than(13.38±2.31)minutes in the control group(P<0.001);the total number of bowel movements in the experimental group was(3.34±0.63)times,more than(2.46±0.58)times in the control group(P<0.001);no adverse events occurred during the study.Conclusion Auricular Gua Sha based on auricular vagus nerve stimulation helps reduce nausea and vomiting symptom scores,improves the total effective rate of the efficacy index,shortens the symptom improvement time,increases the frequency of bowel movements,and it is safe for patients with chronic renal failure.
2.Effect and mechanism of Yiqi tongfu xiere prescription on lipopoly-saccharide-induced acute lung injury in mice

AIM:To investigated the mechanism of action of Yiqi tongfu xiere prescription(YTX)in treating mice with lipopolysaccharide(LPS)-in-duced acute lung injury(ALI).METHODS:According to the random number table,24 C57BL/6 mice were divided into 4 groups:control group(Control),model group(LPS),low dose group(YTX-L)and high dose group(YTX-H).Except for the control group,the mice models of acute lung injury were established by intratracheal instillation of LPS solu-tion(5 mg/kg).The low and high dose treatment groups were given intragastric administration con-tinuously for 14 days.After 24 hours,the lung tis-sue,bronchoalveolar lavage fluid(BALF)and serum of the four groups were taken for follow-up detec-tion.The degree of pulmonary edema was evaluat-ed by wet weight coefficient(wet to dry ratio,W/D)of lung tissue.The degree of alveolar inflamma-tion and pulmonary fibrosis were evaluated by HE and Masson staining,and the contents of BALF and serum inflammatory cytokines IL-1β and IL-6 were detected by ELISA.The protein expressions of α-SMA,FN,Col-Ⅰ and Col-Ⅲ were measured by West-ern blot.Determination of α-SMA,FN,Col-Ⅰ,MAPK,NF-κB mRNA expression by RT-PCR method.RE-SULTS:Compared with LPS group,the contents of BALF,IL-1β and IL-6 in serum,Wmax D ratio,lung pathology,serum α-SMA,FN,Col-Ⅰ,Col-Ⅲ protein expression and α-SMA,FN,Col-Ⅰ,MAPK,NF-κB mRNA expression in treatment group were signifi-cantly lower than those in control group.CONCLU-SION:YTX can significantly reduce the levels of pul-monary fibrosis markers such as α-SMA,FN,Col-Ⅰand Col-Ⅲ by inhibiting the activation of MAPK/NF-κB signal pathway,and improve alveolar inflamma-tion and pulmonary fibrosis in mice with lung inju-ry,suggesting that YTX can treat acute lung injury and provide a theoretical basis for the clinical use of YTX.
3.Correlation analysis between expression of cytokeratin 19 and clinical pathological characteristics and prognosis of dual-phenotype hepatocellular carcinoma

Objective:To investigate the correlation between the expression of the immunohistochemical marker cytokeratin (CK)-19 and the clinical characteristics and prognosis of dual-phenotype hepatocellular carcinoma (DPHCC).Methods:The data of patients diagnosed with hepatocellular carcinoma (HCC) who underwent surgical resection were collected. DPHCC cases were screened by immunohistochemistry, followed up, and grouped. The correlation between the expression of the immunohistochemical marker CK-19 and the clinical pathological characteristics and prognosis of DPHCC was analyzed by statistical methods. The enumeration data were compared using the χ2 test or Fisher's exact probability method between groups. Results:The expression of CK19 was significantly correlated with factors such as the tumor size, histological grade, liver tissue cirrhosis surroundings, microvascular invasion (MVI), and serum carbohydrate antigen 199 (CA-199) levels in DPHCC, and the differences were statistically significant ( P<0.05). There was no significant correlation between the expression of CK19 and the gender, age, tumor necrosis, multiple lesions, liver capsule invasion, serum alpha-fetoprotein (AFP), and immunohistochemical CK7 and mucin 1 (MUC-1) in DPHCC patients, and the differences were not statistically significant ( P>0.05). The results of univariate analysis showed that immunohistochemical CK19 expression, MVI, number of lesions, tumor necrosis, tumor differentiation degree, serum AFP, and carbohydrate antigen 199 levels were related factors affecting the prognosis in DPHCC patients ( P<0.05); while gender, age, capsule invasion, tumor size, and expression of immunohistochemical markers (vascular endothelial growth factor, CK7, MUC-1) were not significantly correlated with the prognosis in DPHCC patients ( P>0.05). The results of multivariate analysis showed that tumor necrosis ( P=0.042, 95% CI: 1.031-5.501) and serum AFP levels were independent risk factors affecting the prognosis in DPHCC patients ( P<0.001, 95% CI: 2.581-24.075). Conclusions:The expression of CK19 is closely related to the prognosis of patients with DPHCC. Patients with high CK19 expression have faster disease progression than those with low CK19 expression. Furthermore, the overall survival rate of patients with high CK19 expression is significantly lower than that of patients with low CK19 expression, which is a risk factor for poor prognosis in patients with DPHCC.

