Objective:
To investigate the trends in incidence and mortality of thyroid cancer in cancer registration areas of Zhengzhou City, Henan Province from 2016 to 2021, so as to provide evidence for developing targeted thyroid cancer prevention and treatment strategies.
Methods:
Data on incidence and mortality of thyroid cancer with qualified registration quality were collected from cancer registration offices in Zhengzhou City. Crude incidence and crude mortality were calculated. The Chinese population-standardized incidence and world population-standardized incidence were calculated using the standard age structure from the Fifth National Population Census in 2 000 and Segi's world standard population structure, respectively. The average annual percentage change (AAPC) was used to analyze the trends in the incidence and mortality of thyroid cancer in Zhengzhou City from 2016 to 2021 across different gender, age, and urban-rural area.
Results:
In 2021, the crude incidence, Chinese population-standardized incidence and world population-standardized incidence of thyroid cancer in Zhengzhou City were 42.55/105, 37.43/105, and 32.56/105, respectively. The crude mortality, Chinese population-standardized mortality and world population-standardized mortality were 1.07/105, 0.77/105, and 0.71/105, respectively. The Chinese population-standardized incidence and mortality in females were 2.64 times and 2.83 times those in males, respectively; those in urban areas were 2.25 times and 1.51 times those in rural areas, respectively. The crude incidence peaked in the group aged 55-<60 years at 92.52/105. From 2016 to 2021, the crude incidence and Chinese population-standardized incidence of thyroid cancer showed an upward trend (AAPC=21.797%, 22.725%, both P<0.05), with trends consistent across different genders and urban-rural areas with the total population. The crude incidence showed upward trends in the groups aged 20-<30 years, 30-<40 years, 40-<50 years, 50-<60 years, and 60-<70 years (AAPC=17.237%, 31.026%, 21.289%, 23.010%, and 11.752%, all P<0.05). From 2016 to 2021, the Chinese population-standardized mortality of thyroid cancer showed an upward trend (AAPC=15.196%, P<0.05), while the trend in crude mortality was not statistically significant (P>0.05). The crude mortality (AAPC=21.078%, 25.669%) and Chinese population-standardized mortality (AAPC=24.122%, 28.865%) in females and rural areas showed upward trends (all P<0.05).
Conclusions
From 2016 to 2021, the Chinese population-standardized incidence and mortality of thyroid cancer in cancer registration areas of Zhengzhou City showed an upward trend. Females, urban residents, and middle-aged and young populations are key groups for thyroid cancer prevention and control.

6.Treatment of blastocytic plasmacytoid dendritic cell tumor with bone marrow involvement and no skin lesions: a case report
Clinical Medicine of China 2024;40(2):120-122
Plasm Blastic Plasmacytoid Dendritic Cell Neoplasm (BPDCN) is a rare and highly invasive hematological malignancies. Typical BPDCN usually starts with skin lesions, involves lymph nodes, bone marrow, central nervous system and other soft tissues. It is not sensitive to chemotherapy and radiotherapy, and the prognosis is very poor. A patient with abdominal pain, anemia, hepatosplenomegalosis as the first manifestations, and lack of typical skin lesions was admitted in Bayannur Hospital on August 28, 2020. BPDCN was finally diagnosed by bone marrow morphology, flow cytometry and bone marrow biopsy. Provide reference for clinical doctors to indentify and treat BPDCN by analyzing the clinical data of rare BPDCN cases with no skin lesions.

Objective:To analyze the burden and benefit finding of illness among home caregivers of patients with cognitive impairment after stroke, so as to provide references for formulating targeted intervention strategies. Methods:Using the intentional sampling method, 15 home caregivers of patients with cognitive impairment after stroke treated in the First Affiliated Hospital of Air Force Medical University from July 2019 to December 2019 were selected as research objects. The semi-structured interview was conducted by phenomenological method, and the interview data were analyzed by Colaizzi analysis method. Results:The burden of home caregivers of patients with cognitive impairment after stroke was mainly manifested as excessive psychological pressure, impaired health, heavy financial burden, and separation from social life. The benefit finding of illness was mainly manifested in the respect for life, the improvement of health behavior ability, and the enhancement of family and social responsibility. Conclusions:Home caregivers of patients with cognitive impairment after stroke have both a negative sense of burden and a positive sense of benefit finding of illness. Medical workers should make a dialectical analysis to help resolve the burden of negative feelings, actively cultivate a sense of benefit finding of illness, and improve their behavioral ability of care.

