1.Evaluation of the quality of Jingangteng capsules based on UPLC fingerprinting combined with multi-component content determination

OBJECTIVE To establish the UPLC fingerprint and the method for multi-component content determination in Jingangteng capsules, and to evaluate its quality by combining chemical pattern recognition analysis. METHODS An UPLC method was established. Separation was performed on a Zorbax SB-C 18 Rapid Resolution HD column, with acetonitrile-0.1% formic acid as the mobile phase for gradient elution.Using the Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicines (2012 edition), UPLC fi ngerprints were established for 10 batches of Jingangteng capsules, and similarity was evaluated. SPSS 22.0 and SIMCA 14.1 software were used to perform hierarchial-cluster analysis and orthogonal partial least squares discriminant analysis (OPLS-DA), respectively. The same UPLC method was employed to determine the contents of chlorogenic acid, 3,5-dihydroxy-2-methylbenzoic acid-3- O -glucoside (M1), caffeic acid, astilbin, oxyresveratrol, quercitrin and resveratrol in the 10 batches of samples. RESULTS A total of 17 common peaks were identified in UPLC fingerprints of the 10 batches of samples, of which 7 were identified as chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol. The similarities of 10 batches of samples ranged from 0.820 to 0.985. The results of hierarchial-cluster analysis showed that 10 batches of samples were grouped into four categories: S1-S4 formed one group, S5 and S6 formed another, S7, S8 and S10 formed a third, and S9 formed a fourth, consistent with the OPLS-DA results; the variable importance projection values for peaks 7, 10, 2, 16 (resveratrol), 13 (oxyresveratrol), 11, 6 (caffeic acid), 5 (M1) and 15 (quercitrin) were >1. Quantitative analysis results showed that the contents of chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol were 1.650 8-4.213 7, 0.636 2-2.161 7, 0.031 0-0.086 5, 0.239 1-1.069 3, 0.211 9-1.104 0, 0.488 8-2.399 2, and 0.164 0-0.699 8 mg/g, respectively. CONCLUSIONS UPLC fingerprint and content determination methods established in this study are simple to operate, accurate, reliable and reproducible; when combined with chemical pattern recognition analysis, they can be used to evaluate the quality of Jingangteng capsules. Nine components, such as resveratrol, oxyresveratrol, caffeic acid, M1 and quercitrin, may serve as markers of quality variation.
2.Pathogens isolated from patients with diabetic foot ulcer infection and expressions of SDF-1α,CXCR4,PCT and CRP in peripheral blood

OBJECTIVE To explore the distribution of pathogens isolated from the patients with diabetic foot ulcer infection and observe the expressions of stromal cell derived factor-1α(SDF-1α),chemokine receptor 4(CXCR4),procalcitonin(PCT)and C-reactive protein(CRP)in peripheral blood.METHODS A total of 172 patients with dia-betic foot ulcer who were treated in Jinan People's Hospital from Nov.2020 to Dec.2023 were enrolled in the study and were divided into the infection group with 141 cases and the non-infection group with 31 cases.The dis-tribution and drug resistance of the pathogens were statistically analyzed.The levels of SDF-1α,CXCR4,PCT and CRP were compared between the two groups.The value of the joint detection of the four indexes in diagnosis of the diabetic foot ulcer infection was analyzed.RESULTS Gram-negative bacteria were dominant among 152 strains of pathogens isolated from the patients of the infection group.Escherichia coli,Staphylococcus aureus and Staph-ylococcus epidermidis were the predominant species.The E.coli strains were highly resistant to ampicillin and piperacillin;the S.aureus and S.epidermidis strains were highly resistant to penicillin and erythromycin.There were significant differences in the levels of serum SDF-1α,CXCR4,PCT and CRP between the infection group and the non-infection group(P<0.05);the serum CRP level of the infection group was(15.25±4.65)mg/L,higher than(10.97±3.42)mg/L of the non-infection group(t=4.840,P<0.001).The area under the curve(AUC)of the joint detection of the four indexes was higher than that of the single detection in diagnosis of the diabetic foot ulcer infection(P<0.05),with the sensitivity 85.80%,the specificity 87.10%.CONCLUSIONS The gram-nega-tive bacteria are dominant among the pathogens isolated from the patients with diabetic foot ulcer infection.E.co-li,S.aureus and S.epidermidis are the predominant species.The pathogens vary in the drug resistance and are highly resistant to the commonly used antibiotics.There is association between the changes of SDF-1α,CXCR4,PCT and CRP levels and the diabetic foot ulcer infection.The joint detection of the indexes may facilitate the diag-nosis.