Objective To establish an expert consensus on home exercise for prevention and rehabilitation of breast cancer related lymphedema.Methods Evidence-based nursing method was used to systematically retrieve and evaluate relevant studies in databases and guideline networks,with a search period from the establishment of the databases to October 2023.Evidence was synthesized to form the expert consensus draft after literature screening,quality evaluation,evidence extraction and evidence integration.Then the final draft was formed through expert discussion meeting and 2 rounds of expert letter consultation.Results The consensus includes 5 dimensions with a total of 22 items,involving evaluation of home exercise,management of home exercise,principles of home exercise,program of home exercise and significance of home exercise.Conclusion This"consensus"was constructed in a scientific and rigorous way,and can provide guidance for promoting the home exercise for prevention and rehabilitation of breast cancer related lymphedema,and ensure the safe and effective implementation of home exercise rehabilitation.
5.Prognostic value of circulating plasma cell in newly diagnosed multiple myeloma treated with bortezomib, lenalidomide, and dexamethasone

Objective:To investigate the prognostic value of circulating plasma cell (CPC) in patients with newly diagnosed multiple myeloma (NDMM) undergoing induction therapy with bortezomib, lenalidomide, and dexamethasone (VRD) regimen.Methods:This study retrospectively analyzed clinical data of 152 patients with NDMM treated with the VRD regimen as induction therapy in the Hematology Department of Jiangsu Provincial People’s Hospital from January 2019 to March 2024. The clinical characteristics, efficacy, and prognosis of patients with high and low CPC proportions are compared. The prognosis of patients in the CPC-positive group, CPC-negative conversion group, and CPC-negative group was analyzed.Results:This study included 152 patients with NDMM, comprising 76 males and 76 females, with a median age at onset of 62 (40–77) years. Compared with the group with CPC proportion of <0.105%, patients with CPC proportion of ≥0.105% demonstrated a higher proportion of International Staging System (ISS) stage Ⅲ ( P<0.001), Revised ISS stage Ⅲ ( P=0.023), HGB≤100 g/L ( P=0.015), β 2-microglobulin ≥3.5 g/L ( P<0.001), shorter median progression-free survival (PFS) period (24 months vs 52 months, P<0.001), and shorter median overall survival (OS) period (52 months vs not achieved, P=0.005). Patients in the CPC-negative group demonstrated a longer median PFS period (not reached vs 41 months vs 19 months, P<0.001) and median OS period (not reached vs not reached vs 26 months, P<0.001) compared with patients in the CPC-negative conversion group and CPC-positive group. Multivariate analysis revealed CPC proportion of ≥0.105% ( HR=3.79, 95% CI: 1.95–7.38, P<0.001), positive CPC after induction therapy ( HR=3.54, 95% CI: 1.41–8.87, P=0.007), and cytogenetic high risk ( HR=3.69, 95% CI: 1.85–7.37, P<0.001) as independent risk factors affecting the PFS of patients. Meanwhile, CPC of ≥0.105% ( HR=3.50, 95% CI: 1.29–9.48, P=0.014) and positive CPC after induction therapy ( HR=4.12, 95% CI: 1.13–15.03, P=0.032) are independent risk factors affecting the OS of patients. Conclusion:Patients with NDMM demonstrating high CPC expression have a worse prognosis, with CPC level as an independent prognostic factor.

Post-stroke spasticity, a common sequelae of upper motor neuron lesions, results in motor control deficits and pathological hypertonia that not only reduce patients' activities of daily living but may also cause impairment of adaptive neuroplasticity. Repetitive peripheral magnetic stimulation (rPMS), as a novel non-invasive neuromodulation technique, demonstrates unique clinical potential through targeted modulation of electromagnetic coupling effects in the peripheral neuromuscular system. Although current international studies have validated the therapeutic potential of rPMS for spasticity, significant heterogeneity persists in elucidating its mechanisms of action, optimizing parameter protocols, and standardizing outcome assessment systems. This review innovatively synthesized recent randomized controlled trials (RCTs) and mechanistic evidence, systematically summarizing rPMS-mediated multidimensional intervention paradigms for upper- and lower-limb spasticity. It rigorously examined the correlations between stimulation frequency parameters (low-frequency vs. high-frequency), anatomical targeting (nerve trunk vs. motor point), and clinical outcomes including spasticity severity, motor function, and quality of life. Crucially, the analysis reveals that rPMS may ameliorate spasticity after stroke through dual mechanisms involving local neuroelectrophysiological modulation and central sensorimotor network reorganization, thereby providing a theoretical foundation for developing individualized rPMS clinical protocols and establishing precision treatment strategies.