Objective:To investigate the clinical characteristics and prognosis of acute myeloid leukemia (AML) patients with positive TLS-ERG fusion gene. Methods:The clinical data of 9 AML patients with positive TLS-ERG fusion gene in the First Hospital of Jilin University from June 2013 to August 2020 were retrospectively analyzed, and the relevant literature was reviewed. Results:Among 9 patients with positive TLS-ERG fusion gene, there were 5 males and 4 females, with a median age of 16 years old (6-40 years old). Five patients received chemotherapy alone, 3 patients received allogeneic hematopoietic stem cell transplantation (allo-HSCT), and 1 patient did not receive systematic treatment. Among 8 patients with systematic treatment, 1 patient had complete remission after the first induction chemotherapy and 5 patients had complete remission after induction therapy. The median overall survival time of 5 patients with chemotherapy alone was 1.5 months (1-11 months), of which 3 patients did not respond to the first course of treatment and died of infection, and 2 patients died after relapse. The median overall survival time of 3 patients with allo-HSCT was 16 months (13-17 months), of which 2 patients died after relapse and 1 patient had sustained molecular complete remission by the end of follow-up.Conclusions:AML with positive TLS-ERG fusion gene has low incidence rate and poor induction efficacy. Hematopoietic stem cell transplantation may partially improve the survival prognosis of patients, but it cannot overcome the adverse effect of positive TLS-ERG fusion gene on prognosis.

Objective:To analyze the genetic characteristics of varicella-zoster virus (VZV) in Liaoning province in 2019.Methods:Vesicle fluid samples were collected from 32 individuals with suspected VZV infection in Liaoning province from 2019. Nucleic acids were extracted, the suspected samples were screened by real-time fluorescent quantitative PCR. Single nucleotide polymorphisms (SNP) sites on ORF22, ORF38 and ORF62 were detected and analyzed in three open reading frames (ORFS) of positive samples. The samples and sequence data were collected and the gene sequences were compared to determine the genotype of the virus strain, and the vaccine strain was identified from the wild strain. Results:In the 32 suspected samples, VZV-positive strains were detected in 27 samples, all belonged to wild-type strain of Clade 2. Compared with Clade 2 referenced strains, the homology of nucleotide and amino acid for all 10 samples were 98.85%-100% and 96.9%-100%, respectively. There was a synonymous mutation (A→G) in position 37990 of LN-14 strain. The nucleotide homology of ORF22 showed that all 27 samples were on the same branch with the Clade 2 referenced strains and had a vaccine breakthrough cases. Conclusions:This study reveals that the VZV strains circulating in Liaoning province in 2019 were all wild-type strains belonging to Clade 2.

OBJECTIVETo explore the differences in the clinical therapeutic effects on xerophthalmia of and deficiency between the needling technique (the combined reinforcing technique to induce warm sensation) and the reinforcing needing technique achieved by rotating needle.
METHODSA total of 54 patients of xerophthalmia were randomized into an observation group (28 cases) and a control group (26 cases). In both of the groups, the main acupoints included Cuanzu (BL 2), Sizhukong (TE 23), Taiyang (EX-HN 5) and Fengchi (GB 20); the combined acupoints were Danzhong (CV 17), Sanyinjiao (SP 6), Taixi (KI 3) and Taichong (LR 3). In the observation group, the needling technique was used and in the control group, the reinforcing needing technique achieved by rotating needle was adopted. The treatment was given once every day. 10 treatments made one course and 2 courses were required. Before and after treatment, the score of eye symptoms, tear secretion and the breakup time of tear film were observed in the two groups. The clinical therapeutic effects were compared between the two groups after treatment and in 4-week follow-up after treatment separately.
RESULTSAfter treatment, the total effective rate was 92.9% (26/28) in the observation group and was 80.8% (21/26) in the control group, without significant difference in comparison (>0.05). In 4-week follow-up after treatment, the total effective rate was 85.7% (24/28) in the observation group and was 61.5% (16/26) in the control group, indicating the significant difference in comparison (<0.05). After treatment, the differences were significant in the score of eye symptoms, tear secretion and the breakup time of tear film as compared with those before treatment in the two groups (all <0.01). After treatment, the improvements of the score of eye symptoms, tear secretion and the breakup time of tear film in the observation group were better remarkably than those in the control group, indicating the significant differences (all <0.05).
CONCLUSIONThe needling technique achieves the significant therapeutic effects on xerophthalmia of and deficiency and the recurrence rate of the disease is low.