4. Application of personalized osteotomy guide plate in medial pivot knee prosthesis replacement based on MRI and three-dimensional CT

BACKGROUND: Personalized positioning guide plate based on three-dimensional CT data can improve the accuracy of posterior-stabilized prosthesis joint replacement, but bone structure reconstruction based on three-dimensional CT scan data ignores the thickness of distal femoral articular cartilage, which will affect the use of personalized osteotomy guide plate and the clinical effect of total knee arthroplasty. OBJECTIVE: To investigate the safety, accuracy and early clinical efficacy of three-dimensional printed screw positioning guide based on MRI and three-dimensional CT assisted medial pivot-total knee arthroplasty in the treatment of osteoarthritis. METHODS: A total of 110 patients with knee osteoarthritis admitted to Department of Orthopedics, Nanjing Hospital Affiliated to Nanjing Medical University from February 2018 to April 2019 were selected. All patients received medial pivot-total knee arthroplasty, and were randomly assigned to two groups. The 58 patients in the experimental group completed knee osteotomy with the assistance of personalized osteotomy guide plate based on MRI and three-dimensional CT. The 52 patients in the control group completed knee osteotomy with the traditional intramedullary and extramedullary positioning methods. The operation time and intraoperative blood loss of the two groups were recorded. Postoperative follow-up was conducted to review the X-ray films. KSS score of knee function and the visual analogue scale score were evaluated. RESULTS AND CONCLUSION: (1) 110 patients were followed up for 6-10 months without serious postoperative complications. (2) The operation time and intraoperative blood loss in the experimental group were less than those in the control group (P < 0. 01). (3) According to the reexamination of X-ray at 1 month after surgery, the deviation between the postoperative values and the theoretical design values of hip-genus-ankle angle, coronal femoral component angle, coronal tibial component angle and sagittal tibial component angle were all smaller in the experimental group than those of the control group (P < 0. 01). (4) KSS score of knee joint function in the experimental group 1, 3 and 6 months after the surgery was higher than that in the control group (P < 0. 05). There was no significant difference in visual analogue scale score between the two groups (P > 0. 05). (5) The results showed that the personalized osteotomy guide plate assisted internal axial knee prosthesis replacement based on MRI and three-dimensional CT could effectively improve the accuracy of prosthesis placement and shorten the operation time. The method was safe and feasible, and the early clinical effect was satisfactory.
5.Academic procrastination status and related factors of undergraduates from a university in Guangzhou

Objective:To explore the academic procrastination (AP) status and related factors in college students. Methods: Totally 828 undergraduates (277 male, 551 female, age 18-25 years) from Sun Yat-Sen University were investigated with a self-made questionnaire for basic information and Undergraduate Academic Procrastination Questionnaire (UAPQ) for AP status. UAPQ included overall learning procrastination (OLP), homework procrastination (HP), exam preparing procrastination (EPP) and self-learning procrastination (SLP). Students who scored more than 3 in OLP, HP, EPP, SLP were regarded as procrastinator. Result: The procrastination rates of OLP, HP, EPP, SLP were 40.7%, 29.7%, 41.5%, and 47.6%. Multiple linear regression analysis results showed that bigger (β = 0.32) and normal (β =0.15) influence for academic by mobile-phone, higher level of computer dependence (β =0.15) were positively related with academic procrastination. Higher (β =-0.27) and normal (β =-0.17) levels of grade satisfaction were negatively correlated with academic procrastination. The seniors' academic procrastination levels were lower than those in other grade (β =-0.12). Conclusion: It suggests that undergraduates procrastinate widely in many academic activities. Bigger and normal influence for academic by mobile-phone, higher level of computer dependence may be risk factors of procrastination, while being satisfied with grades and seniors may be protective factors.
6.Psychotherapy in patients with optic papilledema undergoing optic nerve sheath fenestration