7.Research progress of LSD1 in regulating tumor immune mechanisms

Lysine-specific demethylase 1(LSD1)plays a regulatory role in tumor immunity.LSD1 affects CD8+T cell infiltration by regulating AGO2 protein level,and it is required for normal B cell proliferation and differentiation via reducing local chromatin accessibility.LSD1 inhibition promotes NK cell cytotoxicity of cancer cells through restoring the expression of ULBPs.LSD1 affects the resistance of M1 macrophages to oxidative stress by regulating the level of SOD2;In addition,regulating the expression of LSD1 affects the anti-tumor function of CD 19 CAR-T cells and immune checkpoint blockers.The development of drugs targeting LSD1 will provide new ideas for immunotherapy of tumors.

The writing of pharmacist-managed clinics documents (hereinafter referred to as “outpatient medication record”) is a necessary part of pharmacist-managed clinics service. Outpatient medication record is an important carrier to reflect the quality of pharmacist-managed clinics service. The Chinese Hospital Association Pharmaceutical Specialized Committee was entrusted by the Pharmaceutical Administration Department of the National Health Commission to lead the formulation of the Guidelines for the Pharmacist-managed Clinics Service and Document Writing and Usage (Reference) (hereinafter referred to as Guidelines) according to the compilation method of group standards and the technical route of “documentation combing→framework establishment→draft writing→opinion collection→Guidelines formation”. The Guidelines standardizes the basic requirements of pharmacist-managed clinics record management and the basic content of record, and provides a general template and two specialized templates including pregnant and lactating pharmacist-managed clinics record template and cough and asthma pharmacist-managed clinics record template, which provides a reference for medical institutions to write pharmacist-managed clinics record. This paper introduces the formulation process of Guidelines and analyzes the key contents of Guidelines, which is helpful for the application practice of Guidelines and further improves the quality of pharmacist-managed clinics work.
9.Development and validation of an oral frailty risk prediction model for lung cancer patients undergoing chemotherapy

Objective To investigate the status of oral frailty(OF)in patients who underwent chemotherapy for lung cancer,identify key factors influencing OF,and develop a risk prediction model.Methods Using convenience sampling,431 lung cancer inpatient were recruited from three Tier-IIIA hospitals in Jiangsu Province between September and November 2024 as the training cohort.The patients were divided into OF and non-OF groups.Relevant data were compared between the two groups.Multifactorial logistic regression analysis was performed to determine factors that associated with OF,and a risk prediction model was created accordingly.Receiver operating characteristic(ROC)curve analysis was used to predict model performance.In December 2024,additional 185 patients from one other Tier-IIIA hospitals were recruited to validate the developed model.Results The prevalence of OF among lung-cancer patients undergoing chemotherapy was 58.93%.Following listed items were identified as the risk factors of OF(all P<0.05):older in age(OR=3.420),poor education(OR=0.030),brain metastasis(OR=7.880),high nutritional risk screening 2002 score(OR=1.550),elevated C-reactive protein(OR=1.100),and elevated lactate dehydrogenase(OR=1.010).ROC area under the curve(AUC)of the model was 0.860(95%CI:0.830-0.900)in modelling cohort and 0.840(95%CI:0.780-0.900)in validation cohort.Hosmer-Lemeshow goodness-of-fit test yielded χ 2=4.870,P=0.770 for the training set and χ 2=2.770,P=0.950 for the validation set.Conclusion The risk prediction model for OF developed in this study demonstrates a good predictive performance and can facilitate early identification of high-risk patients,thereby providing a scientific basis for clinical interventions.

Hereditary breast cancer(HBC)is one of the important subtypes of breast cancer(BC),primarily characterized by mutations in the BRCA1/2 genes.Accurate identification of BRCA1/2 mutation carriers remains a significant clinical challenge.Traditional genetic testing methods for BRCA1/2 mutations are often expensive and complex.In recent years,advancements in radiomics and artificial intelligence(AI)have opened new avenues for non-invasive prediction of BRCA1/2 mutations.This review summarized the current research progresses in utilizing radiomics and AI for screening and predicting of BRCA1/2-related HBC.
