1.The application of high resolution vascular wall imaging in the diagnosis and treatment of children's central nervous system vasculitis

Objective To investigate the clinical value of high resolution vascular wall imaging(HRVWI)in the diagnosis and treatment of central nervous system vasculitis(CNSV)in children.Methods A total of 36 children with CNSV underwent HRVWI examination were selected.The HRVWI imaging features,involved vessel types and stenosis degree were analyzed retrospectively and compared with magnetic resonance angiography(MRA).Combined with some patients'follow-up and reviewed imaging after treatment.Results HRVWI showed that all of the 36 children with CNSV had cerebral arteritis,and the affected inflammatory ves-sels showed varying degrees of focal arterial wall thickening and enhancement,mainly uniform thickening and centripetal enhance-ment of the affected arterial wall.Most were mid-enhanced,and the stenosis degree was mostly at level 1(21/36).According to the type of involvement cerebrovascular,there were 26 cases of large-medium vessel type,6 cases of small vessel type and 4 cases of mixed type.Compared with MRA,HRVWI had a higher sensitivity in diagnosing CNSV(P<0.05).13 cases were followed up after treat-ment,and HRVWI showed cerebral vasculitis relief in 8 cases and progression in 5 cases,consistent with changes in clinical symp-toms and laboratory indicators.Conclusion The imaging features of CNSV in children with different causes are different.HRVWI has a high sensitivity in the diagnosis of CNSV in children.HRVWI plays an important role in the diagnosis and treatment of CNSV in children.
2.Practice and reflection on the construction of health care big data platform in Peking University First Hospital

Objective:This study aims to summarize the experience of building a research data platform at Peking University First Hospital and propose pathways for constructing healthcare big data platforms in medical institutions to provide insights for the further expansion of the application of the health care big data platform and construction of smart healthcare.Methods:Hospital multi-domain data was collected through a systematic process. After data classification and cleaning, international medical standards and NLP technology were utilized to achieve data standardization and structuring. At the same time, the eCRF form, data source judgment, and mathematical calculations were optimized to ensure the data quality and application value of the scientific research data platform.Results:By carrying out the construction of a scientific research data platform, Peking University First Hospital achieved the transformation of clinical data into research data, completed data collection, pushing, integration, cleaning, and governance, and conducted health care big data research.Conclusions:It is recommended to subsequently expand the data scope, promote the transformation of clinical data into research data, enhance clinical experts' participation in prospective design and data collection, use an integrated data platform for automatic data pushing and calling, explore the application of large language models and conduct technical exploration and reserves.

4.Study on the correlation between prognosis of patients with chronic hepatitis B under interferon treatment and polymorphism of both calcitonin gene related peptide and receptor activity modifying protein 1

Objective:To analyze the association of two single-nucleotide polymorphisms (SNP) [Calcitonin gene related peptide (CGRP) rs155209 and receptor activity modifying protein 1 (RAMP1) rs3754701] and the prognosis of chronic hepatitis B patients who were under interferon therapy.Methods:A total of 317 patients and their anticoagulant blood samples were collected in this study. The SNPs in the CGRP and region RAMP1 were genotyped using matrix-assisted laser desorption/ionization time of flight mass spectrometry. Logistic regression method was used to assess the results from different phenotypic outcomes between cases and controls, after adjusted for sex and age in co-dominant, dominant and recessive genetic models.Results:Data from this study clearly demonstrated the relevance of CGRP rs155209 and RAMP1 rs3754701 with DNA response and ALT response. RAMP1 rs3754701T was strongly associated with both DNA response and ALT response ( OR=2.277, 95 %CI: 1.386-3.741, P=0.001; OR=1.694, 95 %CI: 1.073-2.675, P=0.024). However, CGRP rs155209C was less prone to DNA response and ALT response ( OR=0.150, 95 %CI: 0.083-0.271, P<0.001; OR=0.583, 95 %CI: 0.367-0.925, P=0.022). Conclusions:Results from our study suggested that both RAMP1 rs3754701 and CGRP rs155209 were associated with the prognosis of patients under interferon therapy in Han population, from the northern parts of China while RAMP1 rs3754701T was a protective factor for both ALT response and DNA response, but CGRP rs155209C carriers were less prone to DNA and ALT responses.