Objective To explore the effect of psychotherapy in patients with optic papilledema undergoing optic nerve sheath fenestration (ONSF).Methods Using convenience sampling method,a total of 64 patients with intracranial hypertension and optic papillary edema who were diagnosed by Ophthalmology Department of Xuanwu Hospital of Capital Medical University from 2013 to 2017 were selected as the subjects of study.All the subjects were randomly divided into two groups,with 32 patients in each group.Patients in the control group received routine ophthalmic nursing.The patients in the observation group were given six-step psychological intervention.Self-rating Anxiety Scale (SAS) were applied within 2 days after admission and before discharge to evaluate the patients.Results The score of SAS before discharge in the observation group was (46.28±4.48),which was lower than that in the control group (56.84±8.25),and the difference was statistically significant (t=6.363,P<0.01).Conclusions Relevant psychotherapy can improve the anxiety of patients,so that patients can cooperate well with the treatment,which is worthy of clinical application.
7.Clinical therapeutic effect of spinal nerve medial branch ablation through transforaminal endoscopic in treatment of lumbar zygapophysial joint pain

Objective To investigate the clinical value and effect of spinal nerve medial branch ablation through transforaminal endoscopic in treatment of lumbar zygapophysial joint pain. Methods From August 2008 to October 2013 ,96 patients diagnosed as lumbar zygapophysial joint pain were included in the research. 36 patients were treated by spinal nerve medial branch ablation through transforaminal endoscopic ,while 60 patients received conservative treatment. The visual analogue scale (VAS) and Japanese Orthopedics Association(JOA) scores before treatment,1,3,6 and 12 months after treatment were recorded and analyzed. The MacNab scores were recorded and analyzed 12 months after treatment. Results The VAS and JOA scores of surgical treatment group showed significant improvement compared with that before operation (P < 0.05),and there were no significant difference at each time point after surgery. The VAS and JOA scores of conservative treatment group in showed significant improved at 1 months after treatment compared with that before treatment(P<0.05),but there were no significant difference at each time after treatment compared with pretreatment. The VAS and JOA scores of surgical treatment group showed significant improvement compared with conservative treatment group at each time after treatment(P<0.05). The MacNab scores of surgical treatment group(94.44%)was significantly higher than that of conservative treatment group(33.33%). Conclusion The spinal nerve branches ablation under transforaminal endoscopic is an effective and safe minimally invasive spine surgery technique with important clinical value in the treatment of lumbar zygapophysial joint pain.
8.Difference of Metallothionein Gene G-201A Polymorphism in Type 2 Diabetes and Essential Hypertension

Objective To investigate the difference of MT4 G-201A of Metallothionein (metallotionein, MT) family gene locus single nucleotide polymorphisms (SNPs) in type 2 diabetes and essential hypertension. Mothods We selected 324 cases of patients with type 2 diabetes, 301 cases of patients with essential hypertension, 301 case of normal physical examination population. Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) analysis technology was used to detect MT4 G-201A polymorphism. Results (1) The genotype GG, GA and AA frequency of MT4 G-201A in 324 cases of patients with type 2 diabetes,301 cases of patients with essential hypertension, 301 case of normal physical examination population were 39.5%, 46.6%, 13.9%;42.2%, 48.5%, 9.3%;and 42.5%, 51.5%, 6.0%, respectively. The genotypes in all the distribution were in Hardy-Weinberg equilibrium (P>0.05) . (2) MT4 G-201A polymorphism:Three groups of genotype and allele frequency distribution (G and A) had differences (P<0.05) ; there was no difference between essential hypertension group and normal group (P>0.05);there was a difference between type 2 diabetes group and the normal group (P<0.05);there was no significant difference in genotype and allele frequency distribution (P>0.05) . (3) MT4 G-201A polymorphism in Men:Three groups of genotype and allele frequency distribution had differences (P<0.05) . In essential hypertension and type 2 diabetes group, there was no significant difference in genotype and allele frequency distribution (P>0.05) .MT4 G-201A polymorphism between women:Three groups of genotype and allele frequency distribution had differences (P<0.05), there was a difference between type 2 diabetes and normal group genotypes (P<0.05) . Conclusions (1) There is correlation of MT gene G-210A polymorphism and type 2 diabetes, no association with essential hypertension. (2) There is a difference of MT gene G-210A polymorphism in type 2 diabetes in women, no difference in men.
