Osteonecrosis of the femoral head(ONFH)is a disabling orthopedic disease, pathologically characterized by progressive collapse of the femoral head accompanied by destruction of the trabecular and articular cartilage structures, leading to hip joint pain and progressive functional impairment.Bone marrow mesenchymal stem cell-derived extracellular vesicles(BMSC-EVs)show great promise in tissue repair and regeneration, with low immunogenicity and tumorigenicity, and play roles in osteogenesis, angiogenesis, and immunomodulation. This review summarizes the therapeutic effects and underlying mechanisms of BMSC-EVs in ONFH,providing a theoretical basis for its treatment.

Internal carotid artery(ICA)occlusion is one of the main causes of ischemic stroke.Endovascular treatment is one of the main surgical methods for symptomatic non-acute ICA occlusion. However, how to optimize the surgical strategy to improve the success rate of recanalization of occluded vessels require further exploration. The authors reported a case of subacute ICA occlusion treated with endovascular treatment.During the operation, intravascular ultrasound(IVUS)was used for high-resolution and precise assessment of the lumen morphology, plaque characteristics and vessel wall structure in the occluded segment. The surgical strategy was individualized, ultimately achieving successful recanalization of the occluded vessel.Combined with literature review of the clinical characteristics of subacute ICA occlusion and the application of IVUS in this type of lesion,in order to provide reference and inspiration for further optimization of interventional surgical strategies.

The clinical diagnosis and treatment of urinary tract infection has long faced the challenges of insufficient standardization of diagnosis and treatment pathways, irrational use of antimicrobial drugs and high recurrence rate. How to optimize the hierarchical diagnosis and treatment pathway of urinary tract infection, standardize the use of antimicrobial drugs, and reduce the recurrence rate have always been the focus of clinical attention. There is significant heterogeneity in the existing diagnostic criteria for urinary tract infection, which seriously affects the comparability and evidence integration of clinical and research studies. In order to solve the above problems,a consensus on global multidisciplinary diagnostic criteria for urinary tract infection has been formed by international multidisciplinary experts after three rounds of Delphi method.Breaking through the traditional classification framework, the consensus innovatively established a four-dimensional quantitative scoring system including local symptoms and signs, systemic inflammatory response, quantitative analysis of pyuria and urine culture results, and established a hierarchical standard for stepwise urinary tract diagnosis according to the scoring threshold.Based on the key citations related to the consensus, this paper interprets in detail the basis for the selection of core indicators and the establishment of thresholds for the diagnosis of urinary tract infection in the consensus, and focuses on the key issues and implementation paths of the consensus in localization practice. This consensus provides a unified standard for standardizing the clinical diagnosis and treatment of urinary tract infection, improving the homogeneity of clinical research through standardized diagnostic processes, and promoting the standardization of UTI drug research and development and the rational use of antibiotics and precision.

Journal of Clinical Hepatology 2024;40(2):397-401
In recent years, NOD-like receptor protein 3 (NLRP3) inflammasome in tumors has become a research hotspot, especially in melanoma, colorectal cancer, lung cancer, and breast cancer, and more and more evidence has shown that inflammation plays a role in the development, progression, angiogenesis, and invasion of cancer. Hepatocellular carcinoma (HCC) is the most common type of primary liver cancer, and there are still controversies over the role of NLRP3 inflammasome in the development and progression of HCC. Therefore, this article reviews the potential impact of NLRP3 inflammasome in the progression of HCC and its mechanism of action in anticancer therapy, and it is believed that NLRP3 inflammasome can be used as an effective therapeutic target for HCC patients.

Objective To investigate the effects of abdominal massage combined with electroacupuncture point-through-point method on the expressions of circadian clock genes of Clock,BMAL1,PER1 and neurotransmitters contents of ACh and Glu in insomnia rats and its possible mechanism. Methods Totally 50 SD male rats were randomly divided into normal group, model group, abdominal massage group, electroacupuncture point-through-point group and combination group, with 10 rats in each group.Except for the normal group,an insomnia rat model was established by intraperitoneal injection of p-chlorophenylalanine.Abdominal massage group received abdominal massage;electroacupuncture point-through-point group was treated with flat acupuncture,"Baihui"through"Shenting","Sanyinjiao"through"Yinlingquan"(bilateral),"Shenmen"through"Neiguan"(bilateral),the electroacupuncture instrument dilatational wave, frequency of 1 Hz/20 Hz was connected;the combination group was treated with electroacupuncture through acupoints after abdominal massage;1 time/d for each treatment group,a total of 7 days. The normal group and model group were not intervened.HE staining was used to observe the pathological changes of neurons in hypothalamic tissue, the expression of Clock,BMAL1 and PER1 mRNA were detected by RT-qPCR,the expressions of Clock,BMAL1 and PER1 in hypothalamic tissue were detected by immunohistochemical staining, the expression of Clock,BMAL1 and PER1 protein in hypothalamic tissue were detected by Western blot, the contents of ACh and Glu in serum were detected by ELISA.Results Compared with the normal group, the model group rats had a longer sleep latency, shorter sleep duration, severe damage to the cellular structure of the hypothalamic tissue, and a vacuolar like change, with a decrease in the number of neuronal cells, the expressions of Clock and BMAL1 mRNA and protein in hypothalamic tissue increased, while the expressions of PER1 mRNA and protein decreased;the contents of serum ACh and Glu increased, with statistical significance(P<0.05).Compared with the model group, the abdominal massage group, electroacupuncture point-through-point group, and combination group could all shorten the sleep latency, prolong sleep duration, and improve the morphology of hypothalamic neurons, reduce the expressions of Clock and BMAL1 mRNA and protein in hypothalamic tissue,up-regulate the expressions of PER1 mRNA and protein, and reduce the contents of serum ACh and Glu,with statistical significance(P<0.05),the combination group showed the most obvious effects(P<0.05).Conclusion Abdominal massage combined with electroacupuncture point-through-point method can improve insomnia. Its mechanism may be related to the regulation of circadian clock genes Clock,BMAL1,PER1 mRNA and protein expression,as well as neurotransmitter content.
